a učinak se dominantno ostvaruje nakon unutarnje konverzije tiroksina u ...aktivniji oblik, trijodotironin, koji pokazuje
veći afinitet za receptorski kompleks te time modificira gensku ekspresiju ciljnih stanica. Amiodaron je jedan od najčešće
upotrebljavanih antiaritmika i rabi se u liječenju širokog spektra aritmija, najčešće tahiaritmija. U svom sastavu sadrži velik
udio joda, što je, uz intrinzični učinak lijeka, temelj utjecaja na tireoidnu funkciju. Smatra se kako 15%-20% bolesnika liječenih
amiodaronom razvija neki oblik tireoidne disfunkcije. Amiodaron može biti uzrokom razvoja amiodaronom izazvane
hipotireoze (amiodarone-induced hypothyroidism, AIH) ili amiodaronom izazvane tireotoksikoze (amiodarone-induced thyrotoxicosis,
AIT). AIT se češće razvija u područjima sa smanjenim, dok se AIH razvija u područjima s dovoljnim unosom joda.
Razlikujemo dva tipa AIT; tip 1 je češći u bolesnika s podležećom tireoidnom patologijom, najčešće nodoznom strumom ili
latentnom Gravesovom (Basedowljevom) bolešću, dok se tip 2 najčešće razvija u prethodno zdravoj štitnjači. AIH je znatno
češća u bolesnika s otprije poznatim Hashimotovim tireoiditisom. Opisani su i miješani oblici bolesti. Bolesnike liječene
amiodaronom potrebno je redovito pratiti, laboratorijski i klinički, kako bi se pravodobno otkrila bilo kakva odstupanja u
tireoidnoj funkciji. Temelj liječenja AIH-a je nadomjesna terapija levotiroksinom. Često u tim slučajevima nije potrebno
izostavljati amiodaron iz terapije. AIT tipa 1 liječi se tireostaticima, kao i ostale tireotoksikoze. Ako je moguće, preporuča se
prekinuti podležeća amiodaronska terapija. Nasuprot AIT tipa 1, temeljni patofiziološki supstrat kojega je povećana sinteza i
otpuštanje tireoidnih hormona, u AIT tipu 2 osnova je destruktivni tireoiditis uzrokovan amiodaronom, dezetilamiodaronom
kao njegovim glavnim metabolitom i povećanim unosom joda. Osnova liječenja tog tipa bolesti je glukokortikoidna terapija.
Objective
Our aim was to characterize Adverse Drug Reactions (ADRs) related to drug–drug interactions (DDIs) related to involvement of cytochrome P450 (CYP450) isoenzymes in a pharmacovigilance ...database.
Methods
ADRs recorded by Midi-Pyrénées PharmacoVigilance center (France) between 1 January and 31 August 2008 were extracted from the French PharmacoVigilance Database (FPVD).
Results
Among the 1,205 reported ADRs, 16 (1.3 %), can be explained by involvement of CYP450 isoenzymes (including 4 “serious”). All interactions involved CYP inhibitors, mainly for CYP3A4/5.
Conclusion
The percentage of ADRs reported in the pharmacovigilance database and related to CYP450-induced DDIs appears to be relatively low (~ 1–2 %).

Amiodaron je derivat benzofurana koji sadržava do 40% joda. Primjenjuje se za liječenje i prevenciju supraventrikularnih i ventrikularnih tahiaritmija opasnih po život. Učinak na štitnjaču očituje se ...od poremećaja funkcionalnih testova štitnjače do klinički izražene hipotireoze potaknute amiodaronom (AIH) i tirotoksikoze (AIT). Bolesnici s AIH liječe se supstitucijskom terapijom L-tiroksinom, a terapija amiodaronom može se nastaviti. Moramo razlikovati dva oblika AIT-a: hipertireoza potaknuta amiodaronom (AIT I) i tiroiditis (AIT II). AIT I liječi se tirostaticima, a metode definitivnog liječenja jesu jod-131 ili totalna tiroidektomija. Oblik II AIT-a liječi se glukokortikoidima. Bolesnici s AIT-om moraju prekinuti terapiju amiodaronom. Dronedaron je manje učinkovit antiaritmik koji je strukturno i farmakološki sličan amiodaronu, ali ne sadržavaju jod, a toksične nuspojave su rjeđe. Dronedaron se može rabiti za liječenje bolesnika s povećanim rizikom od razvoja AIT-a ili AIH.
