An Inaugural Address is a speech made by a speaker when he or she assumes a new position, stating his or her position, views and policies in light of domestic and international political and economic ...situations. Based on Martin's Appraisal Theory, this paper mainly uses the three subsystems of Appraisal Theory: Attitude, Engagement, and Graduation to explore and analyze the evaluative devices in Kennedy's Inaugural Address and its expression of the speaker's views and attitudes, and to fully reveal the reasons for the enduring popularity of this speech.

An 84-year-old woman with depression, who witnessed the suicide of a close friend, presented with symptoms of chest pain, palpitations, and cold and clammy extremities. An electrocardiogram showed ...alternating tachycardia and bradycardia. Urgent transthoracic echocardiogram demonstrated left greater than right ventricular dysfunction, moderate mitral regurgitation, global hypokinesis, and an estimated ejection fraction of 20%. Cardiac catheterization demonstrated non-obstructive coronary artery disease and decreased cardiac output. Findings were consistent with Takotsubo cardiomyopathy complicated with cardiogenic shock, acute mitral regurgitation, and sinus node dysfunction. Management of this patient required the use of a mechanical device intra-aortic balloon pump, and pacemaker insertion for persistent symptomatic arrhythmia. This case highlights the challenging management of potentially fatal acute complications of Takotsubo cardiomyopathy and inadequate data on how to approach them.
IntroductionOver half of the 5 million annual deaths among children aged 0–59 months occur in sub-Saharan Africa. The period immediately after hospitalisation is a vulnerable time in the life of a ...child in sub-Saharan Africa as postdischarge mortality rates are as high as 1%–18%. Identification of neonates and children who are at highest risk for postdischarge mortality may allow for the direction of interventions to target patients at highest risk.Methods and analysisThe Predicting Post-Discharge Mortality study is a prospective, observational study being conducted at Muhimbili National Hospital (Dar es Salaam, Tanzania) and John F. Kennedy Medical Center (Monrovia, Liberia). The aim is to derive and validate two, age population specific, clinical prediction rules for the identification of neonates (n=2000) and children aged 1–59 months (n=2000) at risk for all-cause mortality within 60 days of discharge from the neonatal intensive care unit or paediatric ward. Caregivers of participants will receive phone calls 7, 14, 30, 45 and 60 days after discharge to assess vital status. Candidate predictor variables will include demographic, anthropometric and clinical factors. Elastic net regression will be used to derive the clinical prediction rules. Bootstrapped selection with repetitions will be used for internal validation. Planned secondary analyses include the external validation of existing clinical prediction models, determination of clinicians’ ability to identify neonates and children at risk of postdischarge mortality at discharge, analysis of factors associated with hospital readmission and unplanned clinic visits and description of health-seeking behaviours in the postdischarge period.Ethics and disseminationThis study received ethical clearance from the Tanzania National Institute of Medical Research, Muhimbili University of Health and Allied Sciences, the John F. Kennedy Medical Center Institutional Review Board, and the Boston Children’s Hospital Institutional Review Board. Findings will be disseminated at scientific conferences and as peer-reviewed publications.

Abdominal aortic aneurysms (AAA) are considered giant when they exceed >10cm, and they are rare, with only a few cases described as >14cm. AAAs can be repaired through endovascular aneurysm repair ...(EVAR) or open surgery. EVAR involves the placement of a graft that contacts the aortic wall and the iliac vessels to prevent the aneurysm sac to have blood flow and pressure. One of the complications of EVAR is endoleak, the most common being type II. We describe an uncommon case of a progressively giant AAA with type II endoleak with poor evolution despite multiple repair attempts.

Political historians who seek to evaluate John F. Kennedy's life, career, and legacy face a dilemma: for almost everyone else, his legendary persona overwhelms what he accomplished as a politician. ...That has been true since his death, over five decades ago, and it is still true now, a full century after his birth. Here, Kazin focuses on four issues--sexuality, political culture, civil rights, and the Vietnam War that have always been and remain central to this vibrant corner of political and historical commentary. To understand the intertwining of celebrity and politics, attention should be paid to the three major phases of Kennedy's life and afterlife: his pre-presidential years, his administration of just over one thousand days, the unceasing debate about who assassinated him and why, and the work his memory does in America's public life and the imagination(s) of its inhabitants.
