The perinatal consequences of SARS-CoV-2 infection are still largely unknown. This study aimed to describe the features and outcomes of pregnant women with or without SARS-CoV-2 infection after the ...universal screening was established in a large tertiary care center admitting only obstetric related conditions without severe COVID-19 in Mexico City. This retrospective case-control study integrates data between April 22 and May 25, 2020, during active community transmission in Mexico, with one of the highest COVID-19 test positivity percentages worldwide. Only pregnant women and neonates with a SARS-CoV-2 result by quantitative RT-PCR were included in this study. Among 240 pregnant women, the prevalence of COVID-19 was 29% (95% CI, 24% to 35%); 86% of the patients were asymptomatic (95% CI, 76%-92%), nine women presented mild symptoms, and one patient moderate disease. No pregnancy baseline features or risk factors associated with severity of infection, including maternal age > 35 years, Body Mass Index >30 kg/m2, and pre-existing diseases, differed between positive and negative women. The median gestational age at admission for both groups was 38 weeks. All women were discharged at home without complications, and no maternal death was reported. The proportion of preeclampsia was higher in positive women than negative women (18%, 95% CI, 10%-29% vs. 9%, 95% CI, 5%-14%, P<0.05). No differences were found for other perinatal outcomes. SARS-CoV-2 test result was positive for nine infants of positive mothers detected within 24h of birth. An increased number of infected neonates were admitted to the NICU, compared to negative neonates (44% vs. 22%, P<0.05) and had a longer length of hospitalization (2 2-18 days vs. 2 2-3 days, P<0.001); these are potential proxies for illness severity. This report highlights the importance of COVID-19 detection at delivery in pregnant women living in high transmission areas.

(1) This study aimed to evaluate characteristics, perinatal outcomes, and placental pathology of pregnant women with or without SARS-CoV-2 infection in the context of maternal PCR cycle threshold ...(CT) values. (2) This was a retrospective case-control study in a third-level health center in Mexico City with universal screening by RT-qPCR. The association of COVID-19 manifestations, preeclampsia, and preterm birth with maternal variables and CT values were assessed by logistic regression models and decision trees. (3) Accordingly, 828 and 298 women had a negative and positive test, respectively. Of those positive, only 2.6% of them presented mild to moderate symptoms. Clinical characteristics between both groups of women were similar. No associations between CT values were found for maternal features, such as pre-gestational BMI, age, and symptomatology. A significantly higher percentage of placental fibrinoid was seen with women with low CTs (<25; p < 0.01). Regarding perinatal outcomes, preeclampsia was found to be significantly associated with symptomatology but not with risk factors or CT values (p < 0.01, aOR = 14.72). Moreover, 88.9% of women diagnosed with COVID-19 at <35 gestational weeks and symptomatic developed preeclampsia. (4) The data support strong guidance for pregnancies with SARS-CoV-2 infection, in particular preeclampsia and placental pathology, which need further investigation.
The current COVID-19 pandemic has generated a series of changes in the daily routines of people, including children and teenagers, in an unprecedented way, which constitutes a global challenge in ...public health. Social isolation has been a prophylactic measure to prevent the spread of the virus; however, it has generated negative impacts on the physical and emotional health of parents, caregivers, children and teenagers around the world. The objective of the present study was to evaluate the effects of confinement caused by the COVID-19 pandemic at the level of nutritional status, dietary and behavioural patterns of elementary school children and teenagers in a small town of Colombia. Anthropometric parameters such as BMI Z-score, waist circumference and waist/height ratio were evaluated in 266 school children and teenagers. A questionnaire with socio-demographic, clinical and lifestyle characteristics and the KIDMED were applied to learn about nutritional aspects. A total of 102 students (38⋅3 %) were classified as having altered nutritional status, being 39 (14⋅7 %) classified as overweight and 36 (13⋅5 %) with obesity. The prevalence of high adherence to the Mediterranean diet was 12 %, 95 % CI (0⋅08, 0⋅16). Overweight was more prevalent in women (26/39, 66⋅7 %; P = 0⋅0439), and obesity was discreetly more frequent in men (19/36, 52⋅7 %; P = 0⋅7193). We observed a worrying nutritional, dietary and behavioural situation in the children and teenagers studied during the confinement associated with the COVID-19 pandemic. This unveils the need to establish strategies and/or public policies in our town that help to promote an adequate biopsychosocial development of the paediatric patient and their family group.
