Using a combined approach of seafloor mapping, MAPR and CTD survey, we report evidence for active hydrothermal venting along the 130°‐140°E section of the poorly‐known South‐East Indian Ridge (SEIR) ...from the Australia‐Antarctic Discordance (AAD) to the George V Fracture Zone (FZ). Along the latter, we report Eh and CH4 anomalies in the water column above a serpentinite massif, which unambiguously testify for ultramafic‐related fluid flow. This is the first time that such circulation is observed on an intermediate‐spreading ridge. The ridge axis itself is characterized by numerous off‐axis volcanoes, suggesting a high magma supply. The water column survey indicates the presence of at least ten distinct hydrothermal plumes along the axis. The CH4:Mn ratios of the plumes vary from 0.37 to 0.65 denoting different underlying processes, from typical basalt‐hosted to ultramafic‐hosted high‐temperature hydrothermal circulation. Our data suggest that the change of mantle temperature along the SEIR not only regulates the magma supply, but also the hydrothermal activity. The distribution of hydrothermal plumes from a ridge segment to another implies secondary controls such as the presence of fractures and faults along the axis or in the axial discontinuities. We conclude from these results that hydrothermal activity along the SEIR is controlled by magmatic processes at the regional scale and by the tectonics at the segment scale, which influences the type of hydrothermal circulation and leads to various chemical compositions. Such variety may impact global biogeochemical cycles, especially in the Southern Ocean where hydrothermal venting might be the only source of nutrients.

With more than 15,000 new cases /year in France and 2,000 deaths, cutaneous melanoma represents approximately 4% of incidental cancers and 1.2% of cancer related deaths. In locally advanced (stage ...III) or resectable metastatic (stage IV) melanomas, medical adjuvant treatment is proposed and recent advances had shown the benefit of anti-PD1/PDL1 and anti-CTLA4 immunotherapy as well as anti-BRAF and anti-MEK targeted therapy in BRAF V600 mutated tumors. However, the recurence rate at one year is approximately 30% and justify extensive research of predictive biomarkers. If in metastatic disease, the follow-up of circulating tumor DNA (ctDNA) has been demonstrated, its interest in adjuvant setting remains to be precised, especially because of a lower detection rate. Further, the definition of a molecular response could prove useful to personalized treatment.
PERCIMEL is an open prospective multicentric study executed through collaboration of the Institut de Cancérologie de Lorraine (non-profit comprehensive cancer center) and 6 French university and community hospitals. A total of 165 patients with resected stage III and IV melanoma, eligible to adjuvant imunotherapy or anti-BRAF/MEK kinase inhibitors will be included. The primary endpoint is the presence of ctDNA, 2 to 3 weeks after surgery, defined as mutated ctDNA copy number calculated as the allelic fraction of a clonal mutation relative to total ctDNA. Secondary endpoints are recurrence-free survival, distant metastasis-free survival and specific survival. We will follow ctDNA along treatment, quantitatively through ctDNA mutated copy number variation, qualitatively through the presence of cfDNA and its clonal evolution. Relative and absolute variations of ctDNA during follow-up will be also analyzed. PERCIMEL study aims at provide scientific evidence that ctDNA quantitative and qualitative variations can be used to predict the recurrence of patients with melanoma treated with adjuvant immunotherapy or kinase inhibitors, thus defining the notion of molecular recurrence.

Summary Background A transverse musculocutaneous gracilis flap provides good autologous reconstruction for small- and medium-sized breasts. Although the procedure is well adapted for bilateral breast ...reconstruction, no publication has specifically addressed simultaneous bilateral cases. Methods From 2010 to 2014, the authors performed seven simultaneous bilateral breast reconstructions using transverse musculocutaneous gracilis flaps. The results with respect to operative data, immediate complications, second-stage reconstruction, and patient satisfaction after >1 year of follow-up were studied retrospectively. Results The mean operative time was 7 h and 48 min (range, 6–9 h). Three minor complications occurred: two cases of limited flap necrosis and one case of donor-site wound dehiscence. Surgical revision was not required, and there was no flap failure. A second-stage operation was performed in 71% of the patients to improve the aesthetic results and flap volume. On average, 167 cm3 of fat was injected per breast. After a mean follow-up of 27 months, the satisfaction rate was 86% without significant functional deficits. Conclusions A transverse musculocutaneous gracilis flap is an effective and safe option for simultaneous bilateral reconstruction. The operating time is shorter than that for other autologous procedures with similar complication rates and high patient satisfaction levels.
