Identify and critically evaluate systematic reviews addressing the effectiveness of interventions to reduce the number of prescriptions of potentially inappropriate medication to older patients.
This ...is an overview of systematic reviews. The studies were searched and selected from Medline, Cochrane Library, Embase, CINAHL, Virtual Health Library, and Web of Science databases, combining the terms aged, prescriptions, inappropriate prescribing and potentially inappropriate medication list with their entry terms and other related descriptors, published by June 2017. This study included systematic reviews with or without meta-analysis that addressed the effectiveness of any intervention or combined interventions to reduce the number of prescriptions of potentially inappropriate medications to older patients, without restriction in terms of design, language or date of publication of primary studies. AMSTAR - A MeaSurement Tool to Assess systematic Reviews - was used to evaluate the methodological quality of selected systematic reviews. Study selection and the methodological quality evaluation were performed by two independent evaluators, who resolved any divergence by consensus. The main findings were grouped into thematic categories, defined after a content analysis and discussed qualitatively as narrative synthesis.
This study analyzed 24 systematic reviews. In terms of study design and methodological quality evaluation, most were systematic reviews of randomized controlled clinical trials and studies of moderate quality, respectively. The interventions were analyzed in five thematic categories: medication review services, pharmaceutical interventions, computerized systems, educational interventions, and others. The interventions analyzed showed good results and most of them helped reduce the number of prescriptions of potentially inappropriate medication to older patients.
The systematic reviews included in this overview showed potential benefits of different interventions. However, it was not possible to determine the most effective intervention. Combined interventions are likely to provide better results than isolated interventions.
Abstract Objective To investigate the factors associated with performance equal to or higher than the 95th percentile in the Mini Mental State Examination of elderly people aged 80 years and over. ...Method This is a cross-sectional, descriptive, and quantitative research conducted with a convenience sample composed of 101 cognitively healthy older adults residing in Brazil's Federal District, assessed by scales that measure mood, social support, functional performance, and health. Results Bivariate statistical analyses indicated that older individuals with better cognitive performance in the Mini Mental State Examination, when compared to participants with performance below the 95th percentile, showed better self-perceived health, greater satisfaction with the ability to perform daily tasks, and a more positive evaluation of general social support and in the domain of having someone to talk to when feeling lonely. Conclusion High global cognitive performance in old age is associated with perceived health, satisfaction with functional performance, and social support.
Resumo Objetivo Investigar os fatores associados ao desempenho igual ou superior ao percentil 95 no Miniexame de Estado Mental de pessoas idosas com 80 anos e mais. Método Trata-se de uma pesquisa transversal, descritiva e quantitativa, conduzida com uma amostra de conveniência composta por 101 idosos cognitivamente saudáveis, residentes no Distrito Federal e avaliados por escalas de humor, suporte social, desempenho funcional e saúde. Resultados Análises estatísticas bivariadas indicaram que os longevos com melhor desempenho cognitivo no Miniexame de Estado Mental, quando comparados aos participantes com desempenho abaixo ao percentil 95, apresentaram melhor autopercepção de saúde, maior satisfação com a capacidade de resolução de tarefas cotidianas, avaliação mais positiva do suporte social geral e relacionado ao domínio de ter com quem conversar quando se sente sozinho. Conclusão O elevado desempenho cognitivo global na velhice se associa à saúde percebida, à satisfação em relação ao desempenho funcional e ao suporte social.

(ELSI-Brasil). A fragilidade foi classificada pelo
número de itens positivos entre perda de peso não intencional, exaustão, baixo
nível de atividade física, lentidão da marcha e fraqueza. As análises principais
foram baseadas na regressão logística multinomial estratificada por sexo. A
prevalência da fragilidade foi menor nos homens (8,6%; IC95%: 6,9; 10,7) do que
nas mulheres (11,9%; IC95%: 9,6; 14,8), sendo o item mais frequente o baixo
nível de atividade física em ambos. A idade e a escolaridade foram os fatores
sociodemográficos associados à pré-fragilidade e à fragilidade entre homens e
mulheres. Houve diferença da fração atribuível populacional para fragilidade
entre os sexos. Nos homens, a maior fração atribuível populacional foi para não
ter companheiro (23,5%; IC95%: 7,7; 39,2) e escolaridade baixa (18,2%; IC95%:
6,6; 29,7). Nas mulheres, maiores frações atribuíveis populacionais foram para
déficit de memória (17,1%; IC95%: 7,6; 26,6), déficit da visão (13,4%; IC95%:
5,1; 21,7) e diabetes mellitus (11,4%; IC95%: 4,6; 18,1). Observou-se fração
atribuível populacional semelhante para doença cardíaca (8,9%; IC95%: 3,8; 14,1,
em mulheres; e 8,8%; IC95%: 2,0; 15,6, em homens). Estratégias voltadas para a
prática de atividade física têm o potencial de prevenir a fragilidade em ambos
os sexos, enquanto a prevenção de condições crônicas é mais importante nas
mulheres.
