Resistant bacterial infections are a major health problem in many parts of the world. The major commercial antibiotic classes often fail to combat common bacteria. Although antimicrobial peptides are ...able to control bacterial infections by interfering with microbial metabolism and physiological processes in several ways, a large number of cases of resistance to antibiotic peptide classes have also been reported. To gain a better understanding of the resistance process various technologies have been applied. Here we discuss multiple strategies by which bacteria could develop enhanced antimicrobial peptide resistance, focusing on sub-cellular regions from the surface to deep inside, evaluating bacterial membranes, cell walls and cytoplasmic metabolism. Moreover, some high-throughput methods for antimicrobial resistance detection and discrimination are also examined. Enterobacter cloacae is a Gram-negative bacterium associated with high morbidity and mortality in intensive care patients due to its resistance to multiple antibiotics. Currently, therapy against ...multi-resistant bacteria consists of using colistin, in spite of its toxic effects at higher concentrations. In this context, colistin-resistant E. cloacae strains were challenged with lower levels of colistin combined with other antibiotics to reduce colistin-associated side effects. Colistin-resistant E. cloacae (ATCC 49141) strains were generated by serial propagation in subinhibitory colistin concentrations. After this, three colistin-resistant and three nonresistant replicates were isolated. The identity of all the strains was confirmed by MALDI-TOF MS, VITEK 2 and MicroScan analysis. Furthermore, cross-resistance to other antibiotics was checked by disk diffusion and automated systems. The synergistic effects of the combined use of colistin and chloramphenicol were observed via the broth microdilution checkerboard method. First, data here reported showed that all strains presented intrinsic resistance to penicillin, cephalosporin (except fourth generation), monobactam, and some associations of penicillin and β-lactamase inhibitors. Moreover, a chloramphenicol and colistin combination was capable of inhibiting the induced colistin-resistant strains as well as two colistin-resistant clinical strains. Furthermore, no cytotoxic effect was observed by using such concentrations. In summary, the data reported here showed for the first time the possible therapeutic use of colistin-chloramphenicol for infections caused by colistin-resistant E. cloacae.
A judicialização como fenômeno de garantia do direito social à saúde é uma questão com discussão crescente no Brasil, devido à definição constitucional de saúde no país, que contempla a ...integralidade. Objetivo: A forma como as demandas judiciais vêm impactando nas políticas públicas de saúde nos três níveis de gestão coloca esse item como ponto premente na discussão de uma agenda para o sistema de saúde. Métodos: Esse estudo analisou o perfil do gasto da saúde pública no Distrito Federal com medicamentos não-padronizados, a fim de compreender quais as circunstâncias em que a judicialização de medicamentos ocorre nesse local. Resultados: Foram utilizados registros administrativos de distribuição dos medicamentos no período de Setembro/2014 a Agosto/2016. O gasto apurado foi classificado por item e por grupo de doença conforme CID-10. Foi verificado que o gasto total apurado foi de R$ 43,7 milhões. Dentre os medicamentos com maior gasto, observou-se o fator IX recombinante, utilizado para tratamento de hemofilia, como maior responsável (22,53%), seguido da alfaglicosidase (9,74%), do fingolimode (8,44%) e da abiraterona (6,63%). As doenças com maior demanda de atendimento via judicial foram as doenças do sangue que incluem as hemofilias (26,6%), as neoplasias (24,9%) e as doenças metabólicas (17,5%). Conclusão:Os resultados obtidos permitiram verificar um padrão de demandas particular do DF, com uma participação importante no orçamento destinado à compra de medicamentos.

Após um século da descoberta da Doença de Chagas, ainda não existe medicamento efetivo para o seu tratamento, e os chagásicos continuam sucumbindo às manifestações crônicas dessa grave enfermidade. A possibilidade de descobrir uma enzima ou uma via metabólica crucial para o ciclo de vida do parasita não se resume apenas à satisfação da curiosidade humana, mas também é fundamental para o desenvolvimento de fármacos que sirvam para combater as mais diferentes doenças, entre elas, a Doença de Chagas. Essa dissertação de mestrado descreve o estudo de três proteínas de Trypanosoma cruzi: duas proteases classificadas como serino-protease com domínio α/β hidrolase, pertencente ao clan SC e família S9 e S15, respectivamente - Oligopeptidase B símile (OPBsTc) e X-prolil dipeptidil aminopeptidase (X-PDAPTc) e uma proteína da via metabólica redox - a Glutationa sintetase (GSTc). A OPBsTc é uma enzima putativa de 903 aminoácidos com massa molecular esperada de 103,4 kDa sendo altamente conservada entre os cinetoplastídeos e ausente em humanos. A proteína recombinante foi expressa em E. coli BL21 Star (DE3) One Shot e purificada da fração insolúvel para produção de anticorpos em camundongo. Os soros foram utilizados em técnicas de Immunoblotting e imunocitolocalização na tentativa de confirmar a expressão da protease. Infelizmente, não foi possível confirmar a expressão da OPBsTc em nenhuma das três formas do parasita: epimastigota, tripomastigota e amastigosta. Também realizamos uma RT-PCR a partir do RNA da forma epimastigota, mas o resultado foi negativo. A segunda protease foi a X-PDAPTc, esta serino-protease é largamente estudada em Lactococcus lactis e parece ser um fator de virulência em Streptococci. Análises em bancos de dados mostraram que a distribuição da X-PDAPTc é restrita e não apresenta ortólogos em mamífero, além disso, a enzima tem 677 aminoácidos com massa molecular esperada de 76,8 kDa. A enzima foi produzida em E. coli BL21 Star (DE3) One Shot e purificada da fração insolúvel para produção de anticorpos em camundongo. X-PDAPTc é expressa na forma epimatigosta do parasito. A GSTc tem importante participação na biossíntese da glutationa para manutenção do ambiente redox no parasito. Em relação ao seu ortólogo em humanos, foi observada homologia reduzida, o que pode torná-la um alvo promissor para desenvolvimento de drogas, assim como as duas proteases citadas acima. Essa enzima tem 535 resíduos de aminoácidos e massa molecular predita de 58,6 kDa. A GSTc foi expressa e utilizada para testes de imunização.
