Le ganglioneurome est une tumeur neurogène bénigne, rare, qui se développe à partir des cellules ganglionnaires sympathiques et qui se localise essentiellement dans la région rétropéritonéale. Nous ...rapportons cinq cas de ganglioneuromes rétropéritonéaux, ayant eu une exérèse complète de la tumeur avec évolution favorable. Le diagnostic a été confirmé par l’examen anatomopathologique. La fréquence, le diagnostic, les données de l’imagerie et le traitement de cette tumeur rare sont discutés.
Ganglioneuroma is a rare, benign, neurogenic tumor originating from the neural sheath and frequently located in the retroperitoneum. We report five cases of retroperitoneal ganglioneuroma. Complete resection of this tumor was performed with a good outcome. Our diagnosis was confirmed by histology. The frequency, diagnosis, imaging and treatment of this rare tumor are discussed.
Le cancer prostatique est parmi les cancers les plus pourvoyeurs de métastases Osseuses. Cependant, très peu de cas de métastases orbitaires ont été rapportés dans la littérature. Leur survenue ...constitue un tournant évolutif défavorable de la maladie et un traitement urgent serait nécessaire vu que le pronostic fonctionnel de l‘œil atteint est mis en jeux.
Nous rapportons un cas de métastase orbitaire d‘un adénocarcinome prostatique et nous revoyons les particularités diagnostiques et thérapeutiques de cette affection.
Herein, we report a rare case of metastasis of prostate cancer in the orbit with pertinent literature review. Although, prostate cancer can metastasize in any organ, yet orbital metastasis indicates an aggressive disease course with eminent loss of vision. Very few similar cases have been reported in the literature.

Résumé Introduction Sous nos climats, la circoncision reste l’intervention la plus couramment pratiquée chez le garçon. Elle est souvent réalisée par un personnel paramédical non qualifié, ou même en ...dehors du milieu médical dont les notions de chirurgie et d’asepsie sont aléatoires. Elle est réputée comme chirurgie bénigne, mais des complications majeures parfois très graves, engageant le pronostic sexuel, voire vital, peuvent survenir. Le but de ce travail était de rapporter les amputations du gland secondaires à la circoncision et reçues en milieu hospitalier urologique. Patients et méthode Nous rapportons huit cas d’amputation du gland suite à une circoncision en milieu extrahospitalier selon les méthodes traditionnelles. Résultats L’âge moyen des enfants est de six ans (trois à huit ans). L’amputation du gland était faite lors de la circoncision dans quatre cas, dont deux avec section complète. Dans les autres cas, il s’agissait d’une nécrose du gland à cause d’un arc électrique lors de l’utilisation inadéquate du bistouri électrique. Une reconstruction urgente du gland n’avait pu être réalisable que dans trois cas. Le gland sectionné dans le quatrième cas était de mauvaise qualité. On n’a noté qu’un seul succès de revascularisation du gland. Le délai de consultation en cas de nécrose du gland était de trois jours en moyenne. Le traitement n’avait consisté qu’en la réalisation d’une méatoplastie pour obtenir un méat urétral satisfaisant avec réfection de la peau pénienne. Conclusion La section du gland demeure une complication, certes rare de la circoncision mais très grave, engageant le pronostic sexuel ultérieur de l’enfant. Cela rend cette intervention non anodine et doit inciter à ne la réaliser qu’en milieu spécialisé par un personnel compétent.
Abstract Introduction Circumcision is still the most commonly performed surgery in our society. It’s often performed by an unlicensed paramedic or even by a traditional practitioner outside the hospital under the non sterile conditions. Although, not technically difficult, circumcision may have serious complications that can endanger the sexuality or even the life of male children. The aim of this study is to emphasize the important problem of glandular amputation after circumcision. Patients and methods We report 8 cases of glandular amputation after circumcision performed outside the hospital within traditional methods. Results The mean age of these patients was 6 years (3 to 8 years). Glandular amputation occurred during circumcision in 4 cases including that 2 cases with total glandular amputation. On the other cases it was about glandular necrosis caused by inadequate use of electric lancet. An urgent reparation of glans was performed on only 3 cases. On the forth the amputed glans was unrepairable. We noticed only one case on wich glans revascularization succeeded. In case of glandular necrosis, patients consulted after 3 days. The treatment consisted of meatoplasty. Conclusion Glandular amputation is a rare but very severe complication of circumcision because it endangers the future sexuality of the child. So, the operation should be performed by educated and experienced personnel.