Abstract This study evaluates the knowledge of fourth, fifth and sixth-year medical undergraduates about palliative care and their perceptions regarding teaching of this topic. Cross-sectional ...observational research was conducted with 135 undergraduates from a private medical school in Belo Horizonte by means of an online questionnaire. Of the participants, 40.7% considered themselves prepared to address patient death and the bereavement of family members, 80.7% rated their level of knowledge about palliative care as mediocre and 77% said they received insufficient information on the subject. When evaluating concepts, 20% of the participants had an unsatisfactory performance, 48% acceptable and 3% excellent. Students from more advanced years and who had completed an internship in elderly health had better responses. Despite regular student performance, the internship in elderly health positively impacts the acquisition of palliative care knowledge.
Resumo O objetivo deste trabalho foi avaliar o conhecimento sobre cuidados paliativos de estudantes do quarto, quinto e sexto anos da graduação em medicina de uma faculdade particular de Belo Horizonte, bem como investigar sua percepções sobre o ensino do tema. Trata-se de estudo transversal observacional com aplicação de questionário on-line para 135 acadêmicos. Dentre os participantes, 40,7% consideram-se preparados para lidar com morte de pacientes e luto dos familiares, 80,7% classificam o próprio nível de conhecimento sobre cuidados paliativos como regular e 77% afirmam não ter recebido informações suficientes sobre o assunto. Ao avaliar conceitos, 20% tiveram desempenho insatisfatório, 48% aceitável e 3% excelente. Acadêmicos de anos mais avançados e que fizeram o internato em saúde do idoso apresentaram melhores respostas. Revelou-se que, apesar de o desempenho dos estudantes ser em geral regular, o internato em saúde do idoso tem impacto positivo na aquisição de conhecimento sobre cuidados paliativos.
Resumen El objetivo de este trabajo fue evaluar el conocimiento sobre cuidados paliativos de estudiantes de cuarto, quinto y sexto curso de la carrera de medicina de una facultad privada de Belo Horizonte, así como investigar sus percepciones sobre la enseñanza del tema. Se trata de un estudio observacional transversal que utilizó un cuestionario en línea para 135 académicos. Entre los participantes, el 40,7% se considera preparado para afrontar la muerte de los pacientes y el duelo de los familiares, el 80,7% califica como regular su propio nivel de conocimiento sobre cuidados paliativos, y el 77% afirma no haber recibido suficiente información sobre el tema. Al evaluar conceptos, el 20% tuvo un desempeño insatisfactorio, el 48% aceptable y el 3% excelente. Los académicos de años más avanzados y que completaron el internado en salud del anciano presentaron mejores respuestas. Se reveló que, aunque el desempeño de los estudiantes sea generalmente regular, el internado en salud del anciano tiene impacto positivo en la adquisición de conocimiento sobre cuidados paliativos.

Age-related cortical atrophy and cognitive decline, especially in memory function, are common among older adults. However, a subset of older adults, known as SuperAgers (SA), exhibit exceptional resilience to these effects, displaying an episodic memory capacity equivalent to or exceeding that of individuals 20 to 30 years their junior. Despite this phenomenon, there is no consensus among researchers regarding the most appropriate instruments and criteria for classifying SuperAgers. To address this gap, we conducted a systematic review of the literature to determine which instruments and classification criteria are most frequently utilized. Our search of PubMed/Medline, Scopus, Web of Science, and PsycINFO yielded results suggesting that SuperAgers are generally defined as individuals aged 80 years or older, with anterograde memory tests based on fixed cut-off values for word list recall, as well as executive functioning tests, serving as the primary classification tools. Given the influence of sociocultural and linguistic factors, as well as differences in the choice of memory tests, it is not recommended to establish a single cut-point in global SA studies.
