While we were drafting the recommendations for the joint contingency plan between the Spanish Society of Intensive Care and Coronary Unit Nursing (SEEIUC) and the Spanish Society of Intensive and ...Critical Care Medicine and Coronary Units (SEMICYUC), predictions were overtaken by events with regard to the magnitude of the COVID-19 pandemic. Members informed us of the lack of personal protection equipment (PPE), the rapid provision of improvised ICUs in various hospital areas and the recruitment of nurses to cover shifts. The failure to recognise the specialty of critical care nursing, included in the macro-specialty of medical-surgical nursing and not yet developed, has highlighted the urgent need to learn from our mistakes: specialisation, increase the number of nurses in teams and protect the public health system.

Statistics: descriptive and multivariate analysis.
254 cases were included. Thirty-nine per cent were women and the median Q1–Q3 age was 67 56–77 years. SAPS 3 on admission (median Q1–Q3): 62 51–71 points.
Frail patients on admission (CFS-Es 5–9): 58 (23%). Dependency on admission (n = 254) vs. 3 m after hospital discharge (n = 171) vs. 12 m after hospital discharge (n = 118): 1) Barthel 90–100: 82% vs. 68% vs. 65%. 2) Barthel 60–85: 15% vs. 15% vs. 20%. 3) Barthel 0–55: 3% vs. 17% vs. 15%.
In the multivariate analysis, adjusted for the variables recorded, we observed that frail patients on admission (CFS-Es 5–9) are 2.8 times (95%CI: 1.03–7.58; p = 0.043) more likely to increase dependency (Barthel 90–100 to <90 or Barthel 85–60 to <60) at 3 m post-discharge (with respect to admission) and 3.5 times (95%CI: 1.18–10.30; p = 0.024) more likely to increase dependency at 12 m post-discharge. Furthermore, for each additional CFS-Es point there is a 1.6-fold (95%CI: 1.01–2.23; p = 0.016) greater chance of increased dependency in the 12 m following discharge.
CFS-Es at admission can predict increased dependency at 3 m and 12 m after hospital discharge.

Estudio de cohorte multicéntrico en 2020–2022. Incluidos pacientes con estancia >48 h en unidades de cuidados intensivos (UCI) y no COVID-19. Variables: fragilidad previa al ingreso (CFS-Es). Sexo, edad, días de estancia (UCI y hospital), dependencia al ingreso y a 3 m y 12 m del alta (Índice de Barthel), debilidad muscular (Medical Research Council Scale sum score <48), reingresos hospitalarios. Estadística: descriptiva y análisis multivariante.
Se incluyeron 254 casos. El 39% fueron mujeres y la mediana Q1–Q3 de edad fue de 67 56–77 años. SAPS 3 al ingreso (mediana Q1–Q3): 62 51–71 puntos.

Además, por cada punto adicional de CFS-Es se multiplica por 1,6 (IC95%: 1,01–2,23; p = 0,016) la posibilidad de incrementar la dependencia en los 12 m siguientes al alta.

The outbreak was initially confined to ...Wuhan City, but then spread outside Chinese borders. On 31 January 2020, the first case was declared in Spain. On 11 March 2020, The World Health Organization (WHO) declared the coronavirus outbreak a pandemic. On 16 March 2020, there were 139 countries affected. In this situation, the Scientific Societies SEMICYUC and SEEIUC have decided to draw up this Contingency Plan to guide the response of the Intensive Care Services. The objectives of this plan are to estimate the magnitude of the problem and identify the necessary human and material resources. This is to provide the Spanish Intensive Medicine Services with a tool to programme optimal response strategies.

Cuando redactábamos las recomendaciones para el plan de contingencia conjunto entre la Sociedad Española de Enfermería Intensiva y Unidades Coronarias (SEEIUC) y la Sociedad Española de Medicina Intensiva, Crítica y Unidades Coronarias (SEMICYUC) los hechos iban por delante de las predicciones ante la magnitud de la pandemia del COVID-19. Socios y socias nos informaban de la falta de equipos de protección individual (EPI), la habilitación rápida de UCI improvisadas en zonas varias de hospitales y el reclutamiento de enfermeras para cubrir turnos. La falta de reconocimiento de la especialidad enfermera en cuidados críticos, englobada dentro la macroespecialidad de enfermería medicoquirúrgica, todavía sin desarrollar, ha puesto de relieve la necesidad urgente de aprender de los errores: especialidad, aumentar dotaciones en los equipos de enfermería y proteger el sistema sanitario público.

El 11 de marzo, la Organización Mundial de la Salud declaró el brote de coronavirus como pandemia. El 16 de marzo había 139 países afectados. Ante esta situación, las Sociedades Científicas SEMICYUC y SEEIUC han decidido la elaboración de este plan de contingencia para dar respuesta a las necesidades que conllevará esta nueva enfermedad. Se pretende estimar la magnitud del problema e identificar las necesidades asistenciales, de recursos humanos y materiales, de manera que los servicios de medicina intensiva del país tengan una herramienta que les permita una planificación óptima y realista con que responder a la pandemia.

RESUMEN Las lesiones por humedad representan un importante y grave problema en personas encamadas durante periodos de tiempo prolongados. En las unidades de cuidados intensivos, dichas lesiones ...suponen un importante reto para los profesionales de enfermería. Su prevención y tratamiento temprano con apósitos específicos es una tarea importante para disminuir la morbilidad del enfermo en cuidados intensivos.
El año 2020 nos ha traído un grave problema de salud a nivel mundial al que todavía nos estamos enfrentando; si bien no es la primera vez que una enfermedad o plaga ha afectado a numerosos países, ...la actual pandemia de la Covid- 19 ha supuesto poner en jaque a la práctica totalidad de los equipos y sistemas sanitarios a nivel planetario. Una situación de tal extrema gravedad nos ha dado la posibilidad y nos seguirá dando la oportunidad de estudiar los fallos que pudiesen tener los sistemas sanitarios de los diferentes países con vista a tener la iniciativa tan lógica, por otro lado, de “aprender de los errores” y de captar todas aquellas fuentes de fuerza y reforzamiento de los sistemas sanitarios.
