Background
Type II distal phalanx (P3) fractures are a well‐described cause of lameness in horses. Reports on outcome following internal fixation of type II P3 fractures are lacking, and with little ...emphasis on complications.
Objective
To describe a technique for internal fixation of type II P3 fractures, and evaluate whether specific variables influenced post‐operative complications or a horse's ability to return to work.
Study design
Retrospective case series.
Methods
Medical records of 51 horses with CT‐guided internal fixation of type II P3 fractures were reviewed. Outcome data were acquired from race records and telephone interviews. Associations between independent variables and outcome were analysed using multivariable logistic regression.
Results
Eighty‐six per cent (95% CI 74%‐94%; n = 44) successfully returned to work. Implant infection (n = 15) and distal interphalangeal joint osteoarthritis (n = 9) were the most common complications, with the latter reducing the likelihood of success (OR = 0.09, 95% CI 0.01‐0.7, P = .02). Implant infection increased the time to return to work (HR = 0.5, 95% CI 0.2‐0.9, P = .03). The odds of delayed infection decreased by filling the hoof defect with acrylic hoof adhesive rather than poly(methyl methacrylate) and deeply countersinking the screw head (OR = 0.08, 95% CI 0.02‐0.38, P = .001); the individual effect of each treatment is unknown. Radiographic healing was not associated with likelihood of success.
Main limitations
Study limitations included variation in follow‐up methods, lack of control population for comparison and lack of randomisation of treatment protocols.
Conclusions
Internal fixation of type II P3 fractures is an effective treatment that allows horses to return to athletic use, with similar improved success rates as those reported for conservative management. Infection rates were reduced by deeply countersinking the screw head and filling the hoof defect with an acrylic that mimics hoof wall flexibility and provides a secure seal. Recommencement of training should be based on clinical rather than strictly radiographic findings.
BACKGROUNDProspective clinical study of blood lactate concentration in horses undergoing colic surgery is needed to determine utility in outcome prediction. OBJECTIVESTo evaluate venous lactate ...measurements in horses following colic surgery, including immediately after anesthetic recovery and daily throughout hospitalization, as well as to determine if lactate concentrations were significantly higher in horses that developed postoperative complications or did not survive to hospital discharge. METHODSHorses > 1 year of age undergoing surgery for colic and recovered from general anesthesia were sampled. A portable lactate meter was used to measure venous samples collected immediately following anesthetic recovery and daily throughout hospitalization. Complications arising during hospitalization and survival to hospital discharge were recorded. RESULTSFifty one horses were enrolled, ranging in age from 2 to 29 years. Lactate concentration immediately following anesthetic recovery was higher in horses that developed complications during hospitalization (p = 0.046). The odds of developing complications postoperatively were doubled for horses with a venous lactate concentration > 5 mmol/L. Lactate measurements in non-survivors were significantly higher compared to survivors by 96 h postoperatively (p < 0.006). CONCLUSIONSHigher venous lactate concentrations in the postoperative colic period were associated with an increased risk of complications and death. Results suggest horses with higher venous lactate measurements in recovery are more likely to have postoperative complications, with the odds of developing complications doubled for horses with a venous lactate > 5 mmol/L. Evaluation of venous lactate could provide information on prognosis in the postoperative period for horses with surgical colic.

