There are no proven safe and effective therapies for children who develop life‐threatening complications of severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2). Convalescent plasma (CP) has ...demonstrated potential benefit in adults with SARS‐CoV‐2, but has theoretical risks.We present the first report of CP in children with life‐threatening coronavirus disease 2019 (COVID‐19), providing data on four pediatric patients with acute respiratory distress syndrome. We measured donor antibody levels and recipient antibody response prior to and following CP infusion. Infusion of CP was not associated with antibody‐dependent enhancement (ADE) and did not suppress endogenous antibody response. We found CP was safe and possibly efficacious. Randomized pediatric trials are needed.
Objective Common knowledge implies that individuals engaging in outdoor sports and especially in regular and extreme mountaineering are exceptionally healthy and hardened. Here we investigate aspects ...of regular and extreme mountaineering which go beyond salutogenic effects and might share similarities with psychiatric disorders. Methods This web-based study collected data on exercise and mountaineering addiction (Exercise Addiction Inventory; original and adapted version for mountaineering; Exercise Dependence Scale adapted version for mountaineering). Further parameters included mountaineering habits, the Risk Taking Inventory, the Sensation Seeking/Emotion Regulation/Agency Scale (SEAS), resilience, self-perceived stress, physical activity and mental health. Non-parametric analyses were used. Results Data from 335 regular mountaineers were analysed comparing those with addiction to mountaineering (MA; n = 88) to control participants i.e. subjects without addiction to mountaineering (controls, CO, n = 247). In the MA group there were significantly higher values regarding self-perceived stress (p< 0.001) and higher numbers of individuals affected with depression (p < 0.001), anxiety (p< 0.001), eating disorders (p< 0.001), alcohol abuse or dependence (p< 0.001), illicit drug use (p = 0.050) or history of psychiatric disorders (p< 0.001). No difference was found concerning resilience or somatic disorders. Individuals with MA showed higher values in all SEAS subscales as well as increased risk taking (p < 0.001). No difference between the groups was found for cautiousness. Conclusion Regular and extreme mountaineering can display features of a behavioral addiction and is associated with psychiatric disorders. Increased risk taking in individuals with MA could potentially be a source of accidents.

The aim of ...this study was to document the incidence of clinically important problems, particularly related to blood loss, and perform a risk factor analysis.
Methods: Records of all patients who underwent craniofacial surgery at the Children’s Hospital of Philadelphia between December 1, 2001 and January 1, 2006 were reviewed. Data were collected from the electronic anesthesia record, intensive care unit (ICU) progress notes, and discharge summary. All intraoperative laboratory values and all laboratory values obtained upon arrival in the ICU were recorded. A multivariable analysis was performed to evaluate associations between elements of intraoperative management and the following clinical outcomes: intraoperative hypotension, intraoperative metabolic acidosis, presence of a postoperative coagulation test abnormality, and postoperative administration of hemostatic blood products.
Results: Data for 159 patients were reviewed. The mean volume of packed red blood cells transfused intraoperatively was 51 ml·kg−1. Multivariable analysis revealed that intraoperative administration of albumin was strongly correlated with both an increased incidence of postoperative coagulation derangements and postoperative administration of hemostatic blood products (Odds Ratio 5.9, 2.8, respectively), while intraoperative fresh frozen plasma (FFP) administration was associated with an opposite effect (Odds Ratio 0.94, 0.97, respectively).
Conclusions: In pediatric CFR procedures where the volume of blood loss routinely exceeds one blood volume, intraoperative administration of FFP favorably impacted postoperative laboratory coagulation parameters.

An expert panel reviewed the data and developed recommendations using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) framework.
The AABB recommends that platelets should be transfused prophylactically to reduce the risk for spontaneous bleeding in hospitalized adult patients with therapy-induced hypoproliferative thrombocytopenia. The AABB recommends transfusing hospitalized adult patients with a platelet count of 10 × 109 cells/L or less to reduce the risk for spontaneous bleeding. The AABB recommends transfusing up to a single apheresis unit or equivalent. Greater doses are not more effective, and lower doses equal to one half of a standard apheresis unit are equally effective. (Grade: strong recommendation; moderate-quality evidence).

