Background
Occupational contact dermatitis often results in work disruption. Return‐to‐work (RTW) is an important outcome.
Objective
The objective of this study was to determine RTW outcomes and ...factors associated with such outcomes using a multidisciplinary clinic model.
Methods
Chart ion was performed for 194 workers who received RTW assistance over a 6‐year period. Elements ed included demographic and diagnostic information and information about the RTW program including principles, program components, barriers, and facilitators.
Results
Of the 902 workers seen for dermatologic assessment, 194 received RTW assistance. At initial assessment, 37% were not working because of their skin disease, and at follow‐up, 7% were not working because of their skin disease. The RTW plan components included a graduated or trial of RTW, specific recommendations for avoiding exposure, personal protective equipment, skin management, and ongoing skin monitoring. Principles associated with successful RTW included good communication and the availability of modified work and a worker adherence to the plan. Barriers included lack of modified work, unresponsive employers, and ongoing skin problems.
Conclusions
Specific approaches are important to identify if RTW is to be successful for workers with occupational contact dermatitis.
With an increasing prevalence during the past decades, atopic dermatitis has become a global health issue. A literature search following a targeted approach was undertaken to perform this ...non-systematic review, which intends to provide an overview of the epidemiology, pathophysiology, clinical features, comorbidities, and current therapies for the treatment of atopic dermatitis. In sum, this is a heterogeneous skin disorder associated with variable morphology, distribution, and disease course. Although not completely understood, its pathogenesis is complex and seems to result from a combination of genetic and environmental factors that induce skin barrier dysfunction, cutaneous and systemic immune dysregulation, skin microbiota dysbiosis, and a strong genetic influence. Diagnosis is based on specific criteria that consider patient and family history and clinical manifestations. Overall disease severity must be determined by evaluating both objective signs and subjective symptoms. Therapeutic goals require a multistep approach, focusing on reducing pruritus and establishing disease control. Patients should be advised on basic skin care and avoidance of triggers. Topical anti-inflammatory agents should be considered in disease flares or chronic/recurrent lesions. In case of inadequate response, phototherapy, systemic immunosuppressants and, more recently, dupilumab, should be added. Nevertheless, the treatment of moderate-to-severe atopic dermatitis remains challenging and novel, efficacious, safe and targeted treatments are urgently needed. In conclusion, although the last few years have seen important improvement in the understanding of the disease, future research in atopic dermatitis will continue exploring gene-environment interactions and how it affects pathophysiology, disease severity, and treatment outcomes.

: Antinukleäre Antikörper (ANA) sind ein typischer Befund bei Patienten mit einer Kollagenose und können mittels Immunfluoreszenz auf HEp2‐Zellen nachgewiesen werden. ...Selbst hohe Titer sind jedoch nicht beweisend für das Vorliegen einer Kollagenose und können zu Fehldiagnosen führen. Anti‐DFS70‐Antikörper sind ANA, welche auch bei Menschen ohne Kollagenose in hohen Titern vorliegen können. Daher wollten wir evaluieren, ob sie zu einer Fehldiagnose beitragen können. Da in Japan beschrieben wurde, dass Anti‐DFS70‐Antikörper mit atopischer Dermatitis (AD) assoziiert sind, wollten wir diese Assoziation und ihre potenzielle diagnostische Relevanz auch in Deutschland untersuchen.
Patienten und Methodik
: Wir analysierten retrospektiv die Daten von 40 Patienten, welche zur Mitbeurteilung bei Kollagenose zu uns überwiesen wurden. Prospektiv erhoben wir die Prävalenz von Anti‐DFS70‐Antikörpern bei 110 AD‐Patienten und 89 Kontrollpatienten.
Ergebnisse
: Bei 75% der überwiesenen Patienten konnten wir keine Kollagenose nachweisen. 26% dieser Patienten erhielten im Vorfeld bereits systemische Therapien. Das DFS70‐typische Fluoreszenzmuster konnte bei 35%, Anti‐DFS70‐Antikörper bei 12,5% dieser Patienten nachgewiesen werden. Bei den AD‐Patienten trat in 22% der Fälle das DFS70‐typische Fluoreszenzmuster auf und in 10% der Fälle zeigten sich Anti‐DFS70‐Antikörper (im Vergleich zu 5,6% und 0% der Kontrollpatienten,

Schlussfolgerungen
: Anti‐DFS70‐Antikörper sind signifikant mit AD assoziiert und könnten für die Fehldiagnose einer Kollagenose mitverantwortlich sein.

