Die Etablierung des Kompetenzzentrums Weiterbildung Allgemeinmedizin Bayern (KWAB) erfolgte mit dem Ziel, die Qualität der fachärztlichen Weiterbildung durch das zusätzliche Angebot von Seminar- und ...Mentoring-Programmen für Ärzt*innen in Weiterbildung (ÄiW) sowie für Dozierende durch regelmäßig durchgeführte „Train-the-Trainer“ und Mentoring-Fortbildungen zu erhöhen. Darüber hinaus haben ÄiW die Möglichkeit, ihre Weiterbildung in einem regionalen Weiterbildungsverbund zu absolvieren.
Ziel der vorliegenden Arbeit ist es, das Ausmaß von Burnout und die Bedeutung der Lernumgebung in der stationären Weiterbildungsphase zu untersuchen.
Es wurde eine Querschnittsstudie durchgeführt. Burnout wurde anhand des Maslach Burnout Inventory (MBI), bestehend aus emotionaler Erschöpfung, Depersonalisierung und persönlicher Leistungsfähigkeit, erfasst. Die Qualität der Lernumgebung wurde mithilfe der deutschen Version des Dutch Residency Educational Climate Test (D-RECT German) erhoben. Zudem wurden multivariable lineare Regressionsmodelle erstellt, um den Zusammenhang zwischen den einzelnen Lernumgebungsfaktoren, dem Jahr der Weiterbildung und der Teilnahme an einer Verbundweiterbildung mit dem Grad der Burnout-Belastung abzuschätzen.
129 in das KWAB eingeschriebene ÄiW in der stationären Weiterbildungsphase wurden zur Studie eingeladen, von denen 78 (61 %) teilnahmen. Von diesen wiederum konnten 76 (59 %) in die Analysen eingeschlossen werden. Die vorliegende Studie zeigt eine erhöhte Burnout-Belastung von ÄiW in der stationären Weiterbildungsphase, wobei etwa 40 % einen kritischen Burnout-Score erreichten. Eine als höher bewertete Qualität der Lernumgebung war bei einem Großteil der D-RECT-Skalen mit signifikant geringeren Burnout-Symptomen assoziiert.
ÄiW der Fachrichtung Allgemeinmedizin weisen im stationären Abschnitt eine hohe Burnout-Belastung auf. Eine hohe Qualität der Lernumgebung erscheint als ein wirksames Schlüsselelement, um eine Reduktion der Burnout-Belastung in der stationären Weiterbildung zu erzielen. Dies könnte zu einer Erhöhung der Berufszufriedenheit beitragen, was letztlich auch Abwanderungen aus dem ärztlichen Beruf vorbeugen würde.
The Competence Centre for Residency Training in Family Medicine Bavaria (CCRTB) was established to improve the quality of postgraduate medical training by offering additional seminars and mentoring programmes as well as regular ‘train-the-trainer’ courses for educating physicians. In addition, residents have the opportunity to participate in a regional training network.

We conducted a cross-sectional study. Burnout was assessed using the Maslach Burnout Inventory (MBI), which comprises the scales “Emotional Exhaustion”, “Depersonalisation” and “Personal Accomplishment”. The quality of the learning environment was recorded using the German version of the Dutch Residency Educational Climate Test (D-RECT German). In addition, multivariable linear regressions were performed to estimate the impact of learning environment, year of training and participation in a regional network on the level of burnout.
129 clinical residents enrolled in the CCRTB were invited to participate in the study, 78 (61%) of whom submitted a response. 76 (59%) of these residents were included in the analyses. The present study discloses an increased burden of burnout among residents in the clinical training phase, with approx. 40% reaching a critical burnout score. A higher quality of the learning environment was associated with significantly milder burnout symptoms on the majority of the D-RECT scales.
Family medicine residents in the clinical training phase show a high burden of burnout. Therefore, increasing the quality of the learning environment appears to be an effective key element in achieving a reduction of burnout in clinical training. This might contribute to an increase in professional satisfaction, which finally may also prevent migration from the medical profession.

Das gesundheitsrelevante Verhalten ist eine wichtige Determinante chronischer Krankheiten. Hausärzten kommt dabei die bedeutende Rolle zu, Patienten bei der Verbesserung ihres Gesundheitsverhaltens ...zu unterstützen; Hindernisse müssen aber berücksichtigt werden. Im neuen Beratungsprogramm «Gesundheitscoaching KHM» (www.gesundheitscoaching-khm.ch) arbeiten Patient und Arzt als Team in vier Schritten (sensibilisieren – Motivation aufbauen – planen – ausführen und begleiten) daran, ein selbstgewähltes Ziel der Verhaltensänderung zu erreichen. Entscheidend sind dabei kommunikative Fertigkeiten des Arztes, die im Programm trainiert werden. Ein Pilottest mit 20 Ärzten und 1045 Patienten zeigte eine hervorragende Akzeptanz und Praktikabilität des Programmes; nach vier Beratungssitzungen verbesserte jeder zweite Teilnehmer sein Gesundheitsverhalten im gewählten Ziel um mindestens eine (von zwei möglichen) Kategorien. Für die Weiterverbreitung ist die Schaffung von günstigen Rahmenbedingungen essenziell.
Le comportement lié à la santé est un déterminant principal des maladies chroniques. Les médecins de famille jouent un rôle important dans le soutien des patients à ce sujet, mais les barrières doivent être adressées. Dans le nouveau programme «Coaching Santé KHM» patient et médecin travaillent ensemble en quatre étapes (sensibiliser – motiver – planifier – prendre action). Dans ce processus, les compétences de communication des médecins (formées au sein du programme) sont essentiellement importantes. Une étude pilote avec 20 médecins et 1045 patients a montré une acceptabilité et faisabilité excellente: La moité des participants a amélioré le comportement dans le domaine choisi d'au moins une catégories (des deux possible). Pour la dissémination la création de conditions favorables est essentielle.
Health related behaviour is a main determinant of chronic disease. Family physicians have an important role in the support of patients to improve their health behaviour, but relevant barriers should be addressed. In the new “Health Coaching KHM” program, patients and physicians are working as a team in four steps (sensitize – create motivation – plan – act). In this process, physicians' communication skills (which are trained within the program) are of utmost importance. In a pilot study with 20 familiy physicians and 1045 patients acceptance and feasibility were excellent: Every second participant improved her or his behaviour in the area of choice by at least one (of two possible) categories. For dissemination improving framework is crucial.
