Little is known ...about the early signs of PHP1A, with a delay in diagnosis. We report two PHP1A cases and their clinical and biochemical findings during a 20-year follow-up.
Clinical suspicion was based on obesity, TSH resistance and ectopic ossifications which appeared several months before PTH resistance, at almost 3 years of age. Treatment with levothyroxine, calcitriol and calcium was required in both patients. DNA sequencing of GNAS gene detected a heterozygous pathogenic variant within exon 7 (c.569_570delAT) in patient one and a deletion from XLAS to GNAS-exon 5 on the maternal allele in patient 2. In patient 1, ectopic ossifications that required surgical excision were found. Noticeably, patient 2 displayed adult short stature, intracranial calcifications and psychomotor delay. In terms of weight, despite early diagnosis of obesity, dietary measures were established successfully in both cases.
GNAS mutations should be considered in patients with obesity, ectopic ossifications and TSH resistance presented in early infancy. These cases emphasize the highly heterogeneous clinical picture PHP1A patients may present, especially in terms of final height and cognitive impairment.
Objetivo. Describir a la plataforma Cursos en Línea Masivos del IMSS (CLIMSS) como herramienta de alfabetización en salud, a través de la evaluación de la eficiencia terminal, la ganancia de ...competencias y satisfacción de los usuarios de cursos en línea masivos en el tema de Covid-19. Material y métodos. Se analizaron datos de 20 cursos ofertados entre marzo y octubre de 2020. Se evaluaron las calificaciones pre y pos, el número total de registros, el total de cursos terminados y la satisfacción del usuario. Resultados. Se registraron un total de 4.9 millones de usuarios y 10 millones de inscripciones, en todos los estados de la República mexicana, con una eficiencia terminal de 85%, una ganancia de competencias de 30% y una satisfacción de 9.34 (10). Conclusiones. La plataforma CLIMSS ha mostrado ser una herramienta para la alfabetización en salud con un alcance de millones de mexicanos en temas relacionados con la crisis sanitaria Covid-19.
El presente artículo se desprende del Trabajo Final de Graduación para optar por el grado de Licenciatura en Ciencias de la Educación con énfasis en Orientación de la Universidad de Costa Rica, el ...cual se realizó desde agosto de 2018 y finalizó en el mes de julio del año 2020. En este estudio se pretende reconocer, a partir de los aportes de las personas participantes, las características que fomentan el involucramiento del estudiantado en su proceso educativo. En la revisión documental no se encontraron estudios donde se enfatizará en las características de las personas profesionales en dicha disciplina. De allí la importancia de la investigación realizada. La metodología correspondió a un enfoque cualitativo y método fenomenológico, se realizaron entrevistas semiestructuradas a profesionales en Orientación en ejercicio y se aplicaron grupos focales con estudiantes de secundaria. Como hallazgos, se obtuvo que las características de apertura, empatía, autenticidad y proactividad enriquecieron el trato digno hacia el estudiantado, lo que favoreció el involucramiento. A su vez, la puesta en práctica de dichas características condujo a la satisfacción profesional de las personas orientadoras. Finalmente, las conclusiones reflejan que las características antes mencionadas permitieron considerar las diferencias entre el estudiantado, además de fomentar el papel protagónico para el involucramiento en su proceso educativo. De igual manera, se concluye que la puesta en práctica de estas características; generó bienestar y satisfacción laboral en las personas profesionales participantes en la investigación.

Calcitriol and calcimimetics are used to treat hyperparathyroidism secondary to chronic kidney disease (CKD). Calcitriol administration and the subsequent increase in serum calcium concentration ...decrease parathyroid hormone (PTH) levels, which should reduce bone remodeling. We have previously reported that, when maintaining a given concentration of PTH, the addition of calcimimetics is associated with an increased bone cell activity. Whether calcitriol administration affects bone cell activity while PTH is maintained constant should be evaluated in an animal model of renal osteodystrophy. The aim of the present study was to compare in CKD PTH‐clamped rats the bone effects of calcitriol and calcimimetic administration. The results show that the administration of calcitriol and calcimimetic at doses that induced a similar reduction in PTH secretion produced dissimilar effects on osteoblast activity in 5/6 nephrectomized (Nx) rats with secondary hyperparathyroidism and in Nx rats with clamped PTH. Remarkably, in both rat models, the administration of calcitriol decreased osteoblastic activity, whereas calcimimetic increased bone cell activity. In vitro, calcitriol supplementation inhibited nuclear translocation of β‐catenin and reduced proliferation, osteogenesis, and mineralization in mesenchymal stem cells differentiated into osteoblasts. In conclusion, besides the action of calcitriol and calcimimetics at parathyroid level, these treatments have specific effects on bone cells that are independent of the PTH level.
In vivo studies used 5/6 nephrectomy with parathyroidectomy and replacement of exogenous PTH to eliminate the osteogenic effect of PTH comparing the bone effects of calcitriol vs calcimimetic. The osteogenesis of mesenchymal stem cells in presence of calcitriol was also evaluated. Results show calcimimetic increases bone formation, while calcitriol reduces it. In vitro, calcitriol presence reduces mineralization and osteogenesis of stem cells into osteoblasts. In this experimental study and independently of PTH, we demonstrate that calcitriol administration reduces bone formation.
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) genetic diversity has the potential to impact the virus transmissibility and the escape from natural infection- or vaccine-elicited ...neutralizing antibodies. Here, representative samples from circulating SARS-CoV-2 in Colombia between January and April 2021, were processed for genome sequencing and lineage determination following the nanopore amplicon ARTIC network protocol and PANGOLIN pipeline. This strategy allowed us to identify the emergence of the B.1.621 lineage, considered a variant of interest (VOI) with the accumulation of several substitutions affecting the Spike protein, including the amino acid changes I95I, Y144T, Y145S and the insertion 146 N in the N-terminal domain, R346K, E484K and N501Y in the Receptor binding Domain (RBD) and P681H in the S1/S2 cleavage site of the Spike protein. The rapid increase in frequency and fixation in a relatively short time in Magdalena, Atlantico, Bolivar, Bogotá D.C, and Santander that were near the theoretical herd immunity suggests an epidemiologic impact. Further studies will be required to assess the biological and epidemiologic roles of the substitution pattern found in the B.1.621 lineage.

En el presente artículo se realiza un análisis sobre las formas en que el uso de slurs como “Guerrillo”, “Vándalo” y “Castrochavista”, así como el dogwhistles “Gente de bien”, transforma ...negativamente la realidad social. El uso intencional de estas expresiones conlleva a la institucionalización de prácticas de rechazo, discriminación y odio que movilizan a la colectividad en la toma de decisiones y a la realización de acciones políticas que terminan por atacar la dignidad humana y son una afrenta directa a la democracia. La institucionalización de estas prácticas discursivas da origen a una glotopolítica de orden negativo, lo que hace posible mostrar la intencionalidad de los hablantes y los compromisos pragmático-lingüísticos asumidos por la colectividad.
