Aim
The purpose of this study was to evaluate a novel hernia repair glue fixation device (LiquibandFix8™) in laparoscopic IPOM repair for incisional hernias.
Methods
All ventral incisional hernia ...patients requiring laparoscopic IPOM repair were included in the study. A retrospective review of the data was performed.
Results
137 patients underwent 138 laparoscopic IPOM repairs for incisional hernias using
n
-butyl-2-cyanoacrylate (LiquibandFix8™) for mesh fixation over a 40-month period by a single experienced laparoscopic surgeon. There were 70 males and 67 females, median age 54 years (range 23–80 years). Defects were predominantly midline
n
= 123/138 (89%) and were closed by transfascial sutures in 126/138 (91%) prior to IPOM repair (IPOM plus). Mesh fixation was successful in all 138 repairs. Synthetic meshes (Symbotex™ polyester/collagen composite, Medtronic

= 53) were used. Median follow-up was 32 months (range 12–48 months). There were 16 adverse events in 12 patients (9%): conversion to open surgery (inadvertent enterotomy) 2 (1.5%), bladder perforation 1, intraperitoneal bleeding 2, port-site haematoma 2, port site wound infection 1, post-op ileus 2, chest infection 1, seroma 1 (1%), hernia recurrence 3 (2%) and chronic pain 2 (1.5%).
Conclusion
This retrospective study shows that mesh fixation in laparoscopic IPOM using cyanoacrylate glue with the Liquiband Fix8™ device is feasible, safe, easy to learn, and is associated with a low risk of seroma, hernia recurrence and chronic pain with short-medium term follow-up.
We report 65 T magnetoabsorption spectroscopy of exciton Rydberg states in the archetypal monolayer semiconductor WSe_{2}. The strongly field-dependent and distinct energy shifts of the 2s, 3s, and ...4s excited neutral excitons permits their unambiguous identification and allows for quantitative comparison with leading theoretical models. Both the sizes (via low-field diamagnetic shifts) and the energies of the ns exciton states agree remarkably well with detailed numerical simulations using the nonhydrogenic screened Keldysh potential for 2D semiconductors. Moreover, at the highest magnetic fields, the nearly linear diamagnetic shifts of the weakly bound 3s and 4s excitons provide a direct experimental measure of the exciton's reduced mass m_{r}=0.20±0.01m_{0}.

Document-level sentiment classification aims to automate the task of classifying a textual review, which is given on a single topic, as expressing a positive or negative sentiment. In general, supervised methods consist of two stages: (i) extraction/selection of informative features and (ii) classification of reviews by using learning models like Support Vector Machines (SVM) and Naı¨ve Bayes (NB). SVM have been extensively and successfully used as a sentiment learning approach while Artificial Neural Networks (ANN) have rarely been considered in comparative studies in the sentiment analysis literature. This paper presents an empirical comparison between SVM and ANN regarding document-level sentiment analysis. We discuss requirements, resulting models and contexts in which both approaches achieve better levels of classification accuracy. We adopt a standard evaluation context with popular supervised methods for feature selection and weighting in a traditional bag-of-words model. Except for some unbalanced data contexts, our experiments indicated that ANN produce superior or at least comparable results to SVM’s. Specially on the benchmark dataset of Movies reviews, ANN outperformed SVM by a statistically significant difference, even on the context of unbalanced data. Our results have also confirmed some potential limitations of both models, which have been rarely discussed in the sentiment classification literature, like the computational cost of SVM at the running time and ANN at the training time.
IMPORTANCE: Proton pump inhibitors (PPIs) are among the most commonly used drugs worldwide and have been linked to acute interstitial nephritis. Less is known about the association between PPI use ...and chronic kidney disease (CKD). OBJECTIVE: To quantify the association between PPI use and incident CKD in a population-based cohort. DESIGN, SETTING, AND PARTICIPANTS: In total, 10 482 participants in the Atherosclerosis Risk in Communities study with an estimated glomerular filtration rate of at least 60 mL/min/1.73 m2 were followed from a baseline visit between February 1, 1996, and January 30, 1999, to December 31, 2011. The data was analyzed from May 2015 to October 2015. The findings were replicated in an administrative cohort of 248 751 patients with an estimated glomerular filtration rate of at least 60 mL/min/1.73 m2 from the Geisinger Health System. EXPOSURES: Self-reported PPI use in the Atherosclerosis Risk in Communities study or an outpatient PPI prescription in the Geisinger Health System replication cohort. Histamine2 (H2) receptor antagonist use was considered a negative control and active comparator. MAIN OUTCOMES AND MEASURES: Incident CKD was defined using diagnostic codes at hospital discharge or death in the Atherosclerosis Risk in Communities Study, and by a sustained outpatient estimated glomerular filtration rate of less than 60 mL/min/1.73 m2 in the Geisinger Health System replication cohort. RESULTS: Among 10 482 participants in the Atherosclerosis Risk in Communities study, the mean (SD) age was 63.0 (5.6) years, and 43.9% were male. Compared with nonusers, PPI users were more often of white race, obese, and taking antihypertensive medication. Proton pump inhibitor use was associated with incident CKD in unadjusted analysis (hazard ratio HR, 1.45; 95% CI, 1.11-1.90); in analysis adjusted for demographic, socioeconomic, and clinical variables (HR, 1.50; 95% CI, 1.14-1.96); and in analysis with PPI ever use modeled as a time-varying variable (adjusted HR, 1.35; 95% CI, 1.17-1.55). The association persisted when baseline PPI users were compared directly with H2 receptor antagonist users (adjusted HR, 1.39; 95% CI, 1.01-1.91) and with propensity score–matched nonusers (HR, 1.76; 95% CI, 1.13-2.74). In the Geisinger Health System replication cohort, PPI use was associated with CKD in all analyses, including a time-varying new-user design (adjusted HR, 1.24; 95% CI, 1.20-1.28). Twice-daily PPI dosing (adjusted HR, 1.46; 95% CI, 1.28-1.67) was associated with a higher risk than once-daily dosing (adjusted HR, 1.15; 95% CI, 1.09-1.21). CONCLUSIONS AND RELEVANCE: Proton pump inhibitor use is associated with a higher risk of incident CKD. Future research should evaluate whether limiting PPI use reduces the incidence of CKD.

