The COVID-19 pandemic led to rapid adoption of telemedicine for the care of youth with type 1 diabetes (T1D). We assessed the utility of a primarily virtual care model by comparing glucometrics from ...a pediatric sample with T1D using continuous glucose monitoring (CGM) both before and during the pandemic.
Pediatric patients aged 1 to 17 years with T1D duration ≥ 1 year if ≥ 6 years old or ≥ 6 months if < 6 years old, with ≥ 1 visit with recorded CGM data both prepandemic (April 1, 2019-March 15, 2020) and during the pandemic (April 1, 2020-March 15, 2021) were included. Data were extracted from the electronic health record.
Our sample comprised 555 young people (46% male, 87% White, 79% pump-treated), mean age 12.3 ± 3.4 years, T1D duration 5.9 ± 3.5 years, baseline glycated hemoglobin A1c 8.0 ± 1.0% (64 ± 10.9 mmol/mol). Diabetes visit frequency increased from 3.8 ± 1.7 visits/prepandemic period to 4.3 ± 2.2 visits/pandemic period (P < 0.001); during pandemic period, 92% of visits were virtual. Glucose management indicator (GMI) improved slightly from 7.9% (63 mmol/mol) prepandemic to 7.8% (62 mmol/mol) during the pandemic (P < 0.001). Those with equal or greater visit frequency (n = 437 79% of sample) had significant improvement in GMI (8.0% to 7.8% 64 to 62 mmol/mol, P < 0.001), whereas those with lower visit frequency did not (7.8 62 mmol/mol, P = 0.86).
Children and adolescents with T1D using CGM before and during the pandemic showed an overall increase in visit frequency using primarily telemedicine-based care and improved CGM glucometrics. Further research is needed to understand factors associated with successful use of telemedicine for pediatric T1D.
Aim: The COVID-19 pandemic called for rapid deployment of telehealth. To assess utility of telehealth in pediatric T1D, we compared CGM glucose values in youth with T1D using CGM before and during ...the pandemic. Methods: The EHR provided visit frequency and CGM metrics. Data during pandemic (3/16/20 to 10/29/20) were compared with data from the same calendar months in 2019. Youth (N=469) using CGM and seen in both periods were included based on pre-pandemic characteristics: age 1-18 yr, T1D duration ≥6 mo (if age <6) or ≥1 yr (if age ≥6 yr). CGM metrics included mean and SD glucose, coefficient of variation (CV), and glucose management index (GMI). Each youth's CGM data were averaged in each time period and compared in paired analyses. Results: Youth (46% male) had a mean age 12.4±3.5 yr, T1D duration 6.1±3.6 yr, HbA1c 7.8±0.9%; 80% were pump-treated. Pre-pandemic, 0.1% of visits were telehealth, increasing to 99.6% during pandemic. Mean number of visits during each 7-mo period increased from 2.5 in 2019 to 2.7 in 2020 (p=.006). CGM metrics generally improved during the pandemic period (Table). Those whose visit frequency increased with telehealth had the greatest improvement in CGM glucose, from 192±72 to 183±67 (p<.001). Conclusions: Telehealth allowed increased visit frequency and improved CGM glucose metrics, supporting non-inferiority and potential benefit of telehealth for youth with T1D using CGM.

A total of 80 children aged 2.0-5.9 years used the investigational system in a single-arm study for 13 weeks following 14 days of baseline data collection with their usual therapy.
There were no episodes of severe hypoglycemia or diabetic ketoacidosis. By study end, HbA1c decreased by 0.55% (6.0 mmol/mol) (P < 0.0001). Time with sensor glucose levels in target range 70-180 mg/dL increased by 10.9%, or 2.6 h/day (P < 0.0001), while time with levels <70 mg/dL declined by median 0.27% (P = 0.0204).
Use of the automated insulin delivery system was safe, and participants experienced improved glycemic measures and reduced hypoglycemia during the study phase compared with baseline.
The Medtronic advanced hybrid closed-loop (AHCL) and MiniMed™ 670G hybrid closed-loop (HCL) systems provide the option to temporarily increase the glucose target to 150 mg/dL (8.3 mmol/L). This ...analysis investigated the efficacy of the AHCL compared with that of the HCL after the use of this setting. Data from 60 participants in the Fuzzy Logic Automated Insulin Regulation (FLAIR) study were used to compare the AHCL and HCL systems after the use of the temporary target (TT), and during analogous periods where this setting was not used. Differences in time in range 70-180 mg/dL between the systems were similar after the use of the TT setting and during analogous non-TT periods (interaction
= 0.87). Similar trends were observed for mean glucose, percentage time >180 mg/dL, and percentage time >250 mg/dL. Differences between AHCL and HCL systems were similar after the use of the TT setting compared with those of non-TT periods. ClinicalTrials.gov NCT03040414.
