We reviewed thirty-seven hands of 26 patients with radial ray deficiency. Twelve patients (46%) had associated anomalies in extremities and/or other systems. Following Blauth's method, we classified ...patients into five groups. There was a positive correlation between the severity of the dysplasia of the thumb and the radius. Twelve hands of 10 patients were surgically treated. The mean follow-up time was 45 months. Only six hands had satisfactory cosmesis and function after surgery. We discuss the difficulty of treatment for severe cases.
A case of destructive lesion due to sarcoidosis in the wrist joint is presented. The rarity of this site of occurrence and the rapid progress of this lesion are discussed.
A total of 523 Japanese patients with terminal limb congenital malformations were reviewed for the characteristic features, and especially for type correlation between hand and foot anomalies. There ...were 281 cases of hand involvement alone, 149 cases of foot involvement alone, and 93 cases of combined hand and foot involvement. Polydactyly was commonly preaxial in the hand and postaxial in the foot, in contrast to the well-known postaxial dominance in the hand among whites. Hand anomalies were likely to show the same Swanson type of anomaly as the foot, with the affected digital ray likely to be identical in both hand and foot.
The Telescope Array(TA) experiment located at western desert in Utah USA (N39.3,W112.9) is designed for observation of air shower from extreme high energy cosmic rays. The TA detector consists of 2 ...types of detector to enable a cross check on systematic difference from the two main methods of observation for the energy region. One is a Fluorescence detector (FD) for detecting fluorescence light from air shower and another is surface detector (SD) array for detecting air shower particles at ground level. Each SD consists of 2 layers of plastic scintillator with 3m
2 of surface and more sensitive to electromagnetic component in air shower. The full operation using 3FD stations and full SD array has started. Here we present the updated status of Telescope Array experiment.
For appraisal of anterior wedge-shaped grafts for the humpback deformity of the scaphoid, a retrospective study of 27 cases with old scaphoid fractures or non-unions was carried out. 11 cases were ...treated with Herbert screw fixation and anterior wedge-shaped graft and the other cases with other methods. For the assessment of carpal alignment, radio-lunate and scapho-lunate angles were measured with peri-operative radiographs. For the clinical assessment, the scoring system of Cooney was applied. In 25 cases, primary bone union was obtained with one attempt and in two cases, with the second operation. Union was achieved in a mean of 3.4 months. The post-operative wrist score ranged from 65 to 100 with an average 81.2 points. There was a statistically significant relationship between the wrist score and the post-operative scapho-lunate angulation of the affected wrist. The humpback deformity of scaphoid non-union should be treated precisely with carpal realignment surgery or anterior wedge-shaped bone graft.
We treated a patient with avascular necrosis of the carpal scaphoid associated with progressive systemic sclerosis. Aetiology and histology of this unusual condition are discussed in relation to the ...underlying disease and the anatomical characteristics of the carpal scaphoid. Resection of the carpal scaphoid and replacement were successful.
The tuberculin skin test for immunologic diagnosis of Mycobacterium tuberculosis infection has many limitations, including being confounded by bacillus Calmette-Guérin (BCG) vaccination or exposure ...to nontuberculous mycobacteria. M. tuberculosis-specific antigens that are absent from BCG and most nontuberculous mycobacteria have been identified. We examined the use of two of these antigens, CFP-10 and ESAT-6, in a whole blood IFN-gamma assay as a diagnostic test for tuberculosis in BCG-vaccinated individuals. Because of the lack of an accurate standard with which to compare new tests for M. tuberculosis infection, specificity of the whole blood IFN-gamma assay was estimated on the basis of data from people with no identified risk for M. tuberculosis exposure (216 BCG-vaccinated Japanese adults) and sensitivity was estimated on the basis of data from 118 patients with culture-confirmed M. tuberculosis infection who had received less than 1 week of treatment. Using a combination of CFP-10 and ESAT-6 responses, the specificity of the test for the low-risk group was 98.1% and the sensitivity for patients with M. tuberculosis infection was 89.0%. The results demonstrate that the whole blood IFN-gamma assay using CFP-10 and ESAT-6 was highly specific and sensitive for M. tuberculosis infection and was unaffected by BCG vaccination status.
