Pheochromocytoma and paraganglioma (PPGL) are rare tumours and at least 30% are part of hereditary syndromes. Approximately 20% of hereditary PPGL are caused by pathogenic germ line variants in genes ...of the succinate dehydrogenase complex (SDHx), TMEM127 or MAX. Herein we present guidelines regarding genetic testing of family members and their surveillance based on a thorough literature review. All cases of PPGL are recommended genetic testing for germ line variants regardless of patient and family characteristics. At minimum, FH, NF1, RET, SDHB, SDHD and VHL should be tested. In addition, testing of MEN1, SDHA, SDHAF2, SDHC, TMEM127 and MAX is recommended. Healthy first‐degree relatives (and second‐degree relatives in the case of SDHD and SDHAF2 which are maternally imprinted) should be offered carrier testing. Carriers of pathogenic variants should be offered surveillance with annual biochemical measurements of methoxy‐catecholamines and bi‐annual rapid whole‐body magnetic resonance imaging and clinical examination. Surveillance should start 5 years before the earliest age of onset in the family and thus only children eligible for surveillance should be offered pre‐symptomatic genetic testing. The surveillance of children younger than 15 years needs to be individually designed. Our guidelines will provide a framework for patient management with the possibility to follow outcome via national registries and/or follow‐up studies. Together with improved insights into the disease, this may enable optimisation of the surveillance scheme in order to minimise both anxiety and medical complications while ensuring early disease detection.

Background: Endoscopic removal of recurrent phaeochromocytomas and retroperitoneal paragangliomas (extra‐adrenal phaeochromocytomas) is a challenging surgical procedure.
Methods: In a prospective ...clinical study, five patients (three males, two females; age: 15–63 years) with recurrent phaeochromocytomas and eight patients (three males, five females; age: 21–60 years) with paragangliomas (twice in combination with phaeochromocytomas) were treated endoscopically. Three patients showed multiple (2 to 5) tumours. Tumour size ranged from 1 to 4 cm. Six patients suffered from von Hippel‐Lindau disease. Twelve tumours were removed using the laparoscopic route; the retroperitoneoscopic technique was performed for nine tumours. High‐dosage alpha‐blockage with phenoxybenzamine was routinely used.
Results: Conversion to open surgery was not necessary. Minor perioperative complications occurred in two patients (15 %); mortality was zero. Mean operating time for recurrent phaeochromocytomas was 172 min (range: 120–255 min), and operating time for paragangliomas ranged from 75 to 600 min. Mean blood loss was 130 mL, and median duration of postoperative hospitalization was 5 days. No recurrences were observed at a mean follow‐up of 30 months.
Conclusions: Endoscopic removal of recurrent phaeochromocytomas and retroperitoneal paragangliomas is feasible and safe but requires extensive experience in minimally invasive surgery as well as in endocrine surgery.
Zusammenfassung: Grundlagen: Die endoskopische Exstirpation von Phäochromozytom‐Rezidiven und von retroperitonealen Paragangliomen (extraadrenalen Phäochromozytomen) ist eine besondere chirurgische Herausforderung.
Methodik: Im Rahmen einer prospektiven Studie wurden 5 Patienten (3 m, 5 w, Alter 15–63 Jahren) mit rezidivierten Phäochromozytomen und 8 Patienten (3 m, 5 w, Alter 21–60 Jahre) mit extraadrenalen Phäochromozytomen (zweimal in Kombination mit adrenalen Phäochromozytomen) endoskopisch operiert. Drei Patienten hatten multiple (2 bis 5) Tumoren. Die Tumorgröße schwankte zwischen 1 und 4 cm. Sechs Patienten litten an von Hippel‐Lindau‐Syndrom. Die laparoskopische Methode wurde bei 12 Tumoren angewandt, 9 Neoplasien wurden retroperitoneoskopisch entfernt. Eine hochdosierte α‐Rezeptorblokkade mit Phenoxybenzamin wurde routinemäßig angewandt.