We recently reported that use of an "advanced" hybrid closed-loop system reduced hyperglycemia without increasing hypoglycemia compared to a first-generation system. The aim of this analysis was to ...evaluate whether this improved performance was specifically related to better mealtime glycemic control.
We conducted a secondary analysis of postprandial glycemic control in an open-label, multinational, randomized crossover trial of 112 participants with type 1 diabetes, aged 14-29, of the Medtronic MiniMed™ 670G hybrid closed-loop system (670G) versus the Medtronic advanced hybrid closed-loop (AHCL) system, for 12 weeks each. We compared glycemic and insulin delivery metrics over a 3 h horizon across all meals to assess system performance and outcomes.
Overall meal size and premeal insulin on board were similar during run-in and between 670G and AHCL arms. Compared with 670G arm, premeal, peak, and mean glucose levels were numerically lower in the AHCL arm (167 ± 23, 231 ± 23, and 177 ± 20 mg/dL vs. 175 ± 23, 235 ± 23, and 180 ± 19 mg/dL, respectively), with a trend to lower hyperglycemia level 2 in AHCL arm. Adjusting for premeal glucose level, all postmeal outcomes between 670G and AHCL were statistically similar. Prandial insulin delivery also was similar in both treatment arms (21 ± 9 vs. 23 ± 10 U), with a shift in basal/bolus ratio from 28%/71% in 670G arm to 20%/80% in AHCL arm.
Reduced hyperglycemia with AHCL compared to 670G was not related to early postprandial glycemic excursions after adjusting for premeal glucose level (<3 h after meal), but likely to later (>3 h) postprandial or overnight improvements. Further refinements to mealtime bolus algorithms and strategies may more optimally control prandial glycemic excursions.
The discovery of Toll-like receptors (TLRs) in Drosophila and their subsequent discovery in humans has piqued the scientific community's interest in their evolution. Marine invertebrates are now ...starting to become model organisms for the evolution of immunity and for immunological disease therapeutics. This study sought to determine the identification of a TLR receptor in the comb jellyfish and moon jellyfish by using probes and antibodies specific to the TIR (intracellular) and LRR (extracellular) domain of the canonical Drosophila TLR1 receptor. Western blots using these two antibodies as well as competitive inhibition assays were run. Southern blots using the Drosophila TIR and LRR domains as probes were also run. The results of these experiments suggest that both comb and moon jellyfish may have one or several TIR-domain containing genes. Data such as these can be applied to research developments in the field of evolutionary invertebrate immunology and may be used to assess immune function and disease resistance in these animals.
Lorsqu'un individu tente d'expliquer ou de prédire le comportement d'autrui, son jugement risque d'être fortement influencé par les informations dont il dispose sur les états mentaux (désirs, ...croyances, intentions) de cette personne. C'est ce qu'étudie principalement le domaine de recherche des théories de la pensée. Par ailleurs, un individu peut également expliquer ou anticiper le comportement d'une personne selon les informations qu'il détient sur ses traits de personnalité. Néanmoins, peu de tentatives ont porté sur l'intégration des résultats de ces deux traditions de recherche. L'objectif de cette étude est donc de mettre en relation ces deux domaines de recherche, en examinant l'importance relative accordée aux états mentaux et aux traits chez les enfants et les adultes lorsque ces deux sources d'informations sont contradictoires et qu'elles mènent à des prédictions différentes de comportement. Selon Piaget (1970), ce n'est pas avant que l'enfant ait acquis le concept de conservation qu'il peut comprendre que les attributs d'une personne ou d'un objet peuvent demeurer stables malgré une modification de l'apparence physique. Avant l'âge de 7-8 ans, les enfants auraient donc tendance à privilégier les attributs externes dans la description et l'explication du comportement (Miller et Aloise, 1989). Les études antérieures montrent que lorsque plusieurs sources d'informations sont disponibles, les jeunes enfants ont tendance à accorder davantage d'importance à l'information portant sur l'apparence physique (Hoffner et Cantor, 1985) et sur la situation (Newman, 1991) qu'aux traits. Les enfants plus vieux et les adultes accordent à l'inverse davantage d'importance aux traits. L'hypothèse est donc que l'importance accordée aux traits et aux croyances varie selon l'âge. Puisque les croyances immédiates découlent des éléments du contexte, il est attendu que les jeunes enfants accordent plus d'importance aux croyances. À l'opposé, les enfants plus