Objetivo:
Caracterizar o grau de acometimento neurológico, estabelecendo uma correlação entre o nível funcional da lesão e a aquisição da marcha, além de identificar os fatores que interferem ...nessa capacidade.
Método:
Foram avaliadas prospectivamente vinte e oito crianças portadoras de mielomeningocele operadas com idade igual ou superior a três anos. Foram determinados o nível funcional da lesão, a aquisição ou não da marcha e os fatores associados por meio das medidas do grau de força muscular nos membros inferiores, presença de deformidades articulares, ocorrência de contratura da musculatura flexora e trato iliotibial, eficácia da reação de equilíbrio e presença de hidrocefalia tratada.
Resultados:
Todos os fatores avaliados, exceto a hidrocefalia, foram relacionados de forma significativa à aquisição da marcha.
Conclusão:
A aquisição da marcha em crianças portadoras de mielomeningocele sofre influência de vários fatores, além do nível de acometimento neurológico, o que deve ser considerado no prognóstico e na programação terapêutica.
The thoracolumbar spine fractures constitute a wide spectrum of resultant lesions, with distinct injury mechanisms. In order to reduce the controversies concerning about the management of these ...fractures, a universally accepted classification is necessary. In this study we evaluated retrospectively 33 patients with thoracolumbar spine fracture, with the goal of categorize and evaluate the factors related to this pathology. A complete radiological investigation, complaining of plain radiography, computed tomography and magnetic resonance imaging, was used to classify these fractures. Fall was the more common mechanism, present in 24 cases. In 57.6% of the patients, the fractures located at thoracolumbar transition (T12-L1) and the more frequent neurological presentation was total deficit, present in 45.45%. The neurological presentation was more serious in patients with thoracic lesions regarding lumbar lesions (Fischer's test, p=0.039). A positive correlation was observed between severity of the neurological presentation and gravity of the lesion according to Magerl's classification (Pearson's method, r=0.85, p<0.001). In conclusion, thoracolumbar spine fractures are serious lesions considering the initial neurological presentation; a wide and accurate classification, as we used, is necessary to describe these injuries and may help resolve some of the controversies concerning the management of these lesions.
Transportation of paraffinic crude in a cold environment can result in wax depositions, which are solid deposits, on the pipe wall. The formation of paraffin in the bulk of the petroleum is a ...crystallization process in solution. Such phenomenon occurs along the pipe connected to offshore wells and even onshore. Its main implication is a paraffin blockage with serious economical consequences. This work aims to learn about paraffin deposit formation using a cold finger methodology, in two types of Brazilian crude oils (32 API and 20 API). The polythermal method is used in various experiments using different temperatures in both cold finger and bulk of the fluid. The experiments measure: the thickness of the deposits, as well as the deposition rates, degree of subcooling and, finally, the induction for the onset of wax. The methodology allows the evaluation of the critical time of deposition and the critical temperature difference between the hot petroleum source and the cold seawater temperature. It is shown that the deposit rate depends on this temperature difference, as well as the cooling surface, the nucleation kinetics and the growth of the crystals on the cold finger surface. The work discusses carefully the experiments design under the light of dimensional analysis. The values of critical temperature differences are relatively small and show that the encrustation is easily attained in the offshore petroleum flow. The methodology implemented is simple and allows the prediction of very important information about the mechanism of paraffin encrustation that could pave the way to avoid paraffin deposition in offshore petroleum production due to high temperature differences between the heat petroleum sink source and the cold seawater.
RESUMO Objetivo: Validar a quantificação de T-cell receptor excision circles (TRECs) e kappa-deleting recombination circles (KRECs) por reação em cadeia de polimerase (polymerase chain reaction, PCR) ...em tempo real (qRT-PCR), para triagem neonatal de imunodeficiências primárias que cursam com defeitos nas células T e/ou B no Brasil. Métodos: Amostras de sangue de recém-nascidos (RN) e controles foram coletadas em papel-filtro. O DNA foi extraído e os TRECs e KRECs foram quantificados por reação duplex de qRT-PCR. O valor de corte foi determinado pela análise de Receiver Operating Characteristics Curve, utilizando-se o programa Statistical Package for the Social Sciences (SSPS) (IBM®, Armonk, NY, EUA). Resultados: 6.881 amostras de RN foram analisadas quanto à concentração de TRECs e KRECs. Os valores de TRECs variaram entre 1 e 1.006 TRECs/µL, com média e mediana de 160 e 139 TRECs/µL, respectivamente. Três amostras de pacientes diagnosticados com imunodeficiência grave combinada (severe combined immunodeficiency, SCID) apresentaram valores de TRECs abaixo de 4/µL e um paciente com Síndrome de DiGeorge apresentou TRECs indetectáveis. Os valores de KRECs encontraram-se entre 10 e 1.097 KRECs/µL, com média e mediana de 130 e 108 KRECs/µL, e quatro pacientes com diagnóstico de agamaglobulinemia tiveram resultados abaixo de 4 KRECs/µL. Os valores de corte encontrados foram 15 TRECs/µL e 14 KRECs/µL, e foram estabelecidos de acordo com a análise da Receiver Operating Characteristics Curve, com sensibilidade de 100% para detecção de SCID e agamaglobulinemia, respectivamente. Conclusões: A quantificação de TRECs e KRECs foi capaz de diagnosticar crianças com linfopenias T e/ou B em nosso estudo, validando a técnica e dando o primeiro passo para a implementação da triagem neonatal em grande escala no Brasil.
Em estudo retrospectivo foram analisados 35 prontuários, obtidos ao acaso, de crianças com diagnóstico de Paralisia Cerebral (PC), atendidas no Hospital das Clínicas de Ribeirão Preto, entre 1982 e ...1998, com o objetivo de verificar fatores etiológicos. As infecções congênitas (36%) e agressão hipóxico-isquêmica (28%) foram os mais freqüentes fatores determinantes de PC entre 25 casos com etiologia definida. Estes achados podem ser utilizados para estratégias regionais de prevenção de deficiências. Para os casos sem etiologia definida, lembram-se as formas herdadas de paralisia cerebral e o diagnóstico diferencial com doenças degenerativas.
