Physical activity is a key determinant of public health and contributes to decreasing the prevalence of many diseases. Cancer is the leading cause of death worldwide. Physical activity, accessible to ...the entire population, could prevent up to 25% of cancers, in addition to improving survival rates and quality of life in cancer patients. Physical activity acts via various mechanisms to slow or decrease tumor growth, including the production and bioavailability of sex hormones, insulin resistance and insulin secretion, and inflammation. In primary prevention, physical activity reduces breast cancer risk by 15-20% and colorectal cancer risk by 24%. All-cause mortality is reduced by 33% in cancer survivors who exercise. Health-related quality of life, fatigue and depression are enhanced by the practice of physical activity in cancer patients. In the general population, the global recommendations on physical activity for health, published by the World Health Organisation, are suggested as a means of primary prevention of cancer. In cancer patients, an adapted physical activity routine is promoted from the very beginning of patient care to decrease fatigue as well as improve tolerance and benefits of treatments.
In view of the lack of recommendations on central venous catheter (CVC)-associated thrombosis in cancer patients, we established guidelines according to the well-standardized Standards, Options and ...Recommendations methodology.
A literature review (1990–2007) on CVC-associated thrombosis was carried out. The guidelines were developed on the basis of the corresponding levels of evidence derived from analysis of the 36 of 175 publications selected. They were then peer reviewed by 65 independent experts.
For the prevention of CVC-associated thrombosis, the distal tip of the CVC should be placed at the junction between the superior cava vein and right atrium; anticoagulants are not recommended. Treatment of CVC-associated thrombosis should be based on the prolonged use of low-molecular weight heparins. Maintenance of the catheter is justified if it is mandatory, functional, in the right position, and not infected, with a favorable clinical evolution under close monitoring; anticoagulant treatment should then be continued as long as the catheter is present.
Several rigorous studies do not support the use of anticoagulants for the prevention of CVC-associated thrombosis. Treatment of CVC-associated thrombosis relies on the same principles as those applied in the treatment of established thrombosis in cancer patients.
Les bisphosphonates sont indiqués pour le traitement des lésions osseuses malignes des tumeurs solides et du myélome multiple. Les bisphosphonates ont montré leur intérêt en cancérologie osseuse car ...ils réduisent le nombre d’événements osseux (hypercalcémie, douleurs, fractures pathologiques, compression médullaire, chirurgie osseuse, radiothérapie à visée antalgique ou décompressive) et retardent leur apparition. Ces résultats ont conduit à l’élaboration de recommandations internationales sur l’utilisation des bisphosphonates pour le traitement des lésions osseuses malignes des tumeurs solides et du myélome multiple. Pour aider le clinicien à prescrire des bisphosphonates en cancérologie osseuse, il nous a semblé important d’établir un guide pratique de leur utilisation sous la forme d’arbres décisionnels et de tableaux comportant des informations sur le bilan pré-thérapeutique, les indications de traitement, la surveillance et les précautions d’emploi à prendre avec ces molécules. En 2007, un groupe de travail s’est mis en place dans le cadre du réseau régional de cancérologie Rhône-Alpes (GIP ONCORA) pour établir ce guide qui a été validé en réunion plénière en juillet 2009 à l’occasion des journées de travail pour la mise en commun des référentiels sur les soins de support qui ont réuni le réseau régional de cancérologie de la Lorraine et le réseau régional de cancérologie Rhône-Alpes (GIP ONCORA).
Bisphosphonates are indicated for the treatment of bone lesions in patients with solid tumours or multiple myeloma. Bisphosphonates have proven their effectiveness in reducing the number of bone complications (hypercalcemia, pain, disease-related fractures, spinal cord compression) and delaying their occurrence in patients with bone tumours; they have also been shown to reduce the need for bone surgery and palliative or pain-relieving radiotherapy in these patients. International recommendations for the treatment of bone lesions related to malignant solid tumours and multiple myeloma have been established. We have elaborated clinical practice guidelines on the use of bisphosphonates to assist treatment decision-making in bone oncology. The guide contains decision trees and tables with information to guide pre-treatment evaluation and patient follow-up, as well as indications and conditions of use of bisphosphonates. In 2007, the regional cancer network of Rhône-Alpes, ONCORA, formed a working group (GIP ONCORA) to elaborate the guideline. The final version was then discussed and adopted at a plenary session in July 2009, during a collaborative workshop on supportive care recommendations organized by ONCORA and the regional cancer network of Lorraine.

Une observation de fuite capillaire idiopathique est rapportée chez un patient de 67 ans présentant des lipothymies récidivantes. Le premier épisode comporte des œdèmes retardés et une hypoalbuminémie imputée à une perte digestive induite par orlistat. Le patient est vu tardivement lors du second épisode au stade de réintégration vasculaire responsable d'une hypertension artérielle pulmonaire transitoire. Le troisième accès typique permet de porter le diagnostic.
Le syndrome de fuite capillaire doit être évoqué devant des épisodes récidivants d'hypovolémie inexpliquée. Le diagnostic en est parfois difficile en raison de formes limitées à des lipothymies isolées ou à un œdème fugace, ou du fait de la baisse retardée de l'albumine plasmatique.
Systemic capillary leak syndrome (SCLS) is a rare disorder characterized by recurrent spontaneous episodes of hypovolaemic shock due to marked plasma shifts from the intravascular to the extravascular space. It presents as the characteristic triad of hypotension, haemoconcentration and hypoalbuminemia.
We describe a patient with SCLS with recurrent lipothymia who presented first with delayed oedema that was thought to be due to orlistat treatment. On the second episode the patient was seen with a pulmonary hypertension when plasma came back into vessels. On the third time the characteristic triad led to the diagnosis of SCLS.
SCLS should be considered in the differential diagnosis of recurrent hypovolemic shock without identifiable cause. Nevertheless, symptoms may be restricted to sole lipothymia or transient oedema or delayed hypoalbuminemia rendering the diagnosis difficult.