A client-owned 12-year-old Appaloosa mare.
The mare had a wound to the lateral aspect of the fourth metatarsal bone (MT4) that communicated with the distal tarsal joints. Radiographs revealed a displaced, comminuted fracture of MT4.
The horse underwent aggressive debridement of the wound and MT4 as well as, on 2 occasions, needle joint lavage. Systemic, regional, and IA antibiotic therapy was also performed together with a bone graft from the tuber coxae. The horse's comfort improved, and the wound appeared to be healing. Five weeks following discharge, the horse re-presented with a non-weight-bearing lameness and radiographs revealed marked osteomyelitis of the tarsometatarsal and distal intertarsal joints. Postmortem examination of the limb identified a sequestrum within the proximal articular surface of the third metatarsal bone.
The present report highlights the importance of arthroscopic lavage to visualize the cartilage surface and the benefits of advanced imaging to detect associated changes within the bone earlier than conventional radiographs. To our knowledge, no reports exist of intra-articular subchondral bone sequestra in the tarsometatarsal joint in horses.
Objective
The aim of this study was to assess screw placement in simulated dorsomedial‐plantarolateral central tarsal bone (CTB) fractures using two imaging guidance techniques – computed tomography ...(CT) with fluoroscopy compared to digital radiography alone (DR).
Study design
Experimental study.
Sample population
Equine cadaver hindlimbs (n = 10 pairs).
Methods
One tarsus per pair was randomly assigned to have a 4.5 mm cortical screw placed across the CTB using CT and fluoroscopy (CT/F group) or digital radiography alone (DR group). Postoperative CT was performed on all limbs. Variables related to marker placement, procedure time, and screw positioning were recorded and compared using a paired t‐test for dependent means (p < .05).
Results
Time for marker placement was longer for the CT/F group (p = .001), with no difference in total procedure time (p = .12). CT/F was not superior to radiography alone (p > .05) for parameters related to screw positioning. Based on the 95% CI, there was greater range in relative screw length using radiography (76.5%–91.2%) versus CT/F (78.4%–84.0%).
Conclusion
Internal fixation of CTB fractures can be successfully performed using either technique for imaging guidance. CT and fluoroscopy did not result in faster or more accurate screw placement compared to radiographs alone, except in determining screw length.
Clinical significance
Mild adjustments in fluoroscopic or radiographic angle appeared to be a point of variability in the perception of screw placement. While CT is recommended for improved understanding of fracture configuration and surgical planning, radiographic guidance may be a suitable alternative for internal fixation of dorsomedial‐plantarolateral fractures.

3 adult horses.
Two 15-mm-diameter full-thickness cartilage defects were created on the medial trochlear ridge of each femur. Defects were treated with microfracture and then filled by 1 of 4 techniques: (1) autologous fibrin graft (FG) via subchondral injection of fibrin glue (FG), (2) autologous fibrin graft via direct injection of FG, (3) subchondral injection of a calcium phosphate bone substitute material (BSM) with direct injection of FG, and (4) untreated control. Horses were euthanized after 2 weeks. Patient response was evaluated via serial lameness examination, radiography, magnetic resonance imaging, computed tomography, gross evaluation, microcomputed tomography, and histopathology.
All treatments were successfully administered. The injected material perfused through the underlying bone into the respective defects without adversely affecting the surrounding bone and articular cartilage. Increased new bone formation was seen at the margins of the trabecular spaces containing BSM. There was no treatment effect on the amount or composition of tissue within defects.
The mSCP technique was a simple, well-tolerated technique in this equine articular cartilage defect model without significant adverse effects to host tissues after 2 weeks. Larger studies with long-term follow-ups are warranted.
Objective
To report the radiographic, surgical and postoperative features in horses with unstable oblique mandibular fractures secured with polyaxial pedicle screws (PPS) external fixation construct ...and intraoral wiring.

Three client‐owned horses.

Short case series.
Methods
Two horses each had a unilateral fracture, which did not improve after conservative management, and one horse had bilateral fractures. Clinical and radiographic features were documented. Polyaxial pedicle screw external fixators and intraoral tension band wiring were applied in standing horses after combining sedation and regional nerve anesthesia. Intraoral wires were implanted through a lateral buccotomy between teeth (two horses) or burred through exposed crown (one horse) and then secured around the incisors. The PPS were inserted under radiographic guidance to avoid tooth roots. Healing was assessed with radiographic examination. The PPS external fixator rod and intraoral wires were removed first. The mandible was manipulated, and, if it was stable, the PPS were removed.
Results
Implants were removed at 6, 8, or 10 weeks after the mandibles were palpably stable. Complications included broken wires in one horse, bone sequestration in one horse, and infection in one horse. Follow‐up communication with the owners 12 to 18 months later confirmed complete healing without further complications of the fractured mandibles or teeth.
Conclusion
Polyaxial pedicle screw external fixation led to fracture healing and a return to function in all three horses. The complications encountered did not preclude a successful outcome.
Clinical significance
Polyaxial pedicle screw external fixation coupled with intraoral wiring provides an alternative to treat unstable equine mandibular fractures without general anesthesia.
