Community-acquired pneumonia (CAP) is amongst the leading causes of death worldwide. As inflammatory markers, cytokines can predict outcomes, if interpreted together with clinical data and scoring ...systems such as CURB-65, CRB, and Acute Physiology and Chronic Health Evaluation II (APACHE II). The aim of this study was to determine the impact of inflammatory biomarkers on the early mortality of hospitalized CAP patients. Twenty-seven CAP patients needing hospitalization were enrolled for the study and samples of interleukin-1 (IL-1) and interleukin-6 (IL-6), tumor necrosis factor alpha (TNF-α), C-reactive protein (CRP), and homocystein were collected at the time of admission (day 1) as well as on the seventh day of the treatment. There was a significant reduction in the levels of IL-6 between the first and the second collections. Median IL-6 values decreased from 24 pg/mL (day 1) to 8 pg/mL (day 7) (P=0.016). The median levels of TNF-α were higher in patients: i) with acute kidney injury (AKI) (P=0.045), ii) requiring mechanical ventilation (P=0.040), iii) with short hospital stays (P=0.009), iv) admitted to the intensive care unit (ICU) (P=0.040), v) who died early (P=0.003), and vi) with worse CRB scores (P=0.013). In summary, IL-6 and TNF-α levels were associated with early mortality of CAP patients. Longer admission levels demonstrated greater likelihood of early death and overall mortality, necessity of mechanical ventilation, and AKI.
Objectives: T-cell Brazil project started in April 2017 focusing to collecting epidemiological and clinical data from the most frequent subtypes of PTCL, as an ambispective study collected diagnosis ...data from January 2015 to December 2022. Our goals were to obtain the frequency of subtypes and the clinical and biology characteristic from five Brazilian regions; besides to have a routine pathological revision and to evaluate the Overall Survival (OS), Progression-Free Survival (PFS) in 5 years of follow-up. Method: After 39 centers were approved by their Ethical Committee, they registered their cases using the REDcap Platform by Vanderbilt, being 63% public centers. Here, It was just analyzed a Prospective Cohort (PC), with diagnosis date from April 2017 to December 2022, a total of 461 cases. Descriptive analyses, Kaplan-Meier method, Log-Rank test to compare groups and Cox Regression to identify risk factor for OS using IBM-SPSS software v.24. Results: The median age was 52 years (18-92); 58% male; Almost 72% had advanced stages, 25% ECOG ≥ 2; the distribution of main subtypes was: 30% PTCL-NOS; 18% ALCL, ALK-; 14% ATL; 13% ENKTL-NT; 10.5% AITL; 5% ALCL, ALK+; 9.5% others. A median of follow up of 22 months (0-73), 55% pts were alive and 24-month PFS and OS were 36% and 49%, respectively. OS by main subtypes was 69% ALCL, ALK+; 61% ALCL, ALK; 50% PTCL-NOS; 44% ENKTL-NT; 41% AITL; 27% ATL. Cox regression showed ATL (HR 1.6, p = 0.008), ECOG ≥ 2 (HR 1.86, p < 0.0001) and Ann Arbor Stage III-IV (HR 2.06, p < 0.0001) as bad prognosis for OS; whereas for PFS were ATL (HR 1.9, p < 0.0001); ECOG≥ 2 (HR 1.53, p = 0.002); Ann Arbor Stage III-IV (HR 1.75, p = 0.001) and B symptoms (HR 1.36, p = 0.02). Discussion: PTCL make up approximately 10-15% of lymphoid malignancies and its frequency varies geographically. In Brazil, mainly due to its vast dimension, data collection has limitations and is subject to bias. This is the first experience cover all over the country, focusing also an educational and of interchanging experience network among the multidisciplinary health team in Brazil. Conclusions: Our initial target of 500 pts exceeded, hence the number of prospective cohort is getting close, because we are in a consistency data phase and probably more cases will be available after that. So, it is possible to make analyzes with PC. ATL and ENKTL-NT subtype presented more frequent of all, different of the Occidental countries, but similar with recent data from American Latin.