Objective: To investigate changes in the frailty levels of older adults in a context of high social vulnerability. Methods: We conducted a prospective cohort study. Data were collected from 2 surveys ...conducted in 2015 and 2018. The frailty phenotype and sociodemographic and health characterization instruments were used. Descriptive statistical analysis was performed, including non-parametric tests, test for equality of proportions, and multivariate multinomial logistic regression. The use of the database was authorized, and the research was approved by the Ethics Committee. Results: In 2015, 346 community-dwelling older adults participated in the study. After 36 months, a final sample of 223 participants was obtained. In 2015, the prevalence of non-frail, pre-frail, and frail older adults was 13.0%, 56.5%, and 30.5%, respectively. In 2018, 22.9% were non-frail, 56.0% were pre-frail, and 21.1% were frail. Higher education and better quality of life reduced the likelihood of becoming pre-frail and frail, respectively. Conclusion: There was a change in the pattern of frailty among socially vulnerable older adults over a 36-month period.
The goal of this study is to characterize the population of older adults in Brazil during the COVID-19 pandemic with regard to health, socioeconomic conditions, gender inequality, adherence to social ...distancing and feelings of sadness or depression. It is a cross-sectional study carried out with Brazilian older adults who responded to an online health survey (N = 9,173), using a "virtual snowball" sampling method. Data were collected online via a self-administered questionnaire. Prevalence and confidence interval estimates were performed and verified for independence using Pearson's chi-square test. During the pandemic there was a fall in household income among almost half of older adults. Extreme social distancing was practiced by 30.9% (95%CI: 27.8; 34.1) and 12.2% (95%CI: 10.1; 14.7) did not adhere to it. Older adults who were not working before the pandemic adhered in greater numbers to extreme social distancing measures. Most of them presented comorbidities associated with a higher risk of developing the severe form of COVID-19. Feelings of loneliness, distress and sadness were frequent among older adults, especially women. The COVID-19 pandemic widened the inequality gap by affecting the most vulnerable older people. Strategies to mitigate loneliness and social distancing should consider social vulnerability and the marked difference between men and women in terms of household composition and socioeconomic and working conditions. The development of representative surveys of Brazilian older adults is recommended, investigating the impact of the pandemic on this population.
Introdução: o uso de aplicativos móveis pode facilitar o autogerenciamento em saúde e oportunizar a autonomia dos idosos no seu autocuidado. Objetivo: mapear a produção científica sobre aplicativos ...móveis para autogerenciamento do cuidado direcionados aos idosos. Materiais e Métodos: revisão de escopo realizada no período de setembro de 2020 a janeiro de 2021, a partir das bases de dados: MEDLINE, SciELO, Scopus, Web of Science e Science Direct, mediante a estratégia de busca: (“Self-management” OR “Self-care”) AND (Elderly OR “Old man”) AND (“Mobile Applications” OR Smartphone OR “Cell phone”), com a inclusão de artigos que tratassem do uso de aplicativos móveis por idosos para o autogerenciamento do cuidado, sem delimitação de tempo e idioma. Resultados: a amostra final compôs-se de 14 artigos, categorizados em três vertentes de gerenciamento, a saber: medicamentos, comorbidades e práticas saudáveis. Na maioria dos estudos, os aplicativos foram direcionados ao autogerenciamento dos medicamentos, seguidos dos cuidados de condições crônicas e por último a autoavaliação do risco de quedas e tratamentos não-farmacológicos da dor. Discussão: esta revisão contribui para a prática clínica e pesquisa em enfermagem, uma vez que seus resultados apontam o que há publicado sobre o desenvolvimento e uso de aplicativos móveis por idosos para o autogerenciamento do cuidado. Conclusões: o uso de aplicativos móveis facilita o autocuidado da população idosa, principalmente, na gestão de medicamentos para condições crônicas.
