There is currently no consensus on the combined length of small bowel that should be bypassed as biliopancreatic or alimentary limb for optimum results with Roux-en-Y gastric bypass. A number of ...different limb lengths exist, and there is significant variation in practice amongst surgeons. Inevitably, this means that some patients have too much small bowel bypassed and end up with malnutrition and others end up with a less effective operation. Lack of standardisation poses further problems with interpretation and comparison of scientific literature. This systematic review concludes that a range of 100–200 cm for combined length of biliopancreatic or alimentary limb gives optimum results with Roux-en-Y gastric bypass in most patients.
The interaction of intense particle bunches with plasma can give rise to plasma wakes
capable of sustaining gigavolt-per-metre electric fields

Plasma wakefields can, therefore, strongly accelerate charged particles and offer the opportunity to reach higher particle energies with smaller and hence more widely available accelerator facilities. However, the luminosity and brilliance demands of high-energy physics and photon science require particle bunches to be accelerated at repetition rates of thousands or even millions per second, which are orders of magnitude higher than demonstrated with plasma-wakefield technology
. Here we investigate the upper limit on repetition rates of beam-driven plasma accelerators by measuring the time it takes for the plasma to recover to its initial state after perturbation by a wakefield. The many-nanosecond-level recovery time measured establishes the in-principle attainability of megahertz rates of acceleration in plasmas. The experimental signatures of the perturbation are well described by simulations of a temporally evolving parabolic ion channel, transferring energy from the collapsing wake to the surrounding media. This result establishes that plasma-wakefield modules could be developed as feasible high-repetition-rate energy boosters at current and future particle-physics and photon-science facilities.
Geological investigations planned for the Europa Clipper mission will examine the formation, evolution, and expression of geomorphic structures found on the surface. Understanding geologic features, ...their formation, and any recent activity are key inputs in constraining Europa’s potential for habitability. In addition to providing information about the moon’s habitability, the geologic study of Europa is compelling in and of itself. Here we provide a high-level, cross-instrument, and cross-discipline overview of the geologic investigations planned within the Europa Clipper mission. Europa’s fascinating collection of ice-focused geology provides an unparalleled opportunity to investigate the dynamics of icy shells, ice-ocean exchange processes, and global-scale tectonic and tidal stresses. We present an overview of what is currently known about the geology of Europa, from global to local scales, highlighting outstanding issues and open questions, and detailing how the Europa Clipper mission will address them. We describe the mission’s strategy for searching for and characterizing current activity in the form of possible active plumes, thermal anomalies, evidence for surface changes, and extremely fresh surface exposures. The complementary and synergistic nature of the data sets from the various instruments and their integration will be key to significantly advancing our understanding of Europa’s geology.
The electromagnetic polarizabilities of the nucleon are fundamental properties that describe its response to external electric and magnetic fields. They can be extracted from Compton-scattering ...data-and have been, with good accuracy, in the case of the proton. In contradistinction, information for the neutron requires the use of Compton scattering from nuclear targets. Here, we report a new measurement of elastic photon scattering from deuterium using quasimonoenergetic tagged photons at the MAX IV Laboratory in Lund, Sweden. These first new data in more than a decade effectively double the world data set. Their energy range overlaps with previous experiments and extends it by 20 MeV to higher energies. An analysis using chiral effective field theory with dynamical Δ(1232) degrees of freedom shows the data are consistent with and within the world data set. After demonstrating that the fit is consistent with the Baldin sum rule, extracting values for the isoscalar nucleon polarizabilities, and combining them with a recent result for the proton, we obtain the neutron polarizabilities as αn=11.55±1.25(stat)±0.2(BSR)±0.8(th)×10(-4) fm(3) and βn=3.65∓1.25(stat)±0.2(BSR)∓0.8(th)×10(-4) fm(3), with χ(2)=45.2 for 44 degrees of freedom.
We report the observation of a steepening in the cosmic ray energy spectrum of heavy primary particles at about 8×10(16) eV. This structure is also seen in the all-particle energy spectrum, but is ...less significant. Whereas the "knee" of the cosmic ray spectrum at 3-5×10(15) eV was assigned to light primary masses by the KASCADE experiment, the new structure found by the KASCADE-Grande experiment is caused by heavy primaries. The result is obtained by independent measurements of the charged particle and muon components of the secondary particles of extensive air showers in the primary energy range of 10(16) to 10(18) eV. The data are analyzed on a single-event basis taking into account also the correlation of the two observables.
Abstract
We collect observational evidence that supports the scheme of mass transfer on the nucleus of comet 67P/Churyumov–Gerasimenko. The obliquity of the rotation axis of 67P causes strong ...seasonal variations. During perihelion the southern hemisphere is four times more active than the north. Northern territories are widely covered by granular material that indicates back fall originating from the active south. Decimetre sized chunks contain water ice and their trajectories are influenced by an antisolar force instigated by sublimation. OSIRIS observations suggest that up to 20 per cent of the particles directly return to the nucleus surface taking several hours of traveltime. The back fall covered northern areas are active if illuminated but produce mainly water vapour. The decimetre chunks from the nucleus surface are too small to contain more volatile compounds such as CO2 or CO. This causes a north–south dichotomy of the composition measurements in the coma. Active particles are trapped in the gravitational minimum of Hapi during northern winter. They are ‘shock frozen’ and only re-activated when the comet approaches the sun after its aphelion passage. The insolation of the big cavity is enhanced by self-heating, i.e. reflection and IR radiation from the walls. This, together with the pristinity of the active back fall, explains the early observed activity of the Hapi region. Sobek may be a role model for the consolidated bottom of Hapi. Mass transfer in the case of 67P strongly influences the evolution of the nucleus and the interpretation of coma measurements.