Nous rapportons l’observation d’une patiente de 64 ans, porteuse d’une myélofibrose idiopathique avec une hématopoïèse extramédullaire pleurale, stabilisée par un traitement à base d’hydroxyurée.
Les aspects cliniques, biologiques, histologiques, thérapeutiques et évolutifs de la plèvre myéloïde seront discutés.
Agnogenic myeloid metaplasia, associated with myelofibrosis, is a myeloproliferative disorder. Extramedullary hematopoiesis in the pleura is rare and its prognosis is often severe.
Herein we report a 64-year-old woman, who presented with pleural extramedullary hematopoiesis, treated by hydroxyurea-based chemotherapy with disease control.

Rocket motor nozzle flow geometry is considered through its influence on the thrust vector control (TVC) performances. Extensive research is conducted using theoretical and software simulations and ...compared with experimental results. Cold and hot flow test equipments are used. The main objective of the research is to establish the methodology of flow geometry optimisation on the TVC hardware system. Several geometry parameters are examined in detail and their effects on the system performances are presented. The discovered effects are used as guidelines in the TVC system design process. A numerical method is presented for the determination of dynamic response time upper limit for the TVC system based on the gas flow dynamics performances.
L'augmentation du nombre de cancers et la généralisation de la pose des chambres implantables pour leur traitement favorisent l'augmentation de fréquence des thromboses sur cathéter central longue ...durée. Ces thromboses veineuses de cathéter central (TVKTC) chez les patients cancéreux ont des conséquences médicales
: dysfonctionnement du cathéter et risque d'embolie pulmonaire. Elles ont également des conséquences économiques
: durée d'hospitalisation supérieure à celles des thromboses profondes des membres inférieurs (+6 jours) et supérieurs (+4 jours), coût de remplacement de chambre implantable estimé à 4500 €.
Le principal facteur de risque de TVKTC est l'agression de la paroi veineuse lors de la pose du cathéter à l'origine du thrombus qui le plus souvent va disparaître spontanément, mais peut aussi s'organiser sous l'influence de facteurs thrombogènes locaux ou généraux. Ainsi, les TVKTC asymptomatiques sont dépistées par phlébographie dans 30 à 60
% des insertions de cathéter, mais ne sont symptomatiques que dans 5
% cas. Leur traduction clinique est non spécifique associant à des degrés divers, douleur et œdème du membre supérieur ou du cou, circulation collatérale et dysfonctionnement du cathéter. Actuellement, le diagnostic des TVKTC de longue durée symptomatique repose en première intention sur l'échographie couplée au doppler objectivant une incompressibilité de la veine et la présence d'un thrombus adhérant à la paroi veineuse et non limité à l'extrémité distale du cathéter. La place de l'angioscanner ou de l'IRM n'est pas précisée. La phlébographie est réservée aux cas difficiles. Le traitement curatif fait appel, soit aux héparines de bas poids moléculaire (HBPM) avec relais précoce par les antivitamines K pour une durée d'au moins trois à six mois, soit plutôt aux HBPM au long cours par analogie aux thromboses veineuses des membres inférieurs chez le patient cancéreux, cela avec ou sans ablation du cathéter central.
La prophylaxie primaire des TVKTC par HBPM ou AVK n'est plus indiquée au vu d'études méthodologiquement bien menées n'ayant pas démontré de bénéfice. Cependant, la mise sur le marché des nouveaux antithrombotiques et la définition d'une population à risque de thrombose de cathéter pourraient déboucher sur une prophylaxie ciblée efficace.
Increased incidence of cancers and the development of totally implanted venous access devices that contain their own port to deliver chemotherapy will lead to a greater than before numbers of central venous catheter related thrombosis (CVCT). Medical consequences include catheter dysfunction and pulmonary embolism. Compared with lower extremity deep venous thrombosis (DVT) (3 d) and with non CVC associated thrombosis (5 d), CVCT is associated with an increased duration of hospitalisation (9 d). CVCT oftentimes leads to the need to replace such ports at an average cost of 4500 euros.
Vessel injury caused by the procedure of CVC insertion is the most important risk factor for development of CVCT. This event could cause the formation of a fresh thrombus, which is reversible in the large majority of patients. The incidence of CVC-related DVT assessed by venography has been reported to vary from 30 to 60% but catheter-related DVT in adult patients is symptomatic in only 5% of cases. The majority of patients with CVC-related DVT is asymptomatic or has non-specific symptoms: arm or neck swelling or pain, distal paresthesias, headache, congestion of subcutaneous collateral veins. In the case of clinical suspicion of CVC-related DVT, compressive ultrasonography (US), especially with Doppler and color imaging, currently is first used to confirm the diagnosis. The main criteria of color-Doppler US are visualization of mural thrombi or incompressibility of the veins. Consequently, contrast venography is reserved for clinical trials and difficult diagnostic situations. There is no consensus on the optimal management of patients with CVC-related DVT. Treatment of CVC-related VTE requires a 5- to 7-day course of adjusted-dose unfractionated heparin or LMWH followed by oral anticoagulants. Long-term LMWH that has been shown to be more effective than oral anticoagulant in cancer patients with lower limb DVT could be used in these patients. The optimal duration of oral anticoagulation treatment for CVC-related DVT is unknown, but patients with active cancer should be treated for at least 6 months or indefinitely.
The efficacy and safety of pharmacologic prophylaxis for CVC related thrombosis is not established. Additional studies performed in high risk populations are needed to define if LMWH or oral anticoagulation is indicated in this clinical setting.
