We have conducted two pilot surveys for radio pulsars and fast transients with the Low-Frequency Array (LOFAR) around 140 MHz and here report on the first low-frequency fast-radio burst limit and the ...discovery of two new pulsars. The first survey, the LOFAR Pilot Pulsar Survey (LPPS), observed a large fraction of the northern sky, ~ 1.4 × 104 deg2, with 1 h dwell times. Each observation covered ~75 deg2 using 7 independent fields formed by incoherently summing the high-band antenna fields. The second pilot survey, the LOFAR Tied-Array Survey (LOTAS), spanned ~600 deg2, with roughly a 5-fold increase in sensitivity compared with LPPS. Using a coherent sum of the 6 LOFAR “Superterp” stations, we formed 19 tied-array beams, together covering 4 deg2 per pointing. From LPPS we derive a limit on the occurrence, at 142 MHz, of dispersed radio bursts of < 150 day-1 sky-1, for bursts brighter than S> 107 Jy for the narrowest searched burst duration of 0.66 ms. In LPPS, we re-detected 65 previously known pulsars. LOTAS discovered two pulsars, the first with LOFAR or any digital aperture array. LOTAS also re-detected 27 previously known pulsars. These pilot studies show that LOFAR can efficiently carry out all-sky surveys for pulsars and fast transients, and they set the stage for further surveying efforts using LOFAR and the planned low-frequency component of the Square Kilometer Array.
Some radio pulsars show clear “drifting subpulses”, in which subpulses are seen to drift in pulse longitude in a systematic pattern. Here we examine how the drifting subpulses of PSR B0809+74 evolve ...with time and observing frequency. We show that the subpulse period (P3) is constant on timescales of days, months and years, and between 14–5100 MHz. Despite this, the shapes of the driftbands change radically with frequency. Previous studies have concluded that, while the subpulses appear to move through the pulse window approximately linearly at low frequencies (<500 MHz), a discrete step of ~180° in subpulse phase is observed at higher frequencies (>820 MHz) near to the peak of the average pulse profile. We use LOFAR, GMRT, GBT, WSRT and Effelsberg 100-m data to explore the frequency-dependence of this phase step. We show that the size of the subpulse phase step increases gradually, and is observable even at low frequencies. We attribute the subpulse phase step to the presence of two separate driftbands, whose relative arrival times vary with frequency – one driftband arriving 30 pulses earlier at 20 MHz than it does at 1380 MHz, whilst the other arrives simultaneously at all frequencies. The drifting pattern which is observed here cannot be explained by either the rotating carousel model or the surface oscillation model, and could provide new insight into the physical processes happening within the pulsar magnetosphere.

Conventional sentinel node (SN) mapping is performed by injection of a radiocolloid followed by lymphoscintigraphy to identify the number and location of the primary tumor draining lymph node(s), the ...so-called SN(s). Over the last decade research has focused on the introduction of new imaging agents that can further aid (surgical) SN identification. Different tracers for SN mapping, with varying sizes and isotopes have been reported, most of which have proven their value in a clinical setting. A major challenge lies in transferring this diagnostic information obtained at the nuclear medicine department to the operating theatre thereby providing the surgeon with (image) guidance. Conventionally, an intraoperative injection of vital blue dye or a fluorescence dye is given to allow intraoperative optical SN identification. However, for some indications, the radiotracer-based approach remains crucial. More recently, hybrid tracers, that contain both a radioactive and fluorescent label, were introduced to allow for direct integration of pre- and intraoperative guidance technologies. Their potential is especially high when they are used in combination with new surgical imaging modalities and navigation tools. Next to a description of the known tracers for SN mapping, this review discusses the application of hybrid tracers during SN biopsy and how the introduction of these new techniques can further aid in translation of nuclear medicine information into the operating theatre.

SUMMARY ANSWER
For virtually all couples with RM amniocentesis in all ongoing pregnancies without the knowledge of parental carrier status is less expensive in preventing the birth of a handicapped child than parental chromosome analysis followed by amniocentesis in case of carrier status of one of the parents.
WHAT IS KNOWN ALREADY
One of the causes of RM is a balanced chromosome abnormality in one of the partners. If one of the partners is carrier of a balanced structural chromosomal abnormality, the risk of offspring with an unbalanced structural chromosome abnormality is increased. Like all couples, couples with RM also have an age-dependent risk for fetal aneuploidy, of which trisomy 21 is most common.
STUDY DESIGN, SIZE, DURATION
Model-based economic analysis to compare costs and effects of two strategies in couples with RM to prevent the birth of a handicapped child in case of ongoing pregnancy.
