Our previous studies showed that experienced Tai Chi practitioners had better joint proprioception and balance control during weight shifting. The objective of the present study was to examine ...whether experienced golfers had attained similar improvement when compared with the Tai Chi practitioners, as well as healthy elderly subjects and young university students.
We compared 12 experienced elderly Tai Chi practitioners, with 11 experienced elderly golfers, 12 healthy elderly subjects, and 12 young university students, who were all males, using: 1) passive knee joint repositioning test to assess their joint proprioceptive acuity and 2) limits of stability test to assess their ability to voluntarily weight shift within their base of support.
Both Tai Chi practitioners and golfers had better knee joint proprioceptive acuity than did the elderly control subjects (P < 0.05). Of special interest is that their performance was similar to that of the young subjects. In the limits of stability test, Tai Chi practitioners and golfers had faster reaction time, leaned further without losing stability, and showed better control of leaning trajectory than did elderly control subjects (all P < 0.05). The latter two outcome measures were also comparable to those of the young subjects.
These results demonstrate that both experienced Tai Chi practitioners and golfers had improved knee joint proprioception and limits of stability, when compared with those of elderly control subjects similar in age, gender (male), and physical activity level. Such improved outcome measures were comparable to those of young male subjects. These findings suggest that experienced Tai Chi practitioners and golfers had improved joint proprioceptive acuity and dynamic standing balance control, despite the known aging effects in these specific sensorimotor functions.
Objectives To conduct a territory-wide study of extended-spectrum β-lactamases (ESBLs) among community isolates of urinary Escherichia coli from women in Hong Kong. Methods Up to 50 consecutive ...single-patient E. coli isolates, collected from 13 laboratories in 2004, were studied. The ESBLs were characterized by PCR sequencing using specific primers. The epidemiological relationship of the isolates was studied by PFGE and phylogenetic group PCRs. Results Forty-two ESBL producers were found among 600 consecutive isolates tested. The ESBL prevalence was 7.3% (15/205) for women aged 18–35 years, 5% (11/219) for women aged 36–50 years, 6.3% (4/63) for women aged 51–64 years and 10.6% (12/113) for women aged ≥65 years (P = 0.3). The ESBL-producing isolates were often multidrug-resistant and CTX-M-14 was found in 37 isolates, CTX-M-15 in 3 isolates and CTX-M-3 in 2 isolates. PFGE revealed no significant clusters among the ESBL producers. Overall, CTX-M-14 producers were significantly more likely to belong to group D than non-ESBL producers 18/37 (48.6%) versus 13/57 (22.8%), P = 0.009. However, 7 of 13 (53.8%) CTX-M-14 producers from women aged 18–35 years represented phylogenetic group B2, compared with 7 of 24 (29.2%) for women of all other ages (P = 0.1). Conclusions The study documented the community emergence of CTX-M as the predominant ESBL type among urinary isolates from women. The spread of CTX-M enzymes among isolates from young women is concerning and deserves close monitoring.
A cross-sectional study of the balance control in adolescents with idiopathic scoliosis. OBJECTIVE.: To investigate the relationship among somatosensory function, balance control, and adolescent ...idiopathic scoliosis (AIS).
Balance control requires the contribution of somatosensory, visual, and vestibular inputs. Previous studies have shown that abnormal somatosensory evoked potentials (SSEPs) were associated with AIS.
A total of 105 girls with AIS and 57 normal girls aged 11-14 years entered the study, and were evaluated with bilateral tibial nerve evoked cerebral SSEPs and standing balance control under reduced or conflicting sensory conditions (i.e., the sensory organization test). One-way analysis of variance was conducted to evaluate the effects of scoliosis and somatosensory function tested by SSEPs on the performance of sensory organization test.
There were 15 patients with AIS who had abnormal SSEPs. Postural sway was measured on all subjects under normal, reduced, or conflicting somatosensory, visual, and vestibular conditions. One-way analysis of variance indicated no significant effect of spinal deformity or SSEPs on the balance control when there was an increased reliance on the somatosensory, visual, and vestibular systems during stance (P > 0.05). However, in the patients with abnormal SSEPs, a significant effect was found when subjects had to rely on somatosensory input for their balance control (P = 0.023). The effect of scoliosis by itself was not found to be significant (P = 1.0).
The finding of significantly larger difference in bilateral SSEP latencies in patients with AIS indicated the association of abnormal SSEPs with AIS. The finding of a significant effect of SSEPs on the balance control further indicated the presence of abnormal somatosensory function in a subgroup of patients with AIS.
