CASE RESOLUTION The procedure was performed under general anesthesia and with the administration of the standard dose of sodium heparin to prevent thrombosis or systemic embolisms (high risk ...considering the patient's condition) via venous femoral access. This procedure was planned using 3D reconstruction through rotational angiography (figure 1 and video 1 of the supplementyary data). Embolization was guided using the roadmapping technique that merges the 3D reconstruction simultaneously while overlapped to the 2D reconstruction in the x-ray image projection required during multiple occlusion device implantations. CASE PRESENTATION Among other causes, arteriovenous malformations (AVMs) can be congenital and associated with Rendu-Osler-Weber disease. It is a conglomerate of anomalous vessels of variable size ...and number characterized by the lack of an intermediate normal capillary network between the arterial and the venous circuits. Its prevalence is of approximately 1 AVM for every 2600 individuals, and it is a significant cause of morbidity and mortality. The percutaneous closure of AVMs is the standard of care established. However, there is a significant rate of recanalization of AVMs at the long-term follow-up due to the recanalization of the main occluded vessel and the formation of collateral circulation towards the vascular tangle. A few articles already published support sequential embolization to reduce the risk of the latter. This article describes the percutaneous closure of 1 AMV in a pediatric patient using a technique of sequential embolization through rotational angiography, 3D analysis, and roadmapping (technique based on merging the 3D reconstruction acquired through rotational angiography and x-ray images in such a way that reconstruction acts as a «map» or «flight plan» during the procedure). ABSTRACT Introduction and objectives: The results of the Radcong-21 Registry of the Spanish Society of Pediatric Cardiology and Congenital Heart Disease Working Group on Hemodynamics are described to ...analyze data, establish updated reference parameters, and compare them to other registries. Methods: Retrospective, cross-sectional, observational, multicenter registry of patients with congenital heart disease undergoing cardiac catheterization in 2021. Each cath lab sent the last 100 cases performed prior to January 2022. A descriptive analysis was conducted of anthropomorphic variables, procedural (grouped by type and radiation exposure categories REC) and technical characteristics, and dosimetric parameters with additional review of all values outside the 95%CI of the median. Results: A total of 1090 procedures performed in 11 cath lab of 10 hospital centers were analyzed. Age distribution: 22.8% < 1 year, 60.7% between 1-18 years, and 16.4% > 18 years. In dose area product (DAP)/Kg and DAP/Kg/fluoroscopy, the distribution was very similar regardless of the type of cath lab as is the case with most pediatric patients in terms of age, weight, and REC group. The DAP/Kg was higher in the REC I and III groups compared to other countries with registries and improvement programs in this area (78% and 8,3%, respectively). Conclusions: Representative data of dosimetric parameters by age and procedures in congenital cardiac catheterizations were obtained in Spain back in 2021. DAP/Kg is the parameter with the lowest dispersion in the sample. There is room for improvement compared to other countries with optimization programs in this area.

Deleterious long-term effects of chronic pulmonary regurgitation after repair of tetralogy of Fallot have become evident during the last decades. Subsequently, some groups have developed strategies to spare the pulmonary valve function at the time of repair with good early results. However, mid-term outcomes are scarce in the literature and in some cases controversial. The aim of our study is to report our results mid-term with valve-sparing repair of tetralogy of Fallot. We retrospectively reviewed patients undergoing tetralogy of Fallot repair and having preservation of the pulmonary valve with intraoperative dilation at our institution. From June 2009 through June 2017, 42 patients underwent valve-sparing tetralogy of Fallot repair. Median age and weight at surgery were 5.2 months and 7.2 kg. Median preoperative pulmonary valve diameters and Z scores by echocardiography were 6.4 mm (range 4.5–11 mm) and −2.3 (range −1.3 to −4.5). No patient died in our series. For a median follow-up of 45 months, the pulmonary valve has grown by Z score (P < 0.0001) as well as the pulmonary trunk (P= 0.00216). Significant pulmonary regurgitation has developed in 9 patients (21.4%). No patient has required reintervention/reoperation for recurrent right ventricular outflow tract obstruction. Patients with tetralogy of Fallot who had valve-sparing repair with intraoperative dilation of the pulmonary valve show good early and mid-term results with respect to right ventricular outflow tract obstruction. The pulmonary valve annulus and the pulmonary trunk grow through follow-up. Progressive development of significant pulmonary regurgitation is seen in more than 20% of patients. Long-term data with this approach and comparison with a population of patients undergoing a transannular patch repair are required to establish the real utility of this approach.