PARTICIPANTS/MATERIALS, SETTING, METHODS
Comparison of two strategies in women with RM: strategy (I) parental chromosome analysis followed by amniocentesis in pregnancy in case of carrier status of one of the parents and strategy (II) amniocentesis in all ongoing pregnancies without the knowledge of carrier status. No testing was the reference strategy. Data on probabilities and costs were derived from the literature. Incremental costs and effects were calculated incremental cost-effectiveness ratio (ICER). Effectiveness was expressed as the number of prevented births of handicapped child equivalents compared with no testing. In these calculations, the birth of a handicapped child was valued 10 times worse than the loss of a viable pregnancy due to amniocentesis.
MAIN RESULTS AND THE ROLE OF CHANCE
Depending on the risk for carrier status, the ICER for Strategy I (parental chromosome analysis followed by amniocentesis in case of carrier status of one of the parents) varied between €226 000 and €6 556 000 per prevented handicapped child equivalent. For Strategy II (amniocentesis in all ongoing pregnancies without the knowledge of carrier status), the ICER varied between €2000 and €233 000 per prevented handicapped child equivalent. Strategy I was less expensive than Strategy II only for a small subgroup of couples with maternal age <23 years, three or more previous miscarriages and a family history of RM.
LIMITATIONS, REASONS FOR CAUTION
Our analysis is not a plea for amniocentesis in all women with RM. Individual risk assessment with serum markers and nuchal translucency is probably more effective at lower cost.
WIDER IMPLICATIONS OF THE FINDINGS
This analysis can be used by clinicians to explain the chances of adverse pregnancy outcome in couples with RM, as well as by policy makers in health-care economics. Future guidelines on RM might be more restrictive from the perspective of the limited health-care resources that we have available.

We present the results of a four-month campaign searching for low-frequency radio transients near the North Celestial Pole with the Low-Frequency Array (LOFAR), as part of the Multifrequency Snapshot ...Sky Survey (MSSS). The data were recorded between 2011 December and 2012 April and comprised 2149 11-min snapshots, each covering 175 deg2. We have found one convincing candidate astrophysical transient, with a duration of a few minutes and a flux density at 60 MHz of 15–25 Jy. The transient does not repeat and has no obvious optical or high-energy counterpart, as a result of which its nature is unclear. The detection of this event implies a transient rate at 60 MHz of
$3.9^{+14.7}_{-3.7}\times 10^{-4}$
d−1 deg−2, and a transient surface density of 1.5 × 10−5 deg−2, at a 7.9-Jy limiting flux density and ∼10-min time-scale. The campaign data were also searched for transients at a range of other time-scales, from 0.5 to 297 min, which allowed us to place a range of limits on transient rates at 60 MHz as a function of observation duration.
We describe the APERture Tile In Focus (Apertif) system, a phased array feed (PAF) upgrade of the Westerbork Synthesis Radio Telescope that transforms this telescope into a high-sensitivity, ...wide-field-of-view L-band imaging and transient survey instrument. Using novel PAF technology, up to 40 partially overlapping beams are formed on the sky simultaneously, significantly increasing the survey speed of the telescope. With this upgraded instrument, an imaging survey covering an area of 2300 deg2 is being performed that will deliver both continuum and spectral line datasets, of which the first data have been publicly released. In addition, a time domain transient and pulsar survey covering 15 000 deg2 is in progress. An overview of the Apertif science drivers, hardware, and software of the upgraded telescope is presented, along with its key performance characteristics.
Aims. We present the highest-quality polarisation profiles to date of 16 non-recycled pulsars and four millisecond pulsars, observed below 200 MHz with the LOFAR high-band antennas. Based on the ...observed profiles, we perform an initial investigation of expected observational effects resulting from the propagation of polarised emission in the pulsar magnetosphere and the interstellar medium. Methods. The polarisation data presented in this paper have been calibrated for the geometric-projection and beam-shape effects that distort the polarised information as detected with the LOFAR antennas. We have used RM Synthesis to determine the amount of Faraday rotation in the data at the time of the observations. The ionospheric contribution to the measured Faraday rotation was estimated using a model of the ionosphere. To study the propagation effects, we have compared our low-frequency polarisation observations with archival data at 240, 400, 600, and 1400 MHz. Results. The predictions of magnetospheric birefringence in pulsars have been tested using spectra of the pulse width and fractional polarisation from multifrequency data. The derived spectra offer only partial support for the expected effects of birefringence on the polarisation properties, with only about half of our sample being consistent with the model’s predictions. It is noted that for some pulsars these measurements are contaminated by the effects of interstellar scattering. For a number of pulsars in our sample, we have observed significant variations in the amount of Faraday rotation as a function of pulse phase, which is possibly an artefact of scattering. These variations are typically two orders of magnitude smaller than that observed at 1400 MHz by Noutsos et al. (2009), for a different sample of southern pulsars. In this paper we present a possible explanation for the difference in magnitude of this effect between the two frequencies, based on scattering. Finally, we have estimated the magnetospheric emission heights of low-frequency radiation from four pulsars, based on the phase lags between the flux-density and the PA profiles, and the theoretical framework of Blaskiewicz et al. (1991, ApJ, 370, 643). These estimates yielded heights of a few hundred km; at least for PSR B1133+16, this is consistent with emission heights derived based on radius-to-frequency mapping, but is up to a few times larger than the recent upper limit based on pulsar timing. Conclusions. Our work has shown that models, like magnetospheric birefringence, cannot be the sole explanation for the complex polarisation behaviour of pulsars. On the other hand, we have reinforced the claim that interstellar scattering can introduce a rotation of the PA with frequency that is indistinguishable from Faraday rotation and also varies as a function of pulse phase. In one case, the derived emission heights appear to be consistent with the predictions of radius-to-frequency mapping at 150 MHz, although this interpretation is subject to a number of systematic uncertainties.