Ergebnisse: Eine Konversion zur offenen Technik war in keinem Falle erforderlich. Perioperativ traten bei 2 Patienten (15%) unbedeutende Komplikationen auf, kein Patient ist verstorben. Die mittlere Operationsdauer lag bei den Phäochromozytom‐Rezidiven bei 172 Minuten (Spanne: 120–255 Minuten), bei den extraadrenalen Phäochromozytomen schwankte sie zwischen 75 und 600 Minuten. Der Blutverlust lag bei 130 mL, der mediane postoperative Klinikaufenthalt bei 5 Tagen. Rezidive wurden nach einer medianen Nachbeobachtungszeit von 30 Monaten nicht beobachtet.
Schlußfolgerungen: Die endoskopische Therapie von Phäochromozytom‐Rezidiven und von retroperitonealen Paragangliomen ist unter extensiver Erfahrung in der minimal‐invasiven Methodik und der endokrinen Chirurgie sicher durchführbar.
Phäochromozytome, Paragangliome und Neuroblastome sind seltene neuroendokrine Tumoren, die durch eine erhöhte Katecholaminproduktion charakterisiert sind. Die Katecholamine werden noch in den ...Tumorzellen zu Metanephrinen O-methyliert. Diese Tatsache erklärt die hohe diagnostische Sensitivität und Spezifität der Katecholaminmetabolite zur Diagnostik von Phäochromozytomen und Paragangliomen. Altersangepasste Referenzwerte von Normetanephrin, dem O-methylierten Metabolit von Noradrenalin, erhöhen hierbei noch die diagnostische Spezifität. Gemessen an der höheren diagnostischen Sensitivität sind die auf Flüssigkeitschromatographie beruhenden Methoden, Metanephrine im Patientenplasma zu messen, immunologischen Verfahren überlegen. Für das Neuroblastom-Screening bleibt die Messung der Tumormarker Vanillinmandelsäure und Homovanillinsäure der am häufigsten benutzte Test, obwohl sie eine eingeschränkte diagnostische Sensitivität haben. Für die Diagnose solcher pädiatrischen Tumoren ist die Suche nach neuen, verbesserten Biomarkern notwendig.
Pheochromocytoma, paraganglioma, and neuroblastoma are rare neuroendocrine tumors characterized by elevated production of catecholamines. The catecholamines are O-methylated to metanephrines in tumor cells, explaining their high diagnostic sensitivity and specificity for diagnosis of pheochromocytoma and paraganglioma. Age-adjusted reference values of normetanephrine, the O-methylated metabolite of norepinephrine, are useful for improving diagnostic specificity. Among available methods for measuring plasma metanephrines, liquid chromatographic based analyses are the most accurate and precise. For screening of neuroblastoma, measurements of vanillylmandelic acid and homovanillic acid in urine remain the most commonly used tests, but have limited diagnostic sensitivity. For such childhood tumors there is a need for new and improved biomarkers for diagnosis.
Background: The purpose of this study was to assess the potential of fluorine‐18 fluoro‐2‐deoxy‐d‐glucose positron emission tomography (18F‐FDG‐PET) for monitoring the efficacy of iodine‐131 ...metaiodobenzylguanidine (131I‐MIBG) therapy in neuroendocrine tumours.
Methods: A total of seven 131I‐MIBG therapies with 3.7 to 10.2 GBq were carried out in three patients suffering respectively from a phaeochromocytoma, a paraganglioma and a metastatic neuroendocrine tumour of an unknown primary. The post‐therapeutic whole‐body scintigrams were compared with the results of six 18F‐FDG‐PET studies performed at the time of the therapies. One patient received three PET scans prior to each one of the MIBG therapies, and one patient was studied twice.
