Die Glomeruläre Filtrationsrate (GFR) ist der sensitivste Parameter der exkretorischen Nierenfunktion. Eine Reduktion der GFR ist schon vor dem Auftreten klinischer Symptome einer Niereninsuffizienz ...nachweisbar. Eine genaue Kenntnis der GFR ist hilfreich, um sinnvoll Pharmakotherapie zu betreiben und Kontrastmitteldiagnostik einsetzen zu können. Im klinischen Alltag werden zur Bestimmung der GFR in erster Linie das Serum-Kreatinin und Kreatinin-basierte Schätzformeln eingesetzt. Diese Formeln unterschätzen die GFR jedoch bei Werten über 60/mL/min/1.73 m2, da sie bei Patienten mit fortgeschrittener Niereninsuffizienz entwickelt wurden. Darüber hinaus wird Kreatinin durch den Muskelstoffwechsel, die Ernährung, Alter und das Geschlecht, aber auch durch Medikamente beeinflusst. Ein alternativer endogener Surrogatparameter der GFR ist Cystatin C, ein ca. 13.3 kDa großer Cystein-Proteinasehemmer, der praktisch von allen kernhaltigen Zellen nahezu konstant synthetisiert wird. Es wird glomerulär frei filtriert, im proximalen Tubulus reabsorbiert und zu mehr als 99% im proximalen Tubulus katabolisiert. Seine Serumkonzentration ist praktisch unabhängig von Körpermasse, Geschlecht oder Entzündungsreaktionen. So kann im Alltag ein einziger Referenzbereich für Männer und Frauen unabhängig von ihrer ethnischen Herkunft benutzt werden. Darüber hinaus sind die Cystatin C-Serumkonzentrationen vom 2. Lebensjahr bis zum Ende der 5. Dekade stabil. Bereits geringe Nierenfunktionseinschränkungen im Kreatinin-blinden Bereich können mit Cystatin C sicher diagnostiziert werden. Cystatin C-basierte Clearanceformeln ermöglichen im Vergleich zu Kreatinin eine einfache und gegenüber diversen Fehlerquellen robuste Bestimmung der GFR. Mit Cystatin C können auch prognostische Aussagen getroffen werden: Dies gilt für das Auftreten eines akuten Nierenversagens infolge Schock, Sepsis oder operativen Eingriffs ebenso wie für das Kontrastmittel-assoziierte Nierenversagen. Ebenso lassen sich das Auftreten chronischen Nierenversagens und kardiovaskulärer Ereignisse auf der Basis von Cystatin C prognostizieren. Cystatin C ist ein verlässlicher Parameter zur GFR Bestimmung, der in seiner diagnostischen Genauigkeit deutlich weniger beeinflusst wird als Kreatinin. Seine Bestimmung bietet auch prognostische Informationen hinsichtlich des generellen und kardiovaskulären Risikoprofils der Patienten und eignet sich damit als Screeningparameter.
Glomerular filtration rate (GFR) is the most sensitive parameter of excretory kidney function. GFR reduction is usually detectable prior to the onset of clinical symptoms. Exact knowledge of GFR is helpful for pharmacotherapy and the use of contrast media. Everyday routine usually applies creatinine and creatinine-based equations to estimate GFR. However, because these equations were derived from patient cohorts with advanced renal insufficiency, they underestimate true GFR, if GFR is higher than 60 mL/min/1.73 m2. Moreover, it is well known that serum creatinine concentrations are influenced by muscle mass, dietary protein intake, sex, age, and drugs thus limiting the precision of creatinine-based methods. An alternative endogenous parameter of GFR is cystatin C, which is a 13.3 kDa large cysteine proteinase inhibitor produced at a constant rate in virtually all nucleated cells. It is freely filtered by the glomerular membrane, reabsorbed in the proximal tubule, and catabolized to more than 99%. Its serum concentrations are virtually independent of body composition, sex, and age or inflammation. In routine diagnostics one single reference interval serves for males and females irrespective of ethnicity. Furthermore, serum concentrations of cystatin C are stable from the second year of life until the end of the fifth decade. Even subtle reductions of renal function in the creatinine-blind range can be diagnosed safely and are reproducible. In comparison to creatinine, cystatin C-based clearance formula allow for an easy estimation of GFR which is robust against diverse sources of error. Cystatin C also enables prognostic statements. This applies for acute renal failure due to shock, sepsis or surgical interventions, as well as for contrast media. Additionally, chronic renal failure and cardiovascular events may be predicted. Cystatin C is a reliable marker for GFR estimation and its diagnostic accuracy is less influenced than creatinine. Furthermore, it provides prognostic information on patients regarding all-cause mortality and cardiovascular risk, suggesting cystatin C as a potential screening parameter.
