Our aim was to collect a large number of cases to characterize clinical presentation, outcome, and prognosis of chronic intestinal pseusoobstruction in children. We conducted a retrospective ...multicenter study that included children treated for chronic intestinal pseusoobstruction defined as recurrent episodes of intestinal obstruction with no mechanical obstruction, excluding Hirschsprung's disease. In all, 105 children, 57 boys and 48 girls, were studied, including five familial forms. Prenatal diagnosis was made in 18 patients. Eighty patients were less than 12 months old at onset; the disease began at birth for 37 patients. The most frequent signs were abdominal distension, vomiting, and constipation. Megacystis was noted in myopathies (7 cases), neuropathies (10 cases) and unclassified forms (13 cases). For all but three cases (two patients with CMV infection, one with Munchhausen-by-proxy syndrome), the associated diseases and disorders could not account for chronic intestinal pseusoobstruction as a secondary disorder. At least one full-thickness biopsy from the digestive tract was studied for 99 patients. The diagnosis recorded was visceral neuropathy in 58 cases, visceral myopathy in 17 cases, and uncertain or normal biopsy results in 24 cases. Seventy-eight children were fed intravenously, and only 18 were able to be fed orally throughout their illness. Seventy-one patients underwent surgery during their illness, and 217 surgical procedures, a mean of 3 per patient, were performed. Ostomy was the most performed procedure. Follow-up continued in 89 patients for 3 months to 16 years (mean 85 months). Forty-two patients were still fed by parenteral (39 patients) or enteral nutrition (3 patients) at the time of the study. Eleven patients died between the age of 1 month and 14 years 7 months.
Introduction Le stress oxydant joue un rôle dans le développement des complications cardiovasculaires chez les sujets diabétiques de type 2 (D2) et une consommation élevée en fruits et légumes (F&L) ...pourrait prévenir ces dommages oxydatifs. Le but du travail est d’étudier chez des patients D2 l’impact d’une augmentation de consommation en F&L sur la peroxydation lipidique. Patients et méthodes 29 patients ambulatoires (50 ± 15 ans, BMI : 30,3 ± 2,9 kg/m2 ) D2 (Hb1c : 8,6 ± 1, 28 %) avec une consommation moyenne en F&L sont randomisés pour une période de 2 mois en un groupe interventionnel (I) visant à accroître l’apport en F&L et en un groupe contrôle (C). La peroxydation lipidique est évaluée par la détection dans le plasma des peroxydes lipidiques (POXL), des substances thiobarbituriques acides-réactives (TBARS), des LDL oxydés (LDLox), des anticorps anti-LDLox (Abox-LDL), des oxystérols (17 β-oxycholéstérol et 7 céto-cholestérol), des sous-fractions par taille des LDL, des isoprostanes et d’un métabolite du 4-hydroxynonénal (DHN-MA) dans les urines. Résultats L’apport en F&L augmente dans le groupe I (de 300 ± 85 à 629 ± 17 g/j, p = 0,04) mais également dans le groupe C (de 300 ± 85 à 408 ± 36 g/j, p = 0,05). Cependant, par comparaison aux conditions basales, aucune variation significative des concentrations plasmatiques en antioxydants (vitamine C, β-carotène, polyphénols) n’est observée dans aucun des deux groupes. Concernant les biomarqueurs de la peroxydation lipidique, les concentrations plasmatiques en POXL et en LDLox tendent à s’accroître dans le groupe C tandis qu’elles diminuent dans le groupe I (NS). Conclusion En conclusion, une prise en charge diététique spécifique permet d’accroître l’apport en F&L chez des sujets D2. Néanmoins, aucune modification significative des niveaux plasmatiques des paramètres antioxydants n’est observée. Ceci confirme que l’effet d’une alimentation riche en F & L ingérée afin de réduire les dommages oxydatifs lipidiques demeure incertain.
The right to the city defended by Henri Lefebvre in 1968 has been given a new leash of life in the Americas in the last decade. There are new dimensions to citizen action, in the form of ...neighbourhood associations and NGOs, that the French philosopher did not live to see. They are calling for direct local democracy to change the institutional rules of representative democracy, and their political demands are linked to environmental issues. Citizens live here but yearn to intervene in a broader area, the one where they work, consume, relax and have multiple relationships. The city is spilling over its administrative boundaries and is becoming an urban entity with virtual as well as real frontiers. The "right to the city" associates ordinary practices (participatory budgeting, the choice of facilities, district festivities ...) with planetary aspirations (cyberspace, citizens of the world ...). Re-reading Henri Lefebvre involves updating his ideas and keeping them alive in a world that is already different.
