Pneumothorax and pneumomediastinum have both been noted to complicate cases of coronavirus disease 2019 (COVID-19) requiring hospital admission. We report the largest case series yet described of ...patients with both these pathologies (including nonventilated patients).
Cases were collected retrospectively from UK hospitals with inclusion criteria limited to a diagnosis of COVID-19 and the presence of either pneumothorax or pneumomediastinum. Patients included in the study presented between March and June 2020. Details obtained from the medical record included demographics, radiology, laboratory investigations, clinical management and survival.
71 patients from 16 centres were included in the study, of whom 60 had pneumothoraces (six with pneumomediastinum in addition) and 11 had pneumomediastinum alone. Two of these patients had two distinct episodes of pneumothorax, occurring bilaterally in sequential fashion, bringing the total number of pneumothoraces included to 62. Clinical scenarios included patients who had presented to hospital with pneumothorax, patients who had developed pneumothorax or pneumomediastinum during their inpatient admission with COVID-19 and patients who developed their complication while intubated and ventilated, either with or without concurrent extracorporeal membrane oxygenation. Survival at 28 days was not significantly different following pneumothorax (63.1±6.5%) or isolated pneumomediastinum (53.0±18.7%; p=0.854). The incidence of pneumothorax was higher in males. 28-day survival was not different between the sexes (males 62.5±7.7%
females 68.4±10.7%; p=0.619). Patients aged ≥70 years had a significantly lower 28-day survival than younger individuals (≥70 years 41.7±13.5% survival
<70 years 70.9±6.8% survival; p=0.018 log-rank).
These cases suggest that pneumothorax is a complication of COVID-19. Pneumothorax does not seem to be an independent marker of poor prognosis and we encourage continuation of active treatment where clinically possible.
Cover crops provide a variety of important agroecological services within cropping systems. Typically these crops are grown as monocultures or simple graminoid-legume bicultures; however, ecological ...theory and empirical evidence suggest that agroecosystem services could be enhanced by growing cover crops in species-rich mixtures. We examined cover crop productivity, weed suppression, stability, and carryover effects to a subsequent cash crop in an experiment involving a five-species annual cover crop mixture and the component species grown as monocultures in SE New Hampshire, USA in 2011 and 2012. The mean land equivalent ratio (LER) for the mixture exceeded 1.0 in both years, indicating that the mixture over-yielded relative to the monocultures. Despite the apparent over-yielding in the mixture, we observed no enhancement in weed suppression, biomass stability, or productivity of a subsequent oat (Avena sativa L.) cash crop when compared to the best monoculture component crop. These data are some of the first to include application of the LER to an analysis of a cover crop mixture and contribute to the growing literature on the agroecological effects of cover crop diversity in cropping systems.

Occupational sedentary behaviour is an important contributor to overall sedentary risk. There is limited evidence for effective workplace interventions to reduce occupational sedentary time and ...increase light activity during work hours. The purpose of the study was to determine if participatory workplace interventions could reduce total sedentary time, sustained sedentary time (bouts >30 minutes), increase the frequency of breaks in sedentary time and promote light intensity activity and moderate/vigorous activity (MVPA) during work hours.
A randomised controlled trial (ANZCTR NUMBER: ACTN12612000743864) was conducted using clerical, call centre and data processing workers (n = 62, aged 25-59 years) in 3 large government organisations in Perth, Australia. Three groups developed interventions with a participatory approach: 'Active office' (n = 19), 'Active Workstation' and promotion of incidental office activity; 'Traditional physical activity' (n = 14), pedometer challenge to increase activity between productive work time and 'Office ergonomics' (n = 29), computer workstation design and breaking up computer tasks. Accelerometer (ActiGraph GT3X, 7 days) determined sedentary time, sustained sedentary time, breaks in sedentary time, light intensity activity and MVPA on work days and during work hours were measured before and following a 12 week intervention period.
For all participants there was a significant reduction in sedentary time on work days (-1.6%, p = 0.006) and during work hours (-1.7%, p = 0.014) and a significant increase in number of breaks/sedentary hour on work days (0.64, p = 0.005) and during work hours (0.72, p = 0.015); there was a concurrent significant increase in light activity during work hours (1.5%, p = 0.012) and MVPA on work days (0.6%, p = 0.012).
