ABSTRACT
To explain the mechanism of action of a novel intralesional cryoprobe, thermal behavior measurements, and histological studies were performed in swine muscle specimens after intralesional ...cryosurgery ex vivo. Slow cooling (20°C/min) and thawing (25°C/min) rates, end temperature of −30°C, produced 8 mm wide diffuse coagulative‐type necrosis and a 3 mm‐wide transition zone around the cryoprobe. In contrast, contact cryosurgery showed fast cooling and thawing rates (80°C/min) and an end temperature of −100°C. Efficacy and safety of the intralesional cryoprobe was further assessed in ten recalcitrant auricular keloids in nine Caucasian patients. There was a 67.4 ± 23 percent reduction of scar volume at the end of the 18‐month follow‐up period after a single intralesional treatment (p < 0.005). Significant reduction of hardness, elevation, and redness as well as itching, pain, and tenderness was documented. The histomorphometric analysis, including spectral and fractal analysis, as well as assessment of the fast Fourier transform algorithm, showed parallel alignment and reorganization of the collagen fibers in the treated scar similar to that in the normal dermis. A long hold time in the deep scar core caused minimal damage to the superficial tissue, including melanocytes. There was no evidence of permanent hypopigmentation, active bleeding, infection, or recurrence. The major advantages of the intralesional cryoprobe, including the marked efficacy of a single treatment, may have a major importance in the clinical application of cryosurgery in the treatment of keloids and of other lesions localized deep in the skin.
In this review, we discuss how, due to a variety of different interactions between laser radiation and biological tissue, the laser has become an established instrument in most medical fields. ...Depending on the interaction time and the effective power density, three types of laser tissue interaction can be distinguished: photochemical effects, photothermal effects, and photomechanical and photoionizing effects. After a description of the physical mechanisms, the typical parameters, and the medical applications of these effects, a review of the laser types used in medicine is given. For percutaneous laser disc decompression (PLDD), lasers in the near-infrared region (Nd:YAG, Ho:YAG, and diode lasers) and with visible green radiation (frequency doubled Nd:YAG, called "KTP laser") were reported to be effective.

Bei dem 67-jährigen Patienten bestand seit 3 Jahren eine
chronische, nicht heilende Ulzeration des Zehenzwischenraumes D4/D5 links.
Mehrere Probebiopsien aus dem Ulkus ergaben keinen ...Hinweis auf Malignität.
Vor vier Wochen Entwicklung eines schnell wachsenden Knotens auf der
Ulzeration. Es erfolgte die Resektion des 4. und 5. Strahles des linken
Fußes, histologisch ergab sich ein sarkomatoides Plattenepithelkarzinom
(G3), pT1NoMo. Regelmäßige Nachsorgeuntersuchungen waren
unauffällig.Das Entstehen von Plattenepithelkarzinomen mit
einem untypischen klinischen Verlauf in chronischen Wunden sollte
differenzialdiagnostisch berücksichtigt und histologisch abgeklärt
werden.
In a total of 45 rabbits, knee-joint arthrosis was induced according to the Hulth & Telhag model. Depending on the post-operative survival time, the cartilage was investigated macroscopically, ...histologically and immunohistochemically (within a period of 10 days to 8 months). Thereafter, the influence of laser irradiation at a wavelength of 692.6 nm and energy densities of 1 and 4 J/cm2 on the cartilage morphology seven days following the exposure was examined. After joint instability surgery it was found out that the cartilage changes in the main stress area (MSA) and in regions outside the main stress area (ROMSA) progressed differently. Various qualitative and semi-quantitative changes were found for collagens I, II, IV and V, and for the glycoproteins fibronectin and tenascin. Immunohistochemically, there was a growing expression of collagen I in the apical layers, collagen II showed a stronger pericellular expression, and collagen IV showed, after an initial growth of the pericellular expression, a reduced territorial expression and a stronger apical-interterritorial expression in the osteoarthrotic cartilage. For fibronectin, the cellular expression turned out to grow in the ROMSA. In the MSA it decreased, but at the same time the interterritorial expression grew. For Tanascin, there was a decrease of the interterritorial expression in the radial zone while the pericellular and interterritorial expression of the apical layers of the osteoarthrotic cartilage grew. Lasing proved to significantly influence the osteoarthrotically changed cartilage when applied at an energy density of 1 J/cm2, i.e., the morphological changes had not yet progressed to the extent the control group had. Both the chondrocyte density and the glucosaminoglycan content turned out to be higher. When lasing was applied at higher energy densities, no significant difference among the control groups was found. Thus, it could be demonstrated in vivo that an arthrotic process decelerates through the influence of laser light of low-energy densities.
Bei insgesamt 45 Kaninchen wurde eine Kniegelenksarthrose nach dem Modell von Hulth & Telhag erzeugt. In Abhängigkeit der postoperativen Überlebenszeit wurde der Knorpel makroskopisch, histologisch ...und immunhistochemisch untersucht (Zeitraum 10 Tage bis 8 Monate). Anschließend wurde der Einfluss von Laserbestrahlung mit einer Wellenlänge von 692,6 nm und Energiedichten von 1 und 4 J/cm2 auf die Knorpelmorphologie sieben Tage nach Bestrahlung überprüft. Nach operativ erzeugter Instabilität fand sich ein unterschiedlicher Verlauf der Knorpelveränderungen in der Haupt- (HB) und der Nichthauptbelastungszone (NHB). Für die Kollagene I, II, IV und VI sowie die Glycoproteine Fibronectin und Tenascin konnten verschiedene qualitative und semiquantitative Veränderungen gefunden werden. Immunhistochemisch trat eine zunehmende Expression von Kollagen I in den apikalen Schichten, bei Kollagen II eine verstärkte perizelluläre Expression, bei Kollagen VI nach anfänglicher Zunahme der perizellulären Expression eine verminderte territoriale Expression und verstärkte apikal-interterritoriale Expression beim osteoarthrotischen Knorpel auf. Für Fibronectin wurden eine Zunahme der zellulären Expression in der NHB und eine Abnahme der zellulären Expression zugleich mit einer Zunahme der interterritorialen Expression in der HB gefunden. Bei Tenascin zeigte sich eine Abnahme der interterritorialen Expression in der Radiärzone mit Zunahme der perizellulären und interterritorialen Expression der apikalen Schichten beim osteoarthrotischen Knorpel. Bezüglich des Lasers fand sich für den osteoarthrotisch veränderten Knorpel ein signifikanter Einfluss bei einer Energiedichte von 1 J/cm2, d.h., die morphologischen Veränderungen waren noch nicht so weit fortgeschritten im Vergleich zur Kontrollgruppe. Chondrozytendichte als auch Glucosaminoglycangehalt waren höher. Beim Lasern mit höherer Energiedichte konnte kein signifikanter Unterschied unter den Kontrollgruppen festgestellt werden. Es konnte somit die Verlangsamung eines arthrotischen Prozesses durch Einfluss von Laserlicht niedriger Energiedichte in vivo dargelegt werden.

