Guidelines for prehospital emergency care within the chain of rescue are recommended. At the scene, information about the patient and therapeutical tools are limited. Accidents and emergencies happen ...outside of hospitals and doctor's offices, while environmental factors and logistic problems strongly determine the rescue process. The sequence of interventions or the choice of the right hospital with specific triage criterias are only examples for these problems. There is only limited scientific evidence for therapies performed prehospitally. However, scientific material for the preparation of guidelines should be evaluated by using standardized criteria. The rules of evidence based medicine according to Cook are a practical way to evaluate and classify scientific material and to establish practical guidelines. In this paper, an 8-step-program is introduced to develop practical guidelines for the Emergency Medical Services Cologne.
Air pollution, in particular high concentrations of particulate matter smaller than 1 µm in diameter (PM1), continues to be a major health problem, and meteorology is known to substantially influence ...atmospheric PM concentrations. However, the scientific understanding of the ways in which complex interactions of meteorological factors lead to high-pollution episodes is inconclusive. In this study, a novel, data-driven approach based on empirical relationships is used to characterize and better understand the meteorology-driven component of PM1 variability. A tree-based machine learning model is set up to reproduce concentrations of speciated PM1 at a suburban site southwest of Paris, France, using meteorological variables as input features. The model is able to capture the majority of occurring variance of mean afternoon total PM1 concentrations (coefficient of determination (R2) of 0.58), with model performance depending on the individual PM1 species predicted.
Based on the models, an isolation and quantification of individual, season-specific meteorological influences for process understanding at the measurement site is achieved using SHapley Additive exPlanation (SHAP) regression values.
Model results suggest that winter pollution episodes are often driven by a combination of shallow mixed layer heights (MLHs), low temperatures, low wind speeds, or inflow from northeastern wind directions. Contributions of MLHs to the winter pollution episodes are quantified to be on average ∼5 µg/m3 for MLHs below <500 m a.g.l. Temperatures below freezing initiate formation processes and increase local emissions related to residential heating, amounting to a contribution to predicted PM1 concentrations of as much as ∼9 µg/m3. Northeasterly winds are found to contribute ∼5 µg/m3 to predicted PM1 concentrations (combined effects of u- and v-wind components), by advecting particles from source regions, e.g. central Europe or the Paris region.
Meteorological drivers of unusually high PM1 concentrations in summer are temperatures above ∼25 ∘C (contributions of up to ∼2.5 µg/m3), dry spells of several days (maximum contributions of ∼1.5 µg/m3), and wind speeds below ∼2 m/s (maximum contributions of ∼3 µg/m3), which cause a lack of dispersion.
High-resolution case studies are conducted showing a large variability of processes that can lead to high-pollution episodes.
The identification of these meteorological conditions that increase air pollution could help policy makers to adapt policy measures, issue warnings to the public, or assess the effectiveness of air pollution measures.
Silicon (Si) fertiliser can improve rice (
Oryza sativa
) tolerance to salinity. The rate of Si uptake and its associated benefits are known to differ between plant genotypes, but, to date, little ...research has been done on how the benefits, and hence the economic feasibility, of Si fertilisation varies between cultivars. In this study, a range of rice cultivars was grown both hydroponically and in soil, at different levels of Si and NaCl, to determine cultivar variation in the response to Si. There was significant variation in the effect of Si, such that Si alleviated salt-induced growth inhibition in some cultivars, while others were unaffected, or even negatively impacted. Thus, when assessing the benefits of Si supplementation in alleviating salt stress, it is essential to collect cultivar-specific data, including yield, since changes in biomass were not always correlated with those seen for yield. Root Si content was found to be more important than shoot Si in protecting rice against salinity stress, with a root Si level of 0.5–0.9% determined as having maximum stress alleviation by Si. A cost–benefit analysis indicated that Si fertilisation is beneficial in mild stress, high-yield conditions but is not cost-effective in low-yield production systems.
Purpose: To compare different q-space reconstruction methods for undersampled diffusion spectrum imaging data.Materials and Methods: We compared the quality of three methods: Mean Apparent Propagator ...(MAP); Compressed Sensing using Identity (CSI) and Compressed Sensing using Dictionary (CSD) with simulated data and in vivo acquisitions. We used retrospective undersampling so that the fully sampled reconstruction could be used as ground truth. We used the normalized mean squared error (NMSE) and the Pearson’s correlation coefficient as reconstruction quality indices. Additionally, we evaluated two propagator-based diffusion indices: mean squared displacement and return to zero probability. We also did a visual analysis around the centrum semiovale.Results: All methods had reconstruction errors below 5% with low undersampling factors and with a wide range of noise levels. However, the CSD method had at least 1–2% lower NMSE than the other reconstruction methods at higher noise levels. MAP was the second-best method when using a sufficiently high number of q-space samples. MAP reconstruction showed better propagator-based diffusion indices for in vivo acquisitions. With undersampling factors greater than 4, MAP and CSI have noticeably more reconstruction error than CSD.Conclusion: Undersampled data were best reconstructed by means of CSD in simulations and in vivo. MAP was more accurate in the extraction of propagator-based indices, particularly for in vivo data.
Despite extensive efforts, half of patients with rare movement disorders such as hereditary spastic paraplegias and cerebellar ataxias remain genetically unexplained, implicating novel genes and ...unrecognized mutations in known genes. Non-coding DNA variants are suspected to account for a substantial part of undiscovered causes of rare diseases. Here we identified mutations located deep in introns of POLR3A to be a frequent cause of hereditary spastic paraplegia and cerebellar ataxia. First, whole-exome sequencing findings in a recessive spastic ataxia family turned our attention to intronic variants in POLR3A, a gene previously associated with hypomyelinating leukodystrophy type 7. Next, we screened a cohort of hereditary spastic paraplegia and cerebellar ataxia cases (n = 618) for mutations in POLR3A and identified compound heterozygous POLR3A mutations in ∼3.1% of index cases. Interestingly, >80% of POLR3A mutation carriers presented the same deep-intronic mutation (c.1909+22G>A), which activates a cryptic splice site in a tissue and stage of development-specific manner and leads to a novel distinct and uniform phenotype. The phenotype is characterized by adolescent-onset progressive spastic ataxia with frequent occurrence of tremor, involvement of the central sensory tracts and dental problems (hypodontia, early onset of severe and aggressive periodontal disease). Instead of the typical hypomyelination magnetic resonance imaging pattern associated with classical POLR3A mutations, cases carrying c.1909+22G>A demonstrated hyperintensities along the superior cerebellar peduncles. These hyperintensities may represent the structural correlate to the cerebellar symptoms observed in these patients. The associated c.1909+22G>A variant was significantly enriched in 1139 cases with spastic ataxia-related phenotypes as compared to unrelated neurological and non-neurological phenotypes and healthy controls (P = 1.3 × 10-4). In this study we demonstrate that (i) autosomal-recessive mutations in POLR3A are a frequent cause of hereditary spastic ataxias, accounting for about 3% of hitherto genetically unclassified autosomal recessive and sporadic cases; and (ii) hypomyelination is frequently absent in POLR3A-related syndromes, especially when intronic mutations are present, and thus can no longer be considered as the unifying feature of POLR3A disease. Furthermore, our results demonstrate that substantial progress in revealing the causes of Mendelian diseases can be made by exploring the non-coding sequences of the human genome.