Due to its key position within the Africa–Europe convergence zone, Tunisia is marked by thrusting, folding, and faulting and has a major rupture zones associated with active faults. Consequently, ...most of Tunisian land is seismically active with significant active deformations, showing recent seismic events and their relative surface effects. This paper reports on several aspects of the seismotectonics, historical, and present-day seismicity and places them in the general tectonic and geodynamic framework of Tunisia. Field investigations, based on an integrated multidisciplinary approach, included (1) the identification of active faults, their motion and displacement, geomorphic aspects, and scarps and their relation with the general structural map of Tunisia and (2) an extensive analysis of brittle tectonic deformation affecting Quaternary deposits in several sites throughout Tunisia. The integration of field data within the existing data related to the seismic events that took place during the last decades allowed the establishment of an earthquake distribution map, as well as major seismic zones for better understanding of the seismicity database of Tunisia. To establish microzonation maps in seismic regions such as Gafsa and its surroundings, we have analyzed surface effects and secondary structures associated with active faults and correlated them with deformation rates, reconstructed for significant seismic events. Most faults exhibited typical left-stepping en-echelon with strike-slip component pattern suggesting that Tunisia is presently subjected to NNW–SSE compression. The focal mechanism of most Tunisia earthquakes combined with the existing tectonic and structural information and reconstruction of the Quaternary stress tensor allowed (a) better understanding of seismic zoning, (b) provided better assessment of the seismic hazard, and (c) facilitated the interpretation of the relationship between seismic zones and the geodynamic African–Eurasian plate boundary.
The aim of this research is to describe applications of virtual/augmented reality (VR/AR) to the problems of children with pervasive developmental disorders (PDDs). We will try to verify whether the ...use of these techniques makes interventions in the case of children with PDDs (concerning their learning and communicative abilities and the state of their emotional sphere) expedient or not. This paper focuses on the use of new information and communication technologies (NICTs) in the treatment of these children and current researches. We will also observe the involvement of NICTs (such as VR/AR) in changing activities made with these tools on the didactic and methodological level. These analyzes will allow us to answer the following research problematic: How does the application of VR/AR in the case of children with PDDs contribute to the development of learning, communicative skills, and recognition and expression of the emotions?
Évaluation de l’effet hémostatique de la radiothérapie hypofractionnée dans le traitement palliatif des cancers de la vessie.
Étude rétrospective colligeant 31 cas de patients atteints de cancer de ...la vessie localement évolué ou métastatique traités par irradiation hypofractionnée à visée hémostatique dans le service de radiothérapie oncologique du centre hospitalier universitaire Habib-Bourguiba entre 1993 et 2015. L’âge médian était de 76ans (53–91ans), avec un sex-ratio de 9,3. Le saignement initial était de grades 1, 2 et 3 dans respectivement quatre cas (13 %), 17 (55 %) et dix (32 %). La radiothérapie a été délivrée selon une technique bidimensionnelle par les photons γ du cobalt 60 à la dose de 20Gy en cinq fractions dans 24 cas (77 %). Trois patients ont interrompu leur traitement prématurément. Le contrôle du saignement a été évalué à la dernière séance et à 3–4 mois. Une analyse des facteurs pouvant influencer l’efficacité de la radiothérapie a été réalisée (sexe ; grade initial du saignement grade 1–2 contre 3 ; nécessité de transfusion ; dose de radiothérapie de 20Gy en cinq fractions vs autres schémas).
Le saignement avait disparu dans 71 % des cas à la fin de la radiothérapie et dans 78 % à 1 mois de la fin du traitement. À 4 mois de suivi, 41 % des patients n’avaient pas d’hématurie.
Notre étude confirme l’effet hémostatique de la radiothérapie. La disparition immédiate est obtenue dans 71 % des cas mais cet effet n’est pas durable dans le temps puisqu’à 3 mois de la fin du traitement, seulement 41 % des patients n’avaient pas de saignement.
To evaluate the influence of continent external urinary diversion type Mitrofanoff on male sexuality.
Between 1992 and 2011, 140 patients underwent continent urinary diversion type Mitrofanoff at an ...academic hospital. Among 76 men, 46 were interviewed about their sexuality after this operation. This study was performed using a set of validated questionnaires (IIEF, DAN PSS and Urolife), grouped by the model of the CTMH. Patients were divided according to their marital status: group 1: patients married before surgery (15 cases), group 2: patients married after surgery (7 cases) and group 3: singles (24 cases).
In the first group, the functional dimension of sexuality was positive with an overall score of 81%, the sexual discomfort score was assessed at 26 % and the sexual satisfaction score was 77%. In the second group, sexual function was considered conserved in all cases with a satisfaction score estimated at 98%. These patients reported a feeling of well-being following the disappearance of urinary incontinence with integrity of their body images. In contrast, in the last group, relatively impaired sexual function was noted (65%) with a satisfaction score estimated at 59%. These disorders were multifactorial, mainly related to neurological causal pathology.
To our knowledge, this is the first study about male sexuality in patients with a continent urinary diversion type Mitrofanoff. Marital status has a major role in the sexuality of these patients. A prospective study with pre- and postoperative evaluation will better clarify the factors affecting sexuality in these patients.
