Objective Lithium users are offered routine renal monitoring but few studies have quantified the risk to renal health. The aim of this study was to assess the association between use of lithium ...carbonate and incidence of renal failure in patients with bipolar disorder. Methods This was a retrospective cohort study using the General Practice Research Database (GPRD) and a nested validation study of lithium exposure and renal failure. A cohort of 6360 participants aged over 18 years had a first recorded diagnosis of bipolar disorder between January 1, 1990 and December 31, 2007. Data were examined from electronic primary care records from 418 general practices across the UK. The primary outcome was the hazard ratio for renal failure in participants exposed to lithium carbonate as compared with non-users of lithium, adjusting for age, gender, co-morbidities, and poly-pharmacy. Results Ever use of lithium was associated with a hazard ratio for renal failure of 2.5 (95% confidence interval 1.6 to 4.0) adjusted for known renal risk factors. Absolute risk was age dependent, with patients of 50 years or older at particular risk of renal failure: Number Needed to Harm (NNH) was 44 (21 to 150). Conclusions Lithium is associated with an increased risk of renal failure, particularly among the older age group. The absolute risk of renal failure associated with lithium use remains small.

A major manifestation of chronic allograft failure (CAF) is the accelerated onset of atherosclerotic lesions within the graft. Polymorphisms in the endothelial nitric oxide synthase (eNOS) gene have ...been implicated in the pathogenesis of native atherosclerosis. This study tested the hypothesis that polymorphisms in eNOS are associated with susceptibility to CAF after cadaveric renal transplantation. The patient cohort comprised 140 renal transplant recipients who had Received: their transplants between 1985 and 1997 at the Oxford Transplant Centre and included 61 patients with biopsy‐proven CAF and 79 with stable graft function for at least 10 years (long‐term survivors, LTS). Genotyping for one polymorphism in the promoter region and two polymorphisms in the coding regions of the eNOS gene was performed by polymerase chain reaction with sequence‐specific primers (PCR‐SSP). No association was found between any genetic variant and the development of CAF, even after stratification for other known risk factors. Statistical analysis revealed that all three polymorphisms were closely linked. We conclude that recipient eNOS gene polymorphisms do not alter the risk of CAF after renal transplantation.
Aims and method
To establish the incidence of venous thromboembolism (VTE) in mental health services for older people (MHSOP) in-patient units, and investigate current practice in terms of assessment ...of risk of VTE and treatment given. We conducted an incidence survey of VTE on in-patient units in MHSOP in a National Health Service trust, searching data of in-patients discharged over a 2-year period.
Results
Overall, 1495 individual patient records were searched. There were 17 confirmed VTEs, which is comparable with VTE incidence rates in general hospitals. There were no risk assessments done on admission and no thromboprophylaxis given. Use of compression stockings and mobilisation was limited.
Clinical implications
The study confirmed a significant VTE incidence and the need for increased awareness among MHSOP staff. This can be met by improved training of VTE awareness and management for clinical staff and the use of a VTE assessment tool on MHSOP in-patient units.
The importance of natural plant products and how they can be used as learning aids at senior undergraduate level are highlighted in this experiment. The experiment involves the isolation and ...structural determination of valtrate, a valepotriate occurring naturally in the roots and aerial parts of the medicinal plant, Valeriana officinalis, and its related species, Centranthus ruber. Valerian root extracts have been employed in both traditional and conventional medicine as sedating and antispasmolytic agents and appear to be nature's answer to modern benzodiazepine tranquillizers. We present various isolation and structural elucidation methodologies, including thin layer and flash column chromatography, and detailed spectroscopic analysis by NMR, IR, and mass spectra. The students isolated valtrate with an average recovery of 4.1% and subsequently managed to unravel the structure of valtrate using the various spectra provided, all within a six-hour laboratory (two week) session.
Summary
Lithium is one of the most effective psychotropic drugs we have, but it is underused because of its low therapeutic index, the need for regular blood tests and perceptions about its adverse ...effects, including renal problems. The last include urinary concentration deficits and diabetes insipidus, chronic kidney disease (including renal failure), nephrotic syndrome, hypercalcaemia, hyperparathyroidism and distal tubular acidosis. This article reviews these adverse effects with special emphasis on their management.
The low exposure of many trauma team members to severe trauma underlines the need for alternatives to “learning by doing” when the “doing,” in the early stages, involves a trauma patient. This is ...unacceptable with today’s patient safety requirements and often unobtainable with modern surgical training. Equally in the dynamic, high-pressured environment of managing trauma patients, the risk of errors occurring and miscommunications is rife. Management of a trauma patient, whether in the emergency department, operating theater, or intensive care unit, relies on the performance of the team as a whole and as such will only work with both effective technical skills but also nontechnical skills such as communication or delegation.
The evidence shows that when a trauma team is trained using simulation, there are significant improvements in the quality of teamwork and a reduction in clinical errors in the trained groups.
