Toscano skupia się przede wszystkim na Agambenowskim rozumieniu sekularyzacji, które umożliwia postawienie tezy, że nowoczesność jedynie dopełnia chrześcijańską „ekonomię” opatrzności, czy też że Marksowskie pojęcie praxis „stanowi w zasadzie zeświecczenie teologicznej koncepcji bycia stworzenia jako boskiego działania”. Autor stara się pokazać, że Agamben w zbytnim stopniu polega na pewnym typie historycznego substancjalizmu, który stoi w sprzeczności z jego deklaracją skupienia się na genealogii. Przygląda się także słabym punktom w Agambenowskim podejściu do tak kluczowych zagadnień, jak pieniądz i administracja.

Foucault returned to the text of Sophocles’s
at various critical junctures in his thought. This article homes in on the place of that tragedy in the passage from the analytic of knowledge-power to ...the problematization of government by truth that took place around 1980, namely in Foucault’s Collège de France lectures. Particular attention is placed on the delineation of the political meaning of “alethurgy,” a neologism through which Foucault sought to thematize the significance of practices concerned with the manifestation of truth for the exercise of political power. By drawing on historical and anthropological studies of Greek “pre-law,” Foucault was able to explore the juridical dimensions of the liturgy of truth, especially in the transition from the Homeric centrality of the
to the emergence of the forensic inquiry with its reliance on the extraction of (slave) testimony. The reading of tragedy here doubles as a nuanced critique of the juridical production of truth, one with important connections to Foucault’s contemporaneous political efforts to support practices of legal defense that rejected the demand that the crimi nalized subject reveal himself in an “auto-alethurgy.”

Direct oral anticoagulant (DOAC) therapy has been shown to be safe and effective in patients with atrial fibrillation (AF). However, outcomes in AF patients with bioprosthetic valves are unclear, as ...this population has been underrepresented in clinical trials. The aim of this study was to assess the safety and efficacy of DOACs in this population based on the existing published literature.
A systematic search and review were conducted to identify randomized clinical trials and comparative observational studies published from 2017 to January 2022 that compared DOACs and vitamin K antagonists (VKAs) in AF patients with bioprosthetic valves. Hazard ratios (HR) were collected to compare the 2 treatments in terms of cardiovascular and all-cause mortality, stroke/systemic embolism, and major bleeding. A meta-analysis combining the results was performed.
We included 12 studies (30 283 patients). DOACs and VKAs were compared based on HRs at the 95% confidence interval. DOAC therapy was associated with a significant 9% reduction in all-cause mortality (HR, 0.91; 95%CI, 0.85-0.97; P=.0068; I2=8%), with no significant differences in the risk of stroke/systemic embolism (HR, 0.87; 95%CI, 0.67-1.14; P=.29; I2=45%) or major bleeding (HR, 0.82; 95%CI, 0.67-1.00; P=.054; I2=48.7%).
DOAC therapy in AF patients with bioprosthetic valves may be associated with a significant reduction in all-cause mortality, with no reduction in the efficacy of stroke/systemic embolism prevention or increase in major bleeding risk.
Los anticoagulantes orales directos (ACOD) se han mostrado eficaces y seguros en pacientes con fibrilación auricular; sin embargo, los pacientes con FA y bioprótesis están infrarrepresentados en los ensayos clínicos, por lo que la evidencia en este grupo es menor. Nuestro objetivo fue analizar la seguridad y eficacia de los ACODs en esta población revisando la información existente en la literatura.
Se realizó una búsqueda y revisión sistemática con los ensayos clínicos aleatorizados y estudios observacionales comparativos desde 2017 a enero de 2022, que comparasen ACODs y antagonistas de vitamina K (AVK) en pacientes con FA y bioprótesis. Se utilizó la hazard ratio al 95% del intervalo de confianza para comparar ambos grupos en términos de mortalidad total y cardiovascular, ictus/embolia sistémica y hemorragia mayor. Se realizó un metanálisis combinando los resultados de los estudios incluidos.
Se incluyeron 12 estudios (un total de 30.283 pacientes). Los ACODs se asociaron a una reducción significativa del 9% de la mortalidad total (HR=0,91; IC95%, 0,85-0,97; p=0,0068; I2=8%), sin diferencias significativas en el riesgo de ictus/embolismo sistémico (HR=0,87; IC95%, 0,67-1,14; p=0,29; I2=45%) o hemorragia mayor (HR=0,82; IC95%, 0,67-1,00; p=0,054; I2=48.7%).
En pacientes con FA portadores de bioprótesis, los ACODs podrían asociarse a una reducción de la mortalidad total sin reducción de eficacia en la prevención de ictus/embolia sistémica o aumento del riesgo de hemorragia mayor.

Abstract
The Covid-19 pandemic has further intensified a crisis in the functions and the perception of the state. It has also revealed underlying contradictions in both mainstream and radical ...ideologies of the state. A desire for the state as guarantor of public welfare vies with fear of the state's hypertrophic capacities for surveillance and control. Following a brief exploration of the intimate modern connection between plagues and the state, the article tries to map some of the ways in which the state has been at stake in political and theoretical commentaries on the pandemic. Is an epidemiological politics from below, beyond the plague state, possible? Can recent emergency measures be seen as incomplete or inverted anticipations of a communist use of the state of exception? Or is the primacy of the political we are currently experiencing a mere fetish, indissociable from the rule of capital?
Most theorisations of fascism, Marxist and otherwise, have taken for granted its idolatry of the state and phobia of freedom. This analytical common sense has also inhibited the identification of ...continuities with contemporary movements of the far Right, with their libertarian and anti-statist affectations, not to mention their embeddedness in neoliberal policies and subjectivities. Drawing on a range of diverse sources – from Johann Chapoutot’s histories of Nazi intellectuals to Ruth Wilson Gilmore’s theorisation of the anti-state state, and from Marcuse’s explorations of fascist competitive individualism to debates on neoliberal authoritarianism – this essay sketches the counter-intuitive but disturbingly timely image of a fascism enamoured of freedom and at odds with the state.
