Targeted therapies are a standard of care for first-line treatment of Anaplastic lymphoma kinase (ALK)-rearranged non small cell lung cancer (NSCLC). Giving the rapid pace of drug discovery and ...development in this area, reporting of adverse effects of ALK inhibitors is crucial. Here, we report a case of osteitis induced by an ALK inhibitor mimicking bone metastasis, a previously undescribed side effect of crizotinib.
A 31-year-old woman with stage IV ALK-rearranged NSCLC presented with back pain after 3 months of crizotinib treatment. Diagnostic work-up showed osteitis on the 4th and 5th thoracic vertebrae, anterior soft tissue infiltration and epiduritis, without any sign of infection. Spinal cord decompression, histological removal and osteosynthesis were performed. Histologic examination showed necrosis with abundant peripheral neutrophils, no microorganism nor malignant cell. Symptoms and Computarized Tomography-abnormalities rapidly diseappeared after crizotinib withdrawal and did not recur after ceritinib onset.
This is the first report of crizotinib-induced osteitis. Crizotinib differs from other ALK inhibitors as it targets other kinases as well, which may have been responsible for the osteitis. Crizotinib can induce rapidly extensive osteitis, which can mimic tumor progression.

Solitary pulmonary nodules (SPN) have become increasingly prevalent and diagnostic management remains challenging. We demonstrate a novel technique in which probe-based confocal endomicroscopy (pCLE) ...could be performed to microimage SPN in vivo and in real-time. Two confocal wavelengths (488 and 660 nm with methylene blue (MB)) were used for elastin network and cellular imaging, respectively using pCLE in conjunction with r-EBUS and virtual navigation. In the first case, the 1-mm Alveoflex

was used to image a metastatic melanoma in a subcentimetric nodule in the right middle lobe. In the next case, a malignant 2-cm nodule in the posterior segment of the upper lobe was imaged using the smaller 0.6-mm Cholangioflex
®
. Lastly, we present a benign case revealing confocal characteristics of a nodular lipid pneumonitis. This reports for the first time the feasibility and utility of pCLE in vivo microimaging of SPN using either the Alveoflex or Cholangioflex miniprobes in addition to 660 nm/MB imaging.
Nous sommes depuis quelques années entrés pleinement dans l’ère génomique de l’analyse génétique des cancers à visée médicale. Les bouleversements technologiques en biologie associés à ceux des ...sciences de l’information permettent maintenant une analyse du génome des cancers en pratique courante, avec dans certains cas un impact majeur sur la prise en charge des patients, sur le plan thérapeutique bien sûr (exemple des mutations du gène EGFR, codant pour l’epidermal growth factor receptor, dans l’adénocarcinome pulmonaire), mais aussi dans le diagnostic et le suivi de la maladie. Le génotypage des tumeurs s’effectue le plus souvent à partir du tissu fixé au formol et inclus en paraffine à partir duquel un diagnostic de malignité a été posé. Cependant, de nouvelles approches voient le jour, comme la pratique maintenant de plus en plus répandue des « biopsies liquides ». La diffusion sur tout le territoire français de séquenceurs « à haut débit » rend possible en routine l’analyse de plusieurs dizaines de gènes d’intérêt dans la prise en charge de certains types de cancer, et il est à parier que le perfectionnement de ces automates rendra encore plus facile et accessible le génotypage des tumeurs dans un avenir proche. Le défi actuel n’est cependant principalement plus la détection des anomalies moléculaires, mais leur traitement, leur analyse et leur interprétation pertinente, de manière à être le plus utile possible au patient dans le traitement de sa maladie.

Technological breakthroughs in both biology and information science now allow a genomic analysis of cancers in everyday medical practice with, in some case, a major impact on patient care not only for the choice of therapy (i.e. EGFR mutations in lung adenocarcinoma), but also for diagnosis and monitoring of the disease. Tumour genotyping is performed from formalin-fixed paraffin-embedded tissues used for diagnosis of cancer. However, new approaches have emerged, with for example the more and more spread use of “liquid biopsies”. Genotyping of a gene panel implicated in carcinogenesis is now routinely performed in some cancer types, with the help of high-throughput sequencers, and it is likely that improvement of these machines will make tumour genotyping easier and more accessible in the near future. Nevertheless, the current challenge is not anymore detection of molecular alterations, but their relevant interpretation, so as to be the most useful in patient care.

Les zones hypoxiques sont généralement des zones résistantes au traitement, en particulier à la radiothérapie. Pour le CPNPC, plusieurs études ont proposé d’augmenter la dose de radiothérapie sur ces ...volumes définis par le FMISO. Dans les cancers de la tête et du cou, la même approche a été utilisée avec le FAZA ou le FMISO, mais ces deux traceurs n’ont jamais été comparés dans le CPNPC. Vingt patients ont été inclus avant chirurgie d’un cancer localisé du CPNPC et ont bénéficié de trois tomographies TEP préchirurgicales : TEP-FDG, TEP-FMISO et TEP-FAZA. Pour chaque patient, les données TEP des trois traceurs ont été comparées entre elles et comparées à l’analyse immunohistochimique (CD34, GLUT-1, CAIX, LDH-5, MCT-4, HIF1-Alpha) après extraction tumorale. 19 patients ont été définitivement inclus dans cet essai : 4 femmes et 15 hommes, avec un âge moyen de 67±7,4 ans. Pour la TEP-FDG, le SUVmax était de 12,3 (±5,4) et le volume avec un seuil à 40 % du SUVmax était de 23,2 cc (±19,2). 18 lésions présentaient une absorption significative (SUVmax supérieur à 1,4) pour le MISO et 17 pour le FAZA. Le SUVmax moyen était respectivement de 3 (±1,36) avec un volume moyen de 25,8 cc (±25,8) pour FMISO et de 2,16 (±0,7) avec un volume moyen de 13,06 cc (±13,76) pour le FAZA. Il existait une excellente corrélation entre le SUVmax FMISO et le SUVmax FAZA à 0,88 (0,72 à 0,95) et une bonne corrélation à 0,73 (0,42 à 0,89) pour le volume avec un seuil à 1,4. Il n’existait pas de corrélation entre le SUVmax FDG et les SUVmax des deux traceurs de l’hypoxie. L’analyse immunohistochimique n’était pas corrélée à l’hypoxie définie par TEP quels que soient les anticorps testés. Cette étude confirme la très bonne corrélation des deux traceurs de l’hypoxie et la supériorité du FMISO sur le FAZA. Malheureusement, il n’existe aucune corrélation avec l’analyse immunohistochimique.
This study presents a methodological approach for the visualization of the glycocalyx by electron microscopy. The glycocalyx is a three dimensional network mainly composed of glycolipids, ...glycoproteins and proteoglycans associated with the plasma membrane. Since less than a decade, the epithelial and endothelial glycocalyx proved to play an important role in physiology and pathology, increasing its research interest especially in vascular functions. Therefore, visualization of the glycocalyx requires reliable techniques and its preservation remains challenging due to its fragile and dynamic organization, which is highly sensitive to the different process steps for electron microscopy sampling. In this study, chemical fixation was performed by perfusion as a good alternative to conventional fixation. Additional lanthanum nitrate in the fixative enhances staining of the glycocalyx in transmission electron microscopy bright field and improves its visualization by detecting the elastic scattered electrons, thus providing a chemical contrast.