Background and purpose
To support clinical decision‐making in central neurological disorders, a physical examination is used to assess responses to passive muscle stretch. However, what exactly is ...being assessed is expressed and interpreted in different ways. A clear diagnostic framework is lacking. Therefore, the aim was to arrive at unambiguous terminology about the concepts and measurement around pathophysiological neuromuscular response to passive muscle stretch.
Methods
During two consensus meetings, 37 experts from 12 European countries filled online questionnaires based on a Delphi approach, followed by plenary discussion after rounds. Consensus was reached for agreement ≥75%.
Results
The term hyper‐resistance should be used to describe the phenomenon of impaired neuromuscular response during passive stretch, instead of for example ‘spasticity’ or ‘hypertonia’. From there, it is essential to distinguish non‐neural (tissue‐related) from neural (central nervous system related) contributions to hyper‐resistance. Tissue contributions are elasticity, viscosity and muscle shortening. Neural contributions are velocity dependent stretch hyperreflexia and non‐velocity dependent involuntary background activation. The term ‘spasticity’ should only be used next to stretch hyperreflexia, and ‘stiffness’ next to passive tissue contributions. When joint angle, moment and electromyography are recorded, components of hyper‐resistance within the framework can be quantitatively assessed.
Conclusions
A conceptual framework of pathophysiological responses to passive muscle stretch is defined. This framework can be used in clinical assessment of hyper‐resistance and will improve communication between clinicians. Components within the framework are defined by objective parameters from instrumented assessment. These parameters need experimental validation in order to develop treatment algorithms based on the aetiology of the clinical phenomena.
KASCADE and KASCADE-Grande were multi-detector installations to measure individual air showers of cosmic rays at ultra-high energy. Based on data sets measured by KASCADE and KASCADE-Grande, 90% C.L. ...upper limits to the flux of gamma-rays in the primary cosmic ray flux are determined in an energy range of eV. The analysis is performed by selecting air showers with a low muon content as expected for gamma-ray-induced showers compared to air showers induced by energetic nuclei. The best upper limit of the fraction of gamma-rays to the total cosmic ray flux is obtained at eV with . Translated to an absolute gamma-ray flux this sets constraints on some fundamental astrophysical models, such as the distance of sources for at least one of the IceCube neutrino excess models.
The novel centrally acting analgesic tapentadol (-)-(1R,2R)-3-(3-dimethylamino-1-ethyl-2-methyl-propyl)-phenol hydrochloride combines two mechanisms of action, μ-opioid receptor (MOR) agonism and ...noradrenaline reuptake inhibition (NRI), in a single molecule. Pharmacological antagonism studies have demonstrated that both mechanisms of action contribute to the analgesic effects of tapentadol. This study was designed to investigate the nature of the interaction of the two mechanisms. Dose-response curves were generated in rats for tapentadol alone or in combination with the opioid antagonist naloxone or the α(2)-adrenoceptor antagonist yohimbine. Two different pain models were used: 1) low-intensity tail-flick and 2) spinal nerve ligation. In each model, we obtained dose-effect relations to reveal the effect of tapentadol based on MOR agonism, NRI, and unblocked tapentadol. Receptor fractional occupation was determined from tapentadol's brain concentration and its dissociation constant for each binding site. Tapentadol produced dose-dependent analgesic effects in both pain models, and its dose-effect curves were shifted to the right by both antagonists, thereby providing data to distinguish between MOR agonism and NRI. Both isobolographic analysis of occupation-effect data and a theoretically equivalent methodology determining interactions from the effect scale demonstrated very pronounced synergistic interaction between the two mechanisms of action of tapentadol. This may explain why tapentadol is only 2- to 3-fold less potent than morphine across a variety of preclinical pain models despite its 50-fold lower affinity for the MOR. This is probably the first demonstration of a synergistic interaction between the occupied receptors for a single compound with two mechanisms of action.

), where the data of the astroparticle physics experiment KASCADE-Grande are made available for the interested public. ...The KASCADE experiment was a large-area detector for the measurement of high-energy cosmic rays via the detection of extensive air showers. The multi-detector installations KASCADE and its extension KASCADE-Grande stopped the active data acquisition in 2013 after more than 20 years of data taking. In several updates since our first release in 2013 with KCDC we provide the public measured and reconstructed parameters of more than 433 million air showers. In addition, KCDC provides meta data information and documentation to enable a user outside the community of experts to perform their own data analysis. Simulation data from three different high energy interaction models have been made available as well as a compilation of measured and published spectra from various experiments. In addition, detailed educational examples shall encourage high-school students and early stage researchers to learn about astroparticle physics, cosmic radiation as well as the handling of Big Data and about the sustainable and public provision of scientific data.
