This paper analyzes the main contradictions existing in Ecuador for the consolidation of neo-constitutionalism, which is considered as; the new legal theory, which seeks to transform the rule of law ...into the constitutional rule of law, proposing for it; greater state intervention and creation of egalitarian democratic spaces and respect for human rights among state agencies and institutions. For this reason, the objective of this work is to characterize the factors that influence the consolidation of neo-constitutionalism in Ecuador, as an ideological and methodological theory of law. ; 400 μg/kg body weight/day; 3 times a week) in gelatine pellets, from embryonic day (E)11.5 until weaning (postnatal day 21), led to gut immunological changes in progeny. The results showed an overall increase of lymphocyte number in intestine, a reduction of expression of epithelial genes related to microbial defence, as well as a decrease in cytokine production by intestinal type 2 innate lymphoid cells (ILC2). While susceptibility to chemically induced colitis was not worsened, immune alterations were associated with changes in gut microbiota and with a higher vulnerability to infection by the protozoan Eimeria vermiformis at early-life. Together these results show that maternal dietary exposure to AFB
can dampen intestinal barrier homeostasis in offspring decreasing their capability to tackle intestinal pathogens. These data provide insights to understand AFB
potential harmfulness in early-life health in the context of intestinal infections.
In this paper I propose, based on a critical examination of a comparison between Wittgenstein's notion of secondary meaning and Davidson's notion of metaphor, a defence of the genuineness of the ...distinction developed by Wittgenstein between both notions. I will argue that Davidson's notion of metaphor cannot be assimilated to the one of secondary meaning and that, on the other hand, these philosophers coincide when they claim that the condition of possibility of the secondary meaning as well as the metaphor resides in the distinction between the meaning of the words and the ways in that we use them. En este artículo, a partir de un examen crítico de una comparación entre la noción de significado secundario en Wittgenstein y la metáfora tal como la entiende Davidson, se propone una defensa de la legitimidad de la distinción propuesta por Wittgenstein entre significado secundario y metáfora. Se argumentará que la noción de metáfora de Davidson no puede ser asimilada a la de significado secundario y que, por otra parte, estos filósofos coinciden en afirmar que la condición de posibilidad del significado secundario y la metáfora radica en la distinción entre el significado de las palabras y las maneras en que las utilizamos.
Ante las frías relaciones que se suelen establecer en las unidades de cuidado intensivo pediátricas (UCIP), entre el personal de salud y las familias de los menores internados, algunos médicos del ...hospital Pablo Tobón Uribe, al buscar mejorar la comunicación, desarrollaron una estrategia que propició el diálogo y la escucha asertiva, luego de poner en práctica el modelo de “Atención Centrada en el Paciente y la Familia”. La metodología de sistematización de experiencias permitió un análisis reflexivo de los testimonios recogidos en los múltiples encuentros entre personal de salud y 820 familiares. Los resultados, en términos de aprendizajes, permitieron evidenciar una atención integral que involucró a la familia como sujeto de cuidado, concienció sobre la repercusión psicológica que implica la experiencia de permanencia en la UCIP y estimuló una reflexión crítica del equipo de salud en la atención ofrecida a las familias de los pacientes en la práctica clínica diaria.

Background: infective endocarditis is a relatively infrequent disease, which is often difficult to diagnose and has a high mortality.Objective: to characterize from a clinical viewpoint the patients ...with endocarditis admitted to the services of internal medicine and cardiology of the "Dr. Ernesto Guevara de la Serna" Hospital of Las Tunas, from January 2012 to December 2015.Methods: an observational, descriptive, cross-sectional study was carried out with 22 patients with a diagnosis of infective endocarditis, attended to at the aforementioned institution and during the period herein declared. To gather the information secondary sources were used by reviewing the medical records.Results: there was a prevalence of patients with infective endocarditis belonging to the 41 to 50 age group for 27,3 % of the cases, as well as the male sex with 68,1 %. The personal pathological history mostly manifested was ischemic cardiopathy in 36,4 %. Fever represented 90 % within the clinical manifestations. Infective endocarditis was diagnosed at the time of admission to 14 patients, 63,6 % out of the total. In 8 patients (36,4 %) there was no coincidence between the presumptive diagnosis and the definitive one. Valvular vegetations predominated in the echocardiogram in 40,9 % and Staphylococcus aureus was isolated in 40,9 % of the cases. Conclusions: the patients with infective endocarditis were characterized, presenting fever as the clinical manifestation in almost the entire sample. In more than half of the cases, the presumptive diagnosis on admission coincided with the definitive diagnosis.
Background: renal cancer has a low prevalence but a high aggressiveness, being the clear cell renal cell adenocarcinoma the most frequent type, with unspecific and varied clinical manifestations. ...Objective: to characterize clinical and therapeutic aspects in patients with clear cell renal cell adenocarcinoma attended to at the “Dr. Ernesto Guevara de la Serna” Hospital of Las Tunas, from January 2012 to December 2017.Methods: an observational, descriptive and cross-sectional study was carried out at the aforementioned hospital and during the period herein declared. The universe was made up of the 57 patients with renal cancer and the study sample included 45 patients with clear cell renal cell adenocarcinoma.Results: the male sex (62,2 %), the white skin (77,7 %) and the urban origin (66,7 %) represented the highest rates. The condition was frequent in the 50 to 65 age group for 53,3 %. Hypertension (55,5 %) and chronic renal failure (26,7 %) were the associated diseases with the highest incidence in those patients. The commonest metastatic site was the renal capsule (24,4 %), followed by the lung (13,3 %). Lumbo-abdominal pain was the main symptom (84,4 %) and represented the diagnostic impression at the moment of admission in 42,3 % of the cases, followed by urinary tract infection (28,9 %). 35,5 % of the patients presented Guyon's triad. Total nephrectomy was performed on 66,6 % of the patients.Conclusions: Lumbo-abdominal pain was the main symptom in the patients with clear cell renal cell adenocarcinoma, with more cases of metastasis in the renal capsule and with total nephrectomy as treatment.
Introduction: taking into account the need to manage the internal processes effectively, and giving a greater emphasis to the multidisciplinary approach and understanding the specialty of cancer as a ...process of integral and continuous care in which diverse medical, technical and social processes interact in order to offer a service of quality, complying with the requirements of the National Nuclear Safety Center as a regulatory body, Cancer Patient Care Center in Pinar del Río designed a Quality Management System based on the requirements described in ISO 9001: 2015 Quality Management System-Requirements. Objective: to implement a suitable Quality Management System for the characteristics of the Radiotherapy Service of this entity. Methods: direct observation, interviews, studies and examinations of the Cuban norms and resolutions related to the topic were used for the development of this research, in addition to analytical tools and techniques. Results: this viable solution was confirmed for the public health entities and medical services in general. Currently, the Center has the recognition and endorsement of the National Center for Nuclear Safety to be the first entity of its kind in the country that presents an implemented a Quality Management System. Conclusion: the application in the Center demonstrated a change in its traditional management approach to a management by processes, favoring the efficiency, guaranteeing a greater participation of the interested parties and a considerably increasing of satisfaction showed by the patients with the service received.
