Metaplastic breast cancer (MBC) is a rare and clinically aggressive tumor with a poor prognosis. We report a case of MBC involving an 85-year-old woman who presented our hospital with a left breast ...mass of 18 mm in size. Core-needle biopsy revealed poorly differentiated epithelial cells and an osteoid-like appearance which characterized by high nuclear grade and high mitotic activity, so metaplastic breast cancer was suspected. We informed her and her family about this result, but they declined therapy for this disease because of her age. After 8 months, the tumor enlarged to 70 mm. There were also skin redness and local pain. It was not until then that she agreed to surgery and underwent mastectomy and axillary lymph node dissection. Histologically, the tumor consisted of poorly differentiated adenocarcinoma, pleomorphic and chondroid metaplasia,and it was diagnosed as metaplastic carcinoma with mesenchymal differentiation (chondroid differentiation). Immunohistochemical staining was positive for vimentin, CKAE1/AE3 and S-100 protein and negative for estrogen, progesterone receptor and HER2. She did not wish further treatment, so neither adjuvant chemotherapy nor radiation therapy was performed. Four years have passed after surgery, and she is alive without recurrence.
RESUMEN Objetivos: Los objetivos del estudio fueron determinar la evolución de la capacidad funcional en mayores de 65 años de la Región Sanitaria de Lleida durante su estancia en el programa de ...atención domiciliaria (ATDOM), y analizar los factores predictores de mortalidad. Metodología: Se trata de un estudio longitudinal. La muestra incluye las estancias (procesos) ≥ 6 meses en el programa ATDOM ocurridos durante 5 años. Las variables a estudio fueron la capacidad funcional al ingreso, a la mitad y al final del proceso, las características sociodemográficas, las enfermedades crónicas, el estado de salud, los problemas sociofamiliares y la mortalidad. Resultados: Se incluyeron un total de 8.189 personas, con un total de 8.433 procesos abiertos entre los años 2011 y 2016. La capacidad funcional empeoró más en las primeras fases del proceso, y se mantuvo más estable en la fase final. En las personas más mayores, con demencia senil y con presencia de úlceras por presión, la dependencia aumentaba conforme avanzaba el proceso. Conclusiones: Los resultados obtenidos en este estudio sobre la relación de la dependencia funcional en la valoración inicial y el mayor riesgo de mortalidad son coincidentes con estudios desarrollados.
The number of indoor heatstroke cases aged 60 years or older categorized the "Residential" in the ambulance transport data was aggregated by perception time to investigate the characteristics of ...temporal changes in the number of indoor heatstroke cases based on the operation status of air conditioner. Simultaneously, association between the severity of indoor heatstroke, categorized as "mild" and "moderate or severe" and use of air conditioner was investigated using the ambulance transport data (June to October 2013 - 2019) provided by the Toyohashi Fire Department, Aichi Prefecture, Japan. The obtained results showed that 1) the number of ambulance transports due to indoor heatstroke, in which the air conditioner was recorded as "OFF" in the transport data, was found to increase sharply from 7:00 a.m., peaking at 9:00 a.m. and 1:00 p.m. Moreover, more patients were diagnosed with "moderate or severe" in the morning than in the afternoon. The high number of cases of indoor heatstroke in the morning was attributed to the possibility of heatstroke occurring during the night or while sleeping, as well as to the delay in calling for emergency medical services. 2) There was a significant association between air conditioner operation status and severity of indoor heatstroke. The use of air conditioner may reduce the probability of moderate or severe heatstroke by an average of 26 points compared to when the air conditioner was OFF.

Resumen Objetivos: Conocer y explorar las preocupaciones y experiencias que genera el confinamiento por la COVID-19 en las personas mayores. Metodología: Estudio cualitativo de tipo fenomenológico ...basado en entrevistas semiestructuradas. La información se analizó mediante análisis de contenido y se verificó con la triangulación de los resultados. Resultados: Emergieron 8 categorías coincidentes: sentimientos durante el confinamiento, inquietudes por la salud, cambios en la propia salud, cambios en los hábitos de vida, apoyo profesionales de la salud, estrategias de afrontamiento, información sobre la COVID-19 y preocupaciones sobre el futuro. Conclusiones: Identificar las vivencias de los mayores durante el confinamiento por la COVID-19 ha mostrado, en esta población, las dificultades surgidas del día a día y las capacidades que han tenido que llevar a cabo para soportar y superar este difícil período de la pandemia.
RESUMEN Introducción: La polifarmacia está considerada como un indicador de fragilidad en el adulto mayor. Objetivo: Caracterizar el consumo de medicamentos en adultos mayores. Métodos: Investigación ...cuantitativa descriptiva y transversal en el consultorio médico de la familia 7 de San Diego de los Baños durante el 2018, que cumplieron criterios de inclusión y de exclusión. El universo fue de 280 ancianos y la muestra de 265. Los datos se obtuvieron de las historias clínicas y de encuestas; las variables investigadas fueron: edad, sexo, polifarmacia, medicamentos prescritos o no y reacciones adversas. Se utilizaron frecuencia absoluta y relativa porcentual. Resultados: Predominaron el sexo masculino (52,0 %), las edades 60 - 69 años (46,7 %), la osteoartritis (54,0 %) e hipertensión arterial (32,0 %). La polifarmacia se presentó en 67,9 % de los ancianos; 19,6 % tuvo reacciones adversas que predominaron en los medicamentos prescritos. Conclusiones: El consumo de medicamentos en los ancianos se caracterizó por un incremento de la polifarmacia y medicamentos prescritos, asociado a un conjunto de reacciones adversas.
Recently, the number of traffic accidents involving elderly drivers has been increasing, and it is known that side collisions at the intersection are particularly frequent. Thus, we put on-dash cams ...for 22 drivers aged 50-82 years old to measure their driving behavior at stop intersections. The relation between emergency braking frequency and looking behavior, and cognitive/physical characteristics and driving attitude were investigated. The results show that some elderly drivers can perform compensatory driving behaviors such as looking both ways at stop intersections to reduce the frequency of emergency braking even though their cognitive and physical characteristics have deteriorated.
