Modern homology modeling techniques applied for the genome-wide prediction of drug target protein structures represent a real chance for an enhancement in the effectiveness of the drug discovery ...process in the pharmaceutical industry.
Advances in bioinformatics and protein modeling algorithms, in addition to the enormous increase in experimental protein structure information, have aided in the generation of databases that comprise homology models of a significant portion of known genomic protein sequences. Currently, 3D structure information can be generated for up to 56% of all known proteins. However, there is considerable controversy concerning the real value of homology models for drug design. This review provides an overview of the latest developments in this area and includes selected examples of successful applications of the homology modeling technique to pharmaceutically relevant questions. In addition, the strengths and limitations of the application of homology models during all phases of the drug discovery process are discussed.

Die Anwendung von Cannabis und Cannabinoiden wird als Therapie für ein breites Spektrum von Erkrankungen und Symptomen diskutiert. In diesem Artikel soll die aktuelle Evidenz zur ...Verwendung von Cannabis und Cannabinoiden in der Palliativversorgung dargestellt und Hintergrundwissen vermittelt werden. Dazu wurde aktuelle Literatur gesichtet und ausgewertet. Der Fokus liegt auf einer Übersichtsarbeit von Mücke et al. zur Verwendung von Cannabinoiden in der Palliativmedizin.
In der Zusammenschau liegt keine Evidenz hoher Qualität für die Verwendung von Cannabis und Cannabinoiden in der Palliativversorgung vor. Ein Therapieversuch mit Cannabinoiden kann bei Therapieversagen für die Behandlung von Tumorschmerzen und zur Gewichtssteigerung bei HIV(Humane Immundefizienz-Virus)-Patienten sowie bei anderen Symptomen (z. B. Übelkeit und Appetitmangel) erwogen werden, wenn andere Therapien nicht ausreichend wirksam sind.
Weitere Studien mit höheren Patientenzahlen sind zur Bewertung des therapeutischen Potenzials von Cannabis und Cannabinoiden notwendig. Die besonderen Bedingungen in der Palliativversorgung erschweren allerdings die Durchführung von Studien mit hohem Evidenzniveau.
Zusammenfassung
Pflanzliche Drogen und Extrakte müssen wie alle pharmazeutischen Ausgangsstoffe, die zur Herstellung von Arzneimitteln verwendet werden, eine geeignete pharmazeutische Qualität ...aufweisen. Entsprechende Qualitätsnormen werden in den Einzelmonographien des Arzneibuchs nach § 55 des deutschen Arzneimittelgesetzes beschrieben. Darin enthalten sind u. a. Informationen zu Inhaltsstoffen und Wirkstoffgehalt. In diesem Beitrag sollen zunächst die Entwicklung der Cannabisblütenmonographie für das Deutsche Arzneibuch (DAB) und die darin enthaltenen Qualitätsanforderungen und Lagerungsbedingungen beschrieben werden. Danach wird der Entwicklungstand von Monographien für das Europäische Arzneibuch dargestellt.
Bereits 2015 wurde mit den ersten Arbeiten an der Cannabisblütenmonographie für das DAB begonnen, nachdem bekannt geworden war, dass es gesetzliche Neuregelungen zur medizinischen Anwendung von Cannabisblüten und Cannabisextrakten geben würde. Zunächst wurde im Mai 2016 eine Monographie für Cannabisblüten im Deutschen Arzneimittel-Codex (DAC) veröffentlicht. Diese konnte im Mai 2017 durch die Publikation der DAB-Monographie ersetzt werden. Eine revidierte Version der DAB-Monographie ist seit April 2018 als nationaler Qualitätsstandard in Kraft getreten.
Derzeit wird eine harmonisierte Cannabisblütenmonographie für das Europäische Arzneibuch erarbeitet, die die nationalen Qualitätsstandards ersetzen soll. Außerdem werden im Deutschen Arzneibuch und nachfolgend im Europäischen Arzneibuch Monographien für Zubereitungen aus Cannabisblüten entwickelt. Mit harmonisierten Monographien im Europäischen Arzneibuch können zukünftig Mehrfachuntersuchungen nach jeweiligen nationalen Standards vermieden und Untersuchungen in Labors und Apotheken erleichtert werden.
Law enforcement interventions in drug markets require policy coordination to prevent collateral outcomes that might harm vulnerable people under criminal control and spread crime across places. This ...paper analyzes street-level peace building in the inner city at the frontline of the ongoing urban war on drugs in Colombia. Building on the emergent literature on criminal governance and using crime script models, the paper argues that behind the chaos that in appearance prevails in open-air drug markets, illegal economies in Colombia’s urban centers are ruled by schemes that set parallel mechanisms of order, social control, and distribution. Furthermore, the paper addresses the ways in which street-level bureaucracies and therapeutic policing interventions can become a way of building urban proximity, connectedness, and trust in the context of contested informalities.

The drop in launches of novel antibacterials is noteworthy, and begs the question - where did we go wrong? But this might not be about going ‘wrong’, but rather a complex mix of science, medical, ...social, financial, regulatory, evolutionary and political factors.