Results: 18F‐FDG uptake in tumour sites seemed to correlate well with tumour differentiation, showing no uptake in one patient with a highly differentiated neuroendocrine tumour, and moderate‐to‐intense uptake in the two other patients with metastatic disease. Those tumour sites that had a simultaneous positive uptake in both the MIBG scintigram and the PET scan showed response to therapy as a continuous reduction in MIBG uptake over time. They also showed a qualitative decrease in FDG accumulation during the follow‐up. This was associated with a decrease in the mean and maximum standard uptake values of more than 50 % in some metastases, while the X‐ray computed tomography showed no decrease in tumour volume. Two patients revealed additional metastases that were unknown on the basis of prior diagnostic or therapeutic PET scans and radiological follow‐up.
Conclusions: It may be concluded from these cases that 18F‐FDG‐PET is a valuable tool for an initial metabolic staging of neuroendocrine tumours prior to 131I‐MIBG therapy, as it can reveal tumour sites beyond the reach of radioisotope therapy. It may also be of importance in assessing therapeutic potential in those tumour sites that do show positive MIBG uptake.
Zusammenfassung: Grundlagen: In einer Pilotstudie untersuchten wir das Potential der Positronen‐Emissions‐Tomographie (PET) als Parameter zur Effektivitätskontrolle einer Therapie Jod‐131‐Metaiodbezylguanidin (MIBG) bei metastasierten neuroendokrinen Tumoren.
Methodik: Bei drei Patienten wurden insgesamt sieben MIBG‐Therapien mit Einzelaktivitäten von 3,7 bis 10,2 GBq Jod‐131‐MIBG durchgeführt. Es lag in je einem Fall ein Phäochromozytom, ein Paragangliom sowie ein metastasierender neuroendokriner Tumor unbekannten Ursprunges (CUP) vor. Die mittels der Therapieaktivität angefertigten Ganzkörperszintigramme wurden dabei mit sechs Fluor‐18‐Desoxyglukose (FDG)‐PET‐Studien verglichen. Ein Patient erhielt drei PET jeweils unmittelbar vor einer von insgesamt drei MIBG‐Therapien. Ein Patient erhielt zwei PET bzw. MIBG‐Therapien.
Ergebnisse: Die F18‐FDG‐Anreicherung zeigte eine inverse Korrelation zur Differenzierung des Tumors. Dabei wies der Patient mit dem metastasierten Tumor unbekannten Ursprunges bei deutlicher MIBG‐Speicherung keine FDG‐Anreicherung in den Metastasen auf. Die anderen Patienten zeigten eine moderate bis intensive Speicherung des FDG. Diejenigen Metastasen mit MIBG‐ und FDG‐Anreicherung zeigten eine Therapieresponse im Sinne einer kontinuierlichen Abnahme der MIBG‐Anreicherung im Verlauf mehrerer Behandlungen. Parallel konnte mittels quantitativer Analyse des Standard‐Uptake‐Values (SUV) auch eine Abnahme der FDG‐Speicherintensität in der PET nachgewiesen werden. Dabei nahm der maximale und mittlere SUV in einzelnen Tumorherden um mehr als 50 % ab, während in der CT keine Abnahme des Herdvolumens beschrieben wurde.
Schlußfolgerungen: Diese ersten Beobachtungen erlauben die Schlußfolgerung, daß die FDG‐PET als Instrument zum initialen Staging neuroendokriner Tumore geeignet ist, da hiermit auch nicht MIBG‐speichernde Metastasen nachgewiesen werden können. Die Effektivität einer MIBG‐Therapie kann mit Hilfe dieser Methode möglicherweise ebenfalls kontrolliert werden, was vor dem Hintergrund der erheblichen Kosten der Isotopentherapie eine wichtige ergänzende Information sein kann.