The present study was performed to determine the variations of breath acetone concentrations with age, gender and body-mass index (BMI). Previous investigations were based on a relatively small ...cohort of subjects (see Turner et al 2006 Physiol. Meas. 27 321-37). Since exhaled breath analysis is affected by considerable variation, larger studies are needed to get reliable information about the correlation of concentrations of volatiles in breath when compared with age, gender and BMI. Mixed expiratory exhaled breath was sampled using Tedlar bags. The concentrations of a mass-to-charge ratio (m/z) of 59, attributed to acetone, were then determined using proton transfer reaction-mass spectrometry. Our cohort, consisting of 243 adult volunteers not suffering from diabetes, was divided into two groups: one that fasted overnight prior to sampling (215 volunteers) and the other without a dietary control (28 volunteers). In addition, we considered a group of 44 healthy children (5-11 years old).The fasted subjects' concentrations of acetone ranged from 177 ppb to 2441 ppb, with an overall geometric mean (GM) of 628 ppb; in the group without a dietary control, the subjects' concentrations ranged from 281 ppb to 1246 ppb with an overall GM of 544 ppb. We found no statistically significant shift between the distributions of acetone levels in the breath of males and females in the fasted group (the Wilcoxon-Mann-Whitney test yielded p = 0.0923, the medians being 652 ppb and 587 ppb). Similarly, there did not seem to be a difference between the acetone levels of males and females in the group without a dietary control. Aging was associated with a slight increase of acetone in the fasted females; in males the increase was not statistically significant. Compared with the adults (a merged group), our group of children (5-11 years old) showed lower concentrations of acetone (p < 0.001), with a median of 263 ppb. No correlation was found between the acetone levels and BMI in adults. Our results extend those of Turner et al's (2006 Physiol. Meas. 27 321-37), who analyzed the breath of 30 volunteers (without a dietary control) by selected ion flow tube-mass spectrometry. They reported a positive correlation with age (but without statistical significance in their cohort, with p = 0.82 for males and p = 0.45 for females), and, unlike us, arrived at a p-value of 0.02 for the separation of males and females with respect to acetone concentrations. Our median acetone concentration for children (5-11 years) coincides with the median acetone concentration of young adults (17-19 years) reported by Spanel et al (2007 J. Breath Res. 1 026001).
Background
Drug‐induced immune hemolytic anemia (DIIHA) is a rare, but important condition requiring specialized laboratory testing for diagnosis. We report a case of DIIHA with antibodies against ...carboplatin and vincristine (VCR) in a child with an optic pathway glioma. Platinum‐based drugs are established to cause DIIHA; to our knowledge, this is the first report implicating VCR.
Study Design and Methods
A 35‐month‐old girl with an optic pathway glioma developed hemolytic anemia while receiving carboplatin and VCR. Specialized blood bank testing was performed to determine the presence of drug‐dependent antibodies and thus DIIHA.
Results
Initial direct antiglobulin test (DAT) was negative. A repeat DAT 3 days later was positive, 3+ with polyspecific‐antiglobulin sera, weak+ with anti‐immunoglobulin (Ig)G, and 2+ with anti‐C3d. An eluate from the DAT‐positive red blood cells (RBCs) was nonreactive. The patient's serum reacted without specificity to all RBC tested using papain‐IgG‐antiglobulin test (AGT) and polyethylene glycol‐IgG‐AGT. No alloantibodies to common RBC antigens were detected. When the serum was evaluated for the presence of drug‐specific antibody, reactivity was shown with VCR and carboplatin using the drug addition solution method, but only with carboplatin using the drug‐coating method.
Conclusion
The patient developed hemolytic anemia during chemotherapy. Initial detection of a panagglutinin suggested a warm‐type autoimmune process. However, since DIIHA could not be excluded, chemotherapy was discontinued and further work‐up was initiated. The findings confirmed the presence of antibodies to carboplatin and VCR. This case highlights the importance for clinicians to maintain a high index of suspicion for DIIHA in patients with unexplained hemolysis and the importance of specialized serologic testing.