Due to the wide variability of tissue interactions and the possibility of specific applications, the neodymium-yttrium-aluminum-garnet (Nd:YAG) laser is the most important surgical laser. With the ...adequate choice of application mode and relationship between interaction time and power density, it can be used for precise cutting in the contact mode with the bare fiber, with either a wide or small coagulation seam. With a handpiece a precise focal coagulation for preparation and hemostatic purposes is possible, as well as cutting with the focussed noncontact beam where additionally a wide coagulation seam is necessary. Endoscopically guided coagulation, vaporization, and cutting are possible with bare fibers which can be introduced through nearly all endoscopes. With increasing importance, the possibility for wide and homogeneous volume coagulation is used to destroy diseased tissues either by noncontact irradiation or interstitial placement of the fiber. Thus, the field of laser application in tumor therapy ranges from the treatment of superficial tumors to endoscopic tumor ablation, resection of neoplastic tissue in parenchymatous organs, and interstitial thermotherapy with coagulation of deep-seated primary and secondary malignancies. With its different application modes the laser can be used as a surgical instrument or as a central therapeutical method, whereby perfect control of tissue interactions is always possible by using either visual control for superficial and endoscopic procedures or magnetic resonance imaging and color-coded duplex sonography as a control for interstitial procedures. During 12 years of clinical work we have developed several application modes and have proved the Nd:YAG laser to be an effective instrument in tumor therapy.
The course of arthrosis was investigated on an animal-experimental arthrosis model considering macroscopic aspects, and the proteoglycan and the glycosaminoglycan contents. Based on these parameters, ...the influence of a low-power diode laser of 692.6
nm wavelength on the progress of arthrosis was investigated. Thirty days following joint instability surgery another operation was made during which the femoral condyles were irradiated using different energy densities. Seven days after the second operation, macroscopic findings were made and the proteoglycan content was established based on the quantitative determination according to Taylor and Jeffre. This method is based on various spectrophotometric absorption behaviours of different concentrations of sulphatized glycosaminoglycans in the presence of dimethylmethylene blue.
Macroscopically, a progressively increasing severity of cartilage changes during the course of arthrosis was detected and the proteoglycan content was found to decrease. The changes in the irradiated joints proved to be less severe, with the higher energy density having a greater positive influence of statistical significance.
In einem tierexperimentellen Arthrosemodell wurde der Verlauf der Arthrose nach makroskopischen Gesichtspunkten und nach Aspekten des Proteoglykan- bzw. Glykosaminoglykangehaltes des Knorpels untersucht. Der Einfluss der Knorpelbestrahlung mit niederenergetischem Diodenlaser mit einer Wellenlänge von 692.6
nm auf den Verlauf der Arthrose wurde nach gleichen Parametern evaluiert. 30 Tage nach einer Gelenkinstabilitätsoperation wurden während einer zweiten Operation die Femurkondylen mit unterschiedlichen Energiedichten bestrahlt. Sieben Tage nach der zweiten Operation wurden sowohl makroskopische Befunde erhoben als auch der Proteoglykangehalt auf Basis der quantitativen Bestimmung nach Taylor & Jeffre bestimmt. Dieses Verfahren beruht auf den unterschiedlichen spektrophotometrischen Absorptionsverhalten der verschiedenen Konzentrationen von sulfatierten Glykosaminoglykanen in Anwesenheit von Dimethylmethylenblau.
Makroskopisch fand sich eine progrediente Zunahme der Schwere der Knorpelveränderungen im Verlauf der Arthrose, ebenso konnte eine Abnahme des Proteoglykangehaltes nachgewiesen werden. Des Weiteren fanden sich bei den bestrahlten Gelenken mildere Veränderungen, wobei hier die höhere Energiedichte einen größeren positiven Einfluss mit statistischer Signifikanz hatte.
La evolución de la artrosis se ha examinado en un modelo experimental-animal, considerando los aspectos macroscópicos y el contenido de proteoglicano y glicosaminoglicano del cartílago. La influencia de la radiación del cartílago con láser de diodo de baja energía con una longitud de onda de 692.6 nm en el desarrollo de la artrosis se evaluó con los mismos parámetros. Treinta días después de una operación para estabilizar la articulación, los cóndilos femorales fueron radiados con diferentes densidades de energía en una segunda operación. Siete días después de la segunda operación, se presentó el diagnóstico y se estableció el contenido del proteoglicano sobre la base de la determinación cuantitativa según Taylor y Jaffe. Este procedimiento se basa en el diferente comportamiento de absorción espectrofotométrico de las diferentes concentraciones de glicosaminoglicanos sulfatados en presencia de azul de dimetilmetileno.
Macroscópicamente, durante el transcurso de la artrosis, se observó un incremento progresivo del cuerpo de las alteraciones en el cartílago, también pudo comprobarse un descenso en el contenido del proteoglicano. Además se encontraron en las articulaciones radiadas alteraciones más bajas y la más alta densidad de la energía ejerció una mayor influencia positiva con significado estadístico.

