Inverzna ili obrnuta proteza ramena je implantat koji se posljednjih godina sve češće ugrađuje na mjesto ramenog zgloba. Ima širok spektar indikacija, od višeivernih prijeloma humerusa, ...posttraumatskih artroza pa do artroza uzrokovanih gubitkom rotatorne manžete. U našoj Klinici ugrađuje se od 2004. godine, isprva sporadično, a potom sve više. Tijekom idućih 8 godina ugrađeno je preko 250 ovih proteza. U to nisu uključene proteze koje su ugrađene u drugim bolničkim ustanovama, ali pod vodstvom stručnjaka iz naše ustanove. U poslijeoperacijskom tijeku, prvih 4-6 tjedana, bolesnik je bio imobiliziran, najčešće torakobrahijalnim gipsom. Nakon skidanja imoblizacije uslijedila bi fizikalna terapija. Dužina fizikalna terapije ovisila je o mnogo čimbenika. U pravilu je duže trajala kod bolesnika koji su operirani zbog posttraumatske artroze i prijeoperacijski su imali atrofiju deltoidnog mišića i kontrakturu ramena. Od komplikacija treba spomenuti luksacije proteze u kratkom poslijeoperacijskom tijeku (prva 4 tjedna) i infekcije. Infekcija je osobit problem, a njezino liječenje uključuje i vađenje proteze.

Department of Orthopedics and Physical Medicine. - Data provided by Europeana Collections- Cilj: Ustanoviti učinkovitost ...resekcijske artroplastike po Kelleru uz dotatak osteotomije otvaranja u području baze prve metatarzalne kosti (RAKO) na plantarnu distribuciju opterećenje tijekom stajanja, utvrditi duljine i prosječnu vrijednost opterećanja tijekom faza oslonca pri hodu, te zadovoljstvo bolesnika učinjenim operacijskim zahvatom.
Materijali i metode: Bolesnike u dobi od 65 do 75 godina s jakim stupnjem hallux valgusa podjelili smo nasumce u dvije grupe od po 40 bolesnika. Prvu grupu liječili smo metodom resekcijske artroplastike po Kelleru (RAK), dok smo u drugoj grupi bolesnika dodali još i osteotomiju otvaranja u području baze prve metatarzalne kosti. Bolesnici su obrađivani prije operacije te godinu dana poslije operacijskog zahvata. Bolesnici su klinički obrađivani prema AOFAS skoru. Rendegenološki su mjereni kut hallux valgusa (HV), prvi intermetatarzlani kut (IMK 1.-2.) kut nagiba prve metatarzalne kosti (MT), veličinu resekcije proksimalne falange, kut između 1. i 5. metatarzalne kosti (IMK 1.-5.), te veličinu resekcije pseudoegzostoze. Rapored plantarnog opterećnje po tipičnim zonama izvođen je statičkom i dinamičkom pedobarografijom. Trajanje i prosječna veličina opterećenja u vremenskim intervalima od 0,1 sekunde u pojedinim fazama oslonca tijekom hoda mjerena je platformom za baropodometrijsku analizu aparatom Baropodometro Elettronico proizvođač: PSI, mjesto: Rim, zemlja podrijetla: Italija.
Rezultati su pokazali da u bolesnika operiranih RAKO metodom kut HV prosječno iznosi 16°, IMK 1.-2. MT kosti prosječno 9°, smanjenje nagiba 1MT za prosječno 2º, što odgovara vrijednostima zdrave populacije. Operacijom po RAK metodi i dalje su zaostale visoke vrijednosti navedenih parametara. Postignute anatomske promjene očitovale su se i pri statičkoj i dinamičkoj pedobarografiji, gdje se RAKO metodom značajno povećalo opterećenje u području zone 3C koja odgovara prvoj metatarzalnoj kosti, te rasterećenje lateralnog ruba stopala. Pri analizi hoda značajno je skraćeno vrijeme trajanja opterećenja pete koje je trajalo do polovice faze oslonca za razliku od prijeoperacijskih vrijednosti kod RAK metode, gdje je peta opterećena 2/3 faze oslonca Vremensko trajanje opterećenje srednjeg dijela stopala zadržalo je prijeoperacijske vrijednost, ali veličina opterećenja smanjila se za 50 posto. Vrijeme opterećenje samo prednjeg dijela stopala je značajno dulje u RAKO metodi. U području palca pri hodu također je došlo do povećanja opterećenja.
Zaključak: RAKO metodom zanačajno smo ispravili deformitete palca i prednjeg dijela stopala. Statičkom i dinamičkom pedobarografijom dokazali smo statistički značajne promijene distribucije veličine i vremenskog trajanja opterećenja stopala tijekom stajanja i faze oslonca ciklusa hoda što je za posljedicu imalo poboljšanje funkcije prednjeg dijela stopala. Došlo je i do značajnog poboljšanja kliničkog skora kod ispitanika nakon operacijske procedure RAKO metodom.- Objects: To establish the effectiveness of Keller’s arthroplasty combined with the opening wedge osteotomy of the first metatarsal base area (RAKO) on the plantar distribution of load during standing; to establish load duration and its mean value during each stance phase of the gait cycle to establish patients’ content with the performed surgery.
Materials and methods: Patients, aged 65-75, suffering from severe hallux valgus were divided into two groups of 40. The first group was treated with Keller’s resection arthroplasty (RAK). The second group was treated with RAK combined with the opening wedge osteotomy of the first metatarsal base area. The patients were assessed before the surgery and a year after the surgery. The clinical assessment was carried out with AOFAS scoring system. Hallux valgus angle, first intermetatarsal angle (IMK 1.-2.), first metatarsal inclination angle, the extent of the proximal phalange resection, the angle between the first and fifth metatarsals (IMK 1.-5.), and the extent of the pseudoegsostosis resection, were all measured roentgenologically. Distribution of the plantar pressure over the standard regions was measured with static and dynamic pedobarography. Duration and the mean extent of the pressure for each phase of stance in the gait cycle were measured at the intervals of 0,1 second with the platform for baropodometric analysis, Baropodometro Elettronico.
Results: regarding the patients treated with RAKO surgery method show that the mean value for the HV angle is 16° and for the IMK 1.-5. is 9°, while the mean 1 MT inclination decrease is 2°; these figures correspond to those of the healthy population. Such high figures of the above-mentioned parameters could not be reached with the RAK method. Attained anatomical changes were recorded with both static and dynamic pedobarography: with the RAKO method the load increased in the region area 3C, which corresponds to the first metatarsal, while the lateral region of the foot became unloaded. Gait analysis showed that the heel pressure, which would prolong until the end of the stance phase, decreased; pre-surgical figures, or RAK method, showed that the heel pressure lasted for 2/3 of the stance phase. Middle part of the foot loading period corresponds to the pre-surgical figures, but the extent of the load decreased for 50%. Loading period of the forefoot lasts longer when the RAKO method was performed. Increased loading during walking can also be noticed in the hallux area.
Conclusion: We significantly corrected the deformity of the hallux and the forefoot by using the RAKO method. Static and dynamic pedobarography helped us prove statistically significant distribution changes concerning the extent and duration of the foot load during standing and the stance phase of the gait cycle. The results show function improvement the anterior part of the foot.
