Objective The deterioration of seminal parameters in men with varicocele suggests a vascular lesion producing progressive damage to the testes over time. The aim of this study was to analyze the ...relationship between the duration of infertility in infertile men with varicocele and the postoperative sperm counts to determine whether the infertility period is a parameter for predicting the pregnancy rate. Methods The medical records of 574 patients with palpable varicoceles were retrospectively reviewed. According to the duration of unprotected intercourse without conception (infertility period), the patients were divided into 4 groups: group 1, 0-3 years (n = 353); group 2, 3-6 years (n = 132); group 3, 6-9 years (n = 47); and group 4, >9 years (n = 42). The mean age of these groups was 28.5 ± 5.3 years (range 19-58), 30 ± 4.9 years (range 22-52), 31.6 ± 4.1 years (range 25-42), and 36.4 ± 4.2 years (range 29-50), respectively. The preoperative and postoperative total motile sperm counts (TMCs) and postoperative pregnancy rates among these groups were compared statistically, including multivariate analysis. Results An overall spontaneous pregnancy rate of 41.3% was achieved after microscopic varicocelectomy. The greatest pregnancy rate was achieved in group 1 (43.9%) and the lowest pregnancy rate in group 4 (31.7%; P < .05). The mean postoperative TMC in these groups was 34.9 ± 3.5 × 106 and 24.6 ± 1.7 × 106 , the only statistically significant difference in terms of postoperative TMC and pregnancy among the 4 groups. Conclusions According to our data, the pregnancy rates and TMCs after varicocelectomy correlate negatively with the infertility period. This finding supports the perspective that varicocele has deleterious effects on the testicular biology over time.

Objective: To investigate the regional metabolite abnormalities and changes after treatment in patients with OCD with autogenous and reactive obsessions.
Method: We assessed right anterior cingulate cortex (ACC) and amygdala–hippocampal region (Am + Hpp) N‐acetyl‐aspartate (NAA), choline (Cho) and creatine (Cr) concentrations and NAA/Cr and Cho/Cr ratios using single‐voxel proton magnetic resonance spectroscopy in 15 patients with autogenous obsessions (OCD‐A), 15 patients with reactive obsessions (OCD‐R) and 15 healthy controls (HC). Measurements were repeated after 16 weeks of fluoxetine treatment.
Results: Baseline ACC NAA/Cr ratios of both OCD groups were significantly lower than HC. OCD‐A group had significantly lower baseline NAA/Cr ratios in the Am + Hpp than other groups. These differences were more likely to be explained by higher Cr levels in ACC. We found no significant differences and changes for Cho levels and Cho/Cr ratios between groups and within groups. Significant increase in NAA/Cr ratios of OCD‐A group found in the Am + Hpp was more likely to be explained by increased NAA levels. No significant changes were found in ACC NAA/Cr ratios.
Conclusion: While disturbed energy metabolism in ACC might reflect a common pathology in patients with OCD regardless of symptom dimension, alterations in mesiotemporal lobe are more likely for autogenous obsessions.

