This report describes the experience of a therapist living with Parkinson's disease who conducts two types of support groups within the Parkinson's community: a long-term patient support group and a ...care-partner support group. The psychosocial dynamics and therapeutic factors of the two groups are compared and contrasted, including how the therapist-member relationship differs in the two groups. Countertransference issues and how to handle these therapeutically are also described.
The analysis of the solar potential carried out in a defined territorial context and considered as a case study, intends to estimate the potential production of electricity from solar sources ...starting from a survey on the availability of the present surfaces. The use of energy from renewable sources for public purposes, obtained through the installation of integrated photovoltaic systems in public urban areas available and appropriately identified, can represent a valid energy strategy in response to the provisions of the European Directive 2009/28 / EC. In this study we want to show how the upgrading of public areas for parking, areas of interchange, or for the shelter of pedestrians in holding areas for public transport or in parking ones for cars can become an opportunity for energy production. In fact, all these public spaces can be transformed into places designed to characterize and redevelop the built space with the use of appropriately designed urban furnishing elements that integrate, for example, the function of shelter and energy production. This research project aimed at developing a methodology of systematic approach that can represent a further element that is part of the policies that can be drawn up in the next 30 years towards a decarbonisation in the territorial area taken into consideration. The methodology developed concerned the following points: 1) Identification of the areas and spaces present in the territory taken as a case study, suitable for an upgrading project and/or for the insertion of urban furnishing elements made with integrated photovoltaic panels; 2) Calculation of usable areas and consequent calculation of installable photovoltaic surfaces; 3) Identification of the elements of urban furniture made with integrated photovoltaic panels and present in the market. The work identifies and computes, through the use of specific softwares, the areas and spaces suitable for the insertion of integrated photovoltaic systems. Subsequently, after identifying the areas, the quantities of hypothetically installable photovoltaic surfaces and the quantities of energy that can be produced are calculated. The latter are then compared with the quantities needed to cover the energy requirements for public lighting.
Recurrent aphthous stomatitis (RAS) is one of the most common causes of mouth sores in children so the management of this condition is a matter of great importance. YouTube is increasingly being ...used by patients to obtain health-related information. The aim of this work is to examine the quality of information offered by YouTube about mouth sores in children (MSC). Searching the term mouth sores in children, (MSC) displayed 12.300 results. Of the top 60 videos analyzed, 31 were excluded following exclusion criteria. The major source of upload was from healthcare information channels (HC-41,38%), followed by individual users (HP-25.59%), healthcare professionals (IU-17.24%) and generalist information channels (HC-13.78%); 20.69% of them deal with predisposing factors, and related pathologies, the majority of these propose home remedies (60.72%) rather than topical analgesic drugs (21.43%), antimicrobials (7.14%) and topical steroids (3.57). Most of the videos analyzed were slightly useful (68.97%). Information about mouth sores in children on YouTube was poor regardless of the upload source. Analyzing health content on social platforms is a starting point for providing greater quality of health-related information.

The aim of this work is to ...examine the quality of information offered by this platform about oral thrush in children. Searching term oral thrush in children (OTC) displayed a total of 2.790 results. Of the top 60 videos analyzed, 27 were excluded. The main source of upload was from generalist information YouTube® channels (GC) followed by healthcare professionals (HP), individual users (IU), and healthcare information channels (HC); usefulness of videos is successfully correlated with the number of visualization, number of likes and viewing rate and was interdependent with the number of visualizations, number of likes and VR. However, videos on the oral thrush do not have satisfactory quality information. HP themselves, along with HC, do not seem to provide more appropriate information on COT, than GC or IU.

Candida albicans is a typical commensal microorganism of the oral cavity that can become virulent in certain conditions and cause denture stomatitis (DS). Many conditions can predispose for DS, ...including local and systemic alterations. The therapeutical approach to DS is various and both topical and systemic alternatives can be considered. Azoles molecules are the most common used, both topically and systemically, but in recent years various alternatives have been proposed. This review aims to examine the scientific literature to evaluate the effectiveness of conventional therapies and the potential of the new ones.
Photodynamic Therapy (PDT) is a minimally invasive treatment that has shown promising results in treating preneoplasticlesions. PDT reckons on the administration of a drug (photosensitizer), which ...can be used in a topical or systemic form. Photodynamic therapy has been successfully used in the management of a variety of pathologies from different anatomical sites, including the head, neck, brain and lungs, hepatobiliary tree and other gastrointestinal and urological pathologies, skin, gynaecological conditions and in vascular anomalies. Starting from these same considerations, PDT has also been adopted for oral malignant and premalignant lesions treatment, and should be kept under investigation as a promising therapeutic management of leukoplakia and preneoplastic lesions of the oral cavity.