Abstract The FES-I is an instrument developed to assess concern about falls. The aim of this study was to develop a Chinese version of the 16-item Falls Efficacy Scale International (FES-I(Ch)) and ...evaluate its structure, measurement properties and convergent and predictive validity. The FES-I(Ch) was developed following the recommended 10-step protocol. The FES-I(Ch) was then administered to 399 community-dwelling Chinese older people (61–93 years) in conjunction with a range of other socio-demographic, physical, medical and functional measures. Falls were prospectively monitored over 12 months. Sub-samples were reassessed for determination of the FES-I(Ch)’s test–retest and inter-rater reliability. The overall structure and measurement properties of the FES-I(Ch), as evaluated with factor analysis and item-total correlations, was good. Internal consistency was excellent (Cronbach's α = 0.94), as was test–retest and inter-rater reliability (ICC3,1 = 0.89 and ICC2,1 = 0.95 respectively). FES-I(Ch) scores were significantly higher in participants with poor physical performance, depression, medical conditions associated with falls and disability indicating acceptable congruent validity. FES-I(Ch) scores did not differ between those who did and did not fall in the 12-month follow-up period. We found that the FES-I(Ch) is a valid and reliable measure of concern about falls in Chinese older people. The relatively high level on concern (high FES-I(Ch) scores) as well as relatively few prospective falls may explain the lack of association between FES-I(Ch) scores and falls in this population. Future studies should explore the FES-I(Ch)’s responsiveness to change over time and during intervention studies.
This study compared the sensory organization and standing balance of adolescent tennis players, taekwondo (TKD) practitioners, and healthy control participants. Sixty participants including 12 tennis ...players, 21 TKD practitioners, and 27 healthy control participants were tested. All of the participants underwent the Sensory Organization Test (SOT) and the Unilateral Stance Test (UST) on a Smart Equitest system. Results revealed that tennis players had higher SOT visual ratios than the control participants (p = .005), and TKD practitioners swayed more slowly in the UST than the control participants (p = .039). No differences (p > .05) were found in the composite score, somatosensory ratio, or vestibular ratio between the groups. Tennis players swayed less when they relied more on visual input to maintain balance, whereas TKD practitioners were more stable when standing on one leg. Parents may consider these sports as recreational activities for their children to develop specific balance abilities.
This single-blinded, randomized controlled study investigated the effects of practicing Tai Chi on the postural control and cognitive performance of older women while dual tasking.
Community-dwelling ...older women (n=31) were recruited from community centers. They were randomized to Tai Chi group (n=15, 12-form Yang style Tai Chi training) or a control group (n=16, general interest classes) for 16 weeks. Balance was tested in single leg stance after stepping down from a step, with and without a concurrent auditory response task (auditory Stroop test). Balance was measured by total sway path and sway area of subject's center of pressure (COP). The reaction time and the correctness of the auditory Stroop test were also measured.
Tai Chi subjects made fewer errors in auditory Stroop test under dual-task condition after Tai Chi training (p=0.01). They also showed significant decreases in the COP path (p<0.05) and the COP area (p<0.05) in their postural control after training in both single- and dual-task conditions. The subjects in the control group did not show any significant improvement in dual-task condition after the intervention.
Tai Chi training improves the cognitive and postural control performance of older women when dual tasking.
Adding abiraterone acetate with prednisolone (AAP) or docetaxel with prednisolone (DocP) to standard-of-care (SOC) each improved survival in systemic therapy for advanced or metastatic prostate ...cancer: evaluation of drug efficacy: a multi-arm multi-stage platform randomised controlled protocol recruiting patients with high-risk locally advanced or metastatic PCa starting long-term androgen deprivation therapy (ADT). The protocol provides the only direct, randomised comparative data of SOC+AAP versus SOC+DocP.
Recruitment to SOC+DocP and SOC+AAP overlapped November 2011 to March 2013. SOC was long-term ADT or, for most non-metastatic cases, ADT for≥2years and RT to the primary tumour. Stratified randomisation allocated pts 2:1:2 to SOC; SOC+docetaxel 75mg/m2 3-weekly×6+prednisolone 10mg daily; or SOC+abiraterone acetate 1000mg+prednisolone 5mg daily. AAP duration depended on stage and intent to give radical RT. The primary outcome measure was death from any cause. Analyses used Cox proportional hazards and flexible parametric models, adjusted for stratification factors. This was not a formally powered comparison. A hazard ratio (HR) <1 favours SOC+AAP, and HR>1 favours SOC+DocP.