under the assumption of a spherically symmetric atmosphere. We find that planetary rotational broadening of
km s
−1
is necessary to reproduce the Balmer line transmission profile shapes, where the model including rotation is strongly preferred over the non-rotating model using a Bayesian information criterion comparison. The time series of both metal line and hydrogen absorption show remarkable structure, suggesting that the atmosphere observed during this transit is dynamic rather than static. We detect a relative emission feature near the end of the transit which exhibits a P-Cygni-like shape, evidence of material moving at ≈50–100 km s
−1
away from the planet. We hypothesize that the in-transit variability and subsequent P-Cygni-like profiles are due to a flaring event that caused the atmosphere to expand, resulting in unbound material being accelerated to high speeds by stellar radiation pressure. Further spectroscopic transit observations will help establish the frequency of such events.
The Jovian Auroral Distributions Experiment (JADE) on Juno provides the critical
in situ
measurements of electrons and ions needed to understand the plasma energy particles and processes that fill ...the Jovian magnetosphere and ultimately produce its strong aurora. JADE is an instrument suite that includes three essentially identical electron sensors (JADE-Es), a single ion sensor (JADE-I), and a highly capable Electronics Box (EBox) that resides in the Juno Radiation Vault and provides all necessary control, low and high voltages, and computing support for the four sensors. All four sensors were extensively tested and calibrated in specialized facilities, ensuring excellent on-orbit observations at Jupiter. This paper documents the JADE design, construction, calibration, and planned science operations, data processing, and data products. Finally, the

describes the Southwest Research Institute SwRI electron calibration facility, which was developed and used for all JADE-E calibrations. Collectively, JADE provides remarkably broad and detailed measurements of the Jovian auroral region and magnetospheric plasmas, which will surely revolutionize our understanding of these important and complex regions.
This study investigated the occurrence of fipronil and its metabolites in aquatic environments in residentially-developed landscapes, including five canals and three retention ponds. Fipronil was ...detected at four of the sites, with concentrations of 0.5–207.3ngL−1. Fipronil sulfone and fipronil sulfide were detected at three sampling sites, with concentrations ranging from 0.46 to 57.75 and 0.40–26.92ngL−1, respectively. Multiple risk assessment methods were performed to characterize potential ecological risks, including deterministic screening and probabilistic risk assessment techniques. The deterministic method indicated no risk to certain biotic groups (i.e. aquatic plants, fish, molluscs, and algae–moss–fungi), but did indicate risks to larval insects and crustaceans. Results from the probabilistic risk assessment indicated significant ecological risks (acute and chronic) ranging from 0.75 to 58.9% and 3.9–35.0% when organisms were exposed to the maximum and median concentrations detected, respectively. The potentially affected fraction of species (PAF) likely to be acutely impacted ranged from 4.6 to 8.1% (fipronil), 0.2–1.6% (fipronil sulfone), and 1.9–3.1% (fipronil sulfide) in the ponds with frequent detectable concentrations. The PAF likely to be impacted at chronic toxicity levels ranged from 16.5 to 23.8% for fipronil. Joint probability curve analysis indicated that concentrations exceeded the LC50 of the most sensitive 5% of species 8.5–18.8% of the time at two of the sites with the most frequent detections. Using the more conservative NOEC/LOEC values, there was a 75–78% probability that concentrations were high enough to negatively affect the most sensitive 5% of species at the same two sites, indicating significant risks for chronic toxicity. JPCs indicated a ≤2.6% probability of fipronil sulfone exceeding the LC50 concentrations for the most sensitive 5% of species at the same two sites; and a 4.3–6.8% probability of fipronil sulfide exceeding the LC50 concentrations at the same sites. Results indicate that fipronil and its sulfone and sulfide degradation products may present significant risks to aquatic organisms in some residentially-developed areas.