Aims
To assess adherence to guideline recommendations among a large network of Italian cardiology sites in the management of acute and chronic heart failure (HF) and to evaluate if an ad‐hoc ...educational intervention can improve their performance on several pharmacological and non‐pharmacological indicators.
Methods and results
BLITZ‐HF was a cross‐sectional study based on a web‐based recording system with pop‐up reminders on guideline recommendations used during two 3‐month enrolment periods carried out 3 months apart (Phase 1 and 3), interspersed by face‐to‐face macro‐regional benchmark analyses and educational meetings (Phase 2). Overall, 7218 patients with acute and chronic HF were enrolled at 106 cardiology sites. During the enrolment phases, 3920 and 3298 patients were included, respectively, 84% with chronic HF and 16% with acute HF in Phase 1, and 74% with chronic HF and 26% with acute HF in Phase 3. At baseline, adherence to guideline recommendations was already overall high for most indicators. Among acute HF patients, an improvement was obtained in three out of eight indicators, with a significant rise in echocardiographic evaluation. Among chronic HF patients with HF and preserved or mid‐range ejection fraction, performance increased in two out of three indicators: creatinine and echocardiographic evaluations. An overall performance improvement was observed in six out of nine indicators in ambulatory HF with reduced ejection fraction patients with a significant increase in angiotensin receptor–neprilysin inhibitor prescription rates.
Conclusions
Within a context of an already elevated level of adherence to HF guideline recommendations, a structured multifaceted educational intervention could be useful to improve performance on specific indicators. Extending this approach to other non‐cardiology healthcare professionals, who usually manage patients with HF, should be considered.
O papel da terapia de altas doses (TAD) seguida pelo transplante de células tronco periférico (TCTP) autólogo como consolidação é ainda muito discutido para os pacientes com Linfoma de células T ...periféricas (LCTP), ou seja, não há consenso atualmente sobre o gerenciamento ideal para o tratamento de primeira linha, principalmente devido ao viés de seleção de pacientes e das amostras pequenas nos estudos. Da mesma forma, com a disponibilidade de novos medicamentos, o uso TAD seguida de TCTP na consolidação ou salvamento deixou de serem as únicas opções disponíveis. Assim, nosso objetivo é descrever e verificar os resultados desses procedimentos e compreender o papel da TAD seguida de TCTP nestes pacientes.
Entre janeiro de 2015 e julho de 2021, 414 pacientes foram registrados por 34 centros brasileiros; destes 374 tinha informação disponível das terapias usadas. Um total de 70 (18.5%) casos foi submetido ao TAD seguido TCTP e este grupo é o foco deste estudo. Sobrevida global (SG) foi calculada a partir da data do diagnóstico até a data da última visita ou data de óbito. A sobrevida livre de progressão (SLP) da data do diagnóstico até data do 1° evento ou última consulta. Para o grupo de salvamento (TCTP-S) foi calculado a SLP considerando a data do TCTP até a data da última consulta ou 1° evento, sendo o evento progressão ou morte.