Siklosporin A üzerinde çok araştırılma yapılan immunmodülatör ajanlardan biridir ve bugün immunolojik olarak tetiklenmiş birçok dermatolojik hastalığın tedavisinde yaygın olarak kullanılmaktadır. ...Siklosporin A'nın terapotik etkinliği, T hücrelerinden lenfokin üretiminin baskılamasına dayanmaktadır. <mmunolojik etkilerinden dolayı bu ilaç özellikle nörolojik ve göz tutulumu ile seyreden Behçet hastalığında sıklıkla tercih edilmektedir.
Bu kronik hastalık remisyon ve alevlenmelerle seyrettiğinden ilaç tedavisi de uzun süreli olmaktadır. Dermatolojik hastalıklarda siklosporin A'nın uzun süreli kullanımının güvenilirliği konusunda sınırlı sayıda çalışma bulunmaktadır. Bu çalışmada, böbrek tutulumu olmayan Behçet hastalığında siklosporin A'nın uzun süre kullanımını değerlendirmeyi amaçladık. Oküler Behçet hastalığı nedeniyle en az bir yıl süreyle siklosporin A tedavisi alan toplam 19 hasta çalışmaya alındı. Bu retrospektif çalışmada, renal parametrelerdeki değişiklik (kreatinin klirensi, serum kreatinini), diğer yan etkiler ve bunların siklosporin A'nın kümülatif dozu ile ilişkisi değerlendirildi. Siklosporin A tedavisinin süresi ve total dozun renal fonksiyon parametreleri üzerinde anlamlı etkisi olmadığı ancak klinik olarak yan etkilerde artış olduğu gözlendi. Sonuç olarak renal fonksiyonlar üzerine olan bulgularımız böbrek biyopsileri ile desteklenmelidir.
Background and design: Cyclosporine A, one of the most extensively investigated immunomodulatory agents, has been widely used today in the treatment of various immunologically triggered dermatologic disorders.
The immunologic basis for the theuropeutic action of Cyclosporine A depends on the suppression on T cells lymphokine production. Due to its immunologic effects, this drug is also commonly preferred in Behçet's Disease(BD) especially with neurologic and ocular involvement. Since this chronic disease course with remissions and exacerbations, the drug therapy should be long term. There are limited amounts of study regarding the safety of Cyclosporine A in dermatologic disease on the long term use. In this study, we aimed to evaluate the long-term use of Cyclosporine A in BD without renal involvement. A total of 19 patients with ocular BD receiving Cyclosporine A therapy for at least one year were included in the study.
Material and Methods: In this retrospective study, the change in parameters of renal function (creatinine clearance, creatinine level in serum), other side effects and their relation with cumulative Cyclosporin A dosage were evaluated.
Results: The duration of Cyclosporin A therapy and total dosage were not found to have a significant affect on the parameters of renal function but depending on the clinical observations the side effects seem to increase.
Conclusion: We conclude that our results on renal function should be supported by renal biopsies.