Stem cell transplantation (SCT) is associated with oral microbial dysbiosis. However, long-term longitudinal data are lacking. Therefore, this study aimed to longitudinally assess the oral microbiome ...in SCT patients and to determine if changes are associated with oral mucositis and oral chronic graft-versus-host disease. Fifty allogeneic SCT recipients treated in two Dutch university hospitals were prospectively followed, starting at pre-SCT, weekly during hospitalization, and at 3, 6, 12, and 18 months after SCT. Oral rinsing samples were taken, and oral mucositis (WHO score) and oral chronic graft-versus-host disease (NIH score) were assessed. The oral microbiome diversity (Shannon index) and composition significantly changed after SCT and returned to pre-treatment levels from 3 months after SCT. Oral mucositis was associated with a more pronounced decrease in microbial diversity and with several disease-associated genera, such as Mycobacterium, Staphylococcus, and Enterococcus. On the other hand, microbiome diversity and composition were not associated with oral chronic graft-versus-host disease. To conclude, dysbiosis of the oral microbiome occurred directly after SCT but recovered after 3 months. Diversity and composition were related to oral mucositis but not to oral chronic graft-versus-host disease.
Dispersion in the interstellar medium is a well known phenomenon that follows a simple relationship, which has been used to predict the time delay of dispersed radio pulses since the late 1960s. We ...performed wide-band simultaneous observations of four pulsars with LOFAR (at 40–190 MHz), the 76-m Lovell Telescope (at 1400 MHz) and the Effelsberg 100-m Telescope (at 8000 MHz) to test the accuracy of the dispersion law over a broad frequency range. In this paper we present the results of these observations which show that the dispersion law is accurate to better than 1 part in 105 across our observing band. We use this fact to constrain some of the properties of the interstellar medium along the line-of-sight and use the lack of any aberration or retardation effects to determine upper limits on emission heights in the pulsar magnetosphere. We also discuss the effect of pulse profile evolution on our observations, and the implications that it could have for precision pulsar timing projects such as the detection of gravitational waves with pulsar timing arrays.
The popularity gained by the sentinel lymph node (SLN) procedure in the last two decades did increase the interest of the surgical disciplines for other applications of radioguided surgery. An ...example is the gamma-probe guided localization of occult or difficult to locate neoplastic lesions. Such guidance can be achieved by intralesional delivery (ultrasound, stereotaxis or CT) of a radiolabelled agent that remains accumulated at the site of the injection. Another possibility rested on the use of systemic administration of a tumour-seeking radiopharmaceutical with favourable tumour accumulation and retention. On the other hand, new intraoperative imaging devices for radioguided surgery in complex anatomical areas became available. All this a few years ago led to the delineation of the concept Guided intraOperative Scintigraphic Tumour Targeting (GOSTT) to include the whole spectrum of basic and advanced nuclear medicine procedures required for providing a roadmap that would optimise surgery. The introduction of allied signatures using, e.g. hybrid tracers for simultaneous detection of the radioactive and fluorescent signals did amply the GOSTT concept. It was now possible to combine perioperative nuclear medicine imaging with the superior resolution of additional optical guidance in the operating room. This hybrid approach is currently in progress and probably will become an important model to follow in the coming years. A cornerstone in the GOSTT concept is constituted by diagnostic imaging technologies like SPECT/CT. SPECT/CT was introduced halfway the past decade and was immediately incorporated into the SLN procedure. Important reasons attributing to the success of SPECT/CT were its combination with lymphoscintigraphy, and the ability to display SLNs in an anatomical environment. This latter aspect has significantly been improved in the new generation of SPECT/CT cameras and provides the base for the novel mixed reality protocols of image-guided surgery. In these protocols the generated virtual SPECT/CT elements are visually superimposed in the body of the patient in the operating room to directly facilitate, by means of visualization on screen or using head-mounted devices, the localization of radioactive and/or fluorescent targets by minimal invasive approaches in areas of complex anatomy. All these technological advances will play an increasing role in the future extension and the clinical impact of the GOSTT concept.
