Hospice poskytují péči pacientům v terminálním stádiu onkologických i jiných onemocnění. Vycházejí z myšlenek důstojného umírání, smysluplnosti každé fáze lidského života a úcty k člověku. Na ...personál hospice setkávající se v každodenní práci se smrtí a umíráním jsou tak kladeny vysoké nároky – musí přijmout hodnoty hospicového hnutí a zároveň být schopen smrt uchopit tak, aby se stala jevem neohrožujícím pocit ontologického bezpečí. V uchopení smrti jako neohrožující personálu napomáhá lékařská terminologie umožňující klasifikování etap umírání, dále profesní standardy i zaměření se na období před smrtí (na život v době umírání) a na období po smrti, které je namísto „ukončení“ chápáno jako „pokračování“ předchozího života. Přesahování smrti se objevuje nejčastěji v podobě zaměření se na trvání vztahů i po smrti a na posmrtný život. Ohrožující potenciál si uchovává smrt mladých lidí vybočující z představy „dobré smrti“. Smrt v hospici získává charakter „smrti blízké“ – všem známé a očekávané, i „smrti vzdálené“ – pozornost se soustřeďuje na období před ní a po ní.
This paper deals with the stressful situations in the profession of the nursing staff. The occupation of the nurse belongs to the most demanding ones. In this occupation, the nurse is permanently ...affected by many stressors of different types. She faces death, the suffering, dying and incurably ill people very often. She is there in times of pain, agony and suffering. Personal encounters with these difficult situations require highly professional approach but most of all, they present immense work stress for the nurse. The stressful situations that are the most common and most difficult in the work of the nursing staff are the care of dyeing persons, the care of incurable and suffering persons, also the care of aggressive patients and confused patients. Patient’s family members could cause the stressful situations for nursing staff so as the working and personal conditions of nursing staff. The emotions the nursing personal experiences in such situation influence their behaviour and communication with patients in negative way. Both sides are therefore dissatisfied which could develop in to aggressive situation or burn out syndrome of the personnel. Also such culminate situations could lead to leaving the personnel. The lack of personnel then doesn´t allows enough of needed time for communication with patients in their life difficult situations. The first objective of our survey was to map the manners used by the nursing personnel to cope with the stressful situations brought by their profession. The second objective was to find out the difference in the coping with the stressful situations between the hospital staff and hospice staff. The research section of this survey contains the quantitative research was done by using the technique of an anonymous questionnaire with 20 question prepared by ourselves. The questionnaire was distributed to the nursing staff of all categories in the hospital and hospice care. It was completed by 269 respondents in total. The data was statistically processed using systems Microsoft Excel 2010 and Software R, version 3.0.2 (Chi-squared test, Fisher´s exact test). The research results show that the nursing staff faces the stressful situations very often. They see as the heaviest stress the care of the suffering and aggressive patient, conflicts with the patient´s family, conflicts at work and workload. The manners of coping with the work burden which proved as the best for them are relax, rest and sleep. There were no differences proved in the manners of coping with the stress among respective members of the nursing team. Nevertheless, the coping with the work burden of the nursing staff in the hospital and hospice care is different. A minimum of the respondents notices any support during the stressful situations from the employer. On the other hand, certain support by the management could be viewed in the form of trainings, supervision or contribution to a „recovery“ which are granted to the respondents from the employer according to their answers. However, the respondents would prefer, as the support by the management, the increase in pay or more days off. The respondents view as insufficient the preparation of the graduates for the future stressful situations in their profession, as well. They would recommend mainly various trainings dealing with the stress and its management. We can state that in the transformation and process of improvement of health care service in Czech Republic the nursing personnel in at the last place. The care of caregivers is insufficient the meaning of supervision, support and education of the personnel. A good solution to alleviate the work burden of the nursing staff and quality enhancement of the services provided could be investments into better work conditions, both in terms of sufficient human and material resources so as the very organization of the work processes. This means the investment into safety of patients because continuous stress in work could jeopardize the quality of provided service and care but also the health of providers of care.

