Introduction:
Paragangliomas are neuroendocrine tumors expressing somatostatinergic receptors and, thus, may be imaged by somatostatin receptor scintigraphy (SRS). The purpose of the study was to ...assess the contribution of SRS in the management of paraganglioma of the head and neck.
Material and Methods:
Based on a population of 159 patients with head and neck paraganglioma, 111In-Pente-treotide (148 MBq) was intravenously administered to three groups of patients. Group A consisted of patients with tumors that did not exhibit typical radiologic features of paragangliomas and required further diagnostic evaluation. Group B consisted of patients with known paragangliomas or paraganglioma metastatic lymph nodes who required staging and assessment of multicentricity. Group C consisted of asymptomatic relatives of affected individuals who required screening. All patients underwent clinical, laboratory, radiologic, and audiovestibular evaluation.
Results:
In group A, SRS was positive in all cases of paragangliomas, but also in the case of meningiomas. No false-negative was observed. In group B, SRS was strongly positive in all head and neck masses diagnosed on conventional imaging to be paragangliomas. It localized 1 primary malignant paraganglioma and revealed 9 unexpected foci. In group C, SRS was positive in half of the cases, which led to the diagnosis of paraganglioma in all cases.
Conclusions:
Because of very high sensitivity in detecting paraganglioma, SRS should be included in a multiple-step strategy for patients' management. It is useful to rule out the diagnosis of schwannoma, and furthermore contributes to a precise staging. If biologic genetic testing evidences the predisposing mutation in patients' relatives, SRS could be proposed to characterize the phenotype and hence to detect early disease.
Objectif Une hypertrophie ventriculaire gauche est fréquente chez les obèses. Elle régresserait moins après perte pondérale chez l’obèse diabétique. Notre objectif était d’examiner, chez des obèses ...candidats à la chirurgie bariatrique, les déterminants de la masse ventriculaire gauche (MVG) et le rôle des apnées du sommeil et des anomalies glycémiques dans son évolution après amaigrissement obtenu par chirurgie bariatrique. Patients et méthodes Nous avons exploré 111 patients, 83 femmes, âge 37,6 ± 11,2 ans, IMC 43,3 ± 7,8 kg/m2 , moyenne de pression artérielle 126/74 mmHg, par échocardiographie. MVG était évaluable en préopératoire chez 95 patients et après chirurgie (gastroplastie, sleeve gastrectomy, by-pass) chez 31. Un syndrome d’apnée du sommeil a été recherché chez 55 patients avec score d’Epworth > 8. Résultats Parmi les 95 patients, 13 étaient diabétiques connus et, selon la charge en glucose, 22 étaient prédiabétiques (hyperglycémie à jeun et/ou intolérance au glucose) et 7 diabétiques. MVG indexée sur la taille corrélait à IMC, masse maigre, pression artérielle systolique et tour de taille/tour de hanches (p = 0,04 à 0,001). MVG/T était plus élevé chez les patients prédiabétiques ou diabétiques que chez les normoglycémiques (p = 0,004). Chez les non diabétiques, MVG/T corrélait aux glycémies à jeun et post-charge (p < 0,0001, p = 0,049). À la seconde échocardiographie, pratiquée après une perte pondérale moyenne de 29,2 kg, MVG a diminué chez 18 patients et progressé chez 13 sur les 31 patients réexaminés. En préopératoire, 5 avaient un index d’apnées-hypopnées (IAH) > 20/h et seul un de ces patients a diminué sa MVG ; un IAH élevé semblait associé à la progression de MVG (p = 0,058). Parmi les patients avec IAH < 20/h sans diabète connu, 5 étaient prédiabétiques ; leur MVG progressait tandis qu’elle régressait chez 7 des 15 patients normoglycémiques avant chirurgie. Conclusion La présence d’un SAS ou d’anomalies glycémiques semble s’opposer à la régression de la MVG après amaigrissement.
The aims of this study were to describe the unbound and total lopinavir (LPV) pharmacokinetics in pregnant women in order to evaluate if a dosing adjustment is necessary during pregnancy. Lopinavir ...placental transfer is described, and several genetic covariates were tested to explain its variability. A total of 400 maternal, 79 cord blood, and 48 amniotic fluid samples were collected from 208 women for LPV concentration determinations and pharmacokinetics analysis. Among the maternal LPV concentrations, 79 samples were also used to measure the unbound LPV concentrations. Population pharmacokinetics models were developed by using NONMEM software. Two models were developed to describe (i) unbound and total LPV pharmacokinetics and (ii) LPV placental transfer. The pharmacokinetics was best described by a one-compartment model with first-order absorption and elimination. A pregnancy effect was found on maternal clearance (39% increase), whereas the treatment group (monotherapy versus triple therapy) or the genetic polymorphisms did not explain the pharmacokinetics or placental transfer of LPV. Efficient unbound LPV concentrations in nonpregnant women were similar to those measured during the third trimester of pregnancy. Our study showed a 39% increase of maternal total LPV clearance during pregnancy, whereas unbound LPV concentrations were similar to those simulated in nonpregnant women. The genetic polymorphisms selected did not influence the LPV pharmacokinetics or placental transfer. Thus, we suggest that the LPV dosage should not be increased during pregnancy.
Introduction L’IMS, en particulier si elle est associée à des SC, et la dysfonction endothéliale sont prédictives de la survenue d’événements cardiovasculaires chez les DT2. Le but de l’étude était ...d’évaluer chez des DT2 asymptomatiques la FEC et la FEP selon leur statut coronarien. Matériels et méthodes Une IMS, définie par la positivité de l’épreuve effort ou de la scintigraphie myocardique ou de l’échographie de stress, a été recherchée chez 85 patients dont 56 hommes, âgés de 61 ± 8 ans, diabétiques de type 2 depuis 15 ± 8 ans, asymptomatiques sur le plan cardiaque, à ECG de repos normal et présentant des facteurs de risque associés. Une coronarographie a été effectuée en cas d’IMS. La FEC a été explorée (n = 40, faisabilité 47 %) par échocardiographie trans-thoracique en mesurant lors d’un test au froid la vitesse de flux au niveau de l’artère inter-ventriculaire antérieure, qui augmente physiologiquement en réponse à l’augmentation du double produit pression artérielle x fréquence cardiaque (ΔPAxFC). La FEP a été évaluée en mesurant la vasodilatation de l’artère humérale lors d’une épreuve d’hyperhémie post-occlusive (n = 85). Résultats Trois groupes ont été constitués en fonction du statut coronarien : sans IMS (n = 46), IMS sans SC (n = 24) et IMS avec SC significatives (n = 15). La corrélation physiologique entre variation de flux coronaire et ΔPAxFC après test au froid retrouvée chez les patients IMS- (r = 0,481, p < 0,05) était perdue chez les patients IMS+, avec ou sans SC. La FEP, évaluée par le rapport diamètre diastolique huméral 1 min après/avant hyperhémie, différait selon le statut IMS-, IMS+CS-, IMS+CS+ : 102 ± 4, 100 ± 3, 98 ± 4 % respectivement (p < 0,05), avec une différence significative entre le groupe IMS+CS+ et les autres (p < 0,05). Discussion Les traitements améliorant la fonction endothéliale pourraient être utiles chez les patients présentant une IMS. Conclusion Les diabétiques de type 2 avec IMS ont une FEC inadaptée et une FEP altérée. La FEP pourrait être utilisée comme un marqueur d’IMS et de SC.
