Health status measurement is a common feature of studies in chronic obstructive pulmonary disease (COPD). This review assesses recent evidence for the validity of these measurements and their role as ...measures of the overall impact of the disease on the patient's daily life and wellbeing. It reviews the mostly widely used COPD specific questionnaires and examines the contribution that they make to an assessment of the overall effect of treatment. Finally, it addresses the question of how symptomatic benefit may be assessed in individual patients in routine practice.
Cells chemically isolate molecules in compartments to both facilitate and regulate their interactions. In addition to membrane-encapsulated compartments, cells can form proteinaceous and membraneless ...organelles, including nucleoli, Cajal and PML bodies, and stress granules. The principles that determine when and why these structures form have remained elusive. Here, we demonstrate that the disordered tails of Ddx4, a primary constituent of nuage or germ granules, form phase-separated organelles both in live cells and in vitro. These bodies are stabilized by patterned electrostatic interactions that are highly sensitive to temperature, ionic strength, arginine methylation, and splicing. Sequence determinants are used to identify proteins found in both membraneless organelles and cell adhesion. Moreover, the bodies provide an alternative solvent environment that can concentrate single-stranded DNA but largely exclude double-stranded DNA. We propose that phase separation of disordered proteins containing weakly interacting blocks is a general mechanism for forming regulated, membraneless organelles.

Evidence is presented of a reduction in relative humidity over low‐latitude and midlatitude land areas over a period of about 10 years leading up to 2008, based on monthly anomalies in surface air ...temperature and humidity from comprehensive European Centre for Medium‐Range Weather Forecasts reanalyses (ERA‐40 and ERA‐Interim) and from Climatic Research Unit and Hadley Centre analyses of monthly station temperature data (CRUTEM3) and synoptic humidity observations (HadCRUH). The data sets agree well for both temperature and humidity variations for periods and places of overlap, although the average warming over land is larger for the fully sampled ERA data than for the spatially and temporally incomplete CRUTEM3 data. Near‐surface specific humidity varies similarly over land and sea, suggesting that the recent reduction in relative humidity over land may be due to limited moisture supply from the oceans, where evaporation has been limited by sea surface temperatures that have not risen in concert with temperatures over land. Continental precipitation from the reanalyses is compared with a new gauge‐based Global Precipitation Climatology Centre (GPCC) data set, with the combined gauge and satellite products of the Global Precipitation Climatology Project (GPCP) and the Climate Prediction Center (CPC), Merged Analysis of Precipitation (CMAP), and with CPC's independent gauge analysis of precipitation over land (PREC/L). The reanalyses agree best with the new GPCC and latest GPCP data sets, with ERA‐Interim significantly better than ERA‐40 at capturing monthly variability. Shifts over time in the differences among the precipitation data sets make it difficult to assess their longer‐term variations and any link with longer‐term variations in humidity.
Global climate change is expected to affect the frequency, intensity and duration of extreme water-related weather events such as excessive precipitation, floods, and drought. We conducted a ...systematic review to examine waterborne outbreaks following such events and explored their distribution between the different types of extreme water-related weather events. Four medical and meteorological databases (Medline, Embase, GeoRef, PubMed) and a global electronic reporting system (ProMED) were searched, from 1910 to 2010. Eighty-seven waterborne outbreaks involving extreme water-related weather events were identified and included, alongside 235 ProMED reports. Heavy rainfall and flooding were the most common events preceding outbreaks associated with extreme weather and were reported in 55·2% and 52·9% of accounts, respectively. The most common pathogens reported in these outbreaks were Vibrio spp. (21·6%) and Leptospira spp. (12·7%). Outbreaks following extreme water-related weather events were often the result of contamination of the drinking-water supply (53·7%). Differences in reporting of outbreaks were seen between the scientific literature and ProMED. Extreme water-related weather events represent a risk to public health in both developed and developing countries, but impact will be disproportionate and likely to compound existing health disparities.

It is known that the concept of limiting polarization introduced 70 yr ago by K. G. Budden has the capacity to explain the magnitude of circular polarization seen in normal pulsars with ...light-cylinder radii of the order of 109–10 cm under the assumption of a high-multiplicity electron–positron plasma. However, a review of limiting polarization under the same assumption in millisecond pulsars indicates that it is inapplicable there because the region of limiting polarization lies far outside the light cylinder. This paper, using the ion–proton model, evaluates circular polarization both generally and specifically for J2144−3933, and gives a fairly detailed understanding of the observations in normal pulsars including the change of sign as a function of frequency seen in J0908−4913. But it also fails to explain circular polarization in millisecond pulsars owing to the smaller particle number densities and birefringence of the magnetosphere in these objects. However, the review of limiting polarization finds that, within the ion–proton model, this distinct process can describe their circular polarization. It is argued that certain features of millisecond pulsar Stokes profiles are clearly consistent with limiting polarization.