Článek je sondou do světa dobrovolníků, kteří doprovázejí umírající v mobilním hospici Cesta domů. V článku je porovnáván tento alternativní diskurz smrti a umírání s dominantním diskurzem ...společnosti. Cílem článku je zviditelnit a odhalit zamlčené předpoklady dominantního diskurzu a to, jakými praktikami sám sebe může legitimizovat a umocňovat. Výzkum používá analýzu diskurzu, pomocí které chce ukázat alternativní možnost vztahování se k tématům smrti a umírání porovnáním dominantního diskurzu s postoji dobrovolníků hospice. Při analýze diskurzů se opírá o výzkumné zprávy, polostrukturované rozhovory s dobrovolníky hospice a také o autorovu zkušenost s diskurzivními praktikami. Zaměřuje se konkrétně na témata výkonu, vztahů, pojetí času, nacházení smyslu, osvobození, demaskování praktik vymísťujících smrt a umírání. Tyto kategorie vzniklé z analýzy rozhovorů dobrovolníků porovnává s tím, jak se k těmto tématům v rámci diskurzu smrti a umírání staví většinová společnost. Článek není zamýšlen jako exotický exkurz do marginální skupiny, ale s porovnáním širšího kontextu dominantního diskurzu má umožnit pochopení diskurzivního zápasu o uhájení jiné „normálnosti“. Cílem není přesvědčit čtenáře o správnosti postojů dobrovolníků, ale spíše ukázat nové možnosti uvažování o tématech umírání lidem, kteří nejsou spokojeni s tím, jak je „dovoleno“ či vůbec možno v dominantních diskurzech o tématech umírání a smrti přemýšlet.

In soziologischen Studien über Hospize und Palliativstationen wird häufig kritisiert, dass die Organisationsförmigkeit solcher Einrichtungen das Erleben der Sterbenden überformt. ...Anhand von problemzentrierten Experteninterviews mit Ärzten und Ärztinnen, Pflegefachkräften sowie narrativen Interviews mit Sterbenden wird in dieser Studie untersucht, nach welchen Kriterien alle Beteiligten Handlungs- und Erlebensmöglichkeiten des Sterbens sich und anderen zurechnen. Auf diese Weise können wir zeigen, dass Sterbeverläufe von professionellen Akteuren als „gut“ erlebt werden, wenn sie als gemeinsames Erleben von einerseits professionellen Akteuren und andererseits Sterbenden, die es so gewollt haben, gedeutet werden können. Dies ist bemerkenswert vor dem Hintergrund eines weiteren Befundes dieser Studie, nämlich der systematischen Perspektivendifferenzen im Erleben des Sterbeverlaufs zwischen professionellen Akteuren einerseits und Sterbenden andererseits. Während Sterbende in unserer Studie ihr Sterben als bedrohliche Diskontinuität erleben, erleben die professionellen Akteure Sterben als kontinuierlich erwartbaren Verlauf. Die Konsensfiktion des gemeinsamen Erlebens des Sterbens stellt offenbar eine hilfreiche Erweiterung der Handlungsmöglichkeiten im Umgang mit dem Sterben dar, da hiermit die Unversöhnlichkeit zwischen Ideal und Praxis, die Perspektivendifferenzen zwischen Sterbenden und professionellen Akteuren und damit auch das unlösbare Problem der Endlichkeit unsichtbar gehalten werden können. Wer Hospize und Palliativstationen untersucht, sollte mit enormen Freiräumen der Deutungsarbeit rechnen, die dem Sterben Latenz verschaffen.

Hintergrund
Menschen vor dem Hintergrund einer progredienten, wahrscheinlich tödlich verlaufenden Erkrankung zu größtmöglicher Lebensqualität zu verhelfen, ist das Ziel von Palliative ...Care. Der Einsatz der Maßnahmen orientiert sich dabei radikal an den Bedürfnissen der Betroffenen und ihrer Angehörigen. Obwohl Altenpflegeeinrichtungen in Deutschland bereits der zweithäufigste Sterbeort sind, erfahren Bewohner*innen hier noch nicht verlässlich die hospizliche Begleitung und palliative Versorgung, wie sie diese benötigen.
Ziel der Arbeit
In einem Forschungsprojekt wurde die Kooperation zwischen einer Altenpflegeeinrichtung und einem ambulanten Hospizdienst untersucht. Dieser Beitrag geht der Frage nach, wie die Kooperation mit dem ambulanten Hospizdienst und die damit einhergehende Begleitung dazu beitragen können, Hospizkultur und palliative Kompetenz in einer Altenpflegeeinrichtung positiv zu verändern.
Material und Methoden
Es wurden 23 leitfadengestützte Interviews mit Bewohnern/Bewohnerinnen, Angehörigen und Mitarbeitenden der Altenpflegeeinrichtung geführt. Die Auswertung erfolgte mittels qualitativer Inhaltsanalyse nach Mayring.
Ergebnisse
Die Kooperation mit dem ambulanten Hospizdienst kann einen wichtigen Beitrag zur Enttabuisierung von Sterben und Tod in der Einrichtung leisten. Sie kann dabei helfen, das Bewusstsein der Mitarbeitenden zu erhöhen und eine hospizliche Haltung zu entwickeln.
Schlussfolgerung
Für eine gelingende Kooperation müssen die Rollen der an der Versorgung und Begleitung beteiligten Personen geklärt werden. Bewohner*innen sollten dabei unterstützt werden, ihre eigenen Vorstellungen einzubringen.
