Unique structural and optical properties of atomically thin two-dimensional semiconducting transition metal dichalcogenides enable in principle their efficient coupling to photonic cavities having ...the optical mode volume close to or below the diffraction limit. Recently, it has become possible to make all-dielectric nano-cavities with reduced mode volumes and negligible non-radiative losses. Here, we realise low-loss high-refractive-index dielectric gallium phosphide (GaP) nano-antennas with small mode volumes coupled to atomic mono- and bilayers of WSeFormula: see text. We observe a photoluminescence enhancement exceeding 10Formula: see text compared with WSeFormula: see text placed on planar GaP, and trace its origin to a combination of enhancement of the spontaneous emission rate, favourable modification of the photoluminescence directionality and enhanced optical excitation efficiency. A further effect of the coupling is observed in the photoluminescence polarisation dependence and in the Raman scattering signal enhancement exceeding 10Formula: see text. Our findings reveal dielectric nano-antennas as a promising platform for engineering light-matter coupling in two-dimensional semiconductors.
Fragestellung:
Die Cervixgravidität macht ca. 0,1% aller ektopen Schwangerschaften aus. Es liegen bislang keine größeren aussagekräftigen Studien bezüglich der Überlegenheit verschiedener ...Therapiestrategien bei Cervixgravidität vor. In unserem Fallbeispiel wird ein erweitertes konservatives Therapiemanagement bestehend aus MTX-Applikation i.m. kombiniert mit intraamnialer MTX-Injektion beschrieben.
Methoden:
Wir berichten über eine 36-jährige II G I P (Z.n. Sectio), die sich mit vaginalen Blutungen in der 7+3 SSW vorstellte. Die Konzeption erfolgte spontan. Sonographisch zeigte sich bei geschlossenem äußerem Muttermund eine intakte entsprechend der 8. SSW entwickelte Cervixgravidität. Der β-HCG Wert bei Aufnahme betrug 15520 mIU/ml
Ergebnisse:
Es erfolgte zunächst die i.m.-Applikation von 50 mg MTX/m
2
KOF und β-HCG Verlaufskontrollen. Bei steigenden β-HCG Werten und weiterhin positiver Herzaktion erfolgte am 5. Behandlungstag die intraamniale MTX-Injektion (10 mg/kg KG). Im Anschluss zeigten sich fallende β-HCG Werte bei negativer Herzaktion und sonographischem Regress.
Zusammenfassung:
Auch in unserem Fallbeispiel zeigte sich ein erweitertes konservatives Therapiemanagement, bestehend aus MTX-Gabe i.m. gefolgt von der sonographisch-gesteuerten intraamnialen Applikation von MTX unter laborchemischer und sonographischer Verlaufskontrolle als erfolgreich und komplikationslos.

In seltenen Fällen können Erkrankungen des Omentum majus Ursache einer akuten abdominellen Schmerzsymptomatik sein. Diese Diagnose wird immer noch häufig erst intraoperativ gestellt.
Eine primäre ...Torsion erfolgt durch Rotation eines Netzteils an einem Fixpunkt. Als prädisponierende Faktoren werden Anomalien der Netzkontur, irreguläre Netzgefäße, mechanische Alteration, Husten und körperliche Überanstrengung angesehen. Die Torsion bedingt meist eine Verlegung der venösen omentalen Gefäße, in der Folge einen Rückstau des Blutes mit Ödem, später eine Ischämie und Nekrose. Als Komplikationen sind sanguinolenter Aszites sowie eine aseptische Peritonitis möglich.
Die sekundäre Torsion erfolgt durch Rotation eines Netzteils zwischen seiner Basis und einem zweiten Fixpunkt. Hierfür kommen in erster Linie vorbestehende Pathologien wie Tumoren, Zysten, Entzündungen, Narbengewebe, Hernien oder eine Peritonealkarzinose in Betracht.
Die Diagnose sollte idealerweise anhand nicht-invasiver Untersuchungen und hier speziell einer Computertomografie gestellt werden. Hierbei finden sich häufig charakteristische Zeichen der Omentumtorsion. Daneben können gleichzeitig eventuell vorhandene Primärtumore oder sekundäre Raumforderungen des Omentums detektiert werden.
In vorgestelltem Fall wirkte die Raumforderung in axialer wie in koronarer Projektion initial wie ein Lipom, aufgrund des verdrängenden Wachstums wurde auch ein Liposarkom oder ein Pseudomyxoma peritonei erwogen. In sagittaler Projektion fiel dann jedoch die für eine omentale Gefäßtorquierung typische, zentral spiralförmige Struktur auf („whirl sign“).
Lernziele:
Primäre Erkrankungen des großen Netzes sind insgesamt selten, sollten jedoch bei der Differenzialdiagnose von akuten Abdominalbeschwerden oder Raumforderungen unklarer Genese in Betracht gezogen werden. Eine invasive Diagnostik kann vielen Patienten durch eine CT-Untersuchung erspart werden, wenn dem Befunder die typischen Zeichen bekannt sind.