In the late 1960s, the medical need for new antibiotics began to be questioned, and the pharmaceutical industry shifted its emphasis of antibacterials from that of a therapeutic leader to a low-priority research area. Although infectious diseases, in particular those caused by bacterial infections, are still among the top causes of mortality in the world, industrial support continues to wane. The shift from this important area of antimicrobial research has been attributed to a combination of science, medical, marketing and business reasons. This decline in antibacterial drug discovery, coupled with increasing risk as a result of infections caused by drug-resistant bacterial pathogens, represents a clear public heath threat.
Zusammenfassung
Aktuelle systematische Übersichtsarbeiten (SRs) kommen zu unterschiedlichen Schlussfolgerungen zur Wirksamkeit und Sicherheit von Cannabispräparaten bei chronischen Schmerzen. Dieser ...Artikel stellt eine methodenkritische Übersicht aktueller SRs von randomisierten kontrollierten Studien (RCTs) mit Cannabispräparaten bei chronischen Schmerzen dar.
Selektive Literatursuche sowie verzerrte Datenanalysen und Ergebnisdarstellungen zugunsten der Wirksamkeit von Cannabispräparaten lassen sich sowohl bei einzelnen RCTs als auch bei SRs nachweisen. Je umfangreicher die Literatursuche und je höher die Kriterien für den Einschluss von Studien und die klinische Relevanz der Studienergebnisse, umso ernüchternder sind die Schlussfolgerungen der SRs bezüglich Wirksamkeit und Sicherheit. Es besteht eine mäßige Qualität der Evidenz für eine moderate Schmerzreduktion bei chronischen neuropathischen Schmerzen. Für diese können Cannabispräparate daher als Drittlinientherapie gelten. Für alle übrigen chronischen Schmerzsyndrome liegen Hinweise auf eine unzureichende Wirksamkeit vor.
Um den Stellenwert von Cannabispräparaten in der Therapie chronischer Schmerzen besser zu definieren, sind neue methodisch hochwertige RCTs notwendig, die den Fokus auf Ergebnisparameter jenseits von Schmerzlinderung legen sowie auf innovative Ansätze wie Netzwerkmetaanalysen mit verschiedenen Therapien und Kombination von kontrollierten und Beobachtungsstudien.
In the 1960s, Thailand was the biggest opium producing country in the world. This article presents Thailand’s evolving strategy in solving the problem of illicit poppy cultivation through poverty ...alleviation and long-term national development. It argues that the root causes of drug crop cultivation and proliferation are poverty, insecurity, and the lack of livelihood opportunities for marginalized communities. Thus, the problem is more a ‘development problem’ rather than a ‘drug problem,’ requiring the addressing of multi-dimensional human development facets in response to the geo-socio-economic conditions of the area. The “Thai approach” is focused on improving the overall well-being of communities, before rule of law can be strengthened, and is very importantly part of long-term broader national development plans. A brief close-up is provided of an example of Thailand’s long-term development project, the Doi Tung Development Project, to explain more concretely how Thailand’s approach to solve drug crop production translated into practice. Some of these lessons learned from Thailand can and have been shared with the international community in shaping attitudes and policies to drugs and development that are more people-centered, balanced, and sustainable.
We use a unique data set detailing the financial activities of a drug-selling street gang to analyze gang economics. On average, earnings in the gang are somewhat above the legitimate labor market ...alternative. The enormous risks of drug selling, however, more than offset this small wage premium. Compensation within the gang is highly skewed, and the prospect of future riches, not current wages, is the primary economic motivation. The gang engages in repeated gang wars and sometimes prices below marginal cost. Our results suggest that economic factors alone are unlikely to adequately explain individual participation in the gang or gang behavior.
This report assesses the magnitude, flows and drivers of illicit trade and the illegal economy including: narcotics, human trafficking, wildlife, sports betting, counterfeit medicines, alcohol and ...tobacco. The negative socio-economic impacts that these markets have in consumer countries are as worrisome as the goverance gaps that are exploited in source countries. This report examines each illicit sector in terms of the geographic sources, destinations and key trade routes, the current trend of infiltration by organized crime networks, and good practices or future policy solutions with which to combat illicit trade within the various sectors.
The opioid epidemic led to the creation of state Prescription Drug Monitoring Programs (PDMPs) that eventually mandated access. We examine how these “must-access” PDMPs influenced prescribing after ...an emergency department (ED) visit and in the long term for the working-age population. By using data from a large multistate commercial insurance database from 2010 to 2014 and estimating difference-in-differences models, we show that only the broadest must-access PDMPs reduced opioid prescribing after an ED visit and in the long term. We then compared changes in prescribing rates for opioid naïve relative to non–opioid naïve individuals to disentangle the influence of information from administration costs on prescriber behavior. Findings suggest that hassle cost explains the majority of the decline in initial prescribing, and that the information value drives most of the reduction in long-term outcomes.