Araştırmada, kronik böbrek yetmezliği (KBY) gözlenen kedilerde hematolojik ve biyokimyasal değişikliklerin incelenmesi ve bulguların hastalığın derecelendirilmesinde kullanılması amaçlanmıştır. KBY ...tespit edilen ve yaşları 3-12 arasında değişen 22 dişi kedi ile aynı yaşlarda ve klinik olarak sağlıklı görünen 10 dişi kedi (kontrol) araştırmanın hayvan materyalini oluşturmuştur. Hayvanlara ait idrar örneklerinde protein varlığı ve dansite; kan örneklerinde hemoglobin, hematokrit değer, alyuvar ve akyuvar sayıları ile akyuvarların % dağılımları; serum örneklerinde ise toplam protein, üre, kreatinin, sodyum, potasyum, kalsiyum, fosfor, osteokalsin (OC), toplam ALP (t-ALP) ve kemik spesifik ALP (k-ALP) değerleri ölçülmüştür. KBY’li kedilerde poliüri, idrar dansitesinde düşüş, azotemi, hipokalemi, hiponatremi, hiperfosfatemi, hiperproteinemi, normositik normokromik anemi ve akyuvar artışı saptanmış; serum osteokalsin değerleri hasta ve kontrol grupta ölçüm sınırlannın altında (<1ng/ml) bulunmuştur. Kan serumunda üre ve kreatinin değerlerinin KBY’li kedilerde “üremi” düzeylerine (320.7±38.5 mg/dl) yükseldiği (p<0.001); kreatinin değerlerinin de (6.77±0.75 mg/dl) hastalığın ciddi boyutlarda olduğunu gösterdiği saptanmıştır. Serum Na ve K değerlerinin hasta kedilerde önemli derecede düştüğü (p<0.05); Ca değerlerinde istatistik önemli bir değişiklik gözlenmezken (p>0.05) Pi değerlerinin önemli derecede (p<0.001) artış gösterdiği (hiperfosfatemi) saptanmış ve değişiklikler glomeruler filtrasyon hızında düşüş ve tubuler geri emilimde bozukluk ile ilişkilendirilmiştir. İncelenen parametreler kümeleme testi ile değerlendirilerek kedilerde KBY (1+)-(3+) arasında derecelendirilmiş ve 4 hastada şiddetli, 9 hastada orta ve geri kalan 9 hastada ise hafif seyrettiği saptanmıştır. Kan hematokrit yüzdesi ve alyuvar sayısındaki önemli düşüşler (p<0.01) KBY ile uyumlu bulunmuştur (normositik normokromik anemi) ve akyuvar sayısındaki artış olası bir sekonder enfeksiyona bağlanmıştır. Kan hemoglobin değerlerindeki düşüş belirgin olmasına rağmen istatistik önemli bulunmamıştır (p>0.05). Serum t-ALP ve k-ALP düzeylerindeki önemli düşüşler; Ca/P dengesindeki bozukluk ve düşük osteokalsin düzeyleri kemik metabolizmasının kronik yetmezlikten henüz belirgin şekilde etkilenmediği yönünde değerlendirilmiştir.
Özet: Bu araştırmada köpeklerde deneysel olarak safra kanalı ligature edilerek oluşturulan karaciğer sirozunda gelişen hepato-renal sendromun klinik belirtileri incelenerek tanı ve tedavi yöntemleri ...araştırıldı. Safra kanalı ligature edilen (n = 14) ve kontrol grubundaki (n = 14) denekler tekrar ikişer gruba ayrılarak aldosteron reseptör antagonist! bir ilaç olan spironalakton türevi (Aldoctan® tablet) günlük 1,5 mg/kg dozunda 5 hafta süreyle verildi. Karaciğer sirozunun 4-6. haftalarında geliştiği bildirilen dekompenzasyon devresinde gerekli kontrollerin yapılması ve kontrol grupları ile karşılaştırılması sonucu ilacın hepato-renal değişiklikler ve sistemik etkileri belirlendi. Çalışma sonunda 1. Gruptaki köpeklerde nekropsi yapılarak makroskobik ve mikroskobik değişiklikleri belirlendi. Yapılan laboratuar muayeneleri sonucunda bütün köpeklerde serum AST, ALT, T. bilirubin, D. bilirubin, ALP ve GGT düzeylerinin operasyon sonrası istatistik! bakımdan anlamlı olarak arttığı, T. protein ve albumin değerlerinin ise azaldığı saptandı (P < 0,001-0,01). Serum aldosteron, sodyum ve potasyum düzeylerinin ise bütün gruplarda benzer olarak normal sınırlar içinde olduğu tespit edildi. Yapılan idrar muayenesinde hepatik siroz oluşturulan deneklerin idrarında bilirubin ve urobilinojen düzeyleri kantitatif olarak saptanırken, yapılan değerlendirmelerde sistemik hemodinamiklerin ve kalp ritminin bütün gruplarda benzer olduğu saptandı. Bununla birlikte glomeruler filtrasyon kapasitesinin özellikle aldakton kullanılmayan karaciğer sirozlu deneklerde azaldığı tespit edildi. Ultrasonografik muayenede hepato-kardiyak değerlendirmelerde önemli bir kardiyak değişikliğin meydana gelmediği gözlendi. Çalışma sonunda, sirotik hayvanlarda kullanılan aldosteron reseptör antagonistlerinin erken dönemde glomerular filtrasyon oranının azalmasına engel olduğu ve bu nedenle ödem ve asitesin önlenmesinde kullanılabileceği saptandı.
