Drug hypersensitivity may deprive patients of drug therapy, and occasionally no effective alternative treatment is available. Successful desensitization has been well documented in delayed drug ...hypersensitivity reactions. In certain situations, such as sulfonamide hypersensitivity in HIV‐positive patients or hypersensitivity to antibiotics in patients with cystic fibrosis, published success rates reach 80%, and this procedure appears helpful for the patient management. A state of clinical tolerance may be achieved by the administration of increasing doses of the previously offending drug. However, in most cases, a pre‐existent sensitization has not been proven by positive skin tests. Successful re‐administration may have occurred in nonsensitized patients. A better understanding of the underlying mechanisms of desensitization is needed. Currently, desensitization in delayed hypersensitivity reactions is restricted to mild, uncomplicated exanthems and fixed drug eruptions. The published success rates vary depending on clinical manifestations, drugs, and applied protocols. Slower protocols tend to be more effective than rush protocols; however, underreporting of unsuccessful procedures is very probable. The decision to desensitize a patient must always be made on an individual basis, balancing risks and benefits. This paper reviews the literature and presents the expert experience of the Drug Hypersensitivity Interest Group of the European Academy of Allergy and Clinical Immunology.
ABSTRACT
We present the JWST cycle 1 53.8 h medium program FRESCO, short for ‘First Reionization Epoch Spectroscopically Complete Observations’. FRESCO covers 62 arcmin2 in each of the two ...GOODS/CANDELS fields for a total area of 124 arcmin2 exploiting JWST’s powerful new grism spectroscopic capabilities at near-infrared wavelengths. By obtaining ∼2 h deep NIRCam/grism observations with the F444W filter, FRESCO yields unprecedented spectra at R ∼ 1600 covering 3.8–5.0 µm for most galaxies in the NIRCam field of view. This setup enables emission line measurements over most of cosmic history, from strong PAH lines at z ∼ 0.2–0.5, to Pa α and Pa β at z ∼ 1–3, He i and S iii at z ∼ 2.5–4.5, H α and N ii at z ∼ 5–6.5, up to O iii and H β for z ∼ 7–9 galaxies. FRESCO’s grism observations provide total line fluxes for accurately estimating galaxy stellar masses and calibrating slit-loss corrections of NIRSpec/MSA spectra in the same field. Additionally, FRESCO results in a mosaic of F182M, F210M, and F444W imaging in the same fields to a depth of ∼28.2 mag (5σ in 0${_{.}^{\prime\prime}}$32 diameter apertures). Here, we describe the overall survey design and the key science goals that can be addressed with FRESCO. We also highlight several, early science results, including: spectroscopic redshifts of Lyman break galaxies that were identified almost 20 yr ago, the discovery of broad-line active galactic nuclei at z > 4, and resolved Pa α maps of galaxies at z ∼ 1.4. These results demonstrate the enormous power for serendipitous discovery of NIRCam/grism observations.
The usefulness of a distance-dependent reduction of background error covariance estimates in an ensemble Kalman filter is demonstrated. Covariances are reduced by performing an elementwise ...multiplication of the background error covariance matrix with a correlation function with local support. This reduces noisiness and results in an improved background error covariance estimate, which generates a reduced-error ensemble of model initial conditions. The benefits of applying the correlation function can be understood in part from examining the characteristics of simple 2 x 2 covariance matrices generated from random sample vectors with known variances and covariance. These show that noisiness in covariance estimates tends to overwhelm the signal when the ensemble size is small and/or the true covariance between the sample elements is small. Since the true covariance of forecast errors is generally related to the distance between grid points, covariance estimates generally have a higher ratio of noise to signal with increasing distance between grid points. This property is also demonstrated using a two-layer hemispheric primitive equation model and comparing covariance estimates generated by small and large ensembles. Covariances from the large ensemble are assumed to be accurate and are used a reference for measuring errors from covariances estimated from a small ensemble. The benefits of including distance-dependent reduction of covariance estimates are demonstrated with an ensemble Kalman filter data assimilation scheme. The optimal correlation length scale of the filter function depends on ensemble size; larger correlation lengths are preferable for larger ensembles. The effects of inflating background error covariance estimates are examined as a way of stabilizing the filter. It was found that more inflation was necessary for smaller ensembles than for larger ensembles.