After a century of the discovery of Chagas’ disease, there is still no effective medicine for its treatment, and patients still succumb to severe manifestations of the chronic disease. The possibility of discovering an enzyme or metabolic pathway crucial to the parasite life cycle is not only to satisfy the human curiosity, but it is also fundamental to the development of drugs used to combat many different diseases, including Chagas’ disease. This master's thesis describes the study of three proteins of Trypanosoma cruzi: two proteases classified as serine-protease with α / β hydrolase domain that belong to SC clan and family S9 and S15, respectively - Oligopeptidase B like (OPBsTc) and X-prolil dipeptidil aminopeptidase (X-PDAPTc) and a redox metabolic pathway enzyme - Glutathione synthetase (GSTc). The OPBsTc is a putative enzyme of 903 amino acids and molecular mass of 103.4 kDa. It is highly conserved among kinetoplastids and it is absent in humans. The recombinant protein was expressed in E. coli BL21 Star (DE3) One Shot and purified from the insoluble fraction for production of antibodies in mice. The sera were used in immunoblotting and immunocytology techniques in an attempt to confirm the expression of the protease. Unfortunately, we could not confirm the expression of OPBsTc in any of the three forms of the parasite: epimastigote, trypomastigote and amastigoste. We also performed a RT-PCR from total epimastigote RNA, but the result was negative. The second protease was X-PDAPTc, this serine-protease is extensively studied in Lactococcus lactis and it seems to be a factor of virulence in Streptococci. Analysis in databases showed that the distribution of X-PDAPTc is limited and it does not have any orthologs in mammalian. In addition, the enzyme has 677 amino acids with molecular mass of 76.8 kDa. The enzyme was produced in E. coli BL21 Star (DE3) One Shot and purified from the insoluble fraction for production of antibodies in mice. X-PDAPTc is expressed in the epimastigote form of the parasite. The GSTc has an important participation in the biosynthesis of glutathione in order to maintain redox environment of the parasite. The human ortholog shows low homology which may make it a promising target for drug development like the two proteases mentioned above. This enzyme has 535 amino acid residues and a predicted molecular mass of 58.6 kDa. The GSTc was expressed and used for immunization tests.