Resumo: Com o objetivo de avaliar a incidência cumulativa de depressão e seus fatores associados na população idosa, residente na zona urbana do Município de Bagé, Rio Grande do Sul, Brasil, ...realizou-se um estudo de coorte, prospectivo, entre 2008 e 2016/2017. A análise foi restrita a 615 idosos com informações completas na Escala de Depressão Geriátrica (GDS-15), tanto na linha de base como no seguimento, que não apresentavam depressão no ano de 2008. Para calcular as razões de incidência bruta e ajustadas e intervalo de 95% de confiança, foi utilizada a regressão de Poisson com ajuste robusto de variância, incluindo as variáveis da linha de base. Empregou-se um modelo hierárquico de quatro níveis de determinação. As variáveis foram controladas para aquelas do mesmo nível ou dos níveis superiores, sendo estabelecido o valor de p ≤ 0,20 para permanecer no modelo de análise. Observou-se que, em 2008, 523 idosos não tinham depressão e 92 haviam sido diagnosticados com a doença. Em 2016/2017, dos 523 indivíduos sem depressão na medida de linha de base, 10,3% apresentaram resultado positivo no rastreamento (casos incidentes), enquanto 89,7% dos idosos permaneceram livres do problema. Dos 92 idosos com depressão em 2008, 32,6% continuaram referindo a sintomatologia depressiva no acompanhamento e 67,3% apresentaram remissão dos sintomas. Sair de casa uma ou nenhuma vez e apresentar incapacidades para o desenvolvimento de atividades funcionais e instrumentais da vida diária se associaram com maior risco de apresentar rastreamento positivo para depressão. Os resultados reforçam o caráter multidimensional e dinâmico da depressão, que alterna episódios curtos e longos, podendo se tornar recorrente e de curso crônico.
Abstract: To evaluate the cumulative incidence of depression and its associated factors in the older population living in the urban area of the municipality of Bagé, Rio Grande do Sul State, Brazil, a prospective cohort study was conducted from 2008 to 2016/2017. The analysis was restricted to 615 older adults with complete information on the Geriatric Depression Scale (GDS-15), both at baseline and at follow-up, and who did not present depression in 2008. To calculate crude and adjusted incidence ratios and 95% confidence interval, Poisson regression with robust variance adjustment was used, including the baseline variables. A hierarchical model of four levels of determination was used. The variables were controlled for those of the same or of higher levels, and the p-value ≤ 0.20 was established to remain in the analysis model. In 2008, 523 older people did not have depression and 92 had been screened with the disease. In 2016/2017, of the 523 individuals without depression in the baseline measure, 10.3% tested positive at screening (incident cases), whereas 89.7% of the older adults remained free of the condition. Of the 92 older adults with depression in 2008, 32.6% continued to present depressive symptoms at follow-up and 67.3% had remission of symptoms. Leaving home once or never and being incapable of doing functional and instrumental activities of daily living were associated with a higher risk of positive screening for depression. The results reinforce the multidimensional and dynamic character of depression, which alternates short and long episodes, and may become recurrent and chronic.
Resumen: Con el objetivo de evaluar la incidencia acumulada de la depresión y sus factores asociados en la población anciana que vive en el área urbana de la ciudad de Bagé, Rio Grande do Sul, Brasil, se realizó un estudio de cohorte, prospectivo, en el período entre 2008 y 2016/2017. Se analizaron a 615 ancianos que tenían información completada en la Escala de Depresión Geriátrica (GDS-15), tanto en la línea de base como en el seguimiento, y aquellos que no tuvieron depresión en 2008. Para calcular los índices de incidencia brutos y ajustados y el intervalo de 95% confianza, se utilizó la regresión de Poisson con un ajuste robusto de la varianza, incluidas las variables de la línea de base. Se empleó un modelo jerárquico de cuatro niveles de determinación. Las variables se controlaron por aquellas del mismo nivel o niveles superiores, y se estableció un valor de p ≤ 0,20 para permanecer en el modelo de análisis. Se observó que 523 ancianos en 2008 no tuvieron depresión y 92 recibieron diagnóstico de la enfermedad. Para el 2016-2017, de los 523 individuos sin depresión en la medida de línea de base, el 10,3% de ellos se detectó con la enfermedad en el cribado (casos incidentes), mientras que el 89,7% no la manifestó. De los 92 ancianos con depresión en 2008, el 32,6% informaron tener síntomas depresivos en el seguimiento y el 67,3% presentó remisión de los síntomas. Salir de casa una sola vez o ninguna y sentir incapaz de realizar las actividades funcionales e instrumentales de la vida diaria estuvieron asociadas a un mayor riesgo de tener cribado positivo para la depresión. Los resultados evidenciaron el carácter multidimensional y dinámico de la depresión, que oscila entre episodios cortos y largos, y puede llegar a ser recurrente y crónica.