Dysbiosis plays an important role in the development of bacterial infections in the gastric mucosa, particularly Helicobacter pylori. The international guidelines for the treatment of H. pylori ...infections suggest standard triple therapy (STT). Nevertheless, because of the increasing resistance rates to clarithromycin, metronidazole has been widely considered in several countries. Unfortunately, the non-justified administration of antibiotics induces dysbiosis in the target organ. We characterized the gastric microbiota of patients diagnosed with follicular gastropathy and pangastropathy attributed to H. pylori infection, before and after the administration of STT with metronidazole. Dominant relative abundances of Cutibacterium were observed in pre-treatment patients, whereas H. pylori was observed at <11%, suggesting the multifactor property of the disease. The correlation of Cutibacterium acnes and H. pylori with gastric infectious diseases was also evaluated using quantitative real-time polymerase chain reaction. The dominance of C. acnes over H. pylori was observed in gastritis, gastropathies, and non-significant histological alterations. None of the microorganisms were detected in the intestinal metaplasia. Post-treatment alterations revealed an increase in the relative abundances of Staphylococcus, Pseudomonas, and Klebsiella. Non-H. pylori gastrointestinal bacteria can be associated with the initiation and development of gastric diseases, such as pathobiont C. acnes.

Abstract BK virus nephropathy in kidney transplantation is widely recognized as an important cause of graft dysfunction and loss. In the case of transplants of organs other than kidney, BK virus ...nephropathy in native kidneys has been recognized as a cause of chronic kidney disease, which is related with immunosuppression; however, the diagnosis is usually late because the renal dysfunction is attributed to other causes, such as toxicity by anticalcineurinic drugs, interstitial nephritis due to medications, hemodynamic changes, diabetes, hypertension, etc. We report a case of BK virus nephropathy in a patient who underwent heart transplantation due to peripartum cardiomyopathy. The kidney biopsy reported active chronic tubulointerstitial nephritis associated with late stage polyomavirus nephritis and the blood viral load for BK virus was positive (logarithm 4.5). The immunosuppressive treatment was reduced, and after two years of follow-up, the patient had stable renal function with a serum creatinine of 2.5 mg/dL (GFR of 23.4 mL/min/1.73m2). We recommend that the BK virus be considered as a cause of renal dysfunction in heart transplant recipients, with the aim of detecting its replication in time to reduce immunosuppressive therapy before irreversible compromise of renal function may manifest.
Resumo A nefropatia pelo vírus BK no transplante renal é amplamente reconhecida como uma importante causa de disfunção e perda do enxerto. No caso de transplantes de órgãos que não sejam rins, a nefropatia pelo vírus BK em rins nativos tem sido reconhecida como uma causa de doença renal crônica, que está relacionada com imunossupressão; entretanto, o diagnóstico é geralmente tardio porque a disfunção renal é atribuída a outras causas, tais como toxicidade por drogas anticalcineurínicas, nefrite intersticial devido a medicamentos, alterações hemodinâmicas, diabetes, hipertensão, etc. Relatamos um caso de nefropatia pelo vírus BK em um paciente que foi submetido a transplante cardíaco devido à cardiomiopatia periparto. A biópsia renal relatou nefrite túbulo-intersticial crônica ativa associada à nefrite por poliomavírus em estágio avançado e a carga viral sanguínea para o vírus BK foi positiva (logaritmo 4,5). O tratamento imunossupressor foi reduzido, e após dois anos de acompanhamento, o paciente apresentava função renal estável com creatinina sérica de 2,5 mg/dL (TFG de 23,4 mL/min/1,73m2). Recomendamos que o vírus BK seja considerado como uma causa de disfunção renal em receptores de transplante cardíaco, com o objetivo de detectar sua replicação a tempo de reduzir a terapia imunossupressora antes que um comprometimento irreversível da função renal possa se manifestar.