En una mujer de 34 años se decidió realizar tratamiento percutáneo para una recoartación de aorta. Como antecedentes destacaban comunicación interventricular muscular con cierre espontáneo, válvula ...aórtica bicúspide con insuficiencia moderada asintomática con volumen y función ventricular normales, y coartación de aorta yuxtaductal tratada mediante aortoplastia con parche a los 2 meses de vida. Durante el seguimiento ambulatorio se observaron datos de recoartación progresiva por ecocardiografía y resonancia magnética, así como respuesta hipertensiva en la ergometría, por lo que se presentó el caso en sesión multidisciplinaria y se decidió tratamiento endovascular. Se realizó aortografía mediante acceso arterial femoral derecho y se observó una recoartación en la unión del cayado aórtico con la aorta torácica descendente (figura 1A,B, flecha azul), con un diámetro mínimo de 9 mm y un gradiente pico-pico de 20 mmHg. Asimismo, se descubrió un seudoaneurisma sacular adyacente a la región estenosada (figura 1A,B; figura 2A, flecha amarilla; vídeo 1 del material adicional), con diámetros internos de 8 × 10 mm (figura 2B), por lo que se implantó un stent recubierto (Covered CP Stent, NuMED, Canadá) de 45 mm de longitud (figura 3), con buen resultado angiográfico (vídeos 2 y 3 del material adicional) y sin evidencia de gradiente...
The aim of this study was to collect retrospective data on the prescription of clopidogrel, describe the conditions of its use in the paediatric population of a tertiary referral hospital, and ...evaluate its use based on the current scientific evidence.
We conducted a retrospective, observational and descriptive study between March 2010 and March 2017. We included all patients under the age of 18 who were discharged from our hospital for home treatment with clopidogrel within the study period. We collected data on the following: demographic data, diagnosis, indication for clopidogrel, start and end date of treatment, presence or absence of concomitant treatment with acetylsalicylic acid or other antiplatelet or anticoagulant drugs, concomitant treatment with proton pump inhibitors, effectiveness, and adverse effects.
The study included a total of 11 patients (45% male). The mean age was 3.1 years (range, 1 month to 8 years). The prescribed dose of clopidogrel was 0.2mg/kg/day in all patients, and 10/11 patients received concomitant treatment with acetylsalicylic acid with the purpose of optimising antiplatelet therapy. None of the children received concomitant treatment with anticoagulants, and only one of them received treatment with a proton pump inhibitor. We did not find evidence of haemorrhagic complications or other adverse effects associated with clopidogrel.
Based on our experience, a clopidogrel dose of 0.2mg/kg/day is a safe and effective treatment, regardless of the patient's age. The good tolerance observed in our study could be related to the adjustment of the optimal dose with the aim of achieving antiplatelet aggregation without increasing the risk of adverse effects.
El objetivo de este estudio fue recopilar datos retrospectivos sobre la prescripción de clopidogrel y describir las condiciones de uso en la población pediátrica de un hospital de referencia de nivel terciario y evaluarlas con base en la evidencia disponible.
Estudio observacional, descriptivo y retrospectivo realizado entre marzo de 2010 y marzo de 2017. Los criterios de inclusión fueron: 100% de los pacientes menores de 18 años que fueron dados de alta de nuestro hospital con clopidogrel como tratamiento domiciliario en el período de estudio. Se recogieron los siguientes datos: datos demográficos, diagnóstico, indicación de clopidogrel, fecha de inicio y de finalización del tratamiento, presencia o ausencia de tratamiento concomitante con ácido acetilsalicílico u otros antiagregantes plaquetarios o anticoagulantes, tratamiento concomitante con inhibidores de la bomba de protones, efectividad y efectos adversos.
Un total de 11 pacientes se incluyeron en el estudio (45% hombres). La edad promedio fue de 3,1 años (rango: 1 mes-8 años). La dosis recomendada de clopidogrel fue de 0,2mg/kg/día en todos los individuos, recibiendo tratamiento concomitante con ácido acetilsalicílico 10/11 pacientes para optimizar la terapia antiplaquetaria. Ningún niño recibió tratamiento concomitante con anticoagulantes y solo uno de ellos recibió tratamiento con inhibidores de la bomba de protones. No se observaron trastornos hemorrágicos u otros efectos adversos asociados con clopidogrel.
