Two cases, case 1, 70 and case 2, 31 years old male, of nonhemolitic unconjugated hyperbilirubinemia associated with remarkable retention of both ICG and BSP excretion are presented. Both cases had ...had hepatitis about 20 years ago. Laboratory examinations including transaminase and alkalin phosphotase were normal except for high serum unconjugated bilirubin (about 2mg/dl), remarkable ICG retention (R15 about 80%) and BSP retention (R15 about 18%) in both cases. Case 1 had slightly shortened red cell turnover rate (51Cr-RBC T1/2 22.5 days) and chronic inactive hepatitis in his liver biopsy specimen. Case 2 had normal red cell turnover rate and normal liver histology. The analysis of ICG curve and BSP conjugates suggested abnormalities somewhere from dye uptake to transfer mechanism, and slightly disturbed excretion. Although the influences of hepatitis can be completely excluded, these cases seem to have new type of constitutional hyperbilirubinemia.
The growth of both cancer cells and fetal tissue is rapid; however, cancer cells de-differentiate and proliferate in a disorderly manner, whereas fetal tissues differentiate and proliferate in an ...orderly manner. Thus, there may be both common and different factors that are involved in the process of the uncontrolled cell growth of pancreatic cancers and the development of the fetal pancreas. The common part of the mechanisms should be in the regulation of the cell cycle, resulting in rapid proliferation via such mechanisms as growth stimulation and avoidance of apoptosis. Therefore, in the current study we investigated the expression of apoptosis-related proteins in fetal pancreatic tissues.
Sixteen human embryonic and fetal pancreatic tissues obtained between 6 and 32 wk of gestation were used. We immunohistochemically examined the protein expression of Bcl-2, Bcl-XL, Mcl-1, and Bax. Further, the expression of insulin, glucagon, and proliferting cell nuclear antigen (PCNA), and TdT-mediated dUTP-biotin nick-end labeling (TUNEL) staining were examined.
In embryonic and fetal pancreatic tissues, Bcl-2 was not detected in any type of pancreatic cell (acinar, ductal, or islet). Bcl-XL was expressed in all types of pancreatic cells throughout the gestation. Mcl-1 was expressed in all types of pancreatic components, and strongly expressed in the margin of the islets. Bax, a pro-apoptotic protein, was expressed in all components. PCNA was strongly expressed in the embryonic and fetal pancreas, especially in early stages of gestation; however, TUNEL staining was negative in all samples. At least one antiapoptotic protein was expressed in all types of pancreatic cells.
The results of the current study indicate that active proliferation and avoidance of apoptosis take place in embryonic and fetal pancreatic tissues, which may be controlled by particular combinations of apoptosis-related proteins. Among these proteins, Bcl-XL and Mcl-1 may play an important role in the proliferation and differentiation of the embryonic and fetal pancreas.
IgG from patients with rheumatoid arthritis possess significantly fewer galactose residues in its sugar chains. We made an attempt to analyze the distribution of a galactosylation defect among IgG ...subclasses by immunoassay using PVL, a recently described lectin specific for N-acetylglucosamine (GlcNAc). A distinct difference between RA and normal controls was present in IgG2 (P = 0.0007). A less remarkable but significant difference was seen in IgG1 (P = 0.0118) and in IgG4 (P = 0.022). A significant difference was not observed in IgG3 (P = 0.169). The possible relationship between such a glycosylation abnormality and RF production was discussed.
1. The Wanibuchi “Kuroko” Deposit is located in the western region of Shimane Peninsula. 2. In the western region acidic volcanic activity and folding movement had been active during the Neogene ...period. 3. The western district of the Shimane Peninsula is characterized by the geologic structure, that formed by Taisha Anticline and Kawashimo syncline which are slow plunging (±15°) to east. 4. In the Wanibuchi Mine area, four stratigraphical horizon of mineralization has been recognized. And workable “Kuroko” Deposits are discovered in two horizons. One of these is Wanibuchi horizon, the other is Otoshi-Kanayama horizon. 5. Ore deposits are situated in boundary area between black shale and acidic green pyroclastics. And mainly they consist of gypsum ore and small amount of black ore. 6. The Gypsum Deposits are accompanied with “Kuroko” bodies which are small amount in volume and on hanging wall of gypsum bodies. 7. The sequence of ore arrangement from lower to upper is as follows. Silicified body (partly siliceous ore) -Gypsum ore-Yellow ore-Black ore-Barite-Siliceous rock.