J), an enhanced matrix synthesis (cartilage material from 36 patients) could be achieved. The proof succeeded by applying the radioisotope marking method (
3H-proline).
Interestingly enough it turned out that the application of too high a power density but constant energy density resulted in a reduced matrix synthesis rate (reduction of 28%). In this study, it was demonstrated for the first time that laser radiation may have a positive influence on human cartilage thus contributing to future in-vivo application.
Untersucht wurde der Einfluss von Laserstrahlung verschiedener Wellenlänge, Leistungsdichte und die Abhängigkeit der Einwirkzeit auf humane, osteoarthrotisch veränderte Chondrozyten. Ziel war es, die im Tierexperiment gezeigten positiven Ergebnisse zu bestätigen. Es konnte dargelegt werden, dass bei Vorliegen einer bestimmten Parameterkonstellation (2

3H-Prolin).
Interessant war die Beobachtung, dass bei zu hoher Leistungsdichte, aber gleich bleibender Energiedichte eine verminderte Matrixsyntheserate (ca. 28%) resultiert.
In dieser Arbeit wurde erstmals der mögliche positive Einfluss von Laserstrahlung auf humanen Knorpel bewiesen. Damit könnte eine erste Voraussetzung für die In-vivo-Anwendung gegeben sein.

J) se obtiene un aumento en la síntesis de la matriz (material de cartílago proveniente de 36 pacientes). El éxito de la prueba se alcanzó aplicando el método de marcaje con radioisótopo (
3H-prolina).
Resultó interesante que, utilizando una densidad de potencia muy alta, pero una densidad de energía constante, se produjo una reducción de la tasa (28%) de síntesis de la matriz. En este trabajo se ha demostrado por primera vez que la radiación láser tendría una influencia positiva sobre el cartílago humano, pudiendo contribuir a futuras aplicaciones in vivo.