This study explored novel ways to modify work practices to reduce occupational sedentary behaviour. Participatory workplace interventions can reduce sedentary time, increase the frequency of breaks and improve light activity and MVPA of office workers by using a variety of interventions.

Cover crops are increasingly being used for weed management, and planting them as diverse mixtures has become an increasingly popular strategy for their implementation. While ecological theory ...suggests that cover crop mixtures should be more weed suppressive than cover crop monocultures, few experiments have explicitly tested this for more than a single temporal niche. We assessed the effects of cover crop mixtures (5- or 6-species and 14-species mixtures) and monocultures on weed abundance (weed biomass) and weed suppression at the time of cover crop termination. Separate experiments were conducted in Madbury, NH, from 2014 to 2017 for each of three temporal cover-cropping niches: summer (spring planting–summer termination), fall (summer planting–fall termination), and spring (fall planting–subsequent spring termination). Regardless of temporal niche, mixtures were never more weed suppressive than the most weed-suppressive cover crop grown as a monoculture, and the more diverse mixture (14 species) never outperformed the less diverse mixture. Mean weed-suppression levels of the best-performing monocultures in each temporal niche ranged from 97% to 98% for buckwheat (Fagopyrum esculentum Moench) in the summer niche and forage radish (Raphanus sativus L. var. niger J. Kern.) in the fall niche, and 83% to 100% for triticale (×Triticosecale Wittm. ex A. Camus Secale × Triticum) in the winter–spring niche. In comparison, weed-suppression levels for the mixtures ranged from 66% to 97%, 70% to 90%, and 67% to 99% in the summer, fall, and spring niches, respectively. Stability of weed suppression, measured as the coefficient of variation, was two to six times greater in the best-performing monoculture compared with the most stable mixture, depending on the temporal niche. Results of this study suggest that when weed suppression is the sole objective, farmers are more likely to achieve better results planting the most weed-suppressive cover crop as a monoculture than a mixture.
Necrotising enterocolitis (NEC) is a devastating intestinal disease primarily affecting preterm infants. The underlying mechanisms are poorly understood: mother's own breast milk (MOM) is protective, ...possibly relating to human milk oligosaccharide (HMO) and infant gut microbiome interplay. We investigated the interaction between HMO profiles and infant gut microbiome development and its association with NEC.
We performed HMO profiling of MOM in a large cohort of infants with NEC (n=33) with matched controls (n=37). In a subset of 48 infants (14 with NEC), we also performed longitudinal metagenomic sequencing of infant stool (n=644).
Concentration of a single HMO, disialyllacto-N-tetraose (DSLNT), was significantly lower in MOM received by infants with NEC compared with controls. A MOM threshold level of 241 nmol/mL had a sensitivity and specificity of 0.9 for NEC. Metagenomic sequencing before NEC onset showed significantly lower relative abundance of
and higher relative abundance of
in infants with NEC. Longitudinal development of the microbiome was also impacted by low MOM DSLNT associated with reduced transition into preterm gut community types dominated by
spp and typically observed in older infants. Random forest analysis combining HMO and metagenome data before disease accurately classified 87.5% of infants as healthy or having NEC.
These results demonstrate the importance of HMOs and gut microbiome in preterm infant health and disease. The findings offer potential targets for biomarker development, disease risk stratification and novel avenues for supplements that may prevent life-threatening disease.
Flexible graphene electronics are rapidly gaining interest, but their widespread implementation has been impeded by challenges with ink preparation, ink printing, and postprint annealing processes. ...Laser-induced graphene (LIG) promises a facile alternative by creating flexible graphene electronics on polyimide substrates through the one-step laser writing fabrication method. Herein, we demonstrate the use of LIG, created with a low-cost UV laser, for electrochemical ion-selective sensing of plant-available nitrogen (i.e., both ammonium and nitrate ions: NH4 + and NO3 –) in soil samples. The laser used to create the LIG was operated at distinct pulse widths (10, 20, 30, 40, and 50 ms) to maximize the LIG electrochemical reactivity. Results illustrated that a laser pulse width of 20 ms led to a high percentage of sp2 carbon (77%) and optimal peak oxidation current of 120 μA during cyclic voltammetry of ferro/ferricyanide. Therefore, LIG electrodes created with a 20 ms pulse width were consequently functionalized with distinct ionophores specific to NH4 + (nonactin) or NO3 – (tridodecylmethylammonium nitrate) within poly(vinyl chloride)-based membranes to create distinct solid contact ion-selective electrodes (SC-ISEs) for NH4 + and NO3 – ion sensing, respectively. The LIG SC-ISEs displayed near Nernstian sensitivities of 51.7 ± 7.8 mV/dec (NH4 +) and −54.8 ± 2.5 mV/dec (NO3 –), detection limits of 28.2 ± 25.0 μM (NH4 +) and 20.6 ± 14.8 μM (NO3 –), low long-term drift of 0.93 mV/h (NH4 + sensors) and −5.3 μV/h (NO3 – sensors), and linear sensing ranges of 10–5–10–1 M for both sensors. Moreover, soil slurry sensing was performed, and recovery percentages of 96% and 95% were obtained for added NH4 + and NO3 –, respectively. These results, combined with a facile fabrication that does not require metallic nanoparticle decoration, make these LIG electrochemical sensors appealing for a wide range of in-field or point-of-service applications for soil health management.
