Magnetic resonance (MR) imaging of the breast is currently used for evaluation of both parenchymal disease and silicone gel implants. MR imaging has the potential to address questions raised or ...unanswered with traditional diagnostic imaging methods. However, lesion specificity and cancer sensitivity depend on multiple technical factors (i.e., imaging parameters and contrast agent delivery) and biologic factors (i.e., the menstrual status of the patient and the vascularity of the lesion). Although diagnostic criteria for parenchymal disease have been reported, overlap of malignant and benign enhancement profiles occurs. The accuracy of implant evaluation depends on the imaging parameters and knowledge of the implant type and surgical history. In addition, individual investigative imaging methods are diverse, causing difficulty in protocol development for the practicing radiologist. An awareness of the problematic factors in breast MR imaging will improve diagnostic accuracy and allow understanding of the limitations of the modality and individual patient examinations.

In this paper we introduce a mathematical model to study the group dynamics of birds resting on wires. The model is agent-based and postulates attraction-repulsion forces between the interacting ...birds: the interactions are “topological”, in the sense that they involve a given number of neighbors irrespective of their distance. The model is first mathematically analyzed and then simulated to study its main properties: we observe that the model predicts birds to be more widely spaced near the borders of each group. We compare the results from the model with experimental data, derived from the analysis of pictures of pigeons and starlings taken in New Jersey: two different image elaboration protocols allow us to establish a good agreement with the model and to quantify its main parameters. We also discuss the potential handedness of the birds, by analyzing the group organization features and the group dynamics at the arrival of new birds. Finally, we propose a more refined mathematical model that describes landing and departing birds by suitable stochastic processes.
Inflammatory bowel disease (IBD) is a common inflammatory disorder with complex etiology that involves both genetic and environmental triggers, including but not limited to defects in bacterial ...clearance, defective mucosal barrier and persistent dysregulation of the immune response to commensal intestinal bacteria. IBD is characterized by two distinct phenotypes: Crohn's disease (CD) and ulcerative colitis (UC). Previously reported GWA studies have identified genetic variation accounting for a small portion of the overall genetic susceptibility to CD and an even smaller contribution to UC pathogenesis. We hypothesized that stratification of IBD by age of onset might identify additional genes associated with IBD. To that end, we carried out a GWA analysis in a cohort of 1,011 individuals with pediatric-onset IBD and 4,250 matched controls. We identified and replicated significantly associated, previously unreported loci on chromosomes 20q13 (rs2315008T and rs4809330A; P = 6.30 × 10−8 and 6.95 × 10−8, respectively; odds ratio (OR) = 0.74 for both) and 21q22 (rs2836878A; P = 6.01 × 10−8; OR = 0.73), located close to the TNFRSF6B and PSMG1 genes, respectively.

This paper reports the results from a second characterisation of the 91500 zircon, including data from electron probe microanalysis, laser ablation inductively coupled plasma‐mass spectrometry ...(LA‐ICP‐MS), secondary ion mass spectrometry (SIMS) and laser fluorination analyses. The focus of this initiative was to establish the suitability of this large single zircon crystal for calibrating in situ analyses of the rare earth elements and oxygen isotopes, as well as to provide working values for key geochemical systems. In addition to extensive testing of the chemical and structural homogeneity of this sample, the occurrence of banding in 91500 in both backscattered electron and cathodoluminescence images is described in detail. Blind intercomparison data reported by both LA‐ICP‐MS and SIMS laboratories indicate that only small systematic differences exist between the data sets provided by these two techniques. Furthermore, the use of NIST SRM 610 glass as the calibrant for SIMS analyses was found to introduce little or no systematic error into the results for zircon. Based on both laser fluorination and SIMS data, zircon 91500 seems to be very well suited for calibrating in situ oxygen isotopic analyses.
Cet article présente les résultats d'une nouvelle caractérisation du zircon 91500, dont des données de microanalyse par sonde électronique, d'analyse par ablation laser en couplage à un ICP‐MS, d'analyse par sonde ionique (SIMS) et d'analyse par fluorination laser. Le but de cette étude etait de démontrer que ce large monocristal de zircon pouvait être utilisé pour la calibration d'analyses in situ de Terres Rares et des isotopes de I'Oxygène, et en même temps de fournir des valeurs “de travail” pour un certain nombre de systémes géochimiques cruciaux. En complément des tests systématiques d'homogénéité de I'échantillon, tant chimiquement que structurellement, /'ex/sfence, dans le zircon 91500, de zonages visibles en électrons retro diffusés et en cathodoluminescence, est décrite en détail, line comparaison en aveugle des résultats obtenus par LA‐ICP‐MS et par SIMS, dans des laboratoires différents, montre que les différences systématiques entre les ensembles de données obtenues par ces deux techniques sont très faibles. De plus, I'utilisation du verre NIST SRM 610 comme calibrant lors de I'analyse par SIMS n'introduit qu'une erreur systématique très faible si ce n'est inexistante sur les résultats du zircon. Sur la base des analyses par fluorination laser et par SIMS, le zircon 91500 semble être parfaitement adapte a son utilisation pour la calibration d'analyses isotopiques in situ d'oxygène.
To outline a quality assessment method with peer review for magnetic resonance (MR) imaging.