BACKGROUNDInitial reports from the severe acute respiratory coronavirus 2 (SARS-CoV-2) pandemic described children as being less susceptible to coronavirus disease 2019 (COVID-19) than adults. ...Subsequently, a severe and novel pediatric disorder termed multisystem inflammatory syndrome in children (MIS-C) emerged. We report on unique hematologic and immunologic parameters that distinguish between COVID-19 and MIS-C and provide insight into pathophysiology.METHODSWe prospectively enrolled hospitalized patients with evidence of SARS-CoV-2 infection and classified them as having MIS-C or COVID-19. Patients with COVID-19 were classified as having either minimal or severe disease. Cytokine profiles, viral cycle thresholds (Cts), blood smears, and soluble C5b-9 values were analyzed with clinical data.RESULTSTwenty patients were enrolled (9 severe COVID-19, 5 minimal COVID-19, and 6 MIS-C). Five cytokines (IFN-γ, IL-10, IL-6, IL-8, and TNF-α) contributed to the analysis. TNF-α and IL-10 discriminated between patients with MIS-C and severe COVID-19. The presence of burr cells on blood smears, as well as Cts, differentiated between patients with severe COVID-19 and those with MIS-C.CONCLUSIONPediatric patients with SARS-CoV-2 are at risk for critical illness with severe COVID-19 and MIS-C. One component of the new national kidney allocation system (KAS) in the United States that was implemented on December 4, 2014, was the allocation of kidneys from A2 and A2B (A, non‐A1 and AB, ...non‐A1B) deceased donors into blood group B candidates (A2/A2B → B). In so far as this is an important component of the new KAS that has the potential to further increase the access to transplantation for blood group B candidates on the waiting list, most of whom are minority candidates, we will review the body of evidence and historical perspectives that led to its inclusion in the new KAS. This review will also describe prospects for more widespread use of A2/A2B → B transplantation and a novel mechanism of humoral immunosuppression in B patients before and after transplantation with an A2 or A2B kidney.
In this review, the authors summarize and discuss the experience that led to the inclusion of the A2/A2B → B component into the United States' new kidney allocation system in 2015, describe future directions to further expand A2'A2B → B renal and simultaneous liver‐kidney transplantation, and emphasize that in order for this allocation component to improve access of blood group B candidates, more B candidates must be made eligible in the UNOS system.
Summary
Objective/Aims: To assess the effect of prophylactic administration of fresh‐frozen plasma (FFP) in the form of reconstituted blood in children undergoing craniofacial reconstruction. The ...outcomes of interest included immediate postoperative coagulation laboratory test results, postoperative surgical drain output, and the number of unique blood donor exposures incurred.
Background: We recently changed our intraoperative transfusion strategy in children undergoing craniofacial reconstruction surgery to one in which blood loss is replaced with donor‐matched reconstituted blood rather than traditional blood component therapy.
Methods: We performed a query of our prospective craniofacial surgery perioperative registry for children who underwent fronto‐orbital advancement or posterior cranial vault reconstruction. Registry data from this query were compared to data from a historical cohort.
Results: Data for 46 registry cases were compared to 150 historical cohort cases. The median number of unique donor exposures for the reconstituted blood group was 2 vs 3 in the historical cohort (P = 0.004). The reconstituted blood group had a decreased incidence of postoperative derangements in soluble clotting factor tests (fibrinogen, PT, or aPTT; 2% vs 24%, P = 0.001), while there was no evidence for a difference in the incidence of thrombocytopenia. There was no evidence for differences in postoperative surgical drain output in the reconstituted blood group and historical cohort over the first 12, 24, and 48 h.
Conclusions: Prophylactic administration of FFP in the form of donor‐matched reconstituted blood in children undergoing craniofacial reconstruction was associated with improved postoperative coagulation parameters, reduced blood donor exposures, and unchanged postoperative surgical drain output.