A mediana de idade dos pacientes transplantados foi de 51 anos (20-75), 37 (53%) sexo masculino, 20% teve performance status > 1; 60% apresentaram sintomas B e 67% teve algum sintoma relacionado à doença. 73% apresentou estadio III/IV; 16% dos casos tinham envolvimento extranodal > 1 e 17% tinham doença com bulky; Os subtipos mais frequentes foram 39% Linfoma de células T periféricas não especificado; 28% Linfoma de grandes células T anaplásicas, ALK- e 17% Linfoma angioimunoblástico. O grupo TCTP-C teve 54 pacientes e o status da doença antes do procedimento foi 50 (92,5%) casos com RC e 4 RP (7,5%); o TCTP-S foi realizado em 18 casos e 2 pacientes foram transplantados em ambos os momentos. Em relação ao tipo de TCTP, na consolidação, foram realizados 48 autólogos e 6 alogênicos, enquanto no salvamento foram 13 autólogos e 5 alogênicos. Após uma mediana de seguimento de 23 meses (4-77) para o grupo TCTP-C e 25 meses (6-43) para TCTP-S, a SG do grupo com TCTP-C foi de 81% (IC 95% 69-93%) e 71% (IC 95% 24-88%) para o TCTP-S, P = 0.16; A SLP foi de 61% (IC 95% 45-77%) e 7% (IC 95% 0-19%), respectivamente, p < 0,0001. Analisando o grupo TCTP-S, a resposta após o TCTP foi 4 (22%) RC, 1 (5,5%) RP, 3 (16,5%) doença estável, 5 (28%) progressão e 5 (28%) morreram. A mediana de seguimento foi 10 meses (1-23), e a SG-S foi 54% e 56% SLP-S em dois anos. Conclusões: De acordo com os dados da literatura, o TCTP pode indicar um controle em longo prazo para o LCTP alcançando uma estabilização após 5 anos e nossos dados apresentam curvas de 3 anos para o grupo consolidação, sugerindo que este procedimento é uma boa opção para melhorar os resultados finais. A literatura é escassa no que diz respeito aos aspectos clínicos e epidemiológicos dos linfomas periféricos de células T, principalmente no Brasil. Ademais, o país é altamente heterogêneo em termos ...de clima, geografia e meio ambiente, inclusive com grande diversidade genômica. As diferentes regiões têm particularidades únicas em aspectos culturais, escolaridade, longevidade e renda per capita. Sem falar das grandes disparidades no atendimento à saúde que afetam a epidemiologia das doenças, refletindo no diagnóstico, tratamento e resultados do paciente. Enfim, por estas e outras razões elaboramos o Projeto T-cell Brasil. Com os objetivos de entender a distribuição no país e as especificidades desta doença, suas características clínicas e biológicas; e avaliar a sobrevida global (SG) e livre de progressão (SLP). Após a aprovação do Comitê de ética, 39 centros tem registrado seus casos usando a Plataforma REDcap (criada pela Vanderbilt). Análises descritivas e bivariadas foram feitas, e então aplicou-se o método Kaplan-Meier com o teste log-rank para obter as estimativas das sobrevidas, usando IBM-SPSS software v.24. A mediana de idade foi de 55 anos (19-95); 56% masculino; 77% apresentou sintomas prévios, 72% estádios avançados; 28% ECOG ≥ 2; a distribuição dos principais subtipos foi: 31% PTCL-NOS; 18% ALCL, ALK-; 16% ATL; 13% ENKTL nasal e tipo nasal; 11% AITL; 7% ALCL, ALK+; 6% outros; 50% dos pacientes estavam vivos e a SLP e SG em 24 meses foi de 36% e 50%, respectivamente. SG estratificada pelos principais subtipos foi de: 48% PTCL-NOS; 61% ALCL, ALK-; 33% ATL; 46% ENKTL nasal/tipo nasal; 48% AITL; 80% ALCL, ALK+. Esta é a primeira experiência abrangendo todo o país, contemplando também um plano educacional e a troca de experiências entre as equipes multidisciplinares de saúde no Brasil. Nosso alvo de 500 casos já foi excedido, entretanto, a inserção de casos continuará até Dezembro como planejado. Todos os casos devem ser revisados tanto no registro quanto pelo Comitê de Patologia, e estimamos que alguns casos deverão ser excluídos por diferentes razões e estes casos excedentes auxiliarão nas análises futuras.
Selenium intake and urinary and fecal Se excretion of 10 healthy men from a low Se area in China were determined for three consecutive days, in summer, fall, and winter of 1983, and the spring of ...1984 while self-selected diets were being consumed. Mean daily Se intake was 8.8 µg/day with a range of 2.3–35.5 µg/day, and was far below the recommended range of safe and adequate Se intake of 50–200 µg Se/day (National Academy of Sciences/National Research Council). Mean urinary and fecal Se outputs were 3.7 and 3.4 µg Se/day, respectively. Mean Se balance during this time was +1.8 µg Se/day. Apparent absorption of Se approximated 57%. The low Se intake in this area is a cause for concern since the residents of Molimo may be at risk for Se deficiency diseases.
