A novel approach in combined laparoscopic and vaginal procedures through the posterior cul-de-sac for subtotal hysterectomy is introduced.
Twenty-one women with menometrorrhagia, symptomatic ...adenomyosis, or uterine myomas were enrolled in this study. After laparoscopic dissection of bilateral round ligaments and adnexa, a guiding suture brought the uterine fundus down through the posterior cul-desac into the vagina via a posterior colpotomy. Subtotal hysterectomy and hemostasis of the cervical stump were then performed transvaginally by conventional techniques and equipment.
Mean operative time, blood loss, and length of hospital stay were 111.2 +/- 28.8 minutes, 252.4 +/- 147.9 mL, and 3.2 +/- 0.9 days, respectively. No patients developed serious complications, but 1 patient had a postoperative stump infection and was treated with 2 combined antibiotics, uneventfully.

This study conducts long-term monitoring and analysis on 161kV overhead transmission line. Electricity demand has grown rapidly in recent years, so distribution and power transmission have become the ...center of attention. In order to effectively and safely solve power transmission problems, this study uses the internet of things technology to collect wind speed and ambient temperature information on Shichih-Minquan HV transmission lines and electric towers. It compares the collected data with the data from the Central Meteorological Bureau in Taiwan and a meteorological website to analyze the effectiveness of the actually installed monitoring device. Specifically, this device instantly obtains the results of dynamic thermal rating (DTR), and these results are compared to the results of static thermal rating to verify the reliability of the device. The data collection lasts more than half a year. The results show that using the grid data the DTR calculation would yield large errors due to the high variations of wind speeds. By contrast, the proposed monitoring device can report real-time wind speeds and ambient temperatures. Thus, the monitoring data obtained from the proposed device can be used by power dispatch operators for power transformation scheduling.
Two‐component decay of time‐resolved photoluminescence (TRPL) intensity in three InGaN/GaN multiple quantum well samples were observed. The first‐decay component was attributed to exciton relaxation ...of free‐carrier and localized states; the second‐decay one was dominated by the relaxation of localized excitons. The second‐decay lifetime was related to the extent of carrier localization or indium aggregation and phase separation. The lifetime of free‐carrier states was connected with the defect density. Based on the temperature‐dependent data of PL and stimulated emission (SE), the localization energies of the three samples were calibrated to show the consistent trend with the second‐decay lifetime and previous material analyses.

In this work, an all-in-one system integrated SiC intelligent power module, which integrates the MCU, memory, SRAM, motor control library, ADC, DAC, gate drivers, and SiC MOSFETs was proposed to explore the benefits and potential of SiC MOSFETs in highly integrated BLDC/PMSM motor drive. As a result, SiC solution generally saves over 80% of space, improves the thermal performance by 27.7°C, and enhances 3% of efficiency compared to the silicon counterparts.
In patients with agenesis of the vagina and cervix but with a functional endometrium, the traditional treatment is hysterectomy with construction of a neovagina. We report successful treatment by ...laparoscopically assisted full thickness skin graft for reconstruction in a patient with congenital agenesis of the vagina and uterine cervix concomitant with haematometra and ovarian endometrioma in a 12 year old girl. Postoperatively, the vaginal skin graft healed well, and menstruation first appeared 4 weeks later. In our opinion, a combined laparoscopic and vaginal procedure with full thickness skin graft is an efficacious alternative in managing such genital defects.
Expression of functionally active thrombomodulin (TM) on the luminal surface of endothelial cells is critical for vascular thromboresistance. The 3-hydroxyl-3-methyl coenzyme A reductase inhibitor, ...pravastatin, can protect the vasculature in a manner that is independent of its lipid-lowering activity. We examined the effect of pravastatin on TM expression by human aortic endothelial cells (HAECs) with subsequent tumor necrosis factor alpha (TNFalpha) stimulation and investigated the signaling pathways involved. TNFalpha treatment attenuated TM expression in HAECs in a time-dependent manner. Pravastatin upregulated TM levels in TNFalpha-treated HAECs. Specific inhibition of geranylgeranyltransferase-I or the Rho family by GGTI-286 or TcdB, respectively, enhanced TM expression in TNFalpha-treated HAECs, whereas MAP kinase inhibitors, inactivation of Rho by Clostridium botulinum C3 exoenzyme, or the Rho kinase inhibitor, Y-27632, had no effect. In TNFalpha-treated HAECs, pravastatin inhibited Rac1 and Cdc42 activation and their translocation to the cell membrane. Blocking the transcriptional activation of NF-kappaB prevented the TNFalpha-induced downregulation of TM. The pravastatin-induced increase in TM expression in TNFalpha-treated HAECs was mediated through inhibition of NF-kappaB activation. Pravastatin regulates TM expression by inhibiting the activation of the Rho family proteins, Rac1 and Cdc42, and the transcription factor, NF-kappaB. The increase in endothelial TM activity in response to pravastatin constitutes a novel pleiotropic (nonlipid-related) effect of this commonly used compound and may be of clinical significance in disorders in which deficient endothelial TM plays a pathophysiological role.
Ethylene glycol (EG), a frequently used lithogenic agent, causes calcium oxalate crystal deposits in the renal cortex and causes death if over-dosage. In this study, we compared the protein ...expression pattern of rat renal lithiasis induced by EG with that of normal renal tissues using two-dimensional electrophoresis (2-DE). Sixteen male Sprague-Dawley rats were divided into 2 groups. In control group (n = 8), rats were fed with normal drinking water; in EG group (n = 8), rats were fed with 0.75% EG in drinking water. After 4 weeks, rats fed with EG had calcium oxalate deposited in renal cortex. The renal cortex proteins were extracted and isolated by 2-DE.