Epigenetic markings acquired in early life may have phenotypic consequences later in development through their role in transcriptional regulation with relevance to the developmental origins of ...diseases including obesity. The goal of this study was to investigate whether DNA methylation levels at birth are associated with body size later in childhood.
A study design involving two birth cohorts was used to conduct transcription profiling followed by DNA methylation analysis in peripheral blood. Gene expression analysis was undertaken in 24 individuals whose biological samples and clinical data were collected at a mean ± standard deviation (SD) age of 12.35 (0.95) years, the upper and lower tertiles of body mass index (BMI) were compared with a mean (SD) BMI difference of 9.86 (2.37) kg/m(2). This generated a panel of differentially expressed genes for DNA methylation analysis which was then undertaken in cord blood DNA in 178 individuals with body composition data prospectively collected at a mean (SD) age of 9.83 (0.23) years. Twenty-nine differentially expressed genes (>1.2-fold and p<10(-4)) were analysed to determine DNA methylation levels at 1-3 sites per gene. Five genes were unmethylated and DNA methylation in the remaining 24 genes was analysed using linear regression with bootstrapping. Methylation in 9 of the 24 (37.5%) genes studied was associated with at least one index of body composition (BMI, fat mass, lean mass, height) at age 9 years, although only one of these associations remained after correction for multiple testing (ALPL with height, p(Corrected) = 0.017).
DNA methylation patterns in cord blood show some association with altered gene expression, body size and composition in childhood. The observed relationship is correlative and despite suggestion of a mechanistic epigenetic link between in utero life and later phenotype, further investigation is required to establish causality.

Late onset sepsis (LOS) in preterm infants is associated with considerable morbidity and mortality. While studies have implicated gut bacteria in the aetiology of the disease, functional analysis and ...mechanistic insights are generally lacking. We performed temporal bacterial (n = 613) and metabolomic (n = 63) profiling on extensively sampled stool from 7 infants with LOS and 28 matched healthy (no LOS or NEC) controls.
The bacteria isolated in diagnostic blood culture usually corresponded to the dominant bacterial genera in the gut microbiome. Longitudinal changes were monitored based on preterm gut community types (PGCTs), where control infants had an increased number of PGCTs compared to LOS infants (P = 0.011). PGCT 6, characterised by Bifidobacteria dominance, was only present in control infants. Metabolite profiles differed between LOS and control infants at diagnosis and 7 days later, but not 7 days prior to diagnosis. Bifidobacteria was positively correlated with control metabolites, including raffinose, sucrose, and acetic acid.
Using multi-omic analysis, we show that the gut microbiome is involved in the pathogenesis of LOS. While the causative agent of LOS varies, it is usually abundant in the gut. Bifidobacteria dominance was associated with control infants, and the presence of this organism may directly protect, or act as a marker for protection, against gut epithelial translocation. While the metabolomic data is preliminary, the findings support that gut development and protection in preterm infants is associated with increased in prebiotic oligosaccharides (e.g. raffinose) and the growth of beneficial bacteria (e.g. Bifidobacterium).
Prognosticating idiopathic pulmonary fibrosis (IPF) is challenging, in part due to a lack of sensitive biomarkers. A recent article in

In a population of patients with IPF, we find that the xenon signal from red blood cells diminishes relative to the tissue/plasma signal over a 12-month time period, even when the diffusion factor for carbon monoxide is static over the same time period. Xe MR spectroscopy may be sensitive to short-term changes in interstitial gas diffusion in IPF.