Según nuestra experiencia, la dosis de clopidogrel de 0,2mg/kg/día demostró ser una estrategia segura y efectiva, independientemente de la edad del paciente. La buena tolerancia observada podría estar asociada con el ajuste de la dosis óptima para lograr la antiagregación plaquetaria sin aumentar el riesgo de efectos adversos.
PRESENTACIÓN DEL CASO Las malformaciones arteriovenosas (MAV) pueden tener, entre otros, un origen congénito asociado a la enfermedad de Rendu-Osler-Weber, tratándose de un conglomerado de vasos ...anómalos de tamaño y número variable, caracterizado por la ausencia de una red capilar normal intermedia entre el circuito arterial y el venoso. La prevalencia de las MAV es de aproximadamente 1 por cada 2.600 individuos, y es una importante causa de morbimortalidad. El cierre percutáneo de las MAV es el tratamiento de elección establecido. No obstante, en el seguimiento a largo plazo existe una tasa significativa de recanalización de la MAV, tanto por recanalización del vaso principal ocluido como por la formación de la circulación colateral hacia el ovillo vascular. Hay artículos que apoyan la embolización secuencial para reducir el riesgo de esta última. En este caso se describe la oclusión percutánea de una MAV en una paciente en edad pediátrica con una técnica de embolización secuencial con la ayuda de la angiografía rotacional, el análisis tridimensional y el roadmapping (técnica consistente en fusionar la reconstrucción tridimensional conseguida con la angiografía rotacional y la escopia radiológica, actuando la reconstrucción como un «mapa» u «hoja de ruta» durante el procedimiento). RESOLUCIÓN DEL CASO El procedimiento se realizó bajo anestesia general y con la administración de heparina sódica según la pauta habitual para evitar trombosis o embolias sistémicas (riesgo alto ...teniendo en cuenta la patología de la paciente), con un abordaje femoral venoso. La planificación de la intervención se realizó utilizando una reconstrucción tridimensional mediante angiografía rotacional (figura 1 y vídeo 1 del material adicional) y guiando la embolización con la técnica de roadmapping, que permite fusionar la reconstrucción tridimensional simultáneamente y superpuesta a la bidimensional en la proyección escópica requerida durante los múltiples implantes de dispositivos oclusores. RESUMEN Introducción y objetivos: Se describen los resultados del Registro Radcong-21 del Grupo de Trabajo de Hemodinámica de la Sociedad Española de Cardiología Pediátrica y Cardiopatías Congénitas ...con el objetivo de analizar los datos, establecer parámetros de referencia actualizados y compararlos con otros registros. Métodos: Registro multicéntrico, observacional, transversal y retrospectivo de pacientes con cardiopatías congénitas tratados con cateterismo cardiaco en 2021. Cada sala de hemodinámica remite los últimos 100 casos realizados hasta enero de 2022. Análisis descriptivo de variables antropomórficas, procedimientos (agrupados por tipo y riesgo estimado de radiación RER), características técnicas y parámetros dosimétricos. Revisión adicional de todos los valores fuera del intervalo de confianza del 95% de la mediana. Resultados: Se analizaron 1.090 procedimientos en 11 salas de hemodinámica de 10 centros hospitalarios. De los pacientes, el 22,8% fueron < 1 año, el 60,7% entre 1 y 18 años, y el 16,4% > 18 años. Los valores del producto dosis-área (PDA)/kg y del PDA/kg/fluoroscopia fueron muy similares independientemente del tipo de sala, al igual que ocurre con la edad, el peso y el grupo RER en la mayor parte de las salas pediátricas. El PDA/kg fue superior en los grupos RER I y III comparado con otros países con registros y programas de mejora activos en este campo (un 78 y un 8,3%, respectivamente). Conclusiones: Se obtienen datos representativos de los parámetros dosimétricos por edad y procedimientos en los cateterismos cardiacos congénitos en España en el año 2021. El PDA/kg es el que tiene menor dispersión en la muestra total. Existen áreas de mejora en comparación con otros países con programas de optimización.
