Background
Re-resection for incidental gallbladder cancer (iGBC) is associated with improved survival but little is known about residual disease (RD) and prognostic factors. In this study, survival ...after re-resection, RD, and prognostic factors are analyzed.
Methods
Patients with iGBC were identified from the Netherlands Cancer Registry, and pathology reports of re-resected patients were reviewed. Survival and prognostic factors were analyzed.
Results
Overall, 463 patients were included; 24% (
n
= 110) underwent re-resection after a median interval of 66 days. RD was present in 35% of patients and was most frequently found in the lymph nodes (23%). R0 resection was achieved in 93 patients (92%). Median overall survival (OS) of patients without re-resection was 13.7 (95% confidence interval CI 11.6–15.6), compared with 52.6 months (95% CI 36.3–68.8) in re-resected patients (
p
< 0.001). After re-resection, median OS was superior in patients without RD versus patients with RD (not reached vs. 23.1 months;
p
< 0.001). In patients who underwent re-resection, RD in the liver (hazard ratio HR 5.54;
p
< 0.001) and lymph nodes (HR 2.35;
p
= 0.005) were the only significant prognostic factors in multivariable analysis. Predictive factors for the presence of RD were pT3 stage (HR 25.3;
p
= 0.003) and pN1 stage (HR 23.0;
p
= 0.022).
Conclusion
Re-resection for iGBC is associated with improved survival but remains infrequently used and is often performed after the optimal timing interval. RD is the only significant prognostic factor for survival after re-resection and can be predicted by pT and pN stages.
« La République ne craint pas la diversité ... Diversité des parcours, diversité des origines, diversité des couleurs, mais pas diversité des races. Il n’y a pas de place dans la République pour la ...race », affirmait François Hollande le 10 mars 2012. Le candidat à la présidentielle proposait ainsi de supprimer le mot « race » de la Constitution, considérant que la race était un concept inexistant dans la République française et dans ses politiques publiques. Norovirus (NoV) is highly infectious and is the major cause of outbreak gastroenteritis in adults, with pandemic spread of the virus being reported in 1995 and 2002. The NoV genome is genetically ...diverse, which has hampered development of sensitive molecular biology-based methods. In this study we report on a nested reverse transcriptase PCR (nRT-PCR) that was designed to amplify the highly conserved 3' end of the polymerase region and the 5' end of the capsid gene of NoV genogroup II (GII). The nRT-PCR was validated with strains isolated from sporadic and outbreak cases between 1997 and 2004 in New South Wales, Australia. Phylogenetic analysis identified six genotypes circulating in New South Wales, GII.1, GII.3, GII.4, GII.6, GII.7, and GII.10, with GII.4 being the predominant genotype. In 2004, there was a marked increase in NoV GII activity in Australia, with a novel GII.4 variant being identified as the etiological agent in 18 outbreaks investigated. This novel GII.4 variant, termed Hunter virus, differed by more than 5% at the amino acid level across the capsid from any other NoV strain in the GenBank and EMBL databases. The Hunter virus was subsequently identified as the etiological agent in large epidemics of gastroenteritis in The Netherlands, Japan, and Taiwan in 2004 and 2005.
Emotion differentiation, which involves experiencing and labeling emotions in a granular way, has been linked with well-being. It has been theorized that differentiating between emotions facilitates ...effective emotion regulation, but this link has yet to be comprehensively tested. In two experience-sampling studies, we examined how negative emotion differentiation was related to (a) the selection of emotion-regulation strategies and (b) the effectiveness of these strategies in downregulating negative emotion (Ns = 200 and 101 participants and 34,660 and 6,282 measurements, respectively). Unexpectedly, we found few relationships between differentiation and the selection of putatively adaptive or maladaptive strategies. Instead, we found interactions between differentiation and strategies in predicting negative emotion. Among low differentiators, all strategies (Study 1) and four of six strategies (Study 2) were more strongly associated with increased negative emotion than they were among high differentiators. This suggests that low differentiation may hinder successful emotion regulation, which in turn supports the idea that effective regulation may underlie differentiation benefits.
Consistent with the view that disgust might be involved in persistent body dissatisfaction, there is preliminary evidence showing a positive correlation between measures of negative body image and ...indices of both trait disgust and self-directed disgust. In two correlational studies among undergraduates (N = 577 and N = 346, respectively) we aimed at replicating and extending these findings by testing a series of critical relationships, which follow from our hypotheses that 1) trait disgust propensity would increase the risk of developing a negative body image by increasing the likelihood of feeling self-disgust, and 2) trait disgust sensitivity would heighten the impact of self-disgust on the development of persistent negative body appraisals. Replicating previous research, both studies showed that negative body image was positively related to self-disgust, disgust propensity and disgust sensitivity. Mediation analyses showed that, in line with our model, self-disgust partly accounted for the association between disgust propensity and negative body image. Although disgust sensitivity showed an independent relationship with body image, disgust sensitivity did not moderate the association between self-disgust and negative body image. All in all, findings are consistent with the view that self-disgust-induced avoidance may contribute to persistent negative body appraisals.
