The authors review the literature concerning the possibilities of modifying the mechanical characteristics of the respiratory system with breathing retraining and other chest physiotherapy ...intervention. Breathing retraining techniques with prolonged and active expiration induce increased work of breathing and do not help diaphragmatic function. Only pursed lips breathing seems to produce some mechanical advantages. The seated and forward position with fixed arms may reduce dyspnea but this does not appear to be caused by a decrease in operating lung volumes. By contrast, the dorsal decubitus position may reduce hyperinflation, giving some mechanical advantage to the diaphragm but does not reduce dyspnea and is not a position spontaneously adopted by patients. General muscular training may reduce ventilatory demand, and by reducing respiratory frequency lead to a decrease in the end expiratory lung volume; therefore inducing improvement in exercise tolerance.
Con o senza strumenti, autonome o guidate, le tecniche fisioterapiche sono affrontate attraverso lo studio delle loro basi biomeccaniche. Ciò permette al terapista di comprenderne la pertinenza ...rispetto a un dato obiettivo di fisioterapia, di adattarle ad ogni situazione clinica, ma anche di comprenderne i limiti. Le tecniche sono raggruppate e presentate in funzione dell’obiettivo terapeutico a loro assegnato: migliorare la ventilazione polmonare, migliorare la forma fisica e la qualità di vita, ridurre la dispnea da sforzo e migliorare il disingombro bronchiale. La loro indicazione è solo raramente menzionata in relazione a una patologia. Viceversa, i loro vantaggi e svantaggi sono soppesati per arricchire la riflessione sul rapporto rischio/beneficio, qualunque sia il contesto, dall’acuto al cronico, dalla rianimazione al domicilio.
La fisioterapia respiratoria, raramente curativa di un deficit o di una disfunzione, trova più spesso il suo posto nella prevenzione o nel trattamento delle complicanze di una lesione respiratoria ...acuta o cronica. Palliativa, essa partecipa anche all’educazione dei pazienti verso la gestione di una malattia respiratoria cronica. Trova le sue indicazioni a tutte le età della vita (dal lattante all’anziano), in un contesto acuto o stabile, dalla rianimazione alla riabilitazione in una struttura di cure, in ambulatorio o in ambito privato. Essa si rivolge a un paziente che presenta un handicap temporaneo o definitivo imputabile a una disfunzione che altera le capacità di adattamento allo sforzo, qualunque ne sia l’origine, capacità dove se intrecciano apparato cardiovascolare, polmoni, muscoli, sistemi di comando e così via. I suoi principali campi d’azione sono: il trattamento dell’insufficienza respiratoria acuta o cronica, il trattamento di una disfunzione acuta della meccanica ventilatoria esterna, il trattamento di un’ostruzione e delle sue conseguenze sulla meccanica ventilatoria esterna, ma anche l’apprendimento della padronanza del respiro nella gestione di una malattia ostruttiva cronica, il trattamento di un ingombro acuto o la gestione dell’ingombro cronico nel caso di una malattia ipersecernente, la riabilitazione allo sforzo, la riduzione della dispnea e il miglioramento della qualità di vita. Nella maggior parte di questi obiettivi, essa integra una dimensione di educazione terapeutica. L’iter intellettuale fondamentale che presiede all’esercizio della fisioterapia respiratoria è basato sull’analisi fisiopatologica dei meccanismi che portano alla disfunzione o alla disabilità. Partendo da una diagnosi medica e da una prescrizione di rieducazione, l’iter diagnostico ha l’obiettivo di formulare una rappresentazione della condizione clinica e della sua probabile evolutività, ma anche di valutare le risorse della fisioterapia nel modificarne la prognosi. Esso porta a porre una diagnosi fisioterapica da cui derivano progetto terapeutico, obiettivi di cure e scelte tecnologiche.
