Laparoscopic radical resection of hilar cholangiocarcinoma was complex because of the tumor near to hepatic hilum. The resection of hilar cholangiocarcinoma increased in recent years in large medical ...centers with safety of surgery improved. The current status of laparoscopic radical resection of hilar cholangiocarcinoma was discussed in this article including contraindication of surgery, precision evaluation and obstructive jaundice treated with preoperative biliary duct drainage, coordination among operative team and their experience. The key technology involved both blood vessels and lymphatic nodes dissected and separated which included resection of caudal lobe combined with hepatic lobes and hepatic segments, and then reconstruction of digestive tract. Lastly, there is consideration and prospect of laparoscopic radical resection of hilar cholagiocarcinoma.
Scattering forces in focused light beams push away metallic particles. Thus, trapping metallic particles with conventional optical tweezers, especially those of Mie particle size, is difficult. Here ...we investigate a mechanism by which metallic particles are attracted and trapped by plasmonic tweezers when surface plasmons are excited and focused by a radially polarized beam in a high-numerical-aperture microscopic configuration. This contrasts the repulsion exerted in optical tweezers with the same configuration. We believe that different types of forces exerted on particles are responsible for this contrary trapping behaviour. Further, trapping with plasmonic tweezers is found not to be due to a gradient force balancing an opposing scattering force but results from the sum of both gradient and scattering forces acting in the same direction established by the strong coupling between the metallic particle and the highly focused plasmonic field. Theoretical analysis and simulations yield good agreement with experimental results.
Amifostine is the most clinical used chemical radioprotector, but its effect in patients treated with radiation is not consistent.
By searching Medline, CENTRAL, EMBASE, ASCO, ESMO, and CNKI ...databases, the published randomized controlled trials (RCTs) about the efficacy of amifostine in HNSCC patients treated with radiotherapy were collected. The pooled efficacy and side effects of this drug were calculated by RevMan software.
Seventeen trials including a total of 1167 patients (604 and 563 each arm) were analyzed in the meta-analysis. The pooled data showed that the use of amifostine significantly reduce the risk of developing Grade 3-4 mucositis (relative risk RR,0.72; 95% confidence interval CI,0.54-0.95; p<0.00001), Grade 2-4 acute xerostomia (RR,0.70; 95%CI,0.52-0.96; p = 0.02), or late xerostomia (RR,0.60; 95%CI,0.49-0.74; p<0.00001) and Grade 3-4 dysphagia (RR,0.39; 95%CI,0.17-0.92; p = 0.03). However, subgroup analysis demonstrated that no statistically significant reduction of Grade 3-4 mucositis (RR,0.97; 95% CI,0.74-1.26; p = 0.80), Grade 2-4 acute xerostomia (RR,0.35; 95%CI,0.02-5.44; p = 0.45), or late xerostomia (RR,0.40; 95%CI,0.13-1.24; p = 0.11) and Grade 3-4 dysphagia (RR,0.23; 95%CI,0.01-4.78; p = 0.35) was observed in patients treated with concomitant chemoradiotherapy. Compared with placebo or observation, amifostine does not show tumor protective effect in complete response (RR,1.02; 95%CI,0.89-1.17; p = 0.76) and partial response (RR,0.90; 95%CI, 0.56-1.44; p = 0.66). For the hematologic side effect, no statistical difference of Grade 3-4 leucopenia (RR,0.60; 95%CI,0.35-1.05; p = 0.07), anemia (RR,0.80; 95%CI, 0.42-1.53; p = 0.50) and thrombocytopenia (RR,0.43; 95%CI,0.16-1.15; p = 0.09) were found between amifostine and control groups. The most common amifostine related side effects were nausea, emesis, hypotension and allergic with an average incidence rate (Grade 3-4) of 5%, 6%, 4% and 4% respectively.
This systematic review showed that amifostine significantly reduce the serious mucositis, acute/late xerastomia and dysphagia without protection of the tumor in HNSCC patients treated with radiotherapy. And the toxicities of amifostine were generally acceptable.

Abstract
Intrinsic autophagy is important for the maintenance of intestinal homeostasis and intestinal regeneration. Ionizing radiation suppresses intrinsic autophagy and reduces damage-induced ...regeneration in the intestine, resulting in intestinal injury. Resveratrol, a sirtuin 1 (SIRT1) agonist, promotes autophagy and exerts radioprotective effect. In this study, the protective effect of resveratrol against radiation-induced intestinal injury and its potential mechanism were investigated. Intestinal epithelial cells (IEC-6) were exposed to 10 Gy ionizing radiation and resveratrol (0.1–40.0 μM). Cell viability was investigated using Cell Counting Kit 8 (CCK8), apoptosis was observed by Annexin V-fluorescein isothiocyanate/propidium iodide (PI) staining and flow cytometry, and the expression of apoptotic and autophagic proteins was determined by western blotting. Resveratrol exerted a high toxicity against IEC-6 cells, but at low concentrations, it inhibited ionizing radiation-induced apoptosis. Resveratrol increased SIRT1 expression after irradiation and inhibited ionizing radiation-induced p53 acetylation and pro-apoptotic protein, Bax, expression. Furthermore, resveratrol promoted autophagy via the phosphoinositide 3-kinase (PI3K)/AKT/mammalian target of rapamycin (mTOR) pathway, thereby protecting IEC-6 cells against radiation-induced damage. These results suggest that resveratrol reduces radiation-induced IEC-6 cell damage by inhibiting apoptosis and promoting autophagy via the activation of SIRT1, and that the PI3K/AKT/mTOR signaling pathway is involved in the induction of autophagy.
