Objective: Postpartum ovarian vein thrombosis (POVT) is a rare but very dangerous complication with possible lethal outcome. Here the authors present a case of POVT with propagation in cava inferior ...vein, which was diagnosed fourth day after delivery by cesarean section. Case Report: A 27-year-old female with fever complained of pain in the area of the right side of the abdomen, in the ileocecal region three days after premature delivery by caesarean section. Right ovarian venous thrombosis and partial cava inferior vein thrombosis was demonstrated on sonography and confirmed with computed tomography. She was treated with antibiotics and anticoagulation therapy with good response. Conclusion: If women after delivery have an elevated temperature with abdominal pain and leukocytosis in laboratory analyzes, it is always necessary to think of the POVT. Early recognition and therapy are crucial.

Book Review: Ivana Bašić. Love Threads: Concepts of Love in European and Serbian Culture. Ethnographic Institute SASA, Special Editions. Book 98. Belgrade, 2021. 329 p.
The review presents the monograph by the Serbian researcher Ivana Bašić, who devoted her work to an anthropological analysis of the concepts of love in European and Serbian cultures. Combining romantic love and culture in one study, the author assumes that anthropological viewpoint can reveal many essential qualities of this mystery of human nature, which are peripheral to other sciences. She views sensual emotion as a social phenomenon, generated not only by physiology but also by the intellect. Considering love to be a time‑varying cultural, anthropological, sociological and philosophical concept, the researcher directs her attention to the study of love as a semiotic system, to the search for key lexemes used to express feelings and experiences, as well as lexemes etymologically and semantically related to them. Reviewers, appreciating Ivana Basic’s book, note her innovative methodological approach and the significance of the author’s contribution to the development of the anthropology of emotions.

Background: Depression in multiple sclerosis (MS) is a complex phenomenon, influenced by several biological and psychological as well as local social and cultural factors, with an impact on different ...aspects of disease and life of the patients. Objective: To evaluate the interrelation of depression in multiple sclerosis and its relation with neurological deficit, quality of life, fatigue and other factors in the MS population. Methods: We applied the following measuring instruments: Expanded Disability Status Scale (EDSS), Paced Auditory Serial Addition Task (PASAT), SF-36, MSQOL-54, Beck Depression Inventory (BDI) and Fatigue Severity Scale (FSS) in 120 relapsing-remitting multiple sclerosis (RRMS) patients, EDSS 0-6,5, age 18-55. Results: Incidence of depression in our population is 63.35% (mild 31.7, moderate 21.7 and severe 10%), in female 84.8% (mild 42.4, moderate 27.2 and severe 15.1%), and in male 36.4%(mild 18.5, moderate 14.8 and severe 3.7%). Depression is correlating with EDSS score (r=3,18) and even more with some EDSS subsystems (EDSSAMB and EDSSPYR) based on multiple regression analyses. Depression is most frequent in patients with EDSS 0-1.5 and more frequently severe in group of patients with EDSS 2-3.5 than in patients with either lower or higher EDSS score. Analyzing the prevalence and severity of depression in different age groups (20-29, 30-44 and 45-55 years), in younger patients depression is more severe (severe depressed 20,5 and 5% in each age group respectively) and more frequent (70, 62 and 58% respectively). Depression is correlating with quality of life (p=0,05). Other factors associated with depression in this study are: fatigue, unemployment, two or more children, not married. Conclusions: Depression is more frequent than in other studies, associated with younger age, EDSS, quality of life and fatigue.
Laboratory tests are routinely ordered to identify or rule out a cause in patients with chronic urticaria/angioedema (CUA). The results of these tests are usually within normal limits or ...unremarkable.
To investigate the proportion of abnormal test results in patients with CUA leading to a change in management and in outcomes of care.
Retrospective analysis of a random sample of adult patients with CUA from 2001-2009.
Cases totaled 356: 166 with urticaria and angioedema (AE), 187 with urticaria, and 3 with only AE. Patients were predominately women (69.1%) and white (75.6%), with a mean age of 48 ± 15 years. Abnormalities were commonly seen in complete blood counts (34%) and in complete metabolic panels (9.4%). Among the 1,872 tests that were ordered, results of 319 (17%) were abnormal. Of 356 patients, 30 underwent further testing because of abnormalities in laboratory work. This represented 30 of 1,872 tests (1.60%). Only 1 patient benefited from a subsequent change in management.
Laboratory testing in CUA patients referred for an Allergy and Immunology evaluation rarely lead to changes in management resulting in improved outcomes of care.
Glatiramer acetate is an immunomodulatory drug that is widely prescribed for the treatment of multiple sclerosis. It is frequently associated with local injection site reactions and generalized ...urticaria. It is also associated with immediate postinjection systemic reactions in approximately 10% of patients. To our knowledge, no desensitization protocols for glatiramer acetate have been published to date.
To evaluate the safety and efficacy of glatiramer acetate desensitization in a series of patients with multiple sclerosis.
Six patients with multiple sclerosis and glatiramer acetate-associated local or systemic reactions underwent a 4-hour outpatient desensitization procedure at Cleveland Clinic between 2003 and 2008. Beginning with 20 ng, we administered subcutaneous glatiramer acetate suspension in increasing dosages every 15 minutes. Patient outcomes were monitored by return clinic visit and telephone follow-up.
No episodes of anaphylaxis or serious adverse reactions occurred during or immediately after desensitization. One patient suspended therapy after 14 months due to persistent local injection site reactions. All other patients successfully continued glatiramer acetate therapy.
Glatiramer acetate offers significant benefit to patients with multiple sclerosis. Our experience suggests that patients who suspend its use owing to local or systemic reactions can be successfully and safely desensitized and can resume medication use. To our knowledge, this is the first report of successful desensitization to glatiramer acetate in patients with multiple sclerosis.