Dans le cadre de la prise en charge de patients porteurs d'un trouble ventilatoire obstructif, l'objectif de la kinésithérapie respiratoire est, à court et moyen termes, d'améliorer la qualité de vie ...et, à long terme, de participer à freiner l'aggravation d'une pathologie chronique. Pour ceux qui présentent un encombrement chronique, briser le cercle vicieux : encombrement, infection, hospitalisation puis retour à domicile avec un état respiratoire et fonctionnel aggravé. L'éducation prend une place prépondérante, seule ou incluse dans des programmes de réhabilitation. Des gains non négligeables en termes de morbidité voire de mortalité mais aussi en termes de coût de santé publique peuvent être envisagés. Les résultats des programmes de réhabilitation respiratoire destinés aux patients atteints de bronchopneumopathie chronique obstructive montrent aussi un gain non négligeable en termes de coût de santé publique. Ces programmes centrés sur le réentraînement à l'exercice, comprennent systématiquement, une dimension d'éducation et de prévention qui fait une large part à l'« hygiène bronchique ».
The goal of respiratory kinesiotherapy in patients with an obstructive ventilatory disorder is to improve their quality of life in the short and medium term; for the long term, the objective is to prevent worsening of a chronic disease, particularly, in patients with chronic obstruction, to break the vicious circle: obstruction, infection, hospitalisation, and discharge back home with an aggravated respiratory and functional status. Education is of utmost importance, in an isolated manner or as part of those rehabilitation programs aimed at patients with chronic obstructive pulmonary diseases. The results of such programs show that significant benefits may be expected in terms of morbidity - and even mortality - and also in terms of public health costs. These programs that focus on exercise re-training systematically include also an education and prevention part devoted to bronchial hygiene.

En quelques années, le monde de la santé et celui de la rééducation ont complètement changé de par la technologie et les rapports sociaux. L'évaluation est devenue un point central de la politique de ...santé et une préoccupation quotidienne des professionnels concernés pour développer la qualité des soins. Le concept d'évaluation a, lui aussi, évolué pour couvrir un champ bien distinct de la recherche qui nécessite d'être développé en tant que tel. Les résultats de recherche participent à l'établissement de référentiels partagés. Dans le champ de la rééducation où la relation humaine est présente tout au long du traitement, l'évaluation prend en compte, à travers des critères, la subjectivité et l'objectivité au sein des projets du patient, des thérapeutes et des institutions. La volonté politique de développement de l'évaluation en santé qui se décline du côté qualitatif avec l'Agence nationale d'accréditation et d'évaluation en santé (ANAES) et du côté quantitatif avec le Programme de médicalisation du système d'information (PMSI) ainsi que par l'intermédiaire de la Sécurité sociale, est relayée dans les pratiques d'évaluation par les acteurs que sont les patients et les praticiens. L'amélioration de la qualité des soins passe par cette exigence d'une évaluation « multivoies » sans cesse renouvelée qui repose pour une grande part sur les compétences des praticiens dans une situation clinique singulière.
Within few years, health and rehabilitation have completely changed in relation with changes in technologies and social relationships. Evaluation has become an essential point in terms of health policy, and a daily concern for those professionals responsible for developing the quality of care. The concept of evaluation has evolved also, and has a specific role separate from the research field, which should be more developed. Research results are used to establish shared referentials. Human relationships are present throughout the rehabilitation therapy; therefore, evaluation is based on criteria that take into account both subjective and objective aspects of the patient's, therapists', and institution's projects. The political will of developing evaluation within the health system has resulted, in qualitative terms, in the creation of the ANAES (Agence nationale d'accréditation et d'évaluation en santé), a national agency for health evaluation, and in quantitative terms in the creation of the PMSI (Programme de médicalisation du système d'information) a program for the development of a medical information system; social services as well as patients and practitioners participate in evaluation practices. Improvement of the care quality depends on such multi-way evaluation which in turn depends on the practitioner competence in dealing with any singular clinical situation.