A total of 566 consenting patients were contemporaneously randomised: 189 SOC+DocP and 377 SOC+AAP. The patients, balanced by allocated treatment were: 342 (60%) M1; 429 (76%) Gleason 8–10; 449 (79%) WHO performance status 0; median age 66years and median PSA 56ng/ml. With median follow-up 4years, 149 deaths were reported. For overall survival, HR=1.16 (95% CI 0.82–1.65); failure-free survival HR=0.51 (95% CI 0.39–0.67); progression-free survival HR=0.65 (95% CI 0.48–0.88); metastasis-free survival HR=0.77 (95% CI 0.57–1.03); prostate cancer-specific survival HR=1.02 (0.70–1.49); and symptomatic skeletal events HR=0.83 (95% CI 0.55–1.25). In the safety population, the proportion reporting≥1 grade 3, 4 or 5 adverse events ever was 36%, 13% and 1% SOC+DocP, and 40%, 7% and 1% SOC+AAP; prevalence 11% at 1 and 2 years on both arms. Relapse treatment patterns varied by arm.
This direct, randomised comparative analysis of two new treatment standards for hormone-naïve prostate cancer showed no evidence of a difference in overall or prostate cancer-specific survival, nor in other important outcomes such as symptomatic skeletal events. Worst toxicity grade over entire time on trial was similar but comprised different toxicities in line with the known properties of the drugs.

Coronavirus HKU15 is a deltacoronavirus that was discovered in fecal samples of pigs in Hong Kong in 2012. Over the past three years, Coronavirus HKU15 has been widely detected in pigs in ...East/Southeast Asia and North America and has been associated with fatal outbreaks. In all such epidemiological studies, the virus was generally only detected in fecal/intestinal samples. In this molecular epidemiology study, we detected Coronavirus HKU15 in 9.6% of the nasopharyngeal samples obtained from 249 pigs in Hong Kong. Samples that tested positive were mostly collected during winter. Complete genome sequencing of the Coronavirus HKU15 in two nasopharyngeal samples revealed quasispecies in one of the samples. Two of the polymorphic sites involved indels, but the other two involved transition substitutions. Phylogenetic analysis showed that the two nasopharyngeal strains in the present study were most closely related to the strains PDCoV/CHJXNI2/2015 from Jiangxi, China, and CH/Sichuan/S27/2012 from Sichuan, China. The outbreak strains in the United States possessed highly similar genome sequences and were clustered monophyletically, whereas the Asian strains were more diverse and paraphyletic. The detection of Coronavirus HKU15 in respiratory tracts of pigs implies that in addition to enteric infections, Coronavirus HKU15 may be able to cause respiratory infections in pigs and that in addition to fecal-oral transmission, the virus could possibly spread through the respiratory route. The presence of the virus in respiratory samples provides an alternative clinical sample to confirm the diagnosis of Coronavirus HKU15 infection. Quasispecies were unprecedentedly observed in the 5′-untranslated region of coronavirus genomes.

This study explored the immediate effects of Tai Chi (TC) training on attention and meditation, perceived stress level, heart rate, oxygen saturation level in blood, and palmar skin temperature in ...late middle-aged adults. Twenty TC practitioners and 20 nonpractitioners volunteered to join the study. After baseline measurements were taken, the TC group performed TC for 10 minutes while their cognitive states and cardiovascular responses were concurrently monitored. The control group rested for the same duration in a standing position. Both groups were then reassessed. The participants’ attention and meditation levels were measured using electroencephalography; stress levels were measured using Perceived Stress Scale; heart rate and blood oxygenation were measured using an oximeter; and palmar skin temperature was measured using an infrared thermometer. Attention level tended to increase during TC and dropped immediately thereafter (p<0.001). Perceived stress level decreased from baseline to posttest in exclusively the TC group (p=0.005). Heart rate increased during TC (p<0.001) and decreased thereafter (p=0.001). No significant group, time, or group-by-time interaction effects were found in the meditation level, palmar skin temperature, and blood oxygenation outcomes. While a 10-minute TC training could temporarily improve attention and decrease perceived stress levels, it could not improve meditation, palmar skin temperature, or blood oxygenation among late middle-aged adults.