We compare Rh and Pt as catalysts for the partial oxidation of methane to syngas at millisecond contact times. The basis for the comparison are species and temperature profiles, with a spatial ...resolution of about 300 μm measured along the centerline of an adiabatically operated metal-coated

3 foam using a capillary sampling technique with mass spectrometric species measurement. Gas temperature profiles are measured with a thermocouple. Investigated stoichiometries range from C/O
=
0.6 to 2.6 at constant flow rate of 4.7 slpm and atmospheric pressure. Rh and Pt are compared with respect to (i) profile development at syngas stoichiometry, (ii) profile development at varying stoichiometries from C/O
=
0.6–2.6, (iii) product selectivities and yields in the oxidation zone, (iv) contribution of partial oxidation and steam reforming to the final syngas yield, (v) mass transport limitations, and (vi) approach to thermodynamic equilibrium. Independent of C/O and metal, all profiles show an oxidation zone and a steam-reforming zone. H
2 and CO are formed in the presence of gas-phase oxygen by partial oxidation and in the absence of gas-phase oxygen by steam reforming. CO
2 reforming is not observed. At the same C/O, H
2 and CO selectivities and yields are higher in the oxidation zone on Rh than on Pt. As the C/O ratio increases, the catalyst temperature decreases and selectivities to H
2 and CO in the oxidation zone decrease. The decrease is larger on Pt than on Rh. Because Rh is also the better steam-reforming catalyst, H
2 and CO yields are generally higher on Rh than on Pt. The rate of O
2 conversion at the catalyst entrance is largely mass transport-controlled on Rh but not on Pt. In the oxidation zone on Pt, the methane CPO is kinetically controlled with a constant reaction rate. An average O
2 mass transport coefficient is calculated and compared with literature values on foam catalysts. Finally, exit species flow rates and temperatures are compared with thermodynamic calculations at constant pressure and enthalpy. Rh brings the methane oxidation close to equilibrium if
C/O
⩽
1.0
, whereas Pt reaches equilibrium only at very high catalyst temperatures if
C/O
⩽
0.7
. At higher C/O, deviations from equilibrium are observed mainly because steam-reforming slows, but also because water–gas shift equilibrium is not established.
Zielsetzung:
Die Kommission Uterus der Arbeitsgemeinschaft Gynäkologische Onkologie versucht, die Versorgungsrealität für das Endometriumkarzinom in Deutschland abzubilden.
Material, Methoden:
In den ...Jahren 2006, 2009, 2013 und 2015 wurde allen deutschen Frauenkliniken ein Fragebogen mit multiple Choice Fragen zugesandt. Die Ergebnisse einzelner Jahrgänge wurden mit dem Chi-Quadrat-Test verglichen.
Ergebnisse:
868 Fragebögen wurden insgesamt analysiert. Die Rücklaufquote betrug 35,8%, 33,3%, 40,0% und 24,2% für 2006, 2009, 2013 und 2015. Zunehmend häufiger wurde eine Laparoskopie für eine Hysterektomie (19,7%; 30,4%; 84,6%; 96,2%; p < 0,001), pelvine Lymphadenektomie (19,3%; 31,8%; 63,3%; 70,1%; p < 0,001) und paraaortale Lymphadenektomie (11,4%; 20,7%; 49,1%; 57,6%; p < 0,001) angeboten. Die pelvine bzw. paraaortale Lymphadenektomie wurde von 2006 bis 2015 im low-risk Stadium pT1aG2 zunehmend seltener (38,6%; 22,9%; 16,3%; 10,0%; p < 0,001 bzw. 16,0%; 10,7%; 6,3%; 4,5%; p < 0,001), im intermediate-risk Stadium pT1aG3 dagegen häufiger durchgeführt (88,0%; 85,7%; 93,9%; 98,0%; p < 0,001 bzw. 49,0%; 65,6%; 88,7%; 86,7%; p < 0,001). Seit 2009 wurde die Brachytherapie unverändert selten im low-risk Stadium FIGO IAG2 (16,0%; 13,3%; 14,0%; p = 0,666), aber zunehmend häufiger im intermediate-risk Stadium FIGO IAG3 (79,2%; 87,8%; 93,9%; p < 0,001) empfohlen. Die Teletherapie wurde unverändert im Stadium FIGO II (53,5%; 49,2%; 56,6%; p = 0,367) und im Stadium FIGO III (83,3%; 76,8%; 81,5%; p = 0,194) indiziert. Die adjuvante Chemotherapie wurde dagegen häufiger empfohlen (48,8%; 63,7%; 90,4%; 97,3; p < 0,001). Eine Kombinationstherapie mit Platin- und Taxanderivaten wurde 2015 am häufigsten angewendet.
Zusammenfassung:
Während des Beoabchtungszeitraums wurde zunehmend häufiger der minimal invasive Zugang gewählt. Die Lymphadenektomie und Bestrahlung wurden zunehmend stadien-adaptiert durchgeführt. Eine adjuvante Chemotherapie wurde häufiger empfohlen.