Amiodaron erişkin ve pediatrik populasyonda çeşitli supraventriküler ve hayatı tehdit eden ventriküler aritmilerin tedavisinde kullanılan sınıf 3 antiaritmik bir ajandır. Uzun dönem amiodaron ...kullanımı sonucu QT uzaması sık görülmesine rağmen polimorfik ventriküler taşikardi (PVT) olarak bilinen "Torsade de pointes" (TdP) epizodlarına çok nadir olarak neden olabilir. Bu çalışmada da ventriküler taşikardi (VT) nedeniyle amiodaron kullanılmasına bağlı belirgin QT uzaması gelişen toplam 13 (6 kadın, 7 erkek, yaş ort: 57±17.2 yıl) hasta sunulmuştur. Altta yatan neden; 8 hastada iskemik kardiyomiyopati, 3'ünde dilate kardiyomiyopati iken, 2 hastada herhangi bir etyolojik neden bulunamamıştır. QT uzaması en erken 30. saatte (olgu:6) olurken en geç 7. günde (olgu:10) ortaya çıkmıştır. En kısa QT aralığı 480 msn, en uzunu 720 msn ve ortalama 589.2±60.3 msn olarak hesaplanırken en kısa QTc aralığı 519 msn, en uzunu 682 msn ve ortalama 590.2±45.3 msn olarak hesaplanmıştır. İki hastada aynı zamanda hipopotasemi de mevcuttu. Hipopotasemili bir hastada tedavinin 4. gününde belirgin QT uzaması sonrası spontan olarak TdP epizodu gelişmiş ve bu vaka kaybedilmiştir. Sonuç olarak, amiodaron nadir de olsa uzamış QT intervali ile birlikte TdP epizotlarına neden olabilir. Özellikle de hipopotasemi, hipomagnezemi ve hipokalsemi gibi elektrolit bozukluklarında risk artar. Bu nedenle gerek oral, gerekse İV yüksek dozda amiodaron başlanacak hastalarda sıkı bir EKG monitorizasyonu ve elektrolit takibi yapılmalıdır.
Amiodarone is a clas III antiarrhythmic agent that is used in treatment of both supraventricular and life-threatening ventricular arrhythmias in adult and pediatric population. Although lengthening of QT interval is seen frequently during the long term use of amiodarone, it rarely causes polymorphic ventricular tachycardia (PVT) called torsade de pointes (TdP) episodes. In this study, 13 patients (6 female, 7 male, mean age:57±17.2 yrs) with significant QT prolongation after amiodarone due to ventricular tachycardia in our clinics were presented. Underlying disease was ischemic cardiomyopathy in 8 patients, dilated cardiomyopathy in 3 patients, however, in 2 patients no etiologic cause could be found. The earliest QT prolongation occurred in 30th and the latest in 7th day. While the shortest QT interval was 480 msec, the longest was 720 msec and the mean QT interval was calculated as 589.2±60.3 msec, the shortest QTc was 519 msec, the longest 682 msec and mean QTc was 590.2±45.3 msec. Meanwhile, there was hypokalemia in two of the patients. A spontaneous TdP episode occurred in the patient with hypokalemia due to significant QT prolongation in the 4th day of treatment and this patient died. As a result, although rare, amiodarone can cause QT prolongation and associated TdP episodes. This risk increases especially with electrolyte imbalances like hypokalemia, hypomagnesemia and hypocalcemia. For this reason, close ECG monitorization and follow for electrolyte imbalances should be done in the patients who will be treated with high dose amiodarone either orally or intravenously.
The review of long-term international clinical experience with amiodaron covers pharmacological properties of the drug, mechanisms of development of its pharmacodynamic effects basing on its action ...on ionic channels, receptors, impact on thyroid function. Unique pharmacokinetics of amiodaron, high antiarrhythmic efficacy of the drug in supraventricular and ventricular arrhythmia, positive experience in primary and secondary prevention of sudden death are characterized. Amiodaron is effective in patients with atrial fibrillation including those with organic heart disease and chronic cardiac failure. It is safe in relation to different cardiac side effects which are minimal but has some non-cardiac side effects which require control during long-term treatment.
Highlights of primary prevention of death in patients with ventricular arrhythmias (VA) are discussed. Overview of all main clinical trials exploring various anti-arrhythmic drugs in prevention of ...death in patients with VA is presented. It is emphasized that in patients with organic heart disease and VA only beta-blockers and amiodarone are able to reduce mortality, while other drugs have no effect on mortality, or they even increase mortality mainly due to arrhythmogenic effect. Recent clinical studies of the cardioverter-defibrillators efficacy in these patients are presented. It is shown that the use of cardioverter defibrillators compared with pharmacotherapy is more effective in prevention of fatal outcomes.

Kalp kökenli ani ölüm (ani kalp ölümü), en sık gözlenen ölüm şeklidir. Kardiyovasküler hastalıklar sonucu olan ölümlerin yaklaşık yarısı ani ölüm şeklinde olmaktadır. Ventrikül taşiaritmileri, ...özellikle ventrikül fibrilasyonuna dönüşen monomorfik ventrikül taşikardileri, ani ölümün en sık nedenleridir. Hastaların %80'inde eşlik eden koroner arter hastalığı ani ölüm açısından en önemli risk faktörüdür. Bazı hastalarda ani ölüm koroner arter hastalığının ilk ve son belirtisi olarak ortaya çıkar. İmplante edilen kardiyoverter defibrilatörlerin ani ölümü önlemedeki başarısı, bu durumun önlenmesinin mümkün olduğunu göstermiştir. Bu yazıda ani ölüm risk faktörleri ile çeşitli hastalıklarda ani ölümün önlenmesi konusunda yapılan birincil koruma çalışmaları değerlendirilmiştir.
Sudden cardiac death is the most common mode of death, accounting for approximately half of the mortality from cardiovascular diseases. Ventricular tachyarrhythmias, especially sustained monomorphic ventricular tachycardia degenerating to ventricular fibrillation are the most common cause of sudden death. About 80% of the victims have coronary artery disease and sudden death is the first manifestation of the disease in a substantial number of patients. Implantable cardioverter defibrillators have been shown to be effective in preventing sudden cardiac death in various situations. In this article, the risk factors for sudden cardiac death, and the results of relevant studies on its primary prevention in certain diseases are reviewed.