Thirty-three providers in the Philadelphia area were rated on a random sample of 132 brain, 124 cervical ...spine, and 113 lower extremity MR imaging examinations performed during 1990. Blinded peer review was performed by panels of three subspecialty-trained academic radiologists. Technical performance, completeness, and report appropriateness of each MR imaging examination were evaluated. Aggregated scores were calculated to rate provider performance for each of the three parameters of quality.
Two or three panelists assessed technical performance as inadequate in 15 cases, completeness as incomplete in 58 cases, and the interpretative report as inappropriate and affecting treatment in 72 cases. Eleven providers received an unsatisfactory rating on one or more parameters of quality. The association between unsatisfactory ratings and the use of low-field-strength (< or = 0.6-T) imagers was statistically significant (P < .008).
Substantial deficiencies were identified in the performance of examinations and interpretation of MR images in the Philadelphia area in 1990. These findings indicate the need for a program to monitor quality of MR imaging.
The sensitivity of contrast-enhanced breast MR imaging for malignancy is close to 100%, an improvement over traditional methods of cancer detection. The reported specificity of breast MR imaging, ...however, ranges from approximately 37% to 97%. This range of values is the result of differences in technique and diagnostic criteria. The variation in biologic activity and vascularity of benign and malignant lesions, as well as normal tissue, also complicates MR imaging diagnosis. A review of breast MR imaging research with an emphasis on specificity issues is also presented.
Terminal mono-oxo complexes of the late transition metal elements have long been considered too unstable to synthesize because of repulsion between the oxygen electrons and the mostly filled metal d ...orbitals. A platinum(IV)-oxo compound flanked by two polytungstate ligands, K7Na9O=Pt(H2O)L2, L = PW9O34(9-), has now been prepared and isolated at room temperature as air-stable brown crystals. X-ray and neutron diffraction at 30 kelvin revealed a very short 1.720(18) angstrom Pt-O bond and no evidence of a hydrogen atom at the terminal oxygen, ruling out a better precedented Pt-OH complex. Density functional theory and spectroscopic data account for the stability of the Pt(IV)-oxo unit by electron withdrawal into delocalized orbitals of the polytungstates.
Membranoproliferative GN (MPGN) was recently reclassified as alternative pathway complement-mediated C3 glomerulopathy (C3G) and immune complex-mediated membranoproliferative GN (IC-MPGN). However, ...genetic and acquired alternative pathway abnormalities are also observed in IC-MPGN. Here, we explored the presence of distinct disease entities characterized by specific pathophysiologic mechanisms. We performed unsupervised hierarchical clustering, a data-driven statistical approach, on histologic, genetic, and clinical data and data regarding serum/plasma complement parameters from 173 patients with C3G/IC-MPGN. This approach divided patients into four clusters, indicating the existence of four different pathogenetic patterns. Specifically, this analysis separated patients with fluid-phase complement activation (clusters 1-3) who had low serum C3 levels and a high prevalence of genetic and acquired alternative pathway abnormalities from patients with solid-phase complement activation (cluster 4) who had normal or mildly altered serum C3, late disease onset, and poor renal survival. In patients with fluid-phase complement activation, those in clusters 1 and 2 had massive activation of the alternative pathway, including activation of the terminal pathway, and the highest prevalence of subendothelial deposits, but those in cluster 2 had additional activation of the classic pathway and the highest prevalence of nephrotic syndrome at disease onset. Patients in cluster 3 had prevalent activation of C3 convertase and highly electron-dense intramembranous deposits. In addition, we provide a simple algorithm to assign patients with C3G/IC-MPGN to specific clusters. These distinct clusters may facilitate clarification of disease etiology, improve risk assessment for ESRD, and pave the way for personalized treatment.
Various prediction models have been developed to predict the risk of kidney failure in patients with CKD. However, guideline-recommended models have yet to be compared head to head, their validation ...in patients with advanced CKD is lacking, and most do not account for competing risks.
To externally validate 11 existing models of kidney failure, taking the competing risk of death into account, we included patients with advanced CKD from two large cohorts: the European Quality Study (EQUAL), an ongoing European prospective, multicenter cohort study of older patients with advanced CKD, and the Swedish Renal Registry (SRR), an ongoing registry of nephrology-referred patients with CKD in Sweden. The outcome of the models was kidney failure (defined as RRT-treated ESKD). We assessed model performance with discrimination and calibration.
The study included 1580 patients from EQUAL and 13,489 patients from SRR. The average
statistic over the 11 validated models was 0.74 in EQUAL and 0.80 in SRR, compared with 0.89 in previous validations. Most models with longer prediction horizons overestimated the risk of kidney failure considerably. The 5-year Kidney Failure Risk Equation (KFRE) overpredicted risk by 10%-18%. The four- and eight-variable 2-year KFRE and the 4-year Grams model showed excellent calibration and good discrimination in both cohorts.
Some existing models can accurately predict kidney failure in patients with advanced CKD. KFRE performed well for a shorter time frame (2 years), despite not accounting for competing events. Models predicting over a longer time frame (5 years) overestimated risk because of the competing risk of death. The Grams model, which accounts for the latter, is suitable for longer-term predictions (4 years).