Atopische Dermatitis (AD), eine chronisch‐entzündliche Hauterkrankung, die durch wiederkehrende Ekzeme und starken Juckreiz gekennzeichnet ist, sorgt weltweit für eine erhebliche ...Krankheitslast. Die Diagnose und Behandlung der AD ist aufgrund der großen Heterogenität der Erkrankung oft schwierig, und ihre genaue Ätiologie ist unbekannt. Die Metabolomik bietet die Möglichkeit, kontinuierliche physiologische und pathologische Veränderungen bei Individuen zu verfolgen. Dies ermöglicht eine genaue Diagnose und Behandlung sowie tiefe Einblicke in die Ätiopathogenese der AD. In den letzten Jahren wurden mehrere metabolomische Studien zur AD veröffentlicht. Ziel dieser Übersicht ist es, diese Ergebnisse zusammenzufassen und den Wissenschaftlern zu helfen, die rasche Entwicklung der Metabolomik bei der AD zu verstehen.
Es wurde eine umfassende und systematische Suche in den Datenbanken PubMed, Embase, Cochrane Library und Web of Science durchgeführt. Nach einer Qualitätsbewertung wurden schließlich sechsundzwanzig Arbeiten in die Untersuchung aufgenommen. Es wurden signifikante Unterschiede zwischen den Metabolitenprofilen von Patienten mit AD und gesunden Personen festgestellt.
Diese Studie bietet einen umfassenden Überblick über die Metabolomforschung zur AD. Ein besseres Verständnis der Metabolomik der AD könnte zu neuen diagnostischen, prognostischen und therapeutischen Ansätzen führen.

Purpose of Review
Occupational hand dermatitis is a common work-related disorder of the skin. Prevention and management of this disease is critical to improving workers’ quality of life and for ...occupation-specific retention.
Recent Findings
This is a critical review of the current literature on occupational hand dermatitis. Occupational dermatitis continues to have a high prevalence among workers although the overall incidence may be slowly decreasing. Irritant contact dermatitis due to wet work exposure is the most common cause of occupational hand dermatitis. Healthcare workers, hairdressers, and metal workers are at particularly high risk for this disease. While some prevention programs have been ineffective in mitigating occupational hand dermatitis, other more resource-intensive initiatives may have benefit.
Summary
Continued research is needed on ways to manage wet work exposures and on scalable, effective prevention programs for occupational hand dermatitis. The spectrum of culprit contact allergens continues to evolve, and vigilance for potential occupation-specific allergens remains important.
Allergic contact dermatitis (ACD) used to be considered a rarity in children, but recently has been estimated to effect 4.4 million children in the USA alone, with a notable rise in investigative ...research in the field of pediatric ACD. Researchers have shown that patch testing is safe and effective in afflicted children and that those with atopic dermatitis (AD) have similar sensitization rates, although they have a higher sensitization to certain allergens, thought to be related to the inflammatory (IL-4) milieu. Patch testing assessment guidelines in children include five key considerations: if a patient’s dermatitis worsens, changes distribution, fails to improve with topical therapy, or immediately rebounds after removal of topical treatments; if a patient has a particular distribution of dermatitis; if a working patient has hand eczema that fails to improve with therapy; if the patient has AD that started in adolescence or adulthood with definitely no history of childhood eczema; and importantly, if a patient has severe or widespread atopic dermatitis that will require immunosuppressive systemic medication.