âgés devraient avoir un biais envers les traits. Finalement, les adultes devraient accorder de l'importance tant aux croyances qu'aux traits dans la prédiction du comportement et ainsi avoir un jugement plus nuancé. Des histoires sont présentées à des enfants de 6 ans, de 9 ans et à des adultes. Dans chaque histoire, le trait d'un personnage est présenté, de même que sa croyance ponctuelle à l'égard de la situation qu'il vit. Les histoires sont conçues de sorte que le trait du personnage est en contradiction avec sa croyance immédiate. Il se retrouve donc devant un dilemme comportemental où il doit faire un choix soit en lien avec son trait de personnalité, soit en lien avec sa croyance. Les participants doivent ainsi prédire le comportement du personnage Les résultats montrent que les groupes d'âge se différencient de façon significative quant au type de prédiction. Les adultes semblent effectivement avoir un jugement plus nuancé que les enfants lors de la prédiction du comportement. Ils accordent une plus grande importance aux croyances, sans toutefois négliger les traits. Par contre, à l'inverse des hypothèses, les enfants de 6 ans effectuent une majorité de prédictions selon les traits et les enfants de 9 ans effectuent presque autant de prédictions selon les traits que selon les croyances. Les résultats présentent donc plutôt une tendance développementale croissante à privilégier les croyances lorsqu'elles sont contradictoires aux traits. Avec l'âge, les participants apportent de plus en plus de nuances à leurs réponses et se montrent capables de tenir compte à la fois du trait et de la croyance.
Selon Rholes, Newman et Ruble (1990) et Yuill et Pearson (1998), les jeunes enfants utiliseraient une stratégie de régularité comportementale lors de la prédiction du comportement. Ils comprennent que les comportements sont associés à des étiquettes et que ceux d'une même étiquette ont tendance à se répéter. Bien que cette stratégie leur permette dans plusieurs situations d'arriver à effectuer des prédictions appropriées selon les traits, ils ne seraient pas en mesure de comprendre que les traits constituent des mécanismes internes et causaux du comportement et que ceux-ci sont étroitement reliés aux désirs et aux croyances. La compréhension de la relation entre les traits et les états mentaux (désirs, croyances) n'est donc pas nécessaire à l'utilisation d'une telle stratégie. Les enfants de 6 ans de la présente étude pourraient donc l'avoir appliquée comme si elle correspondait à une règle absolue, c'est-à-dire qu'un individu se comportera de la même façon que par le passé. Les jeunes enfants ne sont probablement pas en mesure de saisir qu'un état mental ponctuel et contradictoire (croyance) peut modifier la règle de la régularité comportementale et engendrer un comportement inverse à celui qui aurait été émis si les comportements étaient toujours en lien avec les traits. Ils ne comprendraient pas encore très bien qu'à la règle existent des exceptions. Par contre, avec l'âge, sa pensée perdrait de sa rigidité. D'ailleurs, ce n'est qu'avec le temps que l'enfant peut admettre l'existence simultanée d'émotions (Harris, 1983) ou de traits de valence différente (Hand, 1982). Il est donc probable qu'il lui faille aussi du temps pour admettre l'existence d'états internes (trait et croyance) contradictoires susceptibles de générer des comportements de valence différente. La présente étude ajoute un élément important aux connaissances sur les théories de la pensée. Bien que bon nombre d'études aient porté sur l'acquisition d'une théorie de la pensée chez les enfants d'âge préscolaire, le développement de la compréhension des états mentaux et des traits semble se poursuivre au-delà de cet âge. Les résultats suggèrent en effet qu'un changement conceptuel important puisse s'opérer entre 6 ans et 9 ans. ABSTRACTThis study was aimed at providing normative data for the Mini-Mental State Examination (MMSE). The norms were built from a sample (n = 2409) of community-dwelling French speaking residents ...from Québec aged 65 and older. The analyses indicated that socio-demographic variables such as education level, age, and gender of individuals influenced significantly the scores of older adults on the MMSE. More precisely, MMSE scores increased with education level and decreased with age. Moreover, women had significantly higher scores than men. On this basis, distinct tables of normative data were produced for women and men. In each table, the MMSE scores corresponding to percentiles 5, 10, 15 and 50 were identified according to four age categories and three education levels. Overall, the use of the present normative data by clinicians will improve their accuracy in detecting cognitive impairment in older adults from Québec.