For the purpose of comparison of the therapeutic effects and side effects between Apalcillin (APPC) and Carbenicillin (CBPC), a comparative clinical study was carried out in 142 patients with chronic ...respiratory tract infections at 31 institutions in Japan. Either of APPC or CBPC was assigned to each patient at random. Administration was performed by intravenous drip infusions for a fixed period of 14 days and the daily dosage was fixed at 2g (APPC) or 4 g (CBPC). A committee consisting of several physicians who had not been informed of the name of actually given drug, made a judgement on the severity, therapeutic results and presence or absence of side effects in each patient based on the detailed subjective and objective symptoms, laboratory findings and chest X-ray films. Subsequently, the key code for the drug administered to each patient was opened and a statistical analysis was carried out by a comparison between two groups (APPC group and CBPC group) with respect to background factors, clinical effectiveness, bacteriological effectiveness, degree of improvement and side effects. Out of 142 cases originally admitted to the trial, 16 cases were excluded bacause of failure to meet the initially established protocol and 126 cases (67 from APPC group and 59 from CBPC group) were used for the analysis of effectiveness while the analysis of side effects was done in 141 cases (75 from APPC group and 66 from CBPC group). It was indicated that there was no significant difference between the two groups regarding background factors except body weight, PaCO2 and ESR. Clinical effects of APPC were good in 40 cases, fair in 7 cases, poor in 19 cases and unknown in 1 case and those of CBPC were excellent in 3 cases, good in 32 cases, fair in 15 cases, poor in 7 cases and unknown in 2 cases. There was no statistically significant difference between the both groups concerning clinical effectiveness, bacteriological effectiveness and the degree of improvement. As to the incidence of side effects, no statistically significant difference was observed between the both groups.
In order to compare the therapeutic effects and side effects of fosfomycin Na (FOM) with those of sulbenicillin Na (SBPC), a comparative clincal trial has been carried out in 115 patients with ...chronic respiratory tract infections at 25 institutions in Japan. Each of the patients was assigned to either of the drugs at random. Both drugs were administered as intravenous infusions at a dosage of 2g twice daily for 14 days. In each patient, on the basis of detailedly recorded subjective and objective symptoms, laboratory test results and chest X-ray film findings, committee members consisting of several physicians who had not been informed of the drug actually given to each patient made an assessment on the severity, therapeutic results, and presence or absence of side effects. Subsequently, the key code for the drug abministered to each patient was opened and statistical analysis was carried out by making a comparison between the two groups (one received FOM and the other SBPC) with respect to background factors, clinical effectiveness, bacteriological effectiveness, degree of improvement and observed side effects. Out of the 115 cases originally admitted to the trial, 13 cases were excluded because of failure to observe the treating regimen initially established, leaving 102 cases (56 from FOM group and 46 from SBPC group) for analysis. It was indicated that regarding background factors there was no significance between the two groups except that more cases of SBPC group had highly accelerated ESR value and that there were more cases in FOM group which had infections due to Pseudomonas and mixed infections, both with significance. The clinical effectiveness were classified as excellent in 5 and 7 cases in FOM and SBPC group, respectively, as good in 22 and 17 cases ; as fair in 8 and 11 cases, as poor in 21 and 14 cases ; and as undecided in 0 and 1 case, respectively, with no significance. In the bacteriological effectiveness and the degree of improvement, there was no significance which is of importance in a comparison of both drugs. In regard to the incidence of side effects no significance was observed.