The majority of gastrointestinal stromal tumours (GISTs) present as solitary gastrointestinal masses in adults aged 50-70 years. A small subset of GISTs (≤5%) occurs in the setting of familial or ...idiopathic multitumour syndrome. In decreasing order of frequency, neurofibromatosis Recklinghausen (NF-1), Carney triad (gastric GIST, pulmonary chondroma and extra-adrenal paraganglioma), familial GIST syndromes resulting from germline mutations in c-Kit/PDGFRA and the Carney-Stratakis syndrome (hereditary GIST paraganglioma syndrome caused by germline mutations in the mitochondrial tumour suppressor gene pathway involving the succinate dehydrogenase subunits SDHD, SDHC and SDHB) represent the four most important GIST syndromes characterized to date. Since affected patients and their family members require special treatment and/or counseling and follow-up, early diagnosis and precise classification of this likely still underdiagnosed diseases is of the utmost importance. This review summarizes the pertinent clinicopathological and molecular features of the main GIST syndromes to facilitate their diagnosis and distinction from their non-syndromic mimics.
십이지장의 소유두에 생기는 종양은 매우 드물고 주로 유암종이나 somatostatinoma와 같은 신경내분비계 종양으로 알려져 왔다. 소유두의 종양은 대부분 점막하종양의 소견을 보이므로 일반적인 조직검사만으로 진단이 어렵기 때문에 주로 수술이나 내시경 절제술에 의해서 진단이 가능하다. 이런 소유두의 종양을 진단하기 위해서는 내시경초음파와 ...내시경역행성췌담관조영술이 필요하고 유암종과 같은 악성 종양의 가능성이 있으므로 복부 전산단층촬영이나 자기공명영상의 검사가 필요하다. 국내에서는 주유두에 발생한 신경절세포 부신경절종과 이소성 췌장의 보고가 있었지만 아직까지 소유두에 발생한 이소성 췌장이나 신경절세포 부신 경절종에 대한 보고는 없었다. 저자들은 우연히 발견된 십이지장 소유두의 점막하종양의 2예에서 진단과 치료를 목적으로 시행한 내시경 절제술에 의해서 제거된 이소성 췌장과 신경절세포 부신경절종을 비슷한 시기에 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
Tumors of the minor papilla are very rare and these tumors have generally been known as neuroendocrine tumors such as carcinoid tumor and somatostatinoma. As these are mostly submucosal tumors, their diagnosis is difficult by just performing endoscopic forceps biopsy, but diagnosis is possible by surgery or endoscopic resection. EUS and ERCP is an essential tool for the diagnosis of these tumors, and abdominal CT or MRI is also useful because there is the possibility of malignant tumors such as carcinoid tumor. For our present two cases, screening endoscopy revealed the polypoid lesion of the minor papilla. EUS disclosed that the submucosal tumor was limited to the submucosal layer and no abnormality was found from the abdominal CT and ERCP. Herein, we performed endoscopic resection for making the diagnosis and treatment. Histologically, these 2 tumors were diagnosed as ectopic pancreas and gangliocytic paraganglioma, respectively. Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu ...feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. Přestože chemodektom je všeobecně považován za radiosenzitivní, hlavní terapeutický postup je chirurgická exstirpace tumoru.
Chemodectoma, paraganglioma of glomus caroticum or carotid glomus tumor occurs in the population with an incidence of 1:30 000. The tumor is macroscopically, microscopically and biologically very similar adrenal pheochromocytoma. An important feature of these tumors is their rich vascularization. Among the most blood supply tumors at all. Histologically, alveolar described arrangement of tumor cells, in contrast to pheochromocytoma, sharply demarcated from each other soft tissue with broad stripes capillaries and cells like Schwann cells. The cytoplasm of tumor cells is a major weakly eosinophilic and finely granular. Nuclei of cells may be pleomorphic and hyperchromic, but must be stated that nuclear atypia is not unequivocal sign of malignancy. Biologically they are mostly benign tumors. Clinically, it is a slow-growing, painless, firm and globular in shape for the angle of the mandible. Although chemodectomas were found is generally considered to be radiosensitive, the main therapeutic approach is surgical extirpation of the tumor.