This paper proposes a method to support the sustainable development of existing cities through the optimization of the usage of their urban surfaces (i.e. roofs, facades, streets, public spaces, ...etc.). Adaptation and mitigation strategies aimed at improving resiliency and sustainability of urban areas are highly related to the utilization of these surfaces. The current trends demonstrate the lack of a systemic approach able to integrate multiple possible functions and avoid sub-optimal solutions by considering the physical and morphological characteristics of the urban environment. For example, in cities, conflicts are arising between the surface uses for renewable energy production, urban agriculture, and green solutions. This study aims at systematizing the existing approaches and proposing a novel method to support the effective application of these solutions. In particular, an analytical procedure is presented to support the integration of different surface uses aiming at the maximization of throughputs, while avoiding conflicts. The method has been tested in a district in Bolzano (Italy) and it is replicable in areas with different morphological characteristics and climate conditions. The final configuration, in which several solutions have been systematically applied and integrated, demonstrate the potentialities of the proposed approach. Outdoor thermal comfort in the district is improved, with a reduction of Universal Thermal Climate Index (UTCI) up to −5.8 °C, by simultaneously guaranteeing the production of renewable energy through solar active systems, and the preservation and expansion of existing green areas.
After decades of focus on molecular genetics in cancer research, the role of metabolic and environmental factors is being reassessed. Here, we investigated the role of microenvironment in the ...promotion of malignant behavior in tumor cells with a different reliance on oxidative phosphorylation (OXPHOS) versus lactic acid fermentation/Warburg effect. To this end, we evaluated the effects of microenvironmental challenges (hypoxia, acidity, and high glucose) on the expression of mitochondrial-encoded cytochrome c oxidase 1 (COX I) and two nuclear-encoded isoforms 4 (COX IV-1 and COX IV-2). We have shown that tumor cells with an "OXPHOS phenotype" respond to hypoxia by upregulating COX IV-1, whereas cells that rely on lactic acid fermentation maximized COX IV-2 expression. Acidity upregulates COX IV-2 regardless of the metabolic state of the cell, whereas high glucose stimulates the expression of COX I and COX IV-1, with a stronger effect in fermenting cells. Our results uncover that "energy phenotype" of tumor cells drives their adaptive response to microenvironment stress.
How microenvironmental stress (hypoxia, acidity, and high glucose) supports tumor growth has not yet been fully elucidated. Here, we demonstrated that these stressors promote malignancy by controlling the expression of cytochrome c oxidase I (COX I), and COX IV-1 and COX IV-2 based on the "energy phenotype" of cancer cells (OXPHOS vs. fermentation). Our results uncover a novel process by which the "energy phenotype" of cancer cells drives the adaptive response to microenvironment stress.
AbstractDisorders of sex development (DSD) are a heterogeneous group of pathologies that result in an alteration in sex determination or differentiation. DSD are estimated to affect 1: 4,500 newborns ...and according to the 2006 Chicago Consensus classification, DSD can be divided into three categories: those with a 46 XX karyotype, those with a 46 XY karyotype and those relating to sex chromosomes. It is crucial to correctly identify the pathology already in the first days of life to direct the patient and his family to the best path of care. For this reason, the role of the pediatrician is fundamental in the correct identification of the clinical picture and in supporting the family during the long process that involves the management of these patients. To make a diagnosis, it is necessary to follow a path led by a multidisciplinary team that includes several steps such as the execution of the genetic analysis, the evaluation with diagnostic imaging methods and laboratory evaluations. The therapeutic management, on the other hand, is still very complex even if in recent years we have moved from an attitude of early gender reassignment to an approach of watchful waiting to let the patient choose when she/he is mature enough to do so, which gender she/he feels to belong. It should not be forgotten that throughout this process the pediatrician must be both supportive and clinically active in the management of the child and his family.
We present a design study of an innovative scheme to generate high rep rate (MHz-class) GeV electron beams by adopting a two-pass two-way acceleration in a superconducting (SC) linac operated in ...continuous wave (CW) mode. The electron beam is accelerated twice by being reinjected in opposite direction of propagation into the linac after the first passage. Acceleration in opposite directions is accomplished thanks to standing waves supported in rf cavities. The task of recirculating the electron beam when it leaves the linac after first pass is performed by a bubble-shaped arc compressor composed by a sequence of double bend achromat. In this paper we address the main issues inherent to the two-pass acceleration process and the preservation of the electron beam quality parameters (emittance, energy spread, peak current) required to operate x-ray free electron lasers (FEL) with low jitters in the amplitude, spectral and temporal domain, as achieved by operating in seeding and/or oscillator mode a CW FEL up to 1 MHz rep rate. Detailed start-to-end simulations are shown to assess the capability of this new scheme to double the electron beam energy as well as to compress the electron bunch length from picoseconds down to tens of femtoseconds. The advantage of such a scheme is to halve the requested linac length for the same final electron beam energy, which is typically in the few GeV range, as needed to drive an x-ray FEL. The AC power to supply the cryogenic plant is also significantly reduced with respect to a conventional single-pass SC linac for the same final energy. We are reporting also x-ray FEL simulations for typical values of wavelengths of interest (in the 200 eV–8 keV photon energy range) to better illustrate the potentiality of this new scheme.