ABSTRACT
We analyse 870 $\mu$m Atacama Large Millimetre Array (ALMA) dust continuum detections of 41 canonically selected $z$ ≃ 3 Lyman-break galaxies (LBGs), as well as 209 ALMA-undetected LBGs, in ...follow-up of SCUBA-2 mapping of the UKIDSS Ultra Deep Survey (UDS) field. We find that our ALMA-bright LBGs lie significantly off the local IRX-beta relation and have relatively bluer rest-frame UV slopes (as parametrized by β), given their high values of the ‘infrared excess’ (IRX ≡ LIR/LUV), relative to the average ‘local’ IRX-β relation. We attribute this finding in part to the young ages of the underlying stellar populations but we find that the main reason behind the unusually blue UV slopes are the relatively shallow slopes of the corresponding dust attenuation curves. We show that, when stellar masses, M*, are being established via SED fitting, it is absolutely crucial to allow the attenuation curves to vary (rather than fixing it on Calzetti-like law), where we find that the inappropriate curves may underestimate the resulting stellar masses by a factor of ≃2–3× on average. In addition, we find these LBGs to have relatively high specific star-formation rates (sSFRs), dominated by the dust component, as quantified via the fraction of obscured star formation $(f_{\rm obs}\equiv {\rm SFR_{\rm IR}/{\rm SFR}_{\rm UV+IR}})$. We conclude that the ALMA-bright LBGs are, by selection, massive galaxies undergoing a burst of a star formation (large sSFRs, driven, for example, by secular or merger processes), with a likely geometrical disconnection of the dust and stars, responsible for producing shallow dust attenuation curves.
Climate variability and global change studies are increasingly focused on understanding and predicting regional changes of daily weather statistics. Assessing the evidence for such variations over ...the last 100 yr requires a daily tropospheric circulation dataset. The only dataset available for the early twentieth century consists of error-ridden hand-drawn analyses of the mean sea level pressure field over the Northern Hemisphere. Modern data assimilation systems have the potential to improve upon these maps, but prior to 1948, few digitized upper-air sounding observations are available for such a “reanalysis.” We investigate the possibility that the additional number of newly recovered surface pressure observations is sufficient to generate useful weather maps of the lower-tropospheric extratropical circulation back to 1890 over the Northern Hemisphere, and back to 1930 over the Southern Hemisphere. Surprisingly, we find that by using an advanced data assimilation system based on an ensemble Kalman filter, it would be feasible to produce high-quality maps of even the upper troposphere using only surface pressure observations. For the beginning of the twentieth century, the errors of such upper-air circulation maps over the Northern Hemisphere in winter would be comparable to the 2–3-day errors of modern weather forecasts.

There is increasing evidence that high-risk human papilloma virus (HPV) is involved in cancers in addition to cervical cancer. For example, it is generally accepted that HPV has a role in a ...significant proportion of head and neck tumours, and it has long been hypothesised that hormone dependent oncogenic viruses, such as HPV may have causal roles in some human breast cancers. A number of reports have identified HPV DNA in breast tissue and breast cancer specimens, but these rely on standard polymerase chain reaction (PCR), which is criticised for its propensity for contamination.
We have used two different technologies, in situ and standard PCR (with sequencing), and histology based on light microscopy.
We unambiguously demonstrate the presence of high-risk HPV in the cells of breast cancer specimens and breast cancer cell lines. In addition, we also show that the oncogenic characteristics of HPV associated breast cancer are very similar to HPV-associated cervical cancer. Specifically, that putative koilocytes are present in some HPV associated breast cancers.
The above observations indicate a likely causal role for high-risk HPV in human breast cancer and offer the possibility of primary prevention of some breast cancers by vaccination against HPV.
High-risk human papilloma viruses (HPVs) are candidates as causal viruses in breast cancer. The scientific challenge is to determine whether HPVs are causal and not merely passengers or parasites. ...Studies of HPV-related koilocytes in breast cancer offer an opportunity to address this crucial issue. Koilocytes are epithelial cells characterised by perinuclear haloes surrounding condensed nuclei and are commonly present in cervical intraepithelial neoplasia. Koilocytosis is accepted as pathognomonic (characteristic of a particular disease) of HPV infection. The aim of this investigation is to determine whether putative koilocytes in normal and malignant breast tissues are because of HPV infection.
Archival formalin-fixed normal and malignant breast specimens were investigated by histology, in situ PCR with confirmation of the findings by standard PCR and sequencing of the products, plus immunohistochemistry to identify HPV E6 oncoproteins.
human papilloma virus-associated koilocytes were present in normal breast skin and lobules and in the breast skin and cancer tissue of patients with ductal carcinoma in situ (DCIS) and invasive ductal carcinomas (IDCs).
As koilocytes are known to be the precursors of some HPV-associated cervical cancer, it follows that HPVs may be causally associated with breast cancer.
Human papilloma viruses (HPVs) are accepted as being carcinogenic in human cervical and anogenital cancers. The suspicion that HPVs may also have a role in human breast cancer is based on the ...identification of HPVs in human breast tumours and the immortalisation of normal human breast cells by HPV types 16 and 18. For this investigation, DNA that had been previously extracted and fresh frozen at -70 degrees C from 50 unselected invasive ductal breast cancer specimens were screened by polymerase chain reaction (PCR) for HPV type 16, 18 and 33 gene sequences. We show that HPV 18 gene sequences are present in DNA extracted from breast tumours in Australian women. Overall, 24 (48%) of the 50 samples were HPV positive. Overall no correlations with tumour grade, patient survival, steroid receptor status, ERB-2, p53 expression and mutation were observed. Human papilloma viruses may have a role in human breast cancer. We speculate that HPVs may be transmitted by hand from the female perineum to the breast.