El derecho a la ciudad que reivindica Henri Lefebvre en 1968 vive una segunda juventud en las Américas desde hace una década. Ha adquirido nuevas dimensiones por la acción ciudadana de las ...asociaciones de vecinos y de las organizaciones no gubernamentales (ONG) que el filósofo francés no llegó a conocer. Se trata de una demanda de democracia directa local que cambia las reglas institucionales de la democracia representativa y que conecta sus reivindicaciones políticas con la cuestión ambiental. El ciudadano reside aquí pero anhela intervenir sobre un territorio más amplio, aquel en el que trabaja, consume, se divierte o mantiene múltiples relaciones. La ciudad sale de sus límites administrativos y se convierte en un urbano con fronteras lo mismo reales que virtuales. El derecho a la ciudad asocia prácticas ordinarias (el presupuesto participativo, la elección de un equipamiento, la fiesta del barrio ) con aspiraciones planetarias (ciberespacio, ciudadano del mundo ). Releer a Henri Lefebvre consiste en actualizar su pensamiento y en mantenerlo vivo en un mundo que ya es otro.
Biologicals have become an important component in the treatment of Crohn's disease in children. Their increased and long term use raises safety concerns. We describe safety and cost of infliximab in ...Belgian pediatric Crohn's disease patients. All patients on infliximab as part of the present or past treatment for Crohn's Disease until January 1st 2011 were selected from an existing database. Information on disease phenotype, medication and adverse events were extracted. Adverse events occurred in 25.9% of patients exposed to infliximab of which 29.6% were severe. In total 31.7% of patients stopped infliximab therapy. The main reasons for discontinuation were adverse events in 45.4% and loss of response in 30.3%. No malignancies or lethal complications occurred over this 241 patient year observation period. Immunomodulators were concomitant medication in 75% of patients and were discontinued subsequently in 38.4% of them. The cost of infliximab infusions per treated patient per year in the Belgian health care setting is approximately 9 474 euro, including only medication and hospital related costs. Even though infliximab is relatively safe in pediatric CD on the short term, close follow-up and an increased awareness of the possible adverse reactions is highly recommended. Adverse reactions appeared in 25.9% of all patients and were the main reason for discontinuation. Treatment cost has to be balanced against efficacy and modifications in disease course. In the Belgian health care system, the medication is available to all patients with moderate to severe CD.
Depuis plusieurs années, l’obésité et les pathologies associées font l’objet d’une attention particulière en santé publique, au vu des conséquences dramatiques sur le confort de vie et le coût ...sociétal lié à la prise en charge de ces patients. Dans ce contexte, le microbiote intestinal semble être une cible thérapeutique intéressante à prendre en compte. En effet, chez l’animal obèse, l’administration de fructanes purifiés a un impact sur l’inflammation, la prise alimentaire et l’adiposité en modulant le microbiote intestinal. Aucune étude à ce jour n’a envisagé l’intérêt des aliments naturellement riches en fructanes pour le contrôle de l’obésité. La région Wallonne belge soutient le projet FOOD4GUT qui propose une approche multidisciplinaire afin d’imprimer des changements sociétaux, comportementaux et diététiques mettant en exergue l’impact de la consommation de légumes riches en fructanes produits localement sur le comportement alimentaire et les désordres métaboliques associés à l’obésité.
Un volet analytique permettra d’établir une cartographie des systèmes de production et de consommation des sources alimentaires de fructanes et de prébiotiques cultivées en Wallonie. D’autre part, une approche expérimentale préclinique et une étude d’intervention clinique randomisée multicentriques (population obèse avant chirurgie bariatrique) permettra d’étudier les mécanismes fondamentaux par lesquels des aliments riches en prébiotiques peuvent moduler les altérations métaboliques, inflammatoires et comportementales liées à l’obésité.
L’originalité et la faisabilité du projet tient de l’alliance d’experts scientifiques reconnus issus de domaine de compétences en sciences humaines, en sciences biomédicales et en sciences technologiques et agronomiques, issus des trois grands centres universitaires et hospitaliers de la communauté Wallonie-Bruxelles.
Le projet FOOD4GUT veut apporter la preuve qu’un changement de comportement individuel et sociétal mettant en avant l’apport de légumes produits localement, et riches en prébiotiques, crée un réel bénéfice de santé publique, en accord avec le développement écologique et économique de la Région Wallonne.
Pancreas transplantation has now become an established option in the treatment of diabetic complications. It normalizes glucose metabolism, prevents, stabilizes and improves the evolution of ...diabetes-associated lesions. Improvements in surgical procedure and in immunosuppression have better defined its indications. Combined kidney-pancreas transplantation appears today as the best treatment for the diabetic patient with end stage renal disease. Isolated pancreas transplantation is reserved to non-uremic patients with severe diabetic complications or with hyperlabile glycaemic control and severe impairment of quality of life.