Accumulating evidence indicate a role for the immune system particularly inflammation and autoimmunity in the aetiology of major psychiatric disorders such as depression and schizophrenia. In this ...paper, we discuss some of the key advances in immunopsychiatry in order to highlight to psychiatrists and other health professionals how an increased understanding of this field might enhance our knowledge of illness mechanism and approaches to treatment. We present a brief overview of clinical research that link inflammation and autoimmunity with depression and psychosis, including potential role of inflammation in treatment response, current evidence for the effectiveness of immune-modulating treatment for depression and psychosis, and possible role of inflammation in common physical comorbidities for these disorders such as coronary heart disease and diabetes mellitus. Gaining a better understanding of the role of immune system could be paradigm changing for psychiatry. We need collaborations between clinicians and scientists to deliver high-quality translational research in order to fully realise the clinical potential of this exciting and rapidly expanding field.
The atmospheric methane burden is increasing rapidly, contrary to pathways compatible with the goals of the 2015 United Nations Framework Convention on Climate Change Paris Agreement. Urgent action ...is required to bring methane back to a pathway more in line with the Paris goals. Emission reduction from “tractable” (easier to mitigate) anthropogenic sources such as the fossil fuel industries and landfills is being much facilitated by technical advances in the past decade, which have radically improved our ability to locate, identify, quantify, and reduce emissions. Measures to reduce emissions from “intractable” (harder to mitigate) anthropogenic sources such as agriculture and biomass burning have received less attention and are also becoming more feasible, including removal from elevated‐methane ambient air near to sources. The wider effort to use microbiological and dietary intervention to reduce emissions from cattle (and humans) is not addressed in detail in this essentially geophysical review. Though they cannot replace the need to reach “net‐zero” emissions of CO2, significant reductions in the methane burden will ease the timescales needed to reach required CO2 reduction targets for any particular future temperature limit. There is no single magic bullet, but implementation of a wide array of mitigation and emission reduction strategies could substantially cut the global methane burden, at a cost that is relatively low compared to the parallel and necessary measures to reduce CO2, and thereby reduce the atmospheric methane burden back toward pathways consistent with the goals of the Paris Agreement.

Methane is a powerful climate warmer, and the amount of methane in the air is growing rapidly. Reducing human‐caused methane emissions is urgent if the 2015 United Nations Paris Agreement to limit climate warming is to succeed. There is hope, though the problem of methane mitigation is very wide and complex. Much of the task is in finding, identifying, and quantifying emissions. Rapid technical advances are making it much easier to locate and thus cut emissions from fossil fuel industries (gas, coal, and oil). Assessing emissions from landfill and sewage facilities is also becoming easier. In particular, poorly regulated landfills in fast‐growing tropical megacities need attention. Agricultural emissions are less tractable but may also be reduced to some extent, especially by improving manure management. Many methane mitigation options offer cost‐effective approaches to cut global warming and bring the amount of methane in the air back to a pathway that is consistent with the aims of the Paris Agreement.

Abstract
A number of previous papers have developed an ion-proton theory of the pulsar polar cap. The basic equations summarizing this are given here with the results of sets of model step-to-step ...calculations of pulse-precursor profiles. The nature of step-to-step profile variations is described by calculated phase-resolved modulation indices. The conditions under which nulls are present in step sequences are analysed. The change of mean null length with neutron-star surface temperature shows a pathway ending in emission similar to the Rotating Radio Transients. The model accommodates exceptional pulsars, the millisecond pulsars (in principle), and the 8.5 s period PSR J2144-3933. These are considered separately and their emission mechanism discussed in some detail.
The Annotated Constitution of Japan: A Handbook for the first time makes the entirety of Japan's constitution accessible in English. This study is an extensive revision of the Climatic Research Unit (CRU) land station temperature database that is used to produce a gridbox dataset of 5° latitude × 5° longitude temperature ...anomalies. The new database comprises 5159 station records, of which 4167 have enough data for the 1961–90 period to calculate or estimate the necessary averages. Apart from the increase in station numbers compared to the earlier study in 1994, many station records have had their data replaced by newly homogenized series that have been produced by several recent studies. New versions of all the gridded datasets currently available on the CRU Web site (http://www.cru.uea.ac.uk) have been developed. This includes combinations with marine (sea surface temperature anomalies) data over the oceans and versions with adjustment of the variance of individual gridbox series to remove the effects of changing station numbers through time.
Hemispheric and global temperature averages for land areas developed with the new dataset differ slightly from those developed in 1994. Possible reasons for the differences between the new and the earlier analysis and those from the National Climatic Data Center and the Goddard Institute for Space Studies are discussed. Differences are greatest over the Southern Hemisphere and at the beginnings and ends of each time series and relate to gridbox sizes and data availability. The rate of annual warming for global land areas over the 1901–2000 period is estimated by least squares to be 0.07°C decade−1(significant at better than the 99.9% level). Warming is not continuous but occurs principally over two periods (about 1920–45 and since 1975). Annual temperature series for the seven continents and the Arctic all show significant warming over the twentieth century, with significant (95%) warming for 1920–44 for North America, the Arctic, Africa, and South America, and all continents except Australia and the Antarctic since 1977. Cooling is significant during the intervening period (1945–76) for North America, the Arctic, and Africa.