In this study, the clinical signs of hepatorenal syndrome that occurs during hepatic cirrhosis were investigated and a variety of diagnosis and therapy methods were investigated in the experimentally ligated bile ducts of dogs. The test group with ligated bile ducts (n = 14) and the sham operated control group (n = 14) were divided into 2 sub groups and administered a derivative of spironalactone (Aldoctan® tablet), an antagonist of the aldosteron receptor, at 1.5 mg/kg daily for 5 weeks. Hepatorenal changes and the systemic effects of the drug were determined by comparing the control and test groups during the decompensation period, which occurred at 4-6 weeks of hepatic cirrhosis. At the end of the study the macroscopic and microscopic changes in the livers of the dogs in Group 1 were determined. It was determined that the serum levels of AST, ALT, T. biluribin, D. biluribin, ALP and GGT significantly increased (P < 0.001-0.01) whereas the serum levels of T. protein and albumin decreased in all the dogs. It was observed that in all groups the serum levels of aldosterone, sodium and potassium were within normal limits.Urine biluribin and urobilinogen levels were quantitatively determined. Test results and examinations of the dogs showed that the systemic hemodynamics and heart rate were the same in all groups. In contrast, it was determined that the glomerular filtration rate was decreased especially in non-aldactonized cirrhotic dogs. In the ultrasonographic examination, hepatocardiac evaluation revealed that there were no significant cardiac changes. At the end of the study, it was determined that aldosterone receptor antagonists prevent decreases in the glomerular filtration rate even in the early stages, and thus can be useful to prevent the ederna and ascites in cirrhotic dogs.
Genetic and nutritional factors are determinants of total plasma homocysteine (tHCy) whose increased levels play a pathogenic role in atherothrombosis and cardiovascular disease. This study ...investigated the influence of sex, age, creatinine, serum folate, vitamin B12 and vitamin B6 (pyridoxal- 5-phosphate, PLP) on fasting (FtHCy) and two hour postmethionine loading levels of tHCy in 147 apparently healthy subjects (M/F= 82/65, age range: 14-94 y). FtHCy was higher in men than women (9.89 vs 8.00 μmol/L, p< 0.01). In males, the main determinants of FtHCy were age and folate levels, respectively explaining 20.5% and 19.0% of FtHCy variance. In women, besides age (22.6%), vitamin B12 (17.8%) rather than folate was a major determinant of FtHCy. Two hour postload tHCy, expressed both as absolute value (PML) and as the difference between 2 hour postload tHCy and FtHCy (delta tHCy) was negatively correlated with folate in both sexes, and with vitamin B12 and age in women only. In males folate was the main determinant, explaining 20% of the variance of postload values, while in females vitamin B12 and PLP were predominant, explaining respectively 40% and 20% of that variance. Age was not among the main determinants of postload values in either sex. These results demonstrate that age and vitamin status differently influence both the fasting and the postmethionine plasma homocysteine levels in normal subjects.
Preventivní opatření zaměřená na redukci tvorby kalciové litiázy by měla vycházet z pečlivého lékařského vyšetření, které zahrnuje podrobnou lékařskou a rodinnou anamnézu, anamnézu výskytu litiázy, ...hodnocení medikace, stravy a prostředí, ve kterém se pacient pohybuje. Tento článek dále popisuje indikaci vhodných zobrazovacích testů a kompletního metabolického vyšetření se zaměřením na specifické metabolické poruchy související se vznikem kalciové litiázy. Pomocí pečlivé anamnézy, zobrazovacích testů a metabolického vyšetření lze zvolit léčbu přesně na míru pro konkrétní abnormalitu.
Preventative strategies to reduce calcium stone formation are predicated on a proper and thorough medical evaluation, which includes a detailed medical, stone, medication, family, dietary and environmental history. Additionally, proper imaging studies and the details of a full metabolic evaluation are described herein, with a discussion of specific metabolic derangements associated with calcium stones. With a thorough patient history, imaging and a metabolic evaluation, the clinician can tailor medical treatment to the specific abnormality identified.