This study included a comparison of the baseline Sn-3.5Ag eutectic to one neareutectic ternary alloy, Sn-3.6Ag-1.0Cu and two quaternary alloys, Sn-3.6Ag-1.0Cu-0.15Co and Sn-3.6Ag-1.0Cu-0.45Co, to ...increase understanding of the beneficial effects of Co on Sn-Ag-Cu solder joints cooled at 1-3 degree C/sec, typical of reflow practice. The results indicated that joint microstructure refinement is due to Co-enhanced nucleation of the Cu sub(6)Sn sub(5) phase in the solder matrix, as suggested by Auger elemental mapping and calorimetric measurements. The Co also reduced intermetallic interface faceting and improved the ability of the solder joint samples to maintain their shear strength after aging for 72 hr at 150 degree C. The baseline Sn-3.5Ag joints exhibited significantly reduced strength and coarser microstructures.
The aim of this study was to investigate the role of diffusion-weighted MR imaging (DWI) in the diagnosis of gastric tumors by means of measuring the apparent diffusion coefficient (ADC) values of ...these lesions, and making a comparison with the endoscopic biopsy results.
Seventy patients having gastric tumor constituted the case group. For the control group 30 healthy individuals were included. Abdominal MRI examinations were performed with a 1,5 Tesla unit. DWI examinations were obtained by single shot spin echoplanar imaging. The ADC was measured based on the tissue of the gastric tumoral entities and normal gastric mucosa in the control group.
Mean ADC values were 0,84 +/- 0,17 x 10(-3) mm2/s and 1,79 +/- 0,08 x 10 mm2/s in gastric tumor group and in control group, respectively, being statistically significant (p<0.05).There was no significance among ADC values of adenocarcinoma subgroups. The comparison of the ADC values in the adenocarcinoma and lymphoma cases were also found to be statistically significant.
DWI is beneficial in the diagnosis of malignant gastric lesions by the aid of ADC measurements. Although ADC quantification seems to be invaluable in the evaluation of histopathologic subgroups of adenocarcinoma, it can help in the diagnosis of gastric lymphoma.
A randomised controlled trial was conducted to investigate efficacy of paracetamol and dexketoprofen trometamol for perineal pain relief after perineal repair. Subjects were randomly assigned to ...receive two doses of either 50 mg of intravenous dexketoprofen trometamol via slow i.v. infusion (Group I, n = 49) or 1,000 mg of paracetamol via intravenous infusion (Group II, n = 46). The main outcome measure was a VAS (visual analogue scale) for pain recorded at 1 h (VAS 1). A total of 82 patients were included in the final analysis (Group I, n = 41; Group II, n = 41). There was no difference among groups in terms of pain scores at the beginning (VAS 0). The pain was decreased in 70% of the patients in Group I and in 62% of the patients in Group II (p = 0.502). Both paracetamol and dexketoprofen are effective in perineal pain relief after episiotomy or perineal tear repair.
Aim To compare ex vivo root canal preparation with conventional stainless steel K‐files and Hero 642 rotary Ni–Ti instruments.
Methodology Mesiobuccal canals of 20 maxillary first molars (with ...angles of curvature between 25° and 35°) were used. After preparation with Hero 642 rotary instruments and stainless steel K‐files, the amount of transportation that occurred was assessed using computed tomography. The teeth were scanned by computed tomography before instrumentation. One millimetre thick slices were prepared from the apical end point to the pulp chamber. The first two sections were 3 mm from the apical end of the root (apical level) and 3 mm below the orifice (coronal level). A further section (mid‐root level) was recorded, dividing the distance between the sections of apical and coronal levels into two equal lengths. Ten teeth were instrumented using Hero 642 rotary instruments and another 10 teeth were instrumented using stainless steel K‐files. Following the completion of the instrumentation, the teeth were again scanned and compared with the cross‐sectional images taken prior to canal preparation. Amount of transportation and centreing ability was assessed. Student's t‐test was used for statistical analysis.
Results Less transportation occurred with Hero 642 rotary instruments than stainless steel K‐files at the mid‐root and coronal levels (mid‐root: P < 0.05 and coronal: P < 0.001). Hero 642 rotary instruments had better centreing ability than K‐files at all three levels (apical: P < 0.05, mid‐root: P < 0.05 and coronal: P < 0.001).
Conclusions Hero 642 rotary instruments transported canals less, especially at the middle and coronal thirds of the root canals than stainless steel K‐files. Hero 642 instruments had better centreing ability.
Abstract Purpose To assess the value of transvaginal sonographic elastography (TSE) in discriminating between endometrial hyperplasia and endometrial carcinoma. Materials and methods A total of 61 ...women with post-menopausal hemorrhage and/or normal TSE were included. There were 32 women (mean age: 53.1 ± 14.1 years) with endometrial hyperplasia, 14 women (mean age: 60.0 ± 14.0 years) with endometrial carcinoma and 15 women (mean age: 51.9 ± 7.8 years) with no endometrial disease who served as a control group. The strain index (SI) values obtained during TSE in each group were compared using Mann-Whitney U test and Kruskal-Wallis analysis of variance test. Results The mean SI values were 0.80 (range: 0.30–1.30) in the endometrial hyperplasia group, 1.80 (range: 0.80–3.20) in the endometrial carcinoma group and 1.00 (range: 0.50–4.00) in the control group. No significant differences were found between endometrial hyperplasia group and control group, but significant differences were found between endometrial carcinoma and hyperplasia groups and between endometrial carcinoma and control groups ( P < 0.0001). TSE had a sensitivity of 81.3%, a specificity of 100%, a positive predictive value of 100% and a negative predictive value of 70% in differentiating endometrial carcinoma from endometrial hyperplasia. The area under ROC curve (AUC) to distinguish between endometrial carcinoma and endometrial hyperplasia was 0.933 (95% CI, 0.853–1.000) using a threshold SI value of 1.05. The AUC to distinguish between endometrial carcinoma and control was 0.881 (95% CI, 0.735–1.000) using a threshold SI value of 1.15. Conclusion Our results indicate that TSE can provide important information that help discriminate between endometrial carcinoma and endometrial hyperplasia.

Objective To evaluate the influence of the abrupt withdrawal of ovarian function on glucose tolerance by studying premenopausal women before and after oophorectomy.
Methods Thirty premenopausal women ...who needed hysterectomy for benign reasons volunteered for the study in our hospital in Istanbul, Turkey. An oral glucose tolerance test (OGTT) was performed before surgery and 3, 6 and 12 months after surgery. Fasting glucose, fasting insulin levels and insulin/glucose indexes, HOMA indexes, insulin and glucose levels were measured during OGTT.
Results The mean fasting and 2-h glucose levels of the women did not change significantly during the 12 months of follow-up. However, the glucose levels during the glucose tolerance tests changed significantly after surgery (p < 0.05). Insulin responses to the glucose tolerance test also increased significantly (p < 0.005). Insulin/glucose indexes were significantly higher after surgery (p < 0.005). Type 2 diabetes mellitus was detected in five women and impaired glucose tolerance was detected in 12 of the 30 patients after surgery in the 12-month period.
Conclusion The impairment of carbohydrate metabolism due to an abrupt decrease in natural estrogen levels should be considered before removing the ovaries during hysterectomy in premenopausal patients.