High performance transparent electrodes (TEs) with figures-of-merit as high as 471 were assembled using ultralong silver nanowires (Ag NWs). A room-temperature plasma was employed to enhance the ...conductivity of the Ag NW TEs by simultaneously removing the insulating PVP layer coating on the NWs and welding the junctions tightly. Furthermore, we developed a general way to fabricate TEs regardless of substrate limitations by transferring the as-fabricated Ag NW network onto various substrates directly, and the transmittance can remain as high as 91% with a sheet resistivity of 13 Ω sq. The highly robust and stable flexible TEs will have broad applications in flexible optoelectronic and electronic devices.
Abstract
In the mechanical vapor recompression (MVR) treatment of landfill leachate, scaling in the evaporator and heat exchanger poses a serious problem. This study explored the reasons for such ...scaling and proposed acid or ion-exchange pre-treatments to reduce the alkalinity of the landfill leachate nanofiltration concentrate (LLNC) to prevent scaling. The feasibility of these two methods was evaluated and the technical and economic parameters for application were obtained via experiments. A large amount of HCO3- in the LLNC was the main cause of scaling. The acid addition experiment and field application demonstrated that this method could prevent fouling problems. The cost of acid addition was USD 0.18/t. LLNC pre-treatment by ion-exchange showed that a weakly acidic cation-exchange resin performed better than a strongly acidic cation-exchange resin did. The amount of solid residue under an alkalinity of 0 mg/L could be decreased by 92.9% compared with that of raw LLNC during evaporation. Both methods could alleviate scaling and enable the wide application of the MVR evaporation process in landfill leachate treatment.
We present a second order accurate, geometrically flexible and easy to implement method for solving the variable coefficient Poisson equation with interfacial discontinuities or on irregular domains, ...handling both cases with the same approach. We discretize the equations using an embedded approach on a uniform Cartesian grid employing virtual nodes at interfaces and boundaries. A variational method is used to define numerical stencils near these special virtual nodes and a Lagrange multiplier approach is used to enforce jump conditions and Dirichlet boundary conditions. Our combination of these two aspects yields a symmetric positive definite discretization. In the general case, we obtain the standard 5-point stencil away from the interface. For the specific case of interface problems with continuous coefficients, we present a discontinuity removal technique that admits use of the standard 5-point finite difference stencil everywhere in the domain. Background
The aim of this study was to simultaneously and quantitatively assess the expression levels of 20 periodontal disease‐related proteins in gingival crevicular fluid (GCF) from normal ...controls (NOR) and severe periodontitis (SP) patients with an antibody array.
Methods
Antibodies against 20 periodontal disease‐related proteins were spotted onto a glass slide to create a periodontal disease antibody array (PDD). The array was then incubated with GCF samples collected from 25 NOR and 25 SP patients. Differentially expressed proteins between NOR and SP patients were then used to build receiver operator characteristic (ROC) curves and compare five classification models, including support vector machine, random forest, k nearest neighbor, linear discriminant analysis, and Classification and Regression Trees.
Results
Seven proteins (C‐reactive protein, interleukin IL‐1α, interleukin‐1β, interleukin‐8, matrix metalloproteinase‐13, osteoprotegerin, and osteoactivin) were significantly upregulated in SP patients compared with NOR, while receptor activator of nuclear factor‐kappa was significantly downregulated. The highest diagnostic accuracy using a ROC curve was observed for IL‐1β with an area under the curve of 0.984. Five of the proteins (IL‐1β, IL‐8, MMP‐13, osteoprotegerin, and osteoactivin) were identified as important features for classification. Linear discriminant analysis had the highest classification accuracy across the five classification models that were tested.
Conclusion
This study highlights the potential of antibody arrays to diagnose periodontal disease.
Intense electric fields at the nanoscale are essential for single molecule surface-enhanced Raman scattering (SERS) detection. Such fields can be achieved in plasmonic nano-gaps between nanoparticles ...and metal films through hybridization of surface plasmons. The nano-gaps could be formed and dynamically controlled by using plasmonic tweezers; however, the aggregation of particles in the plasmonic field degrades each particle's enhancement and spoils the nanosized-spatial resolution. Here, dual-plasmonic tweezers are proposed and demonstrated to accurately control the number of nano-gaps and enhancement by tailoring a crater-shaped potential well in the nano-trap system. As the electric field in the nano-gap is intense, SERS spectral signatures of a single molecular level are probed simultaneously. These advantages point towards the implementation of enhanced Raman spectra, and broad applications in optical molecular detection.