Over the past two decades, ultrasound (US) has become widely accepted to guide safe and accurate insertion of vascular devices in critically ill patients. We emphasize central venous catheter ...insertion, given its broad application in critically ill patients, but also review the use of US for accessing peripheral veins, arteries, the medullary canal, and vessels for institution of extracorporeal life support. To ensure procedural safety and high cannulation success rates we recommend using a systematic protocolized approach for US-guided vascular access in elective clinical situations. A standardized approach minimizes variability in clinical practice, provides a framework for education and training, facilitates implementation, and enables quality analysis. This review will address the state of US-guided vascular access, including current practice and future directions.

with the EDELWEISS-III experiment. A 2D profile likelihood analysis is ...performed on data from eight selected detectors with the lowest energy thresholds leading to a combined fiducial exposure of 496 kg-days. External backgrounds from
γ
- and
β
-radiation, recoils from
206
Pb
and neutrons as well as detector intrinsic backgrounds were modelled from data outside the region of interest and constrained in the analysis. The basic data selection and most of the background models are the same as those used in a previously published analysis based on boosted decision trees (BDT)

. For the likelihood approach applied in the analysis presented here, a larger signal efficiency and a subtraction of the expected background lead to a higher sensitivity, especially for the lowest WIMP masses probed. No statistically significant signal was found and upper limits on the spin-independent WIMP-nucleon scattering cross section can be set with a hypothesis test based on the profile likelihood test statistics. The 90 % C.L. exclusion limit set for WIMPs with

Single-nucleotide polymorphisms (SNPs) in CACNA1C, the α1C subunit of the voltage-gated L-type calcium channel Ca
1.2, rank among the most consistent and replicable genetics findings in psychiatry ...and have been associated with schizophrenia, bipolar disorder and major depression. However, genetic variants of complex diseases often only confer a marginal increase in disease risk, which is additionally influenced by the environment. Here we show that embryonic deletion of Cacna1c in forebrain glutamatergic neurons promotes the manifestation of endophenotypes related to psychiatric disorders including cognitive decline, impaired synaptic plasticity, reduced sociability, hyperactivity and increased anxiety. Additional analyses revealed that depletion of Cacna1c during embryonic development also increases the susceptibility to chronic stress, which suggest that Ca
1.2 interacts with the environment to shape disease vulnerability. Remarkably, this was not observed when Cacna1c was deleted in glutamatergic neurons during adulthood, where the later deletion even improved cognitive flexibility, strengthened synaptic plasticity and induced stress resilience. In a parallel gene × environment design in humans, we additionally demonstrate that SNPs in CACNA1C significantly interact with adverse life events to alter the risk to develop symptoms of psychiatric disorders. Overall, our results further validate Cacna1c as a cross-disorder risk gene in mice and humans, and additionally suggest a differential role for Ca
1.2 during development and adulthood in shaping cognition, sociability, emotional behavior and stress susceptibility. This may prompt the consideration for pharmacological manipulation of Ca
1.2 in neuropsychiatric disorders with developmental and/or stress-related origins.