As Infecções Relacionadas à Assistência à Saúde (IRaS) e o aparecimento de cepas multirresistentes representam um grave problema de saúde pública, sendo notória a necessidade de melhor compreender os mecanismos de resistência bacteriana e assim, elaborar antibióticos mais eficazes. Portanto, o objetivo deste trabalho consistiu em identificar, através de técnicas proteômicas e transcriptômicas, genes e proteínas diferencialmente expressos em linhagens de E. coli ATCC 8739 resistentes à magainina I. Para isso foi realizada a caracterização bioquímica através de microdiluição, VITEK®, MicroScan® e MALDI-TOF MS. Em seguida, foram analisadas a transcrição gênica e a expressão protéica por RNA-seq, nanoUPLC-MSE e 2-DE, respectivamente. Os resultados demonstram que as linhagens resistentes podem ser discriminadas por análises no MALDI-TOF MS e que a resistência à magainina I parece ser bastante específica. Utilizando técnicas proteômicas, foram identificadas 10 proteínas superexpressas dentre as quais, as porinas OmpW, OmpF, OmpC e OmpC fragmento 1b e as proteínas OppA, ZraP e MalM, que participam do transporte pela membrana e transdução de sinal. Por outro lado as proteínas GlpB, GlpA, GdpD, que participam do metabolismo de fosfolipídeos, e o regulador transcricional kdgR, DPS, DnaK, relacionado ao processamento de informação genética, estavam subexpressas. Observou-se também 60 proteínas exclusivas na linhagem resistente, que participam principalmente do metabolismo e processamento de informação genética e ambiental. A fim de aumentar a amplitude da análise, técnicas transcriptômicas também foram aplicadas ao mesmo modelo de estudo observando-se 80 genes com expressões diferenciais, principalmente correlacionados com o metabolismo. Dentre esses, a linhagem resistente versus susceptível com magainina I apresentou 5 genes diferenciais, estando 3 deles superexpressos (pspA, secM, xdhB) e 2 subexpressos (cdp, hldD). Os resultados sugerem que a bactéria foi capaz de desenvolver mecanismos de adaptação em resposta à presença de magainina, provavelmente regulando uma complexa rede de múltiplos genes. ______________________________________________________________________________________________ ABSTRACT
Healthcare Associated Infections (HAIs) and emergence of multidrug-resistant bacteria represent a serious public heath problem, requesting a better understanding of bacterial resistance’s mechanisms in order to develop more effective antibiotics. Therefore, work aims to identify, through proteomics and transcripomics tools, genes and proteins differentially expressed in E. coli ATCC 8739 magainin I resistant strains. Biochemical characterization was performed by broth microdilution, VITEK, MicroScan and MALDI – TOF MS. Then, gene and proteins expression were analyzed by RNA –seq, nanoUPLC – MS and 2-DE. Results demonstrated that the resistant strains could be discriminated by MALDI- TOF MS analysis and resistance to magainin I seems to be quite specific. By using proteomic tools, ten proteins were upregulated, such OmpW, OmpF, OmpC e OmpC 1b fragment and fifteen proteins, sush GlpB, GlpA, GdpD, transcriptional factor regulator kdgR, DPS and DnaK, were downregulated in resistant strains. The upregulated proteins have been associated to membrance transport and sinal transduction and dowregulated proteins seem to participate in phospholipid metabolism and genetic information processing. It were also observed the presence of 60 unique proteins in the resistant strains that participate in bacterial metabolism, genetic and environmental information processing. In order to increase the knowledge of bacterial resistance, transcriptomic analysis was realized by using the same magainin I-resistant E. coli model being 80 genes identified, with differential expression in resistant strain being mainly related to metabolism. Among these, the comparison between resistant versus susceptible group cultured with magainin I showed five differential expressed genes, being three of them overexpressed (pspA, secM, xdhB) and the others subexpressed (cdp, hldD.) These data suggest that the bacterium was able to develop a adaptation mechanism in response to magainin, probably regulating a complex net formed by multiple genes.
COVID-19 has affected more than half a billion people worldwide, with more than 6.3 million deaths, but the pathophysiological mechanisms involved in lethal cases and the host determinants that ...determine the different clinical outcomes are still unclear. In this study, we assessed lung autopsies of 47 COVID-19 patients and examined the inflammatory profiles, viral loads, and inflammasome activation. Additionally, we correlated these factors with the patient’s clinical and histopathological conditions. Robust inflammasome activation was detected in the lungs of lethal cases of SARS-CoV-2. Experiments conducted on transgenic mice expressing hACE2 and infected with SARS-CoV-2 showed that Nlrp3 -/- mice were protected from disease development and lethality compared to Nlrp3 +/+ littermate mice, supporting the involvement of this inflammasome in disease exacerbation. An analysis of gene expression allowed for the classification of COVID-19 patients into two different clusters. Cluster 1 died with higher viral loads and exhibited a reduced inflammatory profile than Cluster 2. Illness time, mechanical ventilation time, pulmonary fibrosis, respiratory functions, histopathological status, thrombosis, viral loads, and inflammasome activation significantly differed between the two clusters. Our data demonstrated two distinct profiles in lethal cases of COVID-19, thus indicating that the balance of viral replication and inflammasome-mediated pulmonary inflammation led to different clinical outcomes. We provide important information to understand clinical variations in severe COVID-19, a process that is critical for decisions between immune-mediated or antiviral-mediated therapies for the treatment of critical cases of COVID-19.

Lateral flow antigen tests have been widely used in the Covid-19 pandemic, allowing faster diagnostic test results and preventing further viral spread through isolation of infected individuals. ...Accomplishment of this screening must be performed with tests that show satisfactory sensitivity in order to successfully detect the target protein and avoid false negatives. The aim of this study was to create a lateral flow test that could detect SARS-CoV-2 nucleocapsid protein in low concentrations that were comparable to the limits of detection claimed by existing tests from the market. To do so, several adjustments were necessary during research and development of the prototypes until they were consistent with these criteria. The proposed alternatives of increasing the test line antibody concentration and addition of an intermembrane between the conjugate pad and the nitrocellulose membrane were able to increase the sensitivity four-fold and generate a new rapid test prototype called "lateral flow intermembrane immunoassay test" (LFIIT). This prototype showed an adequate limit of detection (2.0 ng mL
) while maintaining affordability and simplicity in manufacturing processes.