Introducción. La cromoblastomicosis es una micosis subcutánea, crónica y granulomatosa que ocurre en zonas endémicas y subtropicales con una incidencia de 1 caso cada 6800 habitantes. La evolución de ...sus manifestaciones clínicas es lenta; inicialmente las lesiones son nodulares, pero pueden progresar hasta convertirse en tumores de gran tamaño. El tratamiento de las lesiones iniciales suele incluir la resección quirúrgica y la administración concomitante de antimicóticos orales. Presentación del caso. Hombre de 59 años, agricultor, sin antecedentes personales relevantes que asistió al servicio de urgencias de un hospital de cuarto nivel de Medellín (Colombia) debido a una lesión tumoral en miembro superior que había aparecido 20 años antes (afectación desde la mano hasta el tercio proximal del brazo), contractura en dedos, muñeca y codo, pérdida de peso involuntaria y sudoración nocturna. Se realizaron biopsias de piel, prueba de cultivo fúngico, una tomografía computarizada y una resonancia magnética. Teniendo en cuenta los hallazgos en estas pruebas, se realizó manejo quirúrgico ablativo (amputación transhumeral proximal) y se inició terapia con antifúngicos sistémicos, con lo cual se obtuvo una evolución clínica favorable. Conclusión. Presentamos el caso de un hombre agricultor con cromoblastomicosis con limitación funcional grave del miembro superior derecho debido a la severidad de la infección y a la ausencia de tratamiento médico temprano, en el que la combinación de un tratamiento ablativo (amputación de la extremidad desde el tercio proximal del húmero) y farmacológico (antifúngicos sistémicos) permitió una resolución completa de la infección.

Introduction: Tenosynovitis is the main cause of pain in the upper limbs; it can be originated from infectious and non-infectious causes. 50% of reported cases are tuberculous tenosynovitis. When the ...rice body formation pathogenesis is uncertain after performing all extension studies, it is called idiopathic tenosynovitis. Clinical Case: A 79- year-old male patient with a pathological history of chronic ischemic heart disease treated with cardiac catheterization with stent placement comes to the Plastic Surgery Office due to the presence of multiple masses on the back of the right hand in zone V, VI and VII with 3 years of evolution, without significant accompanying symptoms. Physical examination revealed the presence of multiple non-painful mobile masses of approximately 4x5cm in the extensor zone V, VI and VII of the right hand. In the surgical exploration for biopsy, within the macroscopic characteristics, multiple converging and interconnected masses were found with granular content infiltrating sheath and tendon itself (II, III, IV and V fingers of the right hand). Posterior capsule of masses is in contact with intrinsic and interosseous muscles of the right hand with the appearance of rice bodies. At the moment, the patient shows a favorable evolution and continues in periodic controls. Conclusions: In this case, having no underlying diseases related to this pathology and having ruled out among the differential diagnoses, it was determined that the occurrence of rice body masses in an infiltrative manner in the tendons is of an idiopathic origin, so our patient will remain on long-term follow-up, but so far with favorable evolution.

Resumen La crisis renal es una complicación con importante morbilidad y mortalidad en pacientes con esclerodermia y una rara entidad en trasplante renal, que se puede presentar con manifestaciones ...clínicas muy variables, principalmente hipertensión arterial, falla renal aguda y compromiso multisistémico. A continuación se reporta el caso de una paciente con recurrencia tardía de la crisis esclerodérmica en el trasplante renal, que recibió tratamiento exitoso con inhibidores de la enzima convertidora de angiotensina.(Acta Med Colomb 2020; 45.