Bu çalışmada preeklamptik ve normotansif gebelerde serum ürik asit düzeyi, 24 saatlik idrarda protein, kalsiyum, kreatinin, ürik asit düzeyleri ve kreatinin klerensi arasındaki ilişkiyi saptamak ...amaçlandı. Çalışma 2000-2001 yıllarında 46 preeklamptik ve 35 normotansif proteinürisiz gebe ile yapılmıştır. Tüm hastaların serum ürik asit ve kreatinin düzeyleri saptandı. Hastaların 24 saatlik idrarları toplanarak kalsiyum, ürik asit, kreatinin, protein düzeylerine bakıldı, kreatinin klerensi hesaplandı. Tüm sonuçlar "Student's t test" kullanılarak analiz edildi. Çalışmadaki 46 hastayı içeren preeklamptik grupla (Grup P) 35 hastadan oluşan kontrol grubu (Grup C) arasında anne yaşı, gestasyon yaşı, BMİ, ve gravida bakımından istaistiksel anlamda fark yoktur. Grup P' de serum ürik asit düzeyi 6.7±1.5 mg/dl 24 saatlik proteinüri 4.126 ±2.5 gr, idrar kalsiyumu 94.21±60.88 mg/24h, idrar kreatinini 1130.71±333.1 mg/24h ve kreatinin klerensi 113.5±36.6 olarak tespit edilmişken Grup C' de serum ürik asit düzeyi 4.01±1 mg/dl, proteinüri 0.189±0.1gr/24h, idrar kalsiyumu 272.44±122.1, idrar kreatinini 1110.99±231.7mg/24h ve kreatinin klerensi 158.18±32.8 olarak tespit edildi. İstatistiksel olarak serum ürik asit, 24 saatlik idrar proteini Grup P' de Grup C' den anlamlı olarak daha yüksekti (P<0.05). Yirmi dört saatlik idrarda kalsiyum miktarı ve kreatinin klerensi Grup P'de Grup C'den oldukça düşüktür (P<0.05). İdrar kreatinin düzeylerinde iki grup arasında istatistiksel olarak anlamlı fark saptanmadı (P>0.05). Preeklampsi ülkemizde obstetrik morbidite ve mortalitenin başlıca nedenlerindendir. Erken tanı koyup girişimde bulunmak hayati önem taşımaktadır. Preeklampsi tanısında 24 saatlik idrardaki protein miktarını ölçmek altın standarttır. Unutulmamalıdır ki preeklampsi ve eklampsi görülen hastaların küçük bir kısmında proteinüri görülmemektedir. Biz çalışmamızın verilerine dayanarak 24 saatlik idrardaki kalsiyum miktarının preeklampsi tanısında faydalı bir parametre olacağına inanmaktayız.
OBJECTIVE:The aim of this study was to examine the relation between serum uric acid level, 24 hours urine protein, calcium,creatinine, uric acid levels and cretinine claearance in preeclamptic and normotensive pregnant women. STUDY DESIGN:The present study was conducted with 46 preeclamptic and 35 normotensive patients in 2000-2001. Serum uric acid and creatinine levels of all patients were determined. After collection of 24 hour urine, calcium, uric acid, creatinine, protein levels were determined and creatinine clearance calculated. All results were analysed by "Student's t test". RESULTS: Preeclamptic grup (group P) and control group (Group C) were adjusted for maternal age, getational age, BMI and gravida. The results in group P were as follows: Serum uric acid level 6.7+1.5 mg / dl, 24 hour proteinuria 4.126 + 2.5 gr., urine calcium 94.21 + 60.88 mg / 24 hours, urine creatinine 1130.71 + 333.1 mg / 24h and creatinine clearance 113.5 + 36.6 whilst they were respectively 4.01 ± 1mg / dl, 0.189 ± 0.1gr / 24h, 272.44 ± 122.1, 1110.99 ± 231.7mg / 24h and 58.18 ± 32.8 in the control group. Serum uric acid and 24 hour urine protein, were significantly higher in group P compared to Group C (p<0.05). In 24 hour urine, calcium and creatinine clearance were quite lower in Group P than Group C (p<0.05). In urine creatinine levels, no statisitically significant difference was found between two groups (p > 0.05). CONCLUSION: In the diagnosis of preeclampsia, measurement of protein in 24 hour urine is gold standard. It should be born in mind that proteinuria is not present only in a minority of patients with preeclampia and eclampsia. Bases on our findings, it is our contention that low calcium levels in 24 hour urine may be a useful parameter in the diagnosis of preeclampsia.
